When randomization is not random: Allocation bias in small sample, group sequential randomized clinical trials.

Even in rare diseases, where the sample size is limited and blinding is less frequently implemented, randomized controlled trials are considered the gold standard to prove efficacy. Randomization is used to mitigate bias and regulatory guidance recommend the investigation of the impact of bias on the test decision. We quantified how allocation bias affects the test decision in small-sample two-arm group sequential trials under a biasing policy based on the Blackwell-Hodges convergence strategy. Type I error and power were evaluated under Lan-DeMets spending (Pocock-, O'Brien-Fleming-, Wang-Tsi

Patient Preference for Medical Information in the Emergency Department: Post-Test Survey of a Random Allocation Intervention.

INTRODUCTION: Health literacy can create barriers for ED staff attempting to communicate important information to patients. Video discharge instructions may address some of these barriers by improving patients' comprehension of medical information and addressing health literacy challenges. METHODS: One hundred ninety-six patients diagnosed with either hypertension, asthma, congestive heart failure, or diabetes were randomly assigned to 1 of 2 interventions: watching video medical information followed by reviewing written discharge instructions or written instructions first, followed by video

Clinical effect of naturally random allocation to lower systolic blood pressure beginning before the development of hypertension.

Systolic blood pressure (SBP) rises approximately linearly with age in most societies. The cause of this rise is unclear. We tested the hypothesis that SBP is causally associated with the rate of rise in SBP with age by evaluating the effect of 12 polymorphisms associated with lower SBP on the age-related rate of rise in SBP in a series of meta-regression analyses involving ≤199 477 participants in 63 studies. We then evaluated the effect of these polymorphisms on the odds of coronary heart disease in 22,223 case and 64,762 control subjects and compared it with the effect of lower SBP observed

Impact of Responsibility Allocation Structures on Diagnostic Quality in AI-Assisted Diagnosis: Randomized Controlled Experiment.

BACKGROUND: AI is increasingly used to support clinical diagnosis, but the appropriate allocation of responsibility between clinicians and AI remains unclear. Different responsibility structures may influence how clinicians evaluate AI recommendations and revise their diagnostic judgments. OBJECTIVE: This study aimed to examine how different physician-AI responsibility allocation structures affect diagnostic accuracy and confidence calibration during AI-assisted diagnosis. METHODS: This individually randomized, 4-arm, parallel-group controlled experiment was conducted in a simulated clinical

Determination of the Optimal Volume of Programmed Intermittent Epidural Bolus When Combined With the Dural Puncture Epidural Technique for Labor Analgesia: A Random-Allocation Graded Dose-Response Study.

BACKGROUND: The dural puncture epidural (DPE) and the programmed intermittent epidural bolus (PIEB) techniques are recent innovations for labor analgesia. The optimal volume of PIEB during traditional epidural analgesia has been investigated previously but it is unknown whether these findings are applicable to DPE. This study aimed to determine the optimal volume of PIEB for effective labor analgesia after initiation of analgesia using DPE. METHODS: Parturients requesting labor analgesia received dural puncture with a 25-gauge Whitacre spinal needle and then had analgesia initiated with 15 mL

A data-informed system to manage scarce blood product allocation in a randomized controlled trial of convalescent plasma.

BACKGROUND: Equitable allocation of scarce blood products needed for a randomized controlled trial (RCT) is a complex decision-making process within the blood supply chain. Strategies to improve resource allocation in this setting are lacking. METHODS: We designed a custom-made, computerized system to manage the inventory and allocation of COVID-19 convalescent plasma (CCP) in a multi-site RCT, CONCOR-1. A hub-and-spoke distribution model enabled real-time inventory monitoring and assignment for randomization. A live CCP inventory system using REDCap was programmed for spoke sites to reserve,

Disappointment and adherence among parents of newborns allocated to the control group: a qualitative study of a randomized clinical trial.

BACKGROUND: When a child participates in a clinical trial, informed consent has to be given by the parents. Parental motives for participation are complex, but the hope of getting a new and better treatment for the child is important. We wondered how parents react when their child is allocated to the control group of a randomized controlled trial, and how it will affect their future engagement in the trial. METHODS: We included parents of newborns randomized to the control arm in the Danish Calmette study at Rigshospitalet in Copenhagen. The Calmette study is a randomized clinical trial inves

Optimal Participant Allocation in Individually-Randomized Stepped-Wedge Clinical Trials With One or More Treatments: Extensions to Realistic Settings.

In stepped-wedge clinical trials the patients are randomized into separate groups. Each group transitions from no treatment to treatment at a different time period. The designs are useful when limited resources or geographical constraints make it impossible to treat all patients at the same time. In applications of the design to individuals, it is typical that there is a single treatment and that equal numbers of patients are randomized to each sequence. Results from the methods of optimal experimental design indicate that this equal allocation is not optimal. Building on an information-theore

Pre-emptive hydrocortisone therapy in early septic shock: a double-blind, allocation-concealed, pilot randomized controlled trial.

BACKGROUND: Sepsis, a major global health issue, often progresses to septic shock with high morbidity and mortality. Corticosteroids especially low-dose hydrocortisone have shown promise in septic shock management. However, the role of hydrocortisone as pre-emptive therapy in early septic shock remains unclear. OBJECTIVE: This study investigates the effects of pre-emptive administration of low-dose, short-course hydrocortisone on outcomes in patients with early septic shock. METHODS: A double-blind, randomized controlled trial was conducted, enrolling individuals with early septic shock. Pat

Senolytics To slOw Progression of Sepsis (STOP-Sepsis) in elderly patients: Study protocol for a multicenter, randomized, adaptive allocation clinical trial.

BACKGROUND: Senescent immune cells exhibit altered gene expression and resistance to apoptosis. The prevalence of these cells increases with age and emerging data implicate senescence-associated maladaptive signaling as a potential contributor to sepsis and septic shock. The senolytic drug fisetin promotes clearance of senescent cells and is hypothesized to mitigate septic responses to infection. METHODS: We are conducting a multi-center, randomized, double-blinded, adaptive allocation phase 2 clinical trial to assess the efficacy of the senolytic drug fisetin in preventing clinical deteriora

The effects of integrated single- and dual-task training on automaticity and attention allocation in Parkinson's disease: A secondary analysis from a randomized trial.

OBJECTIVE: People with Parkinson's disease (PwPD) demonstrate impaired automaticity of motor and cognitive tasks, with unclear prioritization strategies when exposed to dual-task situations. However, no randomized trials have investigated the effects of training on automaticity and prioritization strategies in this population. The purpose of this study was to investigate the effects of training on the automaticity of gait and cognitive processing in PwPD and the allocation of attention between gait and a cognitive task. METHOD: One-hundred PwPD were randomized to 10 weeks of challenging gait
