Aspergillus-associated hypersensitivity respiratory disorders.

The mould Aspergillus is responsible for a gamut of respiratory diseases ranging from saprobic colonisation to rapidly invasive disseminated disease. The clinical spectrum of Aspergillus-associated hypersensitivity respiratory disorders includes Aspergillus induced asthma, allergic bronchopulmonary aspergillosis (ABPA), allergic Aspergillus sinusitis (AAS) and hypersensitivity pneumonitis. Inhalant allergens, in patients with allergic asthma, play a key role in bringing about the inflammation present in the airways, and fungi are increasingly being recognised as important inhalant allergens. Prasugrel hypersensitivity with respiratory distress and rash.

PURPOSE: An adverse drug reaction associated with the use of prasugrel for dual antiplatelet therapy after percutaneous coronary intervention (PCI) with stent placement is reported. SUMMARY: About one week after starting prasugrel use following angioplasty and a stent revision procedure, a 61-year-old woman arrived in the emergency department with wheezing, shortness of breath, a feeling of throat closure, and a widespread erythematous, maculopapular, pruritic rash. She reported that the respiratory symptoms had started to develop the previous day, with the onset of rash occurring about 24 ho

Environmental factors and respiratory hypersensitivity: the Americas.

Diverse environmental exposure profiles exist in the Americas because of widely different climates, ambient pollutants, and bioaerosols in these continents. This paper reviews selected studies from the Americas that support the broad hypothesis that environmental factors contribute to respiratory hypersensitivity. Processes influenced by environmental factors include primary immunologic sensitization, the development and exacerbation of specific immunologic diseases and the activation of nonspecific mechanisms with tissue inflammation, injury and remodeling. Endpoints resulting from these proc

Exploring cough hypersensitivity patterns across respiratory diseases.

BACKGROUND: Cough hypersensitivity is increasingly recognized as a key mechanism underlying chronic cough across diverse etiologies. The Cough Hypersensitivity Questionnaire (CHQ) was developed to systematically assess characteristic triggers and abnormal sensory perceptions; however, its structural and clinical relevance across respiratory diseases remains unclear. METHODS: We prospectively enrolled 300 adults with chronic cough from multiple respiratory centers. Participants completed four cough-related questionnaires: the Numeric Rating Scale (NRS), Leicester Cough Questionnaire (LCQ), Cou

The role of the gastrointestinal tract in the development of respiratory hypersensitivities.

Adverse reactions to foods may sometimes cause symptoms from the respiratory tract, including bronchial obstruction and rhinitis. The true prevalence is not known. In adults, it has been estimated to be about 1% of asthmatic patients or less. This would correspond to a prevalence of about 2-4 in 10,000 of the general population. The prevalence in children is higher, as IgE mediated food allergy is more common in this age group than in adults. The most common foods causing immunologically mediated reactions include milk, egg, fish, crustaceans, nuts, wheat, soy, peanut, peas and other legumes.

Hypersensitivity Reactions and the Respiratory System: Imaging Based Review.

Hypersensitivity reactions are characterized by inappropriate response of the immune system to an inciting antigen, which results in damage to various body tissues. Respiratory system can be involved as a part of hypersensitivity reaction by a myriad of conditions ranging from infective pathologies like tuberculosis to non-infective processes such as asthma, graft- versus host disease, sarcoidosis and vasculitic disorders. Recognition of specific imaging features in appropriate clinical setting helps in diagnosing these conditions. We present a review of mechanism of different types of hyperse

Respiratory hypersensitivity and environmental factors: East and West Germany.

We assessed the prevalence of asthma and allergic disorders in 9-11 year old children in Leipzig and Halle in East Germany, and Munich, West Germany. Both East German cities were heavily polluted due to private coal burning and industrial emissions, whereas Munich has moderate industry but heavy car traffic. All fourth grade pupils in Munich (n = 7445) were compared with those in Leipzig, 1990 (n = 1429) and Leipzig and Halle, 1991 (n = 3105). The prevalence of hay fever and skin test reactivity to common aeroallergens was considerably higher in West Germany as compared to East Germany. Furthe

Environmental chemicals relevant for respiratory hypersensitivity: the indoor environment.

The allergenic constituents of non-industrial indoor environments are predominantly found in the biologic fraction. Several reports have related biological particles such as mites and their excreta, dander from pets and other furred animals, fungi and bacteria to allergic manifestations including respiratory hypersensitivity among the occupants of buildings. Also, bacterial cell-wall components and the spores of toxin-producing moulds may contribute to the induction of hypersensitivity, but the relevance for human health is not yet determined. The knowledge regarding hypersensitivity and asthm

Expert opinion on the cough hypersensitivity syndrome in respiratory medicine.

In 2011, a European Respiratory Society Task Force embarked on a process to determine the position and clinical relevance of the cough hypersensitivity syndrome, a disorder characterised by troublesome coughing often triggered by low levels of thermal, mechanical or chemical exposure, in the management of patients with chronic cough. A 21-component questionnaire was developed by an iterative process supported by a literature review. 44 key opinion leaders in respiratory medicine were selected and interviewed as to their opinions. There was a high degree of unanimity in the responses obtained,

Exposure to environmental chemicals relevant for respiratory hypersensitivity: global aspects.

