Quality of resuscitation orders in general medical patients.

BACKGROUND: Documentation of resuscitation status in hospitalized patients has relevance in the management of cardiopulmonary arrest. Its association with mortality, Length Of hospital Stay (LOS) and the patients' primary diagnosis has not been established in general medical inpatients in hospitals in Australia and New Zealand. AIM: To investigate the association of resuscitation orders with in-hospital mortality and LOS in a range of diagnoses, adjusting for severity of illness and other covariates. DESIGN: Retrospective study. METHODS: The admission notes of 1681 medical admissions to fou

Resuscitation of residents with do not resuscitate orders in long-term care facilities.

BACKGROUND: A considerable number of emergency medical services (EMS) responses for cardiac arrest occur in long-term care facilities. In some instances, these responses are for residents who have expressed wishes not to be resuscitated by signing a do not resuscitate (DNR) order. OBJECTIVES: To assess the magnitude of EMS use for cardiac arrest in long-term care facilities for residents with DNR orders and to determine reasons why EMS is called. METHODS: A retrospective study was conducted using data collected from medical incident reports between July 1999 and December 2000 for all persons

'Do not resuscitate order' in neonatology: authority rules.

Ethical dilemmas in medicine should be resolved in light of four essential principles. To specify and guide concrete actions, it is necessary to 'supplement' these principles by certain other (substantive, authority and procedural) rules. The purpose of this paper is to establish and justify the authority rules regarding the order not to resuscitate newborns. The authority rules are intended to indicate who should decide, but they do not determine what should be chosen. Decision regarding newborn's treatment/letting die depends on medical and quality-of-life judgments. Parents, doctors, and so

Completeness and Audibility of Verbal Orders for Medications and Blood Products during Trauma Resuscitation.

INTRODUCTION: Resuscitation of critically injured patients requires effective team leadership. Poor communication is the leading cause of sentinel events. Closed-loop communication reduces error during trauma resuscitations. Nonetheless, previous studies show few verbal orders are audible. Verbal orders during trauma resuscitations have not been studied for completeness. In this project we aimed to assess whether verbal orders for medications and blood products during trauma resuscitations were complete, audible, and used closed-loop communication. METHODS: This was an observational assessmen

Must consent always be obtained for a do-not-resuscitate order?

Using cardiopulmonary resuscitation for cardiac or respiratory arrest unless there is an explicit do-not-resuscitate order is a policy adopted by all hospitals. Such a policy usually requires the patient's (or surrogate's) consent for a do-not-resuscitate order to be instituted. This article, however, presents the argument that consent need not always be obtained. In the case discussed, the well-being of the patient, other patients, and the health care providers all support a unilateral decision by the physician not to attempt resuscitation at the time of death. The medical community and socie

Variation in the use of do-not-resuscitate orders in patients with stroke.

OBJECTIVES: To identify sociodemographic and clinical characteristics associated with the use of do-not-resuscitate (DNR) orders in hospitalized patients with stroke. To examine whether the use of DNR orders varies across hospitals. METHODS: This observational cohort study used data collected for 13337 consecutive eligible patients with a primary diagnosis of stroke. These patients were discharged in 1991 through 1994 from 30 hospitals in a large metropolitan area. Study data were abstracted from patients' hospital records using standard forms. Admission severity of illness was measured using

Discussing and documenting (do not attempt) resuscitation orders in a Dutch Hospital: a disappointing reality.

OBJECTIVE: To determine whether the introduction of a patient information sheet about do not attempt resuscitation (DNAR) orders and personal motivation of the medical staff results in an improvement in the documentation of the DNAR orders in the medical records. DESIGN: Retrospective chart review. METHOD: The medical records for all hospital admissions during February 2005 were checked for age, sex, admission time, admitting specialty, admission type (acute or planned), death, documentation of the DNAR order on the admission form, and if this order was complied with and under whose initiati

[Effect of the do-not-resuscitate orders on the critical patient care plan].

BACKGROUND: Do-not-resuscitate (DNR) orders are physician orders that refer to not initiating cardiopulmonary resuscitation in a patient who is in cardiac or respiratory arrest. However, these orders often imply other treatment modifications. AIMS: To analyze the effect that do-not-resuscitate orders have on the care plan of the critically ill patient; and to analyze if differences exist in the nursing workload (NEMS), before and after DNR prescription. METHOD: This descriptive study analyzed the care plan of 50 critically ill adult patients, before and after an electronic DNR order. RESULT

Impact of Do-Not-Resuscitate Orders on Nursing Clinical Decision Making.

OBJECTIVES: Code status specifies the action that healthcare providers should take in the event of cardiac arrest. Studies have shown, however, that patients with do-not-resuscitate/do-not-intubate (DNR/DNI) orders have worse outcomes and do not consistently receive the standard of care. Several studies have demonstrated that physicians behave differently toward patients with DNR/DNI orders, but little research exists into whether DNR/DNI status affects the practice of other members of the care team. Our objective was to determine whether code status affects decision making by nursing staff.

Survey of do-not-resuscitate orders in surgical intensive care units.

