Respiratory aspiration of stomach contents.

The aspiration of stomach contents is a common clinical problem of concern to all physicians. Its consequences are varied, depending on the amount and distribution of the aspirate, its pH, and the presence or absence of food, particulate matter, and bacteria. Because multiple factors are involved, aspiration of stomach contents can lead to several distinct syndromes of pulmonary injury, all of which unfortunately have been labeled "aspiration pneumonitis." We review the pathophysiology of each of these syndromes and discuss important diagnostic and therapeutic consequences.

Respiratory complications following hydrocarbon aspiration in children.

Accidental hydrocarbon ingestion may lead to aspiration and chemical pneumonitis in children. In this review article, the clinical course of hydrocarbon pneumonitis, chest radiographic abnormalities, complications, and treatment interventions are summarized. Most children remain asymptomatic and without complications following ingestion of a hydrocarbon. In approximately 15% of ingestions, aspiration pneumonitis occurs and evolves over the first 6-8 hr presenting with fever, tachypnea, hypoxemia, and tachycardia. A symptom zenith is reached within 48 hr followed by progressive improvement. Up

Acute respiratory failure following traumatic tooth aspiration.

Foreign body aspiration can infrequently occur following trauma. Tooth aspiration after trauma is a rare clinical scenario. Here, we report a case in which tooth aspiration after trauma led to a presentation of acute respiratory failure with clinical findings mimicking tension pneumothorax. Successful removal of the aspirated tooth was accomplished by rigid bronchoscopy. Tooth aspiration must be considered in the list of differential diagnosis for any patient having signs or symptoms of respiratory distress following trauma especially maxillofacial trauma.

Dysphagia, aspiration, and respiratory problems in people with chronic respiratory illness.

PURPOSE: Dysphagia, aspiration, and respiratory dysfunction are interlinked with shared neural control and biomechanical demands. Recent respiratory-swallow coordination research and scrutiny of long-standing dysphagia interventions necessitate reappraisal of aspiration risk, airway protection, and clinical decision making. These issues are particularly relevant in chronic respiratory illness and palliative care, where dyspnoea, frailty, and quality-of-life considerations strongly influence eating and drinking decisions. RECENT FINDINGS: Breathing and swallowing are coordinated by overlapping

Neonatal Respiratory Distress Secondary to Meconium Aspiration Syndrome.

Infants born through meconium-stained amniotic fluid (MSAF) are 100 times more likely than infants born through clear amniotic fluid to develop respiratory distress in the neonatal period. Meconium aspiration syndrome (MAS) is a common cause of respiratory distress in term and post-mature neonates. MAS is defined as respiratory distress accompanied by a supplemental oxygen requirement in an infant born with MSAF, in the absence of any other identified etiology to explain the symptoms. Therapy for MAS is supportive, and should be tailored to each infant's specific pathophysiology. In cases of M

[Damage to the respiratory tract due to aspirated alendronate].

We describe the rare case of a high-grade chemical irritating-toxic damage to the larynx, the trachea and the bronchial system due to the aspiration of the bisphosphonate alendronate. The aspiration of alendronate was caused by oropharyngeal dysphagia. The 68-year-old female patient was sent to hospital with increasing hoarseness and a tormenting continuous cough of high intensity. X-Ray pictures of the thorax showed dystelectasis of the right middle lobe. Severe damage to the bronchial system caused by the aspirated alendronate was demonstrated by flexible bronchoscopy. Anti-obstructive, anti

Transbronchial needle aspiration in the diagnosis of respiratory diseases.

Thirty-three patients were subjected to transbronchial needle aspiration during the routine diagnostic bronchoscopy. Of the 17 patients with submucosal diseases, Transbronchial needle aspiration (TBNA) had a yield of 88.2% as against 81.2% for bronchial biopsies, 54.5% for bronchial brush smears and 41.1% for bronchial aspirates. TBNA also had a good yield in endobronchially visible growths. Nine patients had mediastinal or hilar lymph nodes as evident in contrast enhanced CT scans. TBNA was attempted from eight lymph node sites in six patients with bronchogenic carcinoma and was positive for

Transthoracic needle aspiration of the lung in respiratory infections.

