As ophthalmologists, we often come across a lot of patients with neuro-ophthalmological disorders that show no positive examination findings except for subtle abnormalities in the pupillary light reflex. However, these subtle changes are often overlooked in a busy OPD setting, especially by junior doctors and nursing staff. Thus, we decided to develop an innovation to simplify the assessment of pupillary reflexes, which we have named PUPILLARY REFLEX AT EASE. Our plan involved magnifying the iris and pupil using biconvex lenses to facilitate the visualization of the response. Additionally, we

With a modified method of infrared pupillography--especially developed for the use in clinical neurological routine diagnosis--the time course of the direct phasic pupillary light reflex in man was investigated under physiological and pathological conditions. The described method allows a high resolution of the time course of the light reflex and is superior to kinematographic as well as video-methods described so far. Normally the physiological time parameters are bilaterally symmetrical, also in cases of so-called physiological anisocoria. On the other hand in pathological anisocorias of var

The size of our pupils changes continuously in response to variations in ambient light levels, a process known as the pupillary light reflex (PLR). The PLR is not a simple reflex as its function is modulated by cognitive brain function and any long-term changes in brain function secondary to injury should cause a change in the parameters of the PLR. We performed a retrospective clinical review of the PLR of our patients using the BrightLamp Reflex iPhone app. The PLR variables of latency, maximum pupil diameter (MaxPD), minimum pupil diameter (MinPD), maximum constriction velocity (MCV), and t

OBJECTIVE: To study the relation of retinal nerve fiber layer thinning to clinical and physiologic measures of visual function in patients with MS or neuromyelitis optica and unilateral optic neuropathy.
METHODS: We studied a cohort of control subjects (n = 64) and patients (n = 24) with evidence of unilateral thinning of their average retinal nerve fiber layer as measured by optical coherence tomography in order to characterize the relationship between ganglion cell axonal degeneration and its impact upon vision and pupillary light reflex metrics using infrared pupillometry.

The pupillary light reflex was investigated using electrical stimulation along the pathway and recording in the short ciliary nerves. The discharge of single units in the ciliary ganglion was compared during diffuse light stimuli and electrical stimuli. It was concluded that the early reflex discharge in the short ciliary nerves following electrical stimulation on the optic tract is due to excitation of fibres active during the pupillary light reflex. The light reflex is conveyed by slow (less than 10 m/sec) optic tract fibres which synapase in the medial part of the pretectal area. In turn, p

OBJECTIVE: We examined the relationship between modeled opioid concentration and quantitative pupillary measures during remifentanil infusion sequences with particular attention to the "light-off" (LO) reflex.
METHODS: Ten volunteer subjects were recruited to undergo two 10-min remifentanil infusion protocols. Pupillary unrest in ambient light (PUAL) and LO were measured at baseline and every 2.5 min during the first 10-min infusion-25-min recovery sequence, and after a wash-out period, the Neurological Pupillary index (NPi) and LO were measured during an identical infusion-recovery sequence.

Background: Leukocoria means white pupil. Normal pupil appears black in children and adults. The typical red reflex is due to retro-illumination of choroidal vessels reflected via the retina, vitreous humor, crystalline lens, aqueous humor, pupil, and cornea. If there is interference in these structures, it would result in a changed red reflex, or leukocoria. Immediate family members are highly likely to detect the first indicator and the pediatrician or general ophthalmologist is usually the first to be visited. The aim of the study was to find out the prevalence of common causes of white pup

Seventy-seven diabetics with a duration of the disease ranging from 2 to 55 years (average 18.5 years) were studied with infrared videopupillometry. The prevalence of diabetic autonomic neuropathy at the pupillary control system (pANP) was studied comparatively using several pupillary tests. The average prevalence using age-dependent parameters was 30.2% [maximal pupillary area: 22.1%, maximal contraction velocity: 24.7%, contraction velocity at 1 s (CV1): 28.6%, and dilation velocity at 6 s (DV6): 45.5%]. Comparing these percentages to prevalences of other diabetic late complications, e.g. re

OBJECTIVE: Traumatic optic neuropathy (TON) refers to a pathological condition caused by direct or indirect injury to the optic nerves. In the case of patients with traumatic brain injury, adequate vision evaluation is difficult in many cases due to altered mentality. In order to address this problem, we investigated preoperative pupillary light reflex in TON patients as a predictive factor of surgical outcomes after optic nerve decompression.
METHODS: From April 2020 to September 2022, we enrolled patients who were diagnosed with TON and underwent endoscopic optic nerve decompression at our

