Adjustment disorder and risk of Parkinson's disease.

BACKGROUND AND PURPOSE: It has been postulated that stress is part of the etiological process of Parkinson's disease (PD). The risk of PD was examined in a cohort of patients with adjustment disorders, a diagnosis made in the presence of a severe response to a stressful life event. METHODS: Using Danish medical registries, PD occurrence was examined in a nationwide population-based cohort of patients with adjustment disorder diagnosed between 1995 and 2011. The standardized incidence ratio of PD was calculated as the ratio of observed to expected cases, stratified by time and potential risk f

Risk-adjusted models for adverse obstetric outcomes and variation in risk-adjusted outcomes across hospitals.

OBJECTIVE: Regulatory bodies and insurers evaluate hospital quality using obstetrical outcomes, however meaningful comparisons should take preexisting patient characteristics into account. Furthermore, if risk-adjusted outcomes are consistent within a hospital, fewer measures and resources would be needed to assess obstetrical quality. Our objective was to establish risk-adjusted models for 5 obstetric outcomes and assess hospital performance across these outcomes. STUDY DESIGN: We studied a cohort of 115,502 women and their neonates born in 25 hospitals in the United States from March 2008 t

[Adjustment to a Stressful Event in the Couple: Depression of the Partner as Risk for Adjustment Disorder].

Maladaptive reactions on stressful experiences justifying a diagnosis of adjustment disorder have high prevalence. Little is known about the possible risk for clinically significant maladaptation that results from the social context. The literature on the effects of depression on communication and altered support conditions in couples is suggesting this. Aim of this study was to investigate whether clinically significant depression in the romantic partner is a risk factor for adjustment disorder following a concept of stress-response disorder. Furthermore, from a dimensional point of view a po

AAHKS Risk Adjustment Initiative: Why Is It Important?

Risk adjustment is critical when any comparison of hospital performance or surgeon performance on patient outcomes is being measured. The purpose of risk adjustment is to level the playing field in making comparisons of outcome either against the benchmark or across individual providers, hospitals, or provider groups. The consequences of not leveling the playing field may lead to the inappropriate inference of high quality, that is outcomes are better not because of better quality but because the patients are healthy. Additional consequence of not leveling the playing field is the inappropriat

Postabortion psychological adjustment: are minors at increased risk?

PURPOSE: To assess whether younger adolescents experience greater adverse psychological outcomes after abortion than those aged 18-21 years, whether abortion places all adolescents at risk for negative sequelae, and what factors predict negative outcomes. METHODS: A total of 96 young women aged 14-21 years seeking counseling for unwanted pregnancies at four clinics completed questionnaires after counseling. These included the Beck Depression Inventory (BDI), an emotion scale, questions regarding sociodemographic and reproductive background, feelings about pregnancy, and decision-making. Sixty

Cesarean delivery risk adjustment for regional interhospital comparisons.

OBJECTIVE: The aim of this study was to determine the effect of adjustment for patient population mix on observed, expected, and standardized cesarean delivery rates in regional hospitals. STUDY DESIGN: Multiple logistic regression was applied to a large regional perinatal database comprising 16 hospitals. Variables significantly associated with cesarean delivery were used to calculate cesarean delivery probabilities for individual patients. Probabilities were summed across hospitals to derive expected hospital cesarean delivery rates. A standardized rate for each hospital was then calculated

Problematic Risk Adjustment in National Healthcare Safety Network Measures.

This article reviews the risk-adjustment models underpinning the National Healthcare Safety Network (NHSN) standardized infection ratios. After first describing the models, the authors focus on hospital intensive care unit (ICU) designation as a variable employed across the various risk models. The risk-adjusted frequency with which ICU services are reported in Medicare fee-for-service claims data was compared as a proxy for determining whether reporting of ICU days is similar across hospitals. Extreme variation was found in the reporting of ICU utilization among admissions for congestive hear

Development of an epilepsy-specific risk adjustment comorbidity index.

