The intermediate respiratory care units (UCRI) are areas of monitoring and specialized care of patients with acute or chronic-exacerbated respiratory failure, whose severity does not require admission to an intensive care unit, but which due to their complexity cannot be treated in conventional hospitalization. Although the COVID-19 pandemic has proven its usefulness in the management of critical respiratory patients, the historical trajectory of the UCRI comes from many years ago, in which its cost-effectiveness has been demonstrated by far. This document presents a series of questions and an

The burden of acute respiratory failure (ARF) has become one of the greatest epidemiological challenges for the modern health systems. Consistently, the imbalance between the increasing prevalence of acutely de-compensated respiratory diseases and the shortage of high-daily cost ICU beds has stimulated new health cost-effective solutions. Respiratory High-Dependency Care Units (RHDCU) provide a specialised environment for patients who require an "intermediate" level of care between the ICU and the ward, where non-invasive monitoring and assisted ventilation techniques are preferentially applie

The ventilator assisted individual (VAI) represents a complex set of medical and social issues that call for the involvement of multiple interest groups. The success of treatment for VAI depends on many factors. These include: appropriate selection of patients for care outside the hospital, clinical and physiological stability of patients, patient and family motivation, and their ability to learn. Assisted ventilation in chronic respiratory failure secondary to chest-wall deformities and neuromuscular disease has shown promising results. Less satisfactory have been those reported in patients w

Patients with intensive care unit-acquired weakness have an increased risk of prolonged mechanical ventilation, which is a risk factor for prolonged stay and mortality. The most common cause of this problem is weakness of the diaphragm, which can derive from phrenic nerve injury associated with critical neuropathy, or with the complex multiorgan failure/systemic respiratory response syndrome causing muscle fiber lesion. Two conventional neurophysiological techniques are useful to investigate the respiratory muscles, phrenic nerve conduction, and needle electromyography of the accessory respira

BACKGROUND AND PURPOSE: Certain physiological characteristics increase the susceptibility of newborns, particularly preterm infants, to pulmonary collapse. In addition, newborns are prone to the accumulation of airway secretions, which, even in small volumes, can compromise respiratory function. Consequently, breathing therapy plays a crucial role in promoting airway clearance and supporting effective ventilation in this population. This scoping review aimed to map the breathing therapy methods used for newborns admitted to intensive care units and also to identify the clinical outcomes assess

Our aim was to determine the frequency of 12 common respiratory viruses in patients admitted to intensive care units with respiratory symptoms, evaluate the clinical characteristics and to compare the results to routine microbiological diagnostics. Throat swabs from 122 intensive care-patients >18 years with acute respiratory symptoms were collected upon admission and analysed with multiplex real-time polymerase chain reaction, for 12 community respiratory viruses. Blood and respiratory tract specimens were analysed for bacteria and fungi upon clinicians' request. Clinical and paraclinical dat

BACKGROUND: Survival outcomes after cardiac or respiratory arrest occurring outside of intensive care units (ICUs) has been well described. We investigated survival outcomes of adults whose arrest occurred in ICUs and determined predictors of decreased survival.
METHODS: We reviewed all records of adults who experienced cardiac or respiratory arrest from Jan. 1, 2000, to Apr. 30, 2005, in ICUs at four hospitals serving Edmonton, Alberta. We evaluated patient and clinical characteristics, as well as survival outcomes during a five-year follow-up period. We determined risk factors for immediate

OBJECTIVE: To evaluate the incidence, treatment and mortality of acute respiratory failure (ARF) in Finnish intensive care units (ICUs).
STUDY DESIGN: Prospective multicentre cohort study.
METHODS: All adult patients in 25 ICUs were screened for use of invasive or non-invasive ventilatory support during an 8-week period. Patients needing ventilatory support for more than 6 h were included and defined as ARF patients. Risk factors for ARF and details of prior chronic health status were assessed. Ventilatory and concomitant treatments were evaluated and recorded daily throughout the ICU stay.

