[Radiosurgery].

Radiosurgery refers to the possibility of the destruction of a small area in the brain with a high dose of radiation, after determination of the target with stereotactic coordinates. Arteriovenous malformations of small size not treatable by direct surgery are obliterated by radiosurgery in nearly 80 p. 100 of the cases. Cerebral metastasis are also effectively treated with a good local control, especially in case of small solitary lesion. In acoustic tumor, of small and medium size, radiosurgery appears to be an alternative to surgery. Other benign tumors (meningioma, pituitary adenoma) have

Neurovascular radiosurgery.

This article focuses on the treatment of neurovascular diseases, in particular brain arteriovenous malformations (BAVMs), with radiosurgery. The target group for this review is physicians who manage patients with neurovascular diseases, but are not actively engaged in radiosurgery. Radiosurgery for BAVMs is an established treatment with clearly defined risks and benefits. The efficacy of radiosurgery for dural arteriovenous shunts (DAVSs) is probably similar but the treatment has not yet gained the same acceptance. Radiosurgical treatment of cavernomas (cavernous hemangiomas) remains controver

[Stereotactic radiosurgery].

Stereotactic radiosurgery (SRS) is a way of treating brain disorders with a precise delivery of a single high dose of radiation in a one-day session. Treatment involves the use of focused radiation beams delivered to a specific area of the brain to treat abnormalities, tumors or other functional disorders. Body stereotactic radiosurgery does not exist because there is no way to immobilize the body to treat a tumor in one session the way the head can be secured. Fractionated stereotactic radiation treatments which are received over a period of days or weeks may be administered in the body with

[Stereotaxic radiosurgery].

Radiosurgery aims at the precise destruction of small, defined volumes of tissue by employing ionizing radiation energy. Its methodologies may be subdivided into closed-skull, external focussed beam radiosurgery, and interstitial radiosurgery (brachytherapy). Focussed beam stereotactic radiosurgery has been used successfully for over two decades to treat cerebral arteriovenous malformations. Complete obliteration ranges from 30% to 50% after one year. After two years, obliteration is observed in up to 90% of patients. Outcome, however, is influenced by patient selection. In the treatment of ac

Radiosurgery imaging.

The most important imaging technology for GKNS continues to be magnetic resonance. The introduction of 3 Tesla machines permits quicker studies with better signal to noise ratio. The more powerful magnet increases the chances of heating the points of contact between patient and frame, but this has been solved with non-conducting nuts. There are several sequences for special functions. CISS studies are optimal for demonstrating cranial nerves in their passage through the subarachnoid space. FLAIR studies facilitate the distinction between CSF and edema due to inflammation. DTI permits the visua

Functional radiosurgery.

Although the application of stereotactic radiosurgery for the management of functional brain disorders began in 1951, almost 50 years elapsed before it received appropriate attention. Radiosurgical techniques are used to create image-guided, physiological inactivity or focally destructive brain lesions without neurophysiological guidance. The lack of neurophysiological guidance remains the greatest argument against the use of radiosurgery for selected disorders. Current anatomic targets include the trigeminal nerve (for trigeminal neuralgia), the thalamus (for tremor or pain), the cingulate gy

Proton Radiosurgery: Current Concepts and Limitations for CNS Radiosurgery.

INTRODUCTION: Stereotactic radiosurgery (SRS) is highly conformal, high-dose radiation therapy delivered in 1-5 fractions, and is considered the standard of care for several central nervous system (CNS) indications. Particle therapies, such as protons, have physical and dosimetric advantageous properties compared to photons. However, proton SRS (PSRS) is not widely performed given the few particle therapy facilities, high-cost, and limited outcomes research as a sole modality and in comparative studies. The data available differs from each pathology. For AVMs, especially those with deep or elo

[Radiology of radiosurgery].

Proper understanding of the fundamentals of radiosurgery and the main principles of radiobiology is a key point for the development and adaptation of therapeutic strategies and for the definition of indications. The prediction of local effects of radiosurgery requires taking into account both the target Volume and the marginal dose (energy,...) and not only the marginal dose which alone is poorly predictive of the outcome. Other additional factors such as topography, age, previous irradiations,... are likely to influence this risk relationship. The comparison of different dose regimens leads t

Radiosurgery of meningiomas.

