Surgical procedures performed after refractive surgery.

BACKGROUND: Refractive surgical procedures have been performed for over one decade. The complications that cause visual disability are abnormalities in wound healing, residual ametropia, and/or severe irregular astigmatism. Many of these complications can be manifested clinically in terms of visually disabling problems (loss of best-corrected vision, glare, monocular diplopia, etc). Some of these complications require additional standard ophthalmic or refractive surgical procedures to restore vision. However, since these secondary procedures have been performed in only a few cases, we known li

Basic knowledge of refractive surgery: correction of refractive errors using modern surgical procedures.

INTRODUCTION: Refractive ophthalmic surgery allows refractive errors to be corrected permanently in a safe, effective, and reliable way with few complications. METHODS: Selective literature review with special reference to the guidelines of the German Commission for Refractive Surgery. RESULTS: With a total of almost 18 million treatments performed to date, laser in-situ keratomileusis (LASIK) is the most commonly used refractive surgical procedure worldwide. Alternatives to LASIK include surface ablation procedures (PRK, LASEK, Epi-LASIK) and phakic intraocular lens implantation. If ocular

LASIK for the correction of residual refractive errors from previous surgical procedures.

BACKGROUND AND OBJECTIVE: To evaluate laser assisted in situ keratomileusis (LASIK) efficacy in correcting residual refractive errors after corneal or intraocular surgery (penetrating keratoplasty, radial keratotomy, photorefractive keratectomy, phacoemulsification with intraocular lens (IOL) implantation, penetrating ocular trauma and phakic IOL implantation). MATERIAL AND METHODS: We performed LASIK in 87 eyes of 62 patients previously operated by means of other surgical techniques to completely correct the residual refractive error. We report the mean refractive error (in terms of spherica

Comparative analysis of IOL power calculations in postoperative refractive surgery patients: a theoretical surgical model for FS-LASIK and SMILE procedures.

BACKGROUND: As the two most prevalent refractive surgeries in China, there is a substantial number of patients who have undergone Femtosecond Laser-assisted In Situ Keratomileusis (FS-LASIK) and Small Incision Lenticule Extraction (SMILE) procedures. However, there is still limited knowledge regarding the selection of intraocular lens (IOL) power calculation formulas for these patients with a history of FS-LASIK or SMILE. METHODS: A total of 100 eyes from 50 postoperative refractive surgery patients were included in this prospective cohort study, with 25 individuals (50 eyes) having undergone

Descemet's stripping automated endothelial keratoplasty (DSAEK) using corneal donor tissue not acceptable for use in penetrating keratoplasty as a result of anterior stromal scars, pterygia, and previous corneal refractive surgical procedures.

PURPOSE: The purpose of this study was to evaluate outcomes of Descemet's stripping automated endothelial keratoplasty (DSAEK) using anterior stromal flawed (ASF) donor corneas that were unsuitable for use in full-thickness penetrating keratoplasty as a result of stromal scars, pterygia, or previous corneal refractive surgery and to compare results with DSAEK using standard tissue. METHODS: We conducted a review of our initial 42 (19 with 6-month follow up) consecutive DSAEK surgeries using ASF tissue compared with 357 (199 with 6-month follow up) time-matched controls using standard tissue.

[Corneal biomechanics].

OBJECTIVE: To review the corneal biomechanic concepts and to analyse, clarify and understand their relevance in refractive surgery. MATERIALS AND METHODS: A literature review has been done using different databases. RESULTS: Corneal biomechanic concepts are not new and are applied implicitly in numerous surgical procedures. Their origin is related to tonometry studies, but they gained in popularity when they were linked to the treatment of keratoconus, a pathology in which the mechanical properties of the cornea are altered. Factors determining corneal stability were thus defined. Corneal bi

Current paradigms in refractive surgery.

Refractive surgeries have evolved from the archaic incisional corneal procedures to the use of sophisticated femtosecond lasers and new-generation phakic intraocular lenses (pIOL) for surgical correction of refractive errors. The armamentarium of modern-day refractive surgery includes corneal-based procedures such as photorefractive keratectomy, laser-assisted in situ keratomileusis and keratorefractive lenticule extraction, as well as lensbased pIOL implantation. The current procedures are associated with a high index of efficacy and predictability, with enhanced safety and a significant redu

Risk profiles of ectasia after keratorefractive surgery.

