Autonomy and autonomy competencies: a practical and relational approach.

This essay will address a general philosophical concern about autonomy, namely, that a conception of autonomy focused on freedom of the will alone is inadequate, once we consider the effects of oppressive forms of socialization on individuals' formation of choices. In response to this problem, I will present a brief overview of Diana Meyers's account of autonomy as relational and practical. On this view, autonomy consists in a set of socially acquired practical competencies in self-discovery, self-definition, self-knowledge, and self-direction. This account provides a distinction between choic

Reconsidering autonomy: Asian Americans' use of relational autonomy in organ donation decisions.

BACKGROUND: As cultural contexts have gained increasing relevance in medical decision-making, the current mainstream definition of autonomy is insufficient. A viable alternative framework, relational autonomy posits that agents' actions are influenced by and embedded in society and culture rather than occurring in isolation. To test the concept's applicability, we examine whether Asian Americans in the study's sample operationalize relational autonomy as a decisional approach in hypothetical scenarios about organ donation, a practice for which there is considerably lower enthusiasm compared to

Managing relational autonomy in interactions: People with intellectual disabilities.

BACKGROUND: This article is about interactions that occur when someone with intellectual disabilities is engaged in everyday activities with a personal assistant (PA) or a support worker. METHOD: We examine the detail of nine hours of naturally occurring video-recorded interactions, to explore how "relational autonomy" is done in practice. Nine people with ID and seven staff took part in the research, which took place in England from 2016-17. RESULTS: We selected six extracts to illustrate different types of joint decision-making. Informed by inclusive research with a drama group of people w

Whose Autonomy Matters? Using Relational Autonomy to Analyze Caregiver Roles in Decision-Making for Cancer Clinical Trials.

BACKGROUND: Participation in a therapeutic clinical trial can be a challenging decision for patients with cancer and their family caregivers. Despite the implications of clinical trials for caregivers, there is little empirical research about how caregivers participate in clinical trial decision-making. This qualitative study sought to identify how caregivers view their role in these decisions and the attitudes and experiences of clinical trial staff regarding inclusion of caregivers in the informed consent process. METHODS: We performed four focus groups-two with clinical trial staff and two

Maternalism in Palliative Care: A Case for Relational Autonomy and Informed Non-Dissent.

BACKGROUND: Clinicians often face situations in which patients or families desire aggressive care that medical teams believe offers no meaningful benefit. Traditional shared decision-making frameworks may inadequately address these tensions. CASE: A 53-year-old woman with metastatic lung cancer desired full resuscitation despite progressive multisystem organ failure. She identified as a "fighter" but faced a clinical dilemma: adequate pain control required accepting respiratory compromise. DISCUSSION: We apply the bioethical construct of maternalism, integrating relational autonomy and infor

Ritualized relational autonomy in Chinese end-of-life care: balancing Confucian benevolence and individual rights.

This article develops a hypothesis-and-theory framework for ethical tensions in contemporary Chinese end-of-life care and family mourning. It examines the relationship between Confucian ren'ai (benevolence), expressed through embodied familial care, and modern rights-based commitments to bodily autonomy, dignity, and informed choice. The article offers a conceptual and normative analysis informed by interpretive cultural inquiry and existing empirical research on family-centered decision-making, advance care planning, caregiver burden, and hospice and palliative care in China. It argues that t

Relational autonomy in the clinical encounter: Female genital cosmetic surgery in Belgium and the Netherlands.

While previous research has examined sociocultural influences on women's motivations for FGCS, less attention has been paid to how clinical encounters are experienced and interpreted in relation to genital concerns and decisions about surgery. Data collection was conducted in 2024-2025 in Belgium and the Netherlands and consisted of in-depth interviews with eleven women who had undergone FGCS and twelve clinicians who perform these procedures, supplemented by focus group discussions with 44 women from the general population. Using reflexive thematic analysis, we identified four interrelated pa

Navigating an ambiguous abortion terrain in Addis Ababa, Ethiopia: autonomy and relational dynamics in pregnancy termination.

BACKGROUND: Ethiopia liberalized its abortion law in 2004 and issued technical guidelines in 2006 to expand access to legal and safe abortion services. However, the implementation of this progressive policy is shaped by stigma, patriarchal norms, and influential actors. As a result, women seeking abortion are required either to negotiate these power dynamics or to go through unsafe procedures. Despite this policy overhaul, there is a paucity of data on how women navigate complex abortion landscape. Therefore, this paper explores how women in Addis Ababa exercise relational autonomy amidst the

Towards a shared and supported decision-making model: fostering relational autonomy in end-of-life care.

