Technique for axillary radiotherapy using computer-assisted planning for high-risk skin cancer.

High-risk skin cancer arising on the upper limb or trunk can cause axillary nodal metastases. Previous studies have shown that axillary radiotherapy improves regional control. There is little published work on technique. Technique standardization is important in quality assurance and comparison of results especially for trials. Our technique, planned with CT assistance, is presented. To assess efficacy, an audit of patients treated in our institution over a 15-month period was conducted. Of 24 patients treated, 13 were treated with radical intent, 11 with this technique. With a follow up of ov

Quantitative computer-assisted analysis vs. visual estimation of MR imaging response of brain metastases to radiotherapy.

Using both quantitative computer-assisted analysis and visual estimation the MR imaging response of brain metastases to whole-brain radiotherapy (WBRT) was prospectively studied in 49 patients. Compared to the computer-assisted analysis, the precision of the visual estimation (by two readers in conference) of tumor response was determined. Also the radiological response was related to the clinical response. Six weeks after WBRT follow-up MR was performed in 27 of 49 patients. Of the remaining 22 patients without follow-up MR, 9 had died or deteriorated due to neurologic decline. In four patien

Accuracy of contour propagation from planning computed tomography to iterative cone-beam computed tomography using a deformable image registration algorithm for assisting head and neck radiotherapy.

BACKGROUND: The deteriorated image quality of cone-beam computed tomography (CBCT) reduces the accuracy of contour propagation. We investigated the accuracy of contour propagation from planning CT (pCT) to iterative CBCT (iCBCT) using deformable image registration and compared it with that of replanning CT (reCT) and Feldkamp-Davis-Kress algorithm-based CBCT (FDK-CBCT). No report exists regarding iCBCT improving the accuracy of this technique for images of the head and neck region. METHODS: We included 29 patients who underwent radiotherapy for head and neck cancer. ReCT and CBCT were perform

Diagnosis of radiosensitive hypothalamic tumors without craniotomy: endocrine and neuroradiologic studies of intracranial atypical teratomas.

Three patients with intracranial atypical teratomas presented with symptoms of endocrine dysfunction (diabetes insipidus, hyperprolactinemia, and anterior hypopituitarism) suggestive of a hypothalamic disorder. Computer-assisted tomography done in two of the three cases enabled us to document the presence of a mass and institute radiotherapy, without prior neurosurgical exploration. Computer-assisted tomography also provided a safe and effective method of assessing the effects of radiotherapy.

Adaptive radiation therapy in head and neck cancer for clinical practice: state of the art and practical challenges.

Modern radiation therapy techniques are characterized by high conformality to tumor volumes and steep dose gradients to spare normal organs. These techniques require accurate clinical target volume definitions and rigorous assessment of set up uncertainties using image guidance, a concept called image-guided radiation therapy. Due to alteration of patient anatomy, changes in tissue density/volumes and tumor shrinkage over the course of treatment, treatment accuracy may be challenged. This may result in excessive irradiation of organs at risk/healthy tissues and undercoverage of target volumes

Development and evaluation of an efficient approach to volumetric arc therapy planning.

An efficient method for volumetric intensity modulated arc therapy (VMAT) planning was developed, where a single arc (360 degrees or less) is delivered under continuous variation of multileaf collimator (MLC) segments, dose rate, and gantry speed. Plans can be generated for any current linear accelerator that supports these degrees of freedom. MLC segments are derived from fluence maps at relatively coarsely sampled angular positions. The beam segments, dose rate, and gantry speed are then optimized using direct machine parameter optimization based on dose volume objectives and leaf motion con

[The need for a paradigm shift in radiotherapy].

The status and indications of radiotherapy have significantly changed in the past decade because novel techniques, radiobiological research and major advances in informatics have made better local control possible. Using supplemented marking of the target volume with computer tomography based other image-making methods adapted made it possible to define the tumor and intact surrounding tissues more precisely. With novel radiotherapy techniques the dosage of the homogenity and the covering in the target volume can be raised optimally, especially with intensity modulated arc radiotherapy (volume

Many-isocenter optimization for robotic radiotherapy.

Despite significant dosimetric gains, clinical implementation of the 4π non-coplanar radiotherapy on the widely available C-arm gantry system is hindered by limited clearance, and the need to perform complex coordinated gantry and couch motion. A robotic radiotherapy platform would be conducive to such treatment but a new conflict between field size and MLC modulation resolution needs to be managed for versatile applications. This study investigates the dosimetry and delivery efficiency of purposefully creating many isocenters to achieve simultaneously high MLC modulation resolution and large

Feasibility of non-coplanar tomotherapy for lung cancer stereotactic body radiation therapy.

