Update on Rectal Neuroendocrine Neoplasms.

Rectal neuroendocrine neoplasms (R-NENs) are increasingly identified in clinical practice, mainly due to the expansion of colorectal cancer screening programs. These tumors are most often detected incidentally as small, well-differentiated lesions. Although many R-NENs exhibit indolent behavior, prognosis and management depend on accurate staging, with particular emphasis on evaluating regional lymph nodes and distant metastases. Cross-sectional imaging is central to this assessment. Endoscopy and endoscopic ultrasound provide essential information regarding tumor size and depth of invasion, w

[Trans-anal resection of low rectal neoplasms].

BACKGROUND: The rising incidence of colorectal neoplasms, and in particular those localised in the lower rectum is stressed and the therapeutic opportunities offered by the trans-anal resection technique are underlined. METHODS: The indispensable conditions for adopting a surgical approach are pointed out. These are identified as the size of the neoplasm, which should not exceed 4 cm, the fixity, site, the polypoid and non-ulcerated nature of the lesion, and the involvement of not more than 1/4 of the circumference of the bowel. Using these inclusion criteria, the authors operated on five pat

Solitary rectal ulcer syndrome mimicking rectal neoplasm on MRI.

Solitary rectal ulcer syndrome (SRUS) is a rare condition that typically affects young adults and describes a spectrum of clinicopathological abnormalities. The diagnosis of SRUS is usually made on the basis of a combination of presenting symptoms and endoscopic and histological appearances. Characteristic radiological appearances have been described on transrectal and endoanal ultrasound, defecating proctography and barium enema. The radiological appearance of solitary rectal ulcer on MRI has not been previously described. MRI appearance of thickened ulcerated mucosa in the anterior rectal wa

[Value of preoperative colonoscopy in colon-rectal neoplasms].

The surgical approach to treat colo-rectal carcinoma is usually based on the findings of barium enema. In 102 patients we reviewed the yield of pre-operative colonoscopy. Barium enema revealed the cancer in 76 of 84 patients (91%), whereas colonoscopy did so in 96 of 102 subjects (94%). In addition, 5 associated cancerous lesions were demonstrated by colonoscopy and none by barium enema. Associated benign lesions were seen in 14 patients. In 5 patients the surgical plan based on the barium enema was modified by the findings at colonoscopy. Eight false negatives to barium enema were correctly d

[Progress in diagnosis and treatment of rectal neuroendocrine neoplasms].

Neuroendocrine neoplasms (NENs) are relatively rare heterogeneous tumors that originate from peptidergic neurons and neuroendocrine cells and have been referred to as "carcinoids" in the past. Although this type of tumor had been previously considered to be indolent tumor with a low degree of malignancy, with the development of medicine and clinical study, researchers found that NENs had the potential to metastasize. They can occur in any part of the body where neuroendocrine cells are distributed and gastro-entero-pancreatic neuroendocrine neoplasms (GEP-NENs) are the most common type of NENs

Robotic transanal surgery for local excision of rectal neoplasms.

Robotic transanal surgery (RTS) has been a natural evolution of transanal minimally invasive surgery. This video demonstrates how RTS is performed for the full-thickness excision of a malignant rectal polyp, with endoluminal robotic suturing of the rectal wall defect. The general approach to RTS, including the approach to docking of the robotic cart, are also illustrated in video format.

[Transanal endoscopic microsurgery for the treatment of localized rectal neoplasms].

OBJECTIVE: To investigate the therapeutic effect of transanal endoscopic microsurgery (TEM) for localized rectal neoplasms. METHODS: Seventy-five patients with localized rectal neoplasms were treated by using TEM between April 2006 and December 2008. The clinical data was summarized and analyzed retrospectively to report the therapeutic effect of TEM in these cases. RESULTS: The mean diameter of the rectal lesions was (1.6 +/- 0.8) cm (range, 0.5-5.0 cm). The average distance of lesions from the anal verge was (7.6 +/- 2.8) cm (range, 5-20 cm). Locations of the lesions at the rectal wall: 25

Transanal endoscopic microsurgery for rectal neoplasms. How I do it.

INTRODUCTION: Transanal endoscopic microsurgery (TEM) has revolutionized the technique and outcomes of transanal surgery, becoming the standard of treatment for large sessile rectal adenomas, and may represent a possible treatment modality for early rectal cancer. METHODS: A full-thickness excision is made on the rectal wall down to the perirectal fatty tissue. The specimen is retrieved transanally. After the parietal defect is disinfected, the wound is closed with one or more running sutures secured with silver clips. RESULTS: Peritoneal perforation during TEM is not associated with adverse

[81 cancers of the rectosigmoid junction. Colonic or rectal neoplasms?].

With the aim of a better understanding of the cancer of the rectosigmoid junction the authors studied a series of 245 patients treated in Surgery Department 4 with the following distribution according to the localization: 113 in the sigmoid, 81 in the rectosigmoid junction and 51 in the superior rectum. The percentage of cases operated for intestinal obstruction in the rectosigmoid junction (37.0%) was higher than those in the superior rectum (7.8%) (p = 0.001) and the resectability rate of tumours located in the rectosigmoid junction (44.4%) was lower than that of those located in the sigmoid

A primary rectal neoplasm with novel DDX5-TFEB fusion.

We report a case of primary rectal neoplasm with a novel DDX5-TFEB fusion in a 14-year-old boy. Histologically, the neoplasm was composed of epithelioid tumor cells with abundant clear to eosinophilic cytoplasm, arranged in nests with rich stromal capillary vasculature. Immunohistochemically, the tumor cells were positive for transcription factor EB (TFEB) and negative for PAX8, TFE3, HMB45, and PCK. TFEB gene rearrangement and low-level amplification were identified using break-apart fluorescence in situ hybridization. Next-generation sequencing identified a heretofore unreported DDX5-TFEB ge

Rectal prolapse of an ileocaecal neoplasm associated with intussusception in a cat.

Intussusception associated with lymphoma of the ileocaecocolic junction was diagnosed in a 12-year-old female domestic short-haired cat that presented with a 3-week history of diarrhoea and a protruding anal mass. Surgical exploration revealed an ileocolonic intussusception proximal to the mass at the ileocaecal junction which was excised. A diagnosis of ileocaecocolic lymphosarcoma was made and euthanasia was later performed. This is an unusual case of an ileocaecal junction tumour that manifested as a rectal prolapse associated with intussusception in a cat.

[Bone metastases of colonic and rectal neoplasms. Apropos of 11 cases].

The authors report 11 personal cases and discuss the characteristics of bony metastases in carcinoma of the colon and rectum. The frequency of these metastases is fairly low (1.3% of all cases of bony metastases) as tumours of the large intestine do not tend to migrate to the bones. Sometimes bony metastases are the presenting symptom but, usually the metastases occur within 5 years of the diagnosis of the primary tumour. The clinical picture shows no special characteristics. Radiologically, these bony metastases may be either single or multiple. Their distribution recalls that of other metast