Respect for autonomy is a central ethical principle in end-of-life care. However, due to cognitive decline, emotional distress, and the existential weight of end-of-life decisions, many patients may struggle to articulate or sustain their preferences without appropriate forms of support. The question, then, is not solely how to respect patients' autonomy in end-of-life care, but how autonomy, understood in relational terms, can be genuinely fostered. To address this question, the article advances a normative and practice-oriented model of decision-making that integrates two approaches often tr

[The stigma of vulnerability and the autonomy fallacy in care. Ethical keys from a relational anthropology].

Vulnerability has become a bridging concept capable of traversing clinical bioethics, research ethics, and contemporary discussions on social justice, without a stable consensus on what the term exactly designates, what it normatively justifies, and how it is operationalized in specific contexts. This article offers an argumentative synthesis based on a systematic literature review (January 2015-August 2025) conducted in PubMed, Scopus, and Web of Science. This study, inspired by PRISMA, identifies four thematic areas encompassing the most significant ethical challenges and emerging approaches

Trajectories of Individuating and Relating Autonomy in Adolescence: A Longitudinal Study in a Relational Cultural Context.

Autonomy is increasingly understood as a relationally embedded process rather than a pursuit of independence, yet few longitudinal studies track how volition develops across cultural and developmental contexts. This study examined the dual trajectories of individuating autonomy and relating autonomy across adolescence and educational transitions. Participants were 777 Taiwanese adolescents (48.8% female; M = 15.22 years, SD = 1.56), including junior high (n = 371) and senior high (n = 406) cohorts, each followed across four waves to yield eight cohort-sequential waves. Relations Between Dual Filial Piety and Life Satisfaction: The Mediating Roles of Individuating Autonomy and Relating Autonomy.

Filial piety (FP) was formerly a Confucian concept that specifies how children should treat their elders. In recent years, some psychologists have postulated that there are considerable overlaps between Chinese FP and notions found in other cultures. They have redefined FP as a contextualized personality emphasizing the psychological schema of parent-child interaction so that it fits universal cultural contexts. Based on this theory construction, this study aimed to examine the effects of reciprocal FP and authoritarian FP on life satisfaction and the mediating roles of individuating autonomy

Street-level bureaucracy and relational autonomy in HIV status-neutral care implementation: A study of jail staff and incarcerated individuals.

In the U.S., nearly 8 million people are incarcerated in jail each year, a majority of whom are structurally vulnerable and at elevated risk for poor health, including HIV. Jails have been identified as strategic settings for health interventions, offering opportunities to deliver "status neutral" HIV services (testing, prevention, and linkage to care) to people who may otherwise have intermittent access to care. To understand barriers and facilitators to integrating HIV care into routine jail health services, we conducted semi-structured, in-person interviews with 15 incarcerated individuals

Frames of decision-making in prenatal consultations in England and France. Towards a sociological, relational, and processual approach to autonomy.

RATIONALE, AIMS, AND OBJECTIVES: The article looks at how, during consultations, pregnant women identified as presenting an increased risk of giving birth to a child with an impairment, and practitioners in the field of prenatal diagnosis, decide whether or not to accept the risk of a miscarriage and proceed with a diagnostic examination. METHODS: We conducted 63 observations of consultations in France and 22 in England. Participants were women for whom an elevated risk of abnormality had been identified and the practitioners involved in their care. Our analytical approach consisted in suspen

Advance care planning with patients of Islamic migration background: a qualitative study identifying the need for relational autonomy and hope-sustaining communication.

BACKGROUND: Advance Care Planning (ACP) is largely based on ideas of individual autonomy and direct communication, which may conflict with the relational, cultural and religious perspectives of patients with an Islamic migration background. We explored the perspectives of patients with an Islamic background on ACP conversations within a Dutch healthcare system and which communication strategies may support more culturally appropriate ACP. METHODS: A qualitative study was conducted consisting of two focus groups with cultural experts and healthcare professionals (N = 14) and interviews with pa

Is autonomy related to the quality of performance of everyday activities in children with spina bifida?

PURPOSE: To investigate the relationship between the level of autonomy and the quality of performance of everyday activities in a population-based cohort of children with spina bifida and to study the agreement between the children's and the parents' ratings of autonomy. METHODS: 50 dyads of children (aged 6-14) with spina bifida and their parents rated the children's level of autonomy with an adapted, Swedish version of the Autonomy Scale from the Arc's Self-Determination Scale. Each child's quality of performance of everyday activities was assessed with the Assessment of Motor and Process S

Discussing treatment options with a minor: the conflicts related to autonomy, beneficence, and paternalism.

A seventeen-year-old male, high-school football player presents to an orthopaedic surgeon because of recurrent right knee pain after having undergone an arthroscopic meniscal repair one year previously. The patient did well initially but now has recurrent medial joint-line pain in the knee, which developed when he planted the right leg to throw a pass during summer training camp. He was evaluated by the team's athletic trainer and by an orthopaedic surgeon, both of whom, on the basis of their physical examination of the boy, believe that he may have sustained a recurrent meniscal tear.