Qualified respiratory therapists have been an integral part of the U.S. health care system for over 50 years now and are an indispensable part of the system. Respiratory therapy involves the entire respiratory system and should not to be confused with breathing therapy. The latter includes only certain breathing techniques. Since 2005, the German Respiratory Society also offers nurses and physiotherapists training in respiratory therapy. The core competencies of the numerous intensive care respiratory therapists employed in German intensive care units include, among others, the diagnosis and t

Disorders of both the central and peripheral nervous systems are important causes of respiratory insufficiency. However, simple clinical observations and pulmonary function measurements may fail to identify the location and type of disorder. This can often be accomplished by the newly-developed technique of phrenic nerve conduction and needle electromyography of the diaphragm which delineate the various disturbances of central drive, axonal or demyelinating neuropathies of the phrenic nerves and certain myopathies. These studies have been preformed safely and with little discomfort on adults,

In an intensive care unit an important role is assigned to respiratory physiotherapy. Its principal task is efficacious toilet of the bronchi by fluidifying the secretions, promoting their ungluing from the respiratory tree and facilitating their evacuation by cough or by aspiration with a catheter or bronchoscope. The technique comprises the inhalation of a secretolytic (e.g. Bisolvon, NaCl 9%) and, in the case of asthma, bronchospasmolytic (e.g. Ventoline) aerosol followed by breathing exercises. The other objectives of physiotherapy are to ensure a better distribution of inspired air, incre

BACKGROUND: In Italy, respiratory intensive care units (RICUs) provide an intermediate level of care between the intensive care unit (ICU) and the general ward for patients with single organ respiratory failure. Because of the lack of official epidemiological data in these units, a two phase study was performed with the aim of describing the work profile in Italian RICUs.
METHODS: A national survey of RICUs was conducted from January to March 1997 using a questionnaire which comprised over 30 items regarding location, models of service provision, staff, and equipment. The following criteria w

INTRODUCTION: Pulmonary aspergilloma is commonly associated with comorbidities that cause immunodeficiency such as diabetes mellitus, tuberculosis, human immunodeficiency virus/acquired immunodeficiency syndrome and/or a pre-existing parenchymal lung disease such as chronic obstructive pulmonary disease. Predisposing factors can further increase the risk of acquiring this mycosis. Our objective was to determine the frequency, clinical and microbiological characteristics of pulmonary aspergilloma in immunocompromised patients.
METHODOLOGY: Retrospective case series of patients diagnosed with p

Atrophy and weakness of the respiratory and peripheral muscles is a common problem in the intensive care unit (ICU). It is difficult to diagnose, particularly in the early stages of critical disease. Consequently, many cases are detected only in advanced stages, for example, when difficulties in mechanical ventilation weaning are encountered. The aim of this review is to describe the main tools that are currently available for evaluation of peripheral and respiratory muscles in the ICU. Techniques of varying complexity and specificity are discussed, and particular emphasis is placed on those w

The incidence of respiratory acid-base abnormalities in the critical care unit (CCU) is unknown, although respiratory alkalosis is suspected to be common in this population. Abnormal carbon dioxide tension can have many physiologic effects, and changes in Pco2 may have a significant impact on outcome. Monitoring Pco2 in CCU patients is an important aspect of critical patient assessment, and identification of respiratory acid-base abnormalities can be valuable as a diagnostic tool. Treatment of respiratory acid-base disorders is largely focused on resolution of the primary disease, although mec

Viral lower respiratory tract infections (LRTIs) are an underappreciated cause of critical illness in adults. Recent advances in viral detection techniques over the past decade have demonstrated viral LRTIs are associated with rates of morbidity, mortality, and health care utilization comparable to those of seen with bacterial community acquired and nosocomial pneumonias. In this review, we describe the relationship between viral LRTIs and critical illness, as well as discuss relevant clinical features and management strategies for the more prevalent respiratory viral pathogens.