Trends of air pollutant concentrations that influence the incidence of respiratory diseases and might be influential for respiratory hypersensitivity are presented and discussed. Data for these trends are collected in UNEP/WHO's Global Environmental Monitoring System GEMS/Air which recently has been revitalized and redirected to become a tool for providing the information necessary for rational air quality management. Data are interpreted with respect to WHO's most recent air quality guidelines, which are an update of the air quality guidelines for Europe published in 1987. A WHO study on the

Exposure to environmental chemicals relevant for respiratory hypersensitivity: European aspects.

The need for assessing air pollution exposure on the individual and population levels is urgent, as health effects are observed at concentrations even below existing air quality guidelines (AQG). Typical European concentrations and trends over time of four main air quality indicators (nitrogen dioxide, sulphur dioxide, ozone and particulate matter) are presented, together with their revised WHO AQG. In order to describe concentrations of pollutants each individual and population groups are exposed to, methods have been developed that estimate these concentrations using different forms of model

Immediate hypersensitivity in adults with IgG deficiency and recurrent respiratory infections.

BACKGROUND: Little is known about the prevalence of atopy in adults with recurrent respiratory infections and IgG deficiency. OBJECTIVE AND METHODS: To elucidate this aspect, we skin-tested 95 consecutive adults with respiratory infections, subnormal levels of IgG subclasses or common variable immunodeficiency and usually poor response to vaccination. In 50 subjects we also measured total IgE. RESULTS: We found 67 subjects with IgG subclass deficiency, 21 subjects with mild (partial) and 5 with usual common variable immunodeficiency, and 2 subjects with functional IgG deficiency. Atopy was e

[Importance of type I hypersensitivity to molds in autumnal respiratory allergies].

The authors examined by means of skin tests for moulds 108 patients with the diagnosis of pollen rhinitis and/or pollen bronchial asthma. All had maximum complaints at the end of summer and in autumn and were formerly hyposensitized only by pollen allergens. Sensitization to fungi and its polyvalent character was significantly greater in subjects unsuccessfully treated by pollen hyposensitization, as moulds acted obviously as an important allergen. The authors found a low sensitivity and specificity of USOL screening preparations "Outdoor moulds" (76% and 81%) and "Indoor moulds" (74% and 93%)

Respiratory hypersensitivity to Ascaris extract in guinea pigs sensitized by aerosol.

Guinea pigs exposed via the respiratory route to Ascaris suum extract in aerosol form became hypersensitive. The hypersensitivity was detectable by inhalational bronchial provocation testing using a noninvasive plethysmography, by chopped lung histamine release, and by passive cutaneous anaplylaxis. The antibody responsible for the hypersensitivity seems to be directly associated with pulmonary tissue. A locally produced cytotropic pulmonary antibody, probably IgG or IgE may be causal. Histopathology included only a moderate pulmonary eosinophilia. Individual variability in susceptibility to e

Intermittent hypercapnic hypoxia induces respiratory hypersensitivity to fentanyl accompanied by tonic respiratory depression by endogenous opioids.

KEY POINTS: Sleep apnoea increases susceptibility to opioid-induced respiratory depression (OIRD). Endogenous opioids are implicated as a contributing factor in sleep apnoea. Rats exposed to sleep-phase chronic intermittent hypercapnic hypoxia (CIHH) for 7 days exhibited exaggerated OIRD to systemic fentanyl both while anaesthetized and artificially ventilated and while conscious and breathing spontaneously, implicating heightened CNS inhibitory efficacy of fentanyl. CIHH also induced tonic endogenous opioid suppression of neural inspiration. Sleep-related episodes of hypercapnic hypoxia, as i

Hypersensitivity Pneumonitis and Acute Respiratory Distress Syndrome From E-Cigarette Use.

Electronic cigarette (e-cigarette) use, or "vaping," is gaining widespread popularity as an alternative to conventional cigarettes among adolescents. Little is known of the health risks of e-cigarette use, especially in children and adolescents. We present a Case Report of a previously healthy 18-year-old woman who presented with dyspnea, cough, and pleuritic chest pain after e-cigarette use. She developed respiratory failure with hypoxia and was intubated, and ultimately met diagnostic criteria for acute respiratory distress syndrome. Chest tubes were placed to drain worsening pleural effusio

Delayed hypersensitivity reaction and acute respiratory distress syndrome following infliximab infusion.

Infliximab, a chimeric human/murine monoclonal antibody directed against the proinflammatory cytokine tumor necrosis factor alpha, is an effective therapy for Crohn's disease (CD) and rheumatoid arthritis refractory to standard medical treatment. We report a case of adult respiratory distress syndrome associated with infliximab therapy. A 33-year-old white male presented with an exacerbation of CD and was treated with his second infliximab infusion (15 months following the first infusion). Within 7 days he developed arthralgias, myalgias, and fever, followed by respiratory failure. He required
