Managing the patient with hypertension and elevated heart rate in the routine care setting: expert opinion and narrative review.

Increased resting heart rate (RHR) due to overactivity of the sympathetic nervous system (SNS) increases the risk of adverse cardiovascular outcomes in hypertension by promoting multiple aspects of hypertension-mediated organ damage (HMOD) such as endothelial dysfunction, arterial stiffness, left ventricular hypertrophy and renal damage. A vicious cycle of SNS overactivity and adverse cardiovascular remodelling leads ultimately to increased risk of adverse clinical cardiovascular outcomes and heart failure with either reduced or preserved left ventricular ejection fraction. Epidemiological stu

Delirium detection tools show varying completion rates and positive score rates when used at scale in routine practice in general hospital settings: A systematic review.

BACKGROUND: Multiple short delirium detection tools have been validated in research studies and implemented in routine care, but there has been little study of these tools in real-world conditions. This systematic review synthesized literature reporting completion rates and/or delirium positive score rates of detection tools in large clinical populations in general hospital settings. METHODS: PROSPERO (CRD42022385166). Medline, Embase, PsycINFO, CINAHL, and gray literature were searched from 1980 to December 31, 2022. Included studies or audit reports used a validated delirium detection tool

Comparison of the Pathological Complete Response Rate and Survival Between HER2-Low and HER2-Zero Breast Cancer in Neoadjuvant Chemotherapy Setting: A Systematic Review and Meta-Analysis.

Although HER2-low breast cancer (BC) constitutes almost 50% of all BC types, its impact on the pathological complete response (pCR) rate and survival in early BC is uncertain. As a result, a systematic review was conducted to compare the pCR rate and survival of HER2-low and HER2-zero BC in the neoadjuvant chemotherapy (NACT) setting. Two reviewers independently performed literature searches using EMBASE, PubMed, and Cochrane Libraries internet databases up to June 2023. Finally, 29 studies with 178,294 patients were included. HER2-low BC had a considerably lower pCR rate compared to HER2-zero

Early neonatal mortality is modulated by gestational age, birthweight and fetal heart rate abnormalities in the low resource setting in Tanzania - a five year review 2015-2019.

BACKGROUND: Early Neonatal mortality (ENM) (< 7 days) remains a significant problem in low resource settings. Birth asphyxia (BA), prematurity and presumed infection contribute significantly to ENM. The study objectives were to determine: first, the overall ENM rate as well as yearly ENM rate (ENMR) from 2015 to 2019; second, the influence of decreasing GA (< 37 weeks) and BW (< 2500 g) on ENM; third, the contribution of intrapartum and delivery room factors and in particular fetal heart rate abnormalities (FHRT) to ENM; and fourth, the Fresh Still Birth Rates (FSB) rates over the same time pe

Outcomes of Cross-Leg Flap Use in Lower Limb Salvage in Resource-Limited Settings: A Systematic Review of Indications, Limitations, and Success Rates.

Cross-leg flaps remain a vital reconstructive option in the management of complex lower limb injuries, particularly in settings where microsurgical techniques are unavailable or unaffordable. This systematic review evaluates the outcomes, indications, limitations, and success rates of cross-leg flap use in lower limb salvage within resource-limited settings. A comprehensive search of PubMed, Google Scholar, Scopus, ScienceDirect, and the Cochrane Library was conducted to identify studies published up to July 2025, following Preferred Reporting Items for Systematic reviews and Meta-Analyses (PR

The Clinical Role of Heart Rate Variability Assessment in Cognitively Impaired Patients and Its Applicability in Community Care Settings: A Systematic Review of the Literature.

Heart rate variability (HRV) correlates well with a person's overall physiological function. Clinically, HRV is successfully used in acute care to identify impending infections, but little is known about its potential in the management of chronic diseases like cognitive decline/dementia. The aim of this study was to identify the best available knowledge about HRV in cognitively impaired populations that might be applied to improve clinical practice in community settings. We conducted a systematic literature search in PubMed, Embase, and Cochrane databases published from January 2009 to August

Setting a baseline: A 7-year review of referral rates and outcomes for serous ovarian cancer prior to implementation of oncologist mediated genetic testing.

OBJECTIVE: Despite guidelines recommending that all women with invasive serous ovarian cancer (SOC) are offered genetic testing, published referral and testing rates have been poor. Many centers have implemented novel genetic counseling service delivery models to increase testing rates. In light of increased awareness and implementation of small process changes at our center, this study aims to establish baseline referral rates and testing outcomes prior to diverging from the traditional model of care. METHODS: A list of women diagnosed with SOC at Princess Margaret Cancer Center (PM) between

Heart rate variability as a strain parameter in maritime settings: a scoping review of methodological approaches to data collection and analysis in field studies on the high seas.

BACKGROUND: Seafarers are exposed to unique occupational stressors that place sustained demands on the autonomic nervous system. Heart rate variability (HRV) represents a promising non-invasive tool for assessing mental and physical strain in maritime settings. However, its methodological application in real field conditions remains poorly characterized, and existing HRV guidelines do not specifically address the methodological challenges of maritime field studies. METHODS: A scoping review was conducted following PRISMA guidelines. Eleven maritime field studies measuring HRV in operational o

Performance of automated respiratory rate counters for respiratory rate assessment to support pneumonia detection among children under five years of age in low- and middle-income settings: a systematic review of qualitative and quantitative evidence.

BACKGROUND: Accurate respiratory rate (RR) assessment is essential for pneumonia identification under World Health Organization Integrated Management of Childhood Illness (IMCI) guidelines; however, manual RR counting is often inaccurate and difficult in routine care. Automated RR counters have been developed to address these challenges, but evidence on their diagnostic performance and implementation in low- and middle-income countries (LMICs) remains limited. This systematic review evaluated the accuracy, usability, acceptability and time efficiency of automated RR counters for RR assessment

Highly active antiretroviral therapy is necessary but not sufficient. A systematic review and meta-analysis of mortality incidence rates and predictors among HIV-infected adults receiving treatment in Ethiopia, a surrogate study for resource-poor settings.

BACKGROUND: Owing to the introduction of highly active antiretroviral therapy (HAART), the trajectory of mortality and morbidity associated with human immunodeficiency virus (HIV) infection has significantly decreased in developed countries. However, this remains a formidable public health challenge for people living with HIV in resource-poor settings. This study was undertaken to determine the pooled person-time incidence rate of mortality, analyze the trend, and identify predictors of survival among HIV-infected adults receiving HAART. METHODS: Quantitative studies were searched in PubMed,

Understanding the context for long-range planning in hospitals.

Using their own experience in an urban hospital of national reputation, the authors describe and illustrate some of the background issues for long-range planning decisions pointing out that managers must steer a clear course among a number of internal and external pressures. The expectations of trustees, philanthropists, reimbursing agencies, the university, medical staff, patients and the surrounding community may sometimes be at odds. Managers can use long-range planning as a tool to ease the hospital into changing modes of operation and to achieve, over a time, a smoother interaction among

Implications of Site-Neutral Rate-Setting Methods for Medicare Spending.

IMPORTANCE: Medicare pays more for some procedures in the hospital outpatient department (HOPD) setting than it does for the same procedures in physician offices because of the facility fees paid to HOPDs under Medicare's Outpatient Prospective Payment System (OPPS). In recent years, Medicare has reduced facility fees for some services and certain types of HOPDs to make reimbursement site neutral, both to rein in Medicare spending and to avoid incentivizing the migration of services from physician offices to HOPDs. OBJECTIVE: To quantify Medicare payments and savings under 2 approaches of set
