High-risk skin cancer arising on the upper limb or trunk can cause axillary nodal metastases. Previous studies have shown that axillary radiotherapy improves regional control. There is little published work on technique. Technique standardization is important in quality assurance and comparison of results especially for trials. Our technique, planned with CT assistance, is presented. To assess efficacy, an audit of patients treated in our institution over a 15-month period was conducted. Of 24 patients treated, 13 were treated with radical intent, 11 with this technique. With a follow up of ov

BACKGROUND: The deteriorated image quality of cone-beam computed tomography (CBCT) reduces the accuracy of contour propagation. We investigated the accuracy of contour propagation from planning CT (pCT) to iterative CBCT (iCBCT) using deformable image registration and compared it with that of replanning CT (reCT) and Feldkamp-Davis-Kress algorithm-based CBCT (FDK-CBCT). No report exists regarding iCBCT improving the accuracy of this technique for images of the head and neck region.
METHODS: We included 29 patients who underwent radiotherapy for head and neck cancer. ReCT and CBCT were perform

Modern radiation therapy techniques are characterized by high conformality to tumor volumes and steep dose gradients to spare normal organs. These techniques require accurate clinical target volume definitions and rigorous assessment of set up uncertainties using image guidance, a concept called image-guided radiation therapy. Due to alteration of patient anatomy, changes in tissue density/volumes and tumor shrinkage over the course of treatment, treatment accuracy may be challenged. This may result in excessive irradiation of organs at risk/healthy tissues and undercoverage of target volumes

An efficient method for volumetric intensity modulated arc therapy (VMAT) planning was developed, where a single arc (360 degrees or less) is delivered under continuous variation of multileaf collimator (MLC) segments, dose rate, and gantry speed. Plans can be generated for any current linear accelerator that supports these degrees of freedom. MLC segments are derived from fluence maps at relatively coarsely sampled angular positions. The beam segments, dose rate, and gantry speed are then optimized using direct machine parameter optimization based on dose volume objectives and leaf motion con

PURPOSE: To calculate an optimal collimator angle at each of sectional arcs in a full-arc volumetric modulated arc therapy (VMAT) plan and evaluate dosimetric quality of these VMAT plans comparing full-arc VMAT plans with a fixed collimator angle.
METHODS: Seventeen patients who had irregularly-shaped target in abdominal, head and neck, and chest cases were selected retrospectively. To calculate an optimal collimator angle at each of sectional arcs in VMAT, integrated MLC apertures which could cover all shapes of target determined by beam's-eye view (BEV) within angular sections were obtained

BACKGROUND: Head and neck (H&N) sarcomas in children can poise numerous challenges to the surgical oncologist and require multidisciplinary input and meticulous surgical planning. The application of computer-assisted design/computer-assisted manufacturing (CAD/CAM) has been extensively examined in H&N reconstruction in adults, but its utility in ablative oncologic surgery in children warrants further examination. We present preliminary results utilizing CAD/CAM techniques to assist in planning tumor resections and the application of intra-operative radiation in children with skull-base sarcoma

BACKGROUND: High-dose rate brachytherapy (HDR BT) has a steep dose gradient that helps sparing surrounding organs, at the same time allowing to irradiate the target organ with more dose. However, the limited fraction count and sharp dose fall-off require high positional accuracy, which currently cannot be verified during HDR BT treatment.
PURPOSE: To address the risk of inaccuracies in low-fraction HDR BT, a semiconductor-based in vivo dosimetry (IVD) system for real-time dose monitoring was developed.
METHODS: The dosimetry system consists of three needles, each containing three 0.7 ×0.7× 0

PURPOSE: With external beam radiation therapy, uncertainties in treatment planning and delivery can result in an undesirable dose distribution delivered to the patient that can compromise the benefit of treatment. Techniques including geometric margins and probabilistic optimization have been used effectively to mitigate the effects of uncertainties. However, their broad application is inconsistent and can compromise the conclusions derived from cross-technique and cross-modality comparisons.
METHODS AND MATERIALS: Conventional methods to deal with treatment planning and delivery uncertaintie