In 46 patients with definite multiple sclerosis (MS) the direct and indirect pupillary light reflex latency (PLRL) and visual evoked potential (VEP) latency of the P100 were measured for each eye separately. The PLRL was measured at both photopic and scotopic illuminance level, using an infrared light reflection technique (IRIS). On average the PLRL increased at the scotopic illuminance level as compared with the photopic (P less than 0.0001). An abnormal VEP was found in 80.4% of the patients, while 26.1% and 29.7% had an abnormal direct PLRL at photopic and scotopic illuminance level respect

BACKGROUND: In intensive care patients with disorders of consciousness, the pupillary light reflex is a measure of pupillary parasympathetic function. By contrast, the pupillary light-off reflex leads to pupil dilation in response to an abrupt change from light to darkness ("light-off") and reflects combined parasympathetic and sympathetic pupillary function. To our knowledge, this reflex has not been systematically investigated in patients with disorders of consciousness. We hypothesized that the pupillary light-off reflex correlates with consciousness levels after acute brain injury.

BACKGROUND: The aim of this study was to characterise the changes of the dynamic pupillary light reflex in patients with unilateral Horner's syndrome by appropriate pupillographic parameters.
METHODS: The dynamic changes of pupillary diameter in response to light flashes of 100 ms were recorded in darkness with two infrared video cameras in 15 patients.
RESULTS: Significant changes in the pupillary light reflex of the affected side were observed only during the phase of redilatation. The decelerated redilatation was defined by the following parameters: (1) time parameters (percentage of redi

OBJECTIVES: To evaluate the pupillary light reflex responses in patients with unilateral strabismic and anisometropic amblyopia using dynamic pupillometry.
MATERIALS AND METHODS: A total of 102 eyes of 51 patients with unilateral amblyopia were included in this cross-sectional study. Of the 51 patients, 37 (72.5%) had strabismic amblyopia and 14 (27.5%) had anisometropic amblyopia. All patients underwent complete ophthalmological examination, and pupillary light reflex responses were measured using a computerized dynamic pupillometry system (MonPack One; Metrovision, France). Autoimmune autonomic ganglionopathy (AAG) is an antibody-mediated form of severe autonomic failure. AAG is associated with serum antibodies against ganglionic acetylcholine receptors (AChR) and appears to result from impaired synaptic transmission in autonomic ganglia. The rabbit model of experimental AAG (EAAG), induced by immunization, reproduces the cardinal features of the human disease. Pupillary dysfunction is a prominent and defining feature in both AAG and EAAG. We adapted infrared computer-assisted video pupillometry to record direct pupil light responses in control and EAAG rabbits.

BACKGROUND: Although many previous studies reported abnormalities of autonomic function in patients with panic disorder (PD), almost all targets in those studies primarily focused on cardiovascular autonomic functions. In the present study, we determined whether PD patients exhibited abnormalities in the pupillary autonomic nervous system (ANS).
METHODS: Before and after audiovisual stimulation (AS), which induced mental stress through exposure to video images of high stress experiences, such as driving motor vehicles, the pupillary light reflex (PLR) was measured by infrared pupillometer in

Background and aims Pupil size and reaction are influenced by opioids, an effect that is not considered to be affected by opioid tolerance. As clinicians have observed patients on high-dose opioids who exhibited seemingly normal pupil sizes, we wanted to dynamically assess the pupillary reflex in cancer patients on high-dose opioids. Methods We performed a dynamic assessment of the pupillary reflex in cancer patients on high-dose opioids and a control group of healthy volunteers using a portable, monocular, infrared pupillometer. We also performed a clinical examination and measured blood conc

This study examined the influence of liquid crystal variable transmission lenses on pupillary light reflexes in response to sudden bright light onset. Participants were exposed to bright light while pupil size was monitored using an eye tracker; eyewear was configured across four transition conditions: constant low-light filtering, constant high-light filtering, variable-light filtering in response to light detection and a control condition without eyewear. Before light onset, pupil diameter was largest in the high-filter condition, medium in the variable- and low-light filtering conditions an

The early components of the pupillary light reflex (PLR) are governed by the parasympathetic nervous system. The use of cheap, portable pupillometry devices may allow for the testing of parasympathetic-system health in field settings. We examined the reliability of two portable instruments for measuring the PLR and their sensitivity to individual differences known to modulate the PLR. Parameters of the PLR were measured in a community sample (N = 108) in a variety of field settings. Measurements were taken using a commercial pupillometer (NeuroLight, IDMED) and an iPhone using the Reflex Pro P

OBJECTIVE: The anatomy of the human pupillary light reflex (PLR) pathway is a matter of debate. The aim of this study was twofold: namely, to investigate the association of a relative afferent pupillary defect (RAPD) in acquired suprageniculate lesions with the location and extent of the cerebral lesions. Further, we suggest a new strategy of lesion analysis by combining established techniques with the stereotaxic probabilistic cytoarchitectonic atlas developed by the Jülich group.
METHODS: Twenty-three patients with homonymous visual field defects participated in this study. The RAPD was qua