Transthoracic needle aspiration of the lung in guinea pigs with experimental pseudomonas pneumonia was evaluated. The number of bacteria in aspirates correlated well with that of bacteria in the lungs which showed diffuse pneumonia (10(7) inoculum group). The number of deaths of experimental animals increased together with an increase of the times of aspiration. This procedure was also investigated in 16 patients of pneumonia and 17 patients of pulmonary abscess. The isolation rate of pathogen from pneumonia was 31.3% and that from pulmonary abscess 58.8%. A higher isolation rate was obtained

Neonatal meconium aspiration syndrome and respiratory morbidity during infancy.

Respiratory morbidity in the first 6 months of life of 35 infants who had had neonatal meconium aspiration syndrome (MAS) was compared to that of 70 controls, also born at term, matched for gender and ethnic origin. The number of infants in the two groups who were symptomatic was compared. Infants were described as symptomatic if, following discharge from hospital, they had at least one episode of wheezing and/or coughing which lasted for 3 days or more. There was no significant difference between the two groups regarding parental smoking or the proportion of infants who had a family history o

Dental prosthesis aspiration: An uncommon cause of respiratory distress.

We present a case of a 66-year-old Caucasian man with acute respiratory distress. The patient had a history of multiple cerebrovascular accidents which resulted in left hemiplegia, swallowing problems, and aphasia. He was tentatively diagnosed with aspiration pneumonia. However, because of clinical deterioration further investigations concluded to the aspiration of a dental prosthesis. After intubation and stabilization, the prosthesis could be manually extracted. However, the patient developed a Staphylococcus epidermidis sepsis and despite adequate antibiotic therapy, he eventually died. Den

Respiratory support in meconium aspiration syndrome: a practical guide.

Meconium aspiration syndrome (MAS) is a complex respiratory disease of the term and near-term neonate. Inhalation of meconium causes airway obstruction, atelectasis, epithelial injury, surfactant inhibition, and pulmonary hypertension, the chief clinical manifestations of which are hypoxaemia and poor lung compliance. Supplemental oxygen is the mainstay of therapy for MAS, with around one-third of infants requiring intubation and mechanical ventilation. For those ventilated, high ventilator pressures, as well as a relatively long inspiratory time and slow ventilator rate, may be necessary to a

Aspiration Risk and Respiratory Complications in Patients with Esophageal Atresia.

Chronic, long-term respiratory morbidity (CRM) is common in patients with a history of repaired congenital esophageal atresia, typically associated with tracheoesophageal fistula (EA/TEF). EA/TEF patients are at high risk of having aspiration, and retrospective studies have associated CRM with both recurrent aspiration and atopy. However, studies evaluating the association between CRM in this population and either aspiration or atopy have reported conflicting results. Furthermore, CRM in this population may be due to other related conditions as well, such as tracheomalacia and/or recurrent inf

Cyclovirus in nasopharyngeal aspirates of Chilean children with respiratory infections.

Some respiratory tract infections remain unexplained despite extensive testing for common pathogens. Nasopharyngeal aspirates (NPAs) from 120 Chilean infants from Santiago with acute lower respiratory tract infections were analysed by viral metagenomics, revealing the presence of nucleic acids from anelloviruses, adenovirus-associated virus and 12 known respiratory viral pathogens. A single sequence read showed translated protein similarity to cycloviruses. We used inverse PCR to amplify the complete circular ssDNA genome of a novel cyclovirus we named CyCV-ChileNPA1. Closely related variants

Aspiration of bile as a cause of respiratory distress in the newborn infant.

Early-onset respiratory distress and a radiographic appearance of an aspiration syndrome occurred in two neonates with gastroschisis who had evidence of inhalation of bile. Hypoxemic respiratory failure developed in both infants, contributing to death or prolonged respiratory morbidity. Bile aspiration syndrome should be considered whenever there is early-onset respiratory distress in a neonate with high intestinal obstruction.

Cytodiagnosis of lower respiratory tract lesions by transthoracic needle aspiration.

A comprehensive prospective and retrospective study of 130 cases was carried out to evaluate the safety, adequacy and diagnostic accuracy of Transthoracic Needle Aspiration (TTNA) in various pulmonary lesions. Using both guided and unguided TTNA diagnostic material was procured in 86.15% cases. Rotex II, Chiba, lumber puncture and ordinary needles were used depending upon the type of lesion. Overall diagnostic accuracy of the procedure was 79.46 percent. Complication after the procedure were transient and self limiting. Haemoptysis was noted in 3.84 percent cases and pneumothorax in a single c