To quantify the dosimetric gains from non-coplanar helical tomotherapy (HT) arcs for stereotactic body radiation therapy (SBRT) of lung cancer, we created oblique helical arcs by rotating patient's CT images. Ten, 20 and 30 degrees of yaws were introduced in the treatment planning for a patient with a hypothetical lung tumor at the upper, middle and lower portion of the right lung, and the upper and middle left lung. The planning target volume (PTV) was 43 cm(3). 60 Gy was prescribed to the PTV. Dose to organs at risk (OARs), which included the lungs, heart, spinal cord and chest wall, was opt

An on-line replanning method for head and neck adaptive radiotherapy.

Daily setup for head and neck (HN) radiotherapy (RT) can vary randomly due to neck rotation and anatomy change. These differences cannot be totally corrected by the current practice of image guided RT with translational repositioning. The authors present a novel rapid correction scheme that can be used on-line to correct both interfractional setup variation and anatomy change for HN RT. The scheme consists of two major steps: (1) Segment aperture morphing (SAM) and (2) segment weight optimization (SWO). SAM is accomplished by applying the spatial relationship between the apertures and the cont

New target volume delineation and PTV strategies to further personalise radiotherapy.

Target volume delineation uncertainty (DU) is arguably one of the largest geometric uncertainties in radiotherapy that are accounted for using planning target volume (PTV) margins. Geometrical uncertainties are typically derived from a limited sample of patients. Consequently, the resultant margins are not tailored to individual patients. Furthermore, standard PTVs cannot account for arbitrary anisotropic extensions of the target volume originating from DU. We address these limitations by developing a method to measure DU for each patient by a single clinician. This information is then used to

An interactive treatment planning system for ophthalmic plaque radiotherapy.

Brachytherapy using removable episcleral plaques containing sealed radioisotope sources is being studied as an alternative to enucleation in the treatment of choroidal melanoma and other tumors of the eye. Encouraging early results have been reported, but late complications which lead to loss of vision continue to be a problem. A randomized national study, the Collaborative Ocular Melanoma Study (COMS) is currently in progress to evaluate the procedure. The COMS specified isotope is 125I. Precise dosimetric calculations near the plaque may correlate strongly with complications and could also b

Accuracy in radiation field alignment in head and neck cancer: a prospective study.

A prospective study has been performed to determine the accuracy of radiation field alignment for a group of 22 patients with tumors in the head and neck. The accuracy was assessed by an analysis of 138 megavolt portal films in comparison to 55 simulation films. The distance (at the patient midplane) between corresponding points at the field edges on verification film and simulation film appeared to be 5 mm on the average and the standard deviation 5 mm. The analysis was extended by translational and rotational matching of the fields in order to separate each error in a translation error of th

Evaluation of the Ethos synthetic computed tomography for bolus-covered surfaces.

PURPOSE: Ethos allows online adaption of radiotherapy treatment plans. Dose is calculated on synthetic computed tomographies (sCT), CT-like images generated by deforming planning CTs (pCT) onto daily cone beam CTs (CBCT) acquired during treatment sessions. Errors in sCT density distribution may lead to dose calculation errors. sCT correctness was investigated for bolus-covered surfaces. METHODS: pCTs were recorded of a slab phantom covered with bolus of different thicknesses and with air gaps introduced by spacer rings of variable diameters and heights. Treatment plans were irradiated followi

Characterization of the output for helical delivery of intensity modulated slit beams.

The UW tomotherapy workbench utilizes a convolution/superposition based dose calculation and optimization program. It specifies the energy fluence that must be delivered from each leaf for each phantom projection angle. This requires that the spectrum of the radiation emitted from the one-dimensional MLC (multileaf collimator) attached to the linear accelerator be determined. The steps involved in that process are described. The spectrum along the central axis of the slit beam was determined, as well as the softening with off-axis position. Moreover, the magnitude of the energy-fluence output

CAD/CAM-assisted ablative surgery and intraoperative brachytherapy for pediatric skull-base sarcomas.

BACKGROUND: Head and neck (H&N) sarcomas in children can poise numerous challenges to the surgical oncologist and require multidisciplinary input and meticulous surgical planning. The application of computer-assisted design/computer-assisted manufacturing (CAD/CAM) has been extensively examined in H&N reconstruction in adults, but its utility in ablative oncologic surgery in children warrants further examination. We present preliminary results utilizing CAD/CAM techniques to assist in planning tumor resections and the application of intra-operative radiation in children with skull-base sarcoma