PURPOSE OF REVIEW: To identify and evaluate the risk factors of iatrogenic ectasia after refractive surgery. RECENT FINDINGS: We reviewed recently published papers that identified various risk factors associated with ectasia after LASIK, photorefractive keratectomy, small incision lenticule extraction, and other refractive surgical procedures. We also attempted to evaluate the relative contributions of these factors to the development of ectasia following refractive surgery. Forme fruste keratoconus, genetic predisposition to keratoconus, low residual stromal bed thickness (through high myopi

Laser eye surgery for refractive errors.

Several laser and non-laser refractive surgical procedures have been used to modify the shape of the cornea and correct myopia, hyperopia, astigmatism, and presbyopia. Introduction of the excimer laser to reshape the cornea has resulted in remarkable developments in the correction of these refractive errors. Combined with other advanced ophthalmological instruments, laser refractive eye surgery has resulted in a substantial rise in the safety, efficacy, and predictability of surgical outcomes. Despite these advances, certain limitations and complications persist. In this review, we describe th

Unraveling the impact of laser refractive surgery on corneal ectasia: an in silico study.

INTRODUCTION: Laser refractive surgeries are a safe option for low-to-moderate refractive corrections, providing excellent visual outcomes. Over the years, various procedures have been introduced into clinical practice, but the most performed today remain Photorefractive Keratectomy (PRK), Laser In-Situ Keratomileusis (LASIK), and Small Incision Lenticule Extraction (SMILE). Although laser refractive treatments are considered safe, clinicians have focused on the risk of post-surgical ectasia, a rare but serious complication. Ectasia is characterized by progressive corneal thinning and steepeni

Cataract surgery in patients with prior refractive surgery.

As the number and types of keratorefractive procedures increase and as the baby boomer population moves into the "cataractous decades," the number of patients requiring cataract surgery following refractive surgery grows larger each year. While technological advances in surgical instrumentation and intraocular lens (IOL) design allow us to perform cleaner, faster, and more reliable cataract extractions, the ultimate postoperative refraction depends primarily on calculations performed before surgery. Third-generation IOL formulas ( Haigis, Hoffer Q, Holladay 2, or SRK/T) provide outstanding acc

[Chinese expert consensus on laser corneal refractive surgery for correction of refractive errors with age-related accommodation deficiency (2021)].

For people receiving laser corneal refractive surgery for refractive errors combined with age-related accommodation deficiency, more attention should be given to avoid wearing reading spectacles prematurely for presbyopia after no spectacles for distant vision are needed. Cautious preoperative screening, personalized and reasonable surgical design and postoperative care should be ensured to achieve satisfactory results. This expert consensus is written by the experts from the Refractive Surgery Group of Chinese Ophthalmologist Association based on the clinical experience and research findings.

[Advancements in machine learning applications in refractive surgery].

Refractive error is a significant factor contributing to visual impairment, imposing a relatively large burden on the social economy. Although refractive surgery is an important corrective method, it faces challenges in clinical practice, such as precise preoperative screening, personalized surgical plan design, and prevention of postoperative complications. This study focuses on the application of machine learning in the field of refractive surgery. Through a comprehensive analysis of relevant literature, it is found that machine learning plays a positive role in multiple key aspects. In preo

Managing residual refractive error after cataract surgery.

We present a review of keratorefractive and intraocular approaches to managing residual astigmatic and spherical refractive error after cataract surgery, including laser in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), arcuate keratotomy, intraocular lens (IOL) exchange, piggyback IOLs, and light-adjustable IOLs. Currently available literature suggests that laser vision correction, whether LASIK or PRK, yields more effective and predictable outcomes than intraocular surgery. Piggyback IOLs with a rounded-edge profile implanted in the sulcus may be superior to IOL exchange, bu

Screening for keratoconus suspects among candidates for refractive surgery.

This review examines methods for estimating the risk of post-surgical ectasia in candidates for refractive surgery by establishing a diagnosis of keratoconus suspect as a contraindication for proceeding with surgery. Notwithstanding the desirability of achieving 100 per cent sensitivity, any associated reduction in specificity and increased numbers of false positives might deny some candidates the opportunity to proceed with refractive surgery. The introduction of a model for the risk of ectasia involving both pre- and post-surgical findings has been followed by a plethora of attempts to achie

[Phakic intraocular lenses].

Refractive surgical procedures are generally divided into additive procedures with, e. g. implantation of an artificial lens and subtractive procedures with ablation of corneal tissue. In this article the current status of phakic intraocular lens (IOL) implantation for correction of refractive errors is reviewed. Phakic IOLs are constructed as angle-supported or iris-fixated anterior chamber lenses and sulcus-fixated posterior chamber lenses. The implantation of phakic IOLs has been demonstrated to be an effective, safe, predictable and stable procedure to correct higher refractive errors. Com