Knowledge-based treatment planning (KBP) has been traditionally employed to improve consistency and efficiency in radiation treatment planning. This work introduces a unique use of the KBP model to build site-specific treatment planning modules to provide inexperienced treatment planners with planning intuitions and enhanced learning experience. Our experimental design divided seven planners into two main categories: (a) three planners with more than 6 months of experience and (b) four planners with less than a month of experience. Prior to going through the training module, each planner optim

AIMS: Advances in radiotherapy have led to increasingly conformal and complex treatment plans. The progressive reduction in safety margins around the target volume and the increased use of hypofractionated radiotherapy further heighten their vulnerability to systematic geometric uncertainties, which may compromise target volume coverage and increase doses to normal tissues. Evaluating treatment plan robustness, therefore, is crucial to ensuring the safe and effective delivery of radiotherapy. This systematic literature review provides a comprehensive overview of current practices for assessing

Scleredema adultorum of Buschke is an unusual manifestation of diabetes mellitus that can result in painful indurations and thickening of the skin with associated limitation of motion, and has been previously reported to be responsive to radiotherapy. We report on the method employed to treat a severely affected patient by means of opposed photon fields and multiple electron fields encompassing his entire neck, arms, and thorax.

A prospective study has been performed to determine the accuracy of radiation field alignment for a group of 22 patients with tumors in the head and neck. The accuracy was assessed by an analysis of 138 megavolt portal films in comparison to 55 simulation films. The distance (at the patient midplane) between corresponding points at the field edges on verification film and simulation film appeared to be 5 mm on the average and the standard deviation 5 mm. The analysis was extended by translational and rotational matching of the fields in order to separate each error in a translation error of th

Despite significant dosimetric gains, clinical implementation of the 4π non-coplanar radiotherapy on the widely available C-arm gantry system is hindered by limited clearance, and the need to perform complex coordinated gantry and couch motion. A robotic radiotherapy platform would be conducive to such treatment but a new conflict between field size and MLC modulation resolution needs to be managed for versatile applications. This study investigates the dosimetry and delivery efficiency of purposefully creating many isocenters to achieve simultaneously high MLC modulation resolution and large

The status and indications of radiotherapy have significantly changed in the past decade because novel techniques, radiobiological research and major advances in informatics have made better local control possible. Using supplemented marking of the target volume with computer tomography based other image-making methods adapted made it possible to define the tumor and intact surrounding tissues more precisely. With novel radiotherapy techniques the dosage of the homogenity and the covering in the target volume can be raised optimally, especially with intensity modulated arc radiotherapy (volume

BACKGROUND: Throughout a patient's course of radiation therapy, maintaining accuracy of their initial treatment plan over time is challenging due to anatomical changes-for example, stemming from patient weight loss or tumor shrinkage. Online adaptation of their RT plan to these changes is crucial, but hindered by manual and time-consuming processes. While deep learning (DL) based solutions have shown promise in streamlining adaptive radiation therapy (ART) workflows, they often require large and extensive datasets to train population-based models.
PURPOSE: This study extends our prior researc

Modern radiotherapy delivers highly conformal dose distributions to irregularly shaped target volumes while sparing the surrounding normal tissue. Due to the complex planning and delivery techniques, dose verification and validation of the whole treatment workflow by end-to-end tests became much more important and polymer gel dosimeters are one of the few possibilities to capture the delivered dose distribution in 3D. The basic principles and formulations of gel dosimetry and its evaluation methods are described and the available studies validating device-specific geometrical parameters as wel

BACKGROUND: Adaptive radiotherapy (ART) involves the continuous adaptation of the radiation plan according to patient- and tumorspecific feedback. In online ART, the plan is optimized in real time during the treatment; in offline ART, the plan is recalculated between treatment sessions. Hybrid linear accelerators with integrated CT, MRI, or PET are required to perform online ART.
METHODS: This review is based on clinically relevant studies on online ART (January 2019 - May 2024) that were retrieved by a selective search in PubMed.
RESULTS: Online ART is a new technique for which no phase 3 t

Target volume delineation uncertainty (DU) is arguably one of the largest geometric uncertainties in radiotherapy that are accounted for using planning target volume (PTV) margins. Geometrical uncertainties are typically derived from a limited sample of patients. Consequently, the resultant margins are not tailored to individual patients. Furthermore, standard PTVs cannot account for arbitrary anisotropic extensions of the target volume originating from DU. We address these limitations by developing a method to measure DU for each patient by a single clinician. This information is then used to
