Rheumatic diseases other than rheumatic fever.

A total of 101 cases of rheumatic diseases collected during a period of six months from a hospital population have been analysed. These did not include rheumatic fever. Paediatric age group up to age of 12 years was not included in the series. Rheumatoid arthritis has been found to be the commonest condition, constituting 60 cases. Cervical spondylosis was next in common (24 cases). Ankylosing spondylitis and osteoarthritis constituted nine and six cases respectively. Emphasis has been laid on the bizarre manifestation of cervical spondylosis. The study, in spite of limitations throws some lig

[Iatrogenic rheumatic diseases].

This article reviews, through illustrative cases, five rheumatic diseases associated with drug treatments: statins and myopathy, quinolones and tendinopathy, viscosupplementation and pseudoseptic arthritis, diuretics and gout, barbiturates and adhesive capsulitis. Due to the rarity of such cases, we lack validated therapeutic recommendations. Announcement to the adverse drug reaction reporting system is the best way to enhance our knowledge and to improve the public health.

Malignancy and rheumatic diseases.

Several systemic autoimmune rheumatic diseases carry an increased risk of malignancy compared to the general population, including dermatomyositis, Sjogren's disease (SjD), rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), and systemic sclerosis (SSc). This is likely a result of chronic inflammation, impaired tumor surveillance related to treatments, and shared risk factors, such as smoking. As treatments for systemic autoimmune rheumatic diseases continue to expand and cancer survival continues to improve, clinicians and patients are increasingly faced with complex decisions rega

Neglected tropical rheumatic diseases.

The complexities of dealing with rheumatic diseases in tropical countries are diverse and likely due to limited health care infrastructure, lack of diagnostic and therapeutic facilities, impact of dominant prevailing diseases, and the challenges of differentiating from infectious and non-infectious disease mimics. Several tropical diseases present with musculoskeletal and rheumatic manifestations and often pose a diagnostic dilemma to rheumatologists. The diagnosis is often delayed or the disease is misdiagnosed, leading to poor patient outcomes. Endemic tropical diseases like tuberculosis and

NETosis in Rheumatic Diseases.

PURPOSE OF REVIEW: Neutrophils are the most numerous and the first responder cells of the innate immune system. Evidence suggests that neutrophils may play an essential role in the pathogenesis of multiple systemic diseases. A novel mechanism of neutrophil extracellular traps (NETs) leading to breaking of self-tolerance and generation of autoimmune responses in predisposed individuals has been described in various autoimmune conditions. The purpose of the review is to identify these important mechanisms of NETs leading to autoimmunity in various rheumatic diseases. RECENT FINDINGS: NETs conta

Osteoporosis and rheumatic diseases.

Numerous rheumatic diseases, including rheumatoid arthritis, juvenile idiopathic arthritis, psoriatic arthritis, ankylosing spondylitis, systemic lupus erythematosus, systemic sclerosis, dermatomyositis/polymyositis and vasculitis are characterized by osteoporosis and fragility fractures. Inflammatory cytokines, glucocorticoid treatment, immobilization and reduced physical activity due to painful joints and muscle weakness are considered the main risk factors that cause low body mass density values in these diseases. Emerging evidence highlights the role of inflammatory cytokines, such as tumo

Radiosynovectomy in rheumatic diseases.

Radiosynovectomy is a safe and repeatable treatment method of chronic synovitis with synovial overgrowth and refractory chronic or acute inflammatory joint effusion. It consist in the intraarticular administration of a radioactive isotope in the form of a colloid causing the extinguishing of active synovitis. The radiocolloid causes permanent irradiation of the synovium with beta ray electron beams, which ultimately leads to its fibrosis and extinguishes the inflammatory process destroying the joint. The main indications for radiosynovectomy include chronic and acute arthritis in the course of

[Tumors in rheumatic diseases].