In early experience, radiosurgery proved to be a relatively safe and effective therapy for selected patients with symptomatic meningiomas, including those for whom surgical resection failed. Radiosurgery also has been an effective primary treatment alternative for patients whose advanced age, medical condition, or high-risk tumor location preclude microsurgery. The long-term response to treatment, as defined by imaging and clinical findings, is not yet available. In addition, further clinical and laboratory work is necessary to determine the appropriate tumoricidal radiosurgical dose, dose-vol

Vestibular schwannoma radiosurgery after previous surgical resection or stereotactic radiosurgery.

OBJECTIVE: To evaluate radiosurgery outcomes in vestibular schwannoma (VS) patients who have undergone prior tumor treatment. METHODS: Retrospective review of 55 consecutive VS patients having radiosurgery for recurrent (n = 22) or residual tumors (n = 33) after prior microsurgery. The median time from the patients' last surgery was 60 months (range, 2-463). Forty-seven patients (84%) had enlarging tumors at the time of radiosurgery. RESULTS: The majority of patients (67%) had facial weakness prior to radiosurgery; 52 patients (95%) were deaf. The median tumor volume was 3.0 cm3 (range, 0.1-

The option of Linac-based radiosurgery in a Gamma Knife radiosurgery center.

OBJECTIVE: Due to the fundamental differences in treatment delivery, linear-accelerator-based radiosurgery can be complementary to Gamma Knife (GK) for intracranial lesions. We reviewed the effect of adding GK to an existing linear accelerator (Linac)-based radiosurgery practice and analyzed case selections for the two modalities. PATIENTS AND METHODS: UC Davis Medical Center installed a Leksell Gamma Knife Model C in October 2003 to supplement an established Linac-based radiosurgery program. Radiosurgery indications for the 15 months before and after installation were compared. RESULTS: Rad

Intracranial stereotactic radiosurgery.

Stereotactic radiosurgery (SRS) is the use of a single high dose of radiation, stereotactically directed to an intracranial region of interest, in order to create a lesion or obliterate a preexisting one. This technology has evolved over the years into the use of multiple radiation sources oriented at a variety of angles, thus permitting the creation of various treatment target shapes. This allows for non-open surgical treatment of intracranial pathologies, which significantly decreases the risk of morbidity. The destruction of pathological tissue following radiosurgery is a stepwise process t

Gamma Knife Radiosurgery.

The concept of radiosurgery and its requirements are presented. Dedicated radiosurgical tools include the modified linear accelerators working with a single rotating beam and the Gamma Knife with multiple stationary radiation sources. The 201 gamma beams from the cobalt 60 sources of the Gamma Knife intersect within 0.3 mm and it is possible to align the selected target point with the focal point within a mechanical accuracy of 0.5 mm. The technical specifications of the Gamma Knife make it possible to safely produce sharply circumscribed lesions in a single session with superior accuracy and

[Gamma knife radiosurgery].

Gamma Knife radiosurgery can be used as an alternative or complementary therapy to neurosurgery or radiotherapy for the treatment of some brain disorders or tumors of small volume. The most frequent indications are brain metastases, vestibular schwannomas, meningiomas, trigeminal neuralgia, arteriovenous malformations, some gliomas, and pituitary adenomas. Created in 1999, the Gamma Knife Center of the ULB remains currently the unique center in Belgium where a Gamma Knife radiosurgery treatment can be performed.

[Radiosurgery: basic principles].

Radiosurgery is a way of treating in-brain lesions which uses convergence of very thin multiple ionisation beams in a unique focus point coinciding with the target. This enables obtaining a therapeutic effect without opening the skull, and thus avoiding eventual complications or contraindications of classical neurosurgery. In certain cases irradiation can be delivered with submillimetric precision. This particularly new approach is historically essentially based on the work of the Swedish neurosurgeon Lars Leksell in the 50s. Since that time new imaging techniques (e.g. digital imaging) have c

[Gamma knife radiosurgery].

Gamma knife radiosurgery has become a new treatment modality in the field of neurosurgery since the first gamma knife was brought into Japan in 1990. Advances in applications of new indications and long-term results have been continued to evolve during the past 12 years. Based on the experience of more than 4,500 cases treated by gamma knife at Komaki City Hospital, long-term results of arteriovenous malformations (AVMs), metastatic brain tumors, acoustic neurinomas, meningiomas and trigeminal neuralgias are presented. Radiosurgery has become a novel treatment modality, especially for AVM, aco