BACKGROUND/PURPOSE: End-of-life decisions are always difficult and complex, especially in the surgical setting. This study examines the epidemiology of do-not-resuscitate (DNR) orders, and the clinical factors influencing DNR consent. The impact of DNR on treatment and resource use in the surgical intensive-care unit (ICU) is also assessed. METHODS: This retrospective observational study was performed at National Taiwan University Hospital, a tertiary medical center in Taipei. A total of 14,698 patients were admitted to the surgical ICUs between January 2003 and December 2006. Of these, 13,82

Independent living following a 'Do Not Resuscitate' order after subarachnoid haemorrhage.

PURPOSE: The recovery and psychosocial adjustment of a 41-year-old right handed women (K.E.) following subarachnoid haemorrhage (SAH) with initially poor predicted prognosis is presented. METHOD: A single case report with an early post surgery 'Do Not Resuscitate' order following SAH with a significant period of impaired consciousness is outlined. RESULTS: Following a right frontal craniotomy and clipping of a left distal anterior communicating artery (AcoA) aneurysm, K.E. was unresponsive for 23 days and 'minimally conscious' for a further 5+ weeks post surgery (36 days). At two months post

Patient reluctance to accept Do Not Resuscitate order: impact on clinical care.

BACKGROUND: A Do Not Resuscitate (DNR) order should only impede the performance of cardiopulmonary resuscitation in case of cardiac or respiratory arrest; it should not interfere with any other treatment decisions. AIMS: To study the impact of DNR order placement on daily clinical care of patients. METHODS: This was a retrospective cohort study of 72 patients in a tertiary care centre in Saudi Arabia. Daily clinical care measures were collected for 2 weeks prior and 2 weeks after DNR order placement and included vital signs, nursing care, comfort measures, documentation, visits by senior and

Intensive Care Unit Physician's Attitudes on Do Not Resuscitate Order in Palestine.

BACKGROUND: There is some ambiguity concerning the do-not-resuscitate (DNR) orders in the Arabic world. DNR is an order written by a doctor, approved by the patient or patient surrogate, which instructs health care providers to not do CPR when cardiac or respiratory arrest occurs. Therefore, this research study investigated the attitudes of Intensive Care Unit physicians and nurses on DNR order in Palestine. MATERIALS AND METHODS: A total of 123 males and females from four different hospitals voluntarily participated in this study by signing a consent form; which was approved by the Ethical C

Structured Workflow to Manage Periprocedural Code Status for Patients With Do-Not-Resuscitate Orders.

Respecting patient wishes regarding resuscitation is fundamental to providing patient-centered care. Despite best practice guidelines for code status management for patients undergoing invasive procedures with existing Do Not Resuscitate (DNR) orders, compliance is low. Our interdisciplinary team created a workflow for code status management of inpatients with active DNR orders undergoing cardiac catheterization (CC) or electrophysiology (EP) procedures. Representatives from nursing, cardiology, surgery, palliative care, internal medicine, and information technology (IT) were involved. We used

Characteristics and Outcomes of Patients with Partial Do Not Resuscitate Orders in a Large Community Hospital.

Background Formal writing of do not resuscitate (DNR) orders first appeared in the literature in the late 20th century. Recently, providers have also noticed the presence of partial DNR orders while caring for patients. We sought to determine the effect of these orders on the clinical outcomes of the patients. Methods The study was a retrospective chart review covering a period of approximately 30 months. Patients included in the study were over 18 years of age and had a partial DNR order (i.e., chemical code, do not defibrillate (DND), do not intubate (DNI), intubate only, no cardiopulmonary

Are hospitalized Parkinson's disease patients more likely to carry a do-not-resuscitate order?

While DNR utilization is a complex subjective phenomenon, the effect of such a decision can collectively influence attitudes of care. The role of palliative care in advanced PD has been under appreciated. We reviewed the Healthcare Cost and Utilization Project's National Inpatient Sample (NIS) database from 2012 for all hospitalizations ⩾65years. We identified PD by using ICD-9-CM code 332.0 and DNR status with ICD code - V49.86 entered during the same admission as a secondary diagnosis. We estimated risk of mortality by the 3M™ All Patient Refined DRG (APR DRG) classification System and gener

The Final Day: What Happens Without a Pre-Existing Do Not Resuscitate Order?

Children with cancer may not have a do not resuscitate (DNR) order, yet absence rarely guarantees cardiopulmonary resuscitation (CPR). In a review of 344 inpatient deaths over a decade, 85% had DNR orders; 20% received CPR, mostly for perceived reversibility. Intensive care unit location and active chemotherapy were associated with fewer DNR orders. Families often declined CPR, despite absence of a DNR order, when death was imminent.

Attitudes of patients' relatives in the end stage of life about do not resuscitate order.

INTRODUCTION AND OBJECTIVE: The do not resuscitate (DNR) order is a decision taken by the patient or other people about medical care in the end stages of life to prevent resuscitation from causing cardiac or respiratory arrest. This study was conducted to evaluate the attitudes of patients' relatives in the end stages about DNR order. MATERIALS AND METHODS: In a cross-sectional study, 150 relatives of patients who had been prescribed DNR orders were included in the educational hospitals affiliated to the Kermanshah University of Medical Sciences. The data collection tool was a researcher-deve