Patients with systemic inflammatory rheumatic diseases (such as rheumatoid arthritis, lupus erythematosus, systemic sclerosis, idiopathic inflammatory myopathies, or Sjögren's syndrome) are at increased risk of cancer, including hematological malignancies (leukemias and lymphomas), as well as non-hematological cancers (e.g. lung, esophageal, prostate or ovarian cancer). This increased risk for cancer development in patients with rheumatic diseases is attributed to the immune (autoimmune) processes and the medical treatment. Due to the often similar symptoms and the occurrence of systemic paran

Diabetes and rheumatic diseases.

PURPOSE OF REVIEW: This review summarizes recent advances in the field of diabetes and rheumatic disease. These conditions exert a significant healthcare burden on our society and much remains to be learned regarding their pathophysiology and treatment. RECENT FINDINGS: We summarize new insights into diabetes and its association with osteoarthritis, rheumatoid arthritis, carpal tunnel syndrome, osteoporosis, diffuse idiopathic skeletal hyperostosis, crystalline arthropathy, neuropathic arthropathy, and tendinopathy. Diabetes has major effects on connective tissues, which have significant impa

Demyelination in rheumatic diseases.

Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS) characterised by multifocal areas of demyelination in the white matter of the brain and spinal cord. Autoantibodies, for example antinuclear antibodies, can also be present. MS and other demyelinating processes, such as transverse myelitis and optic neuritis (which may be clinically isolated cases or be part of the clinical spectrum of MS), are sometimes difficult to differentiate from CNS involvement in systemic autoimmune diseases like systemic lupus erythematosus (SLE), antiphospholipid syndrome (A

Chemokines in rheumatic diseases.

Chemotactic cytokines, termed chemokines, mediate the ingress of leukocytes into the inflamed synovium. In this review, authors discuss the role of the most relevant chemokines and chemokine receptors involved in chronic inflammatory rheumatic diseases. Rheumatoid arthritis was chosen as a prototype to discuss these issues, as the majority of studies on the role of chemokines in inflammatory diseases were carried out in arthritis. However, other rheumatic diseases including systemic lupus erythematosus, systemic sclerosis, Sjögren's syndrome, mixed connective tissue disease, polymyositis/derma

Calcinosis in rheumatic diseases.

BACKGROUND: Calcinosis, or dystrophic soft-tissue calcification, occurs in damaged or devitalized tissues in the presence of normal calcium/phosphorus metabolism. It is often noted in the subcutaneous tissues of connective tissues diseases--primarily systemic lupus erythematosus, scleroderma, or dermatomyositis--and may involve a relatively localized area or be widespread. The calcinotic accumulations may lead secondarily to muscle atrophy, joint contractures, and skin ulceration complicated by recurrent episodes of local inflammation and infection. OBJECTIVES: To review the classification, p

Photosensitivity in rheumatic diseases.

There have been a number of recent advances in the genetic understanding of photosensitive rheumatic diseases, especially subacute cutaneous lupus erythematosus and dermatomyositis. These advances support the concept that increased numbers of ultraviolet light-induced apoptotic cells in skin lead to a supra-threshold concentration of antigenic peptides. The current genetic data suggest that increased keratinocyte apopotosis can result from increased amounts of TNF-alpha that induce apoptosis due to a ultraviolet light-sensitive TNF promoter polymorphism or to decreased clearance of apototic ce

Rheumatic diseases of childhood.

Development of diagnostic criteria for juvenile rheumatoid arthritis, systemic lupus erythematosus, a juvenile dermatomyositis, as well as advances in molecular biology, have assisted epidemiologic study of the rheumatic disorders of childhood. It may be misleading to extrapolate the incidence and prevalence of pediatric forms of arthritis from population studies of adults. Additional study of the frequency of childhood musculoskeletal disorders is very much needed. Classification criteria for Kawasaki syndrome, fibrositis in children, and the juvenile spondyloarthropathies are also desirable.

[Uveitis and rheumatic diseases].

Uveitis occurs among children suffering from juvenile chronic arthropathy (mainly in daughters presenting with the pauciarticular form and accompanied by antinuclear antibodies) and among adults in the group of seronegative spondylarthropathies, among which ankylosing spondylitis and Reiter's syndrome are the most representative diseases. The association between acute anterior uveitis, HLA B27 antigen and seronegative spondylarthropathies is discussed.