PURPOSE: To develop an epilepsy-specific comorbidity risk adjustment index for mortality outcomes research. METHODS: Data were extracted from five linked administrative databases in Calgary, Canada from April 1, 1996 to March 31, 2004. Epilepsy patients were defined using a validated ICD-9-CM- and ICD-10-CA-based case definition. An epilepsy-specific comorbidity index was developed using comorbidities from the Charlson and Elixhauser indexes and other relevant epilepsy comorbidities. In the final model, 14 comorbidities significantly associated with mortality remained and each was assigned a

[Risk adjustment in external quality assurance with multivariate analysis].

Surgical clinics throughout Germany are under political and economical pressure. Comparing outcomes across clinics or ranking become more and more popular although there are no generally accepted and scientifically valid methods how to compare. Using original data of the external quality assurance project of the chamber of physicians of Westfalia-Lippia it is shown in this article that comparing raw data is often misleading. Differences in risk factors (case mix) have to be taken into consideration. A model using logistic regression methods to adjust for case mix in legality and morbidity of h

Risk of First Adjustment Disorder in U.S. Army Soldiers.

INTRODUCTION: Adjustment disorder is the most common mental health diagnosis in the U.S. military and one of the most common globally, yet its patterns and predictors are rarely studied. Identifying career phases and characteristics associated with increased risk of first adjustment disorder (AD) diagnosis among U.S. Army soldiers can inform targeted prevention and intervention efforts. MATERIALS AND METHODS: This longitudinal, retrospective cohort study analyzed data from the Study to Assess Risk and Resilience in Servicemembers-Longitudinal Study (STARRS-LS). Administrative medical and pers

Direct comparison of risk-adjusted and non-risk-adjusted CUSUM analyses of coronary artery bypass surgery outcomes.

OBJECTIVE: We previously applied non-risk-adjusted cumulative sum methods to analyze coronary bypass outcomes. The objective of this study was to assess the incremental advantage of risk-adjusted cumulative sum methods in this setting. METHODS: Prospective data were collected in 793 consecutive patients who underwent coronary bypass grafting performed by a single surgeon during a period of 5 years. The composite occurrence of an "adverse outcome" included mortality or any of 10 major complications. An institutional logistic regression model for adverse outcome was developed by using 2608 cont

Longitudinal Risk Adjustment for Maternal End-Organ Injury and Death.

OBJECTIVE: To determine whether adjusting for healthcare utilization and comorbidity diagnosed in the year before delivery improves the prediction of adverse maternal outcomes. METHODS: The Truven Health MarketScan database was used to determine whether healthcare utilization and comorbidity diagnosed in the year before pregnancy improved prediction of acute organ injury or death during the delivery hospitalization through 30 days postpartum in this retrospective cohort study. In an initial model, we analyzed the risk for adverse outcomes controlling for underlying comorbidity, obesity, and d

Toward Optimizing Risk Adjustment in the Dutch Surgical Aneurysm Audit.

BACKGROUND: To compare hospital outcomes of aortic aneurysm surgery, casemix correction for preoperative variables is essential. Most of these variables can be deduced from mortality risk prediction models. Our aim was to identify the optimal set of preoperative variables associated with mortality to establish a relevant and efficient casemix model. METHODS: All patients prospectively registered between 2013 and 2016 in the Dutch Surgical Aneurysm Audit (DSAA) were included for the analysis. After multiple imputation for missing variables, predictors for mortality following univariable logist

Child maltreatment profiles and adjustment problems in high-risk adolescents.

The purpose of this article is to identify profiles of maltreatment experiences in a sample of high-risk adolescents and to investigate the relationship between the derived profiles and psychological adjustment. Participants are 1,131 youth between the ages of 12 and 18 years involved with publicly funded mental health and social services. Information on physical, sexual, and emotional maltreatment and psychological symptoms are obtained in interviews with adolescents and their primary caregivers. Using latent profile analysis, three maltreatment profiles are identified: "sexual+physical+emoti

Risk adjustment for congenital heart surgery: the RACHS-1 method.

The new health care environment has increased the need for accurate information about outcomes after pediatric cardiac surgery to facilitate quality improvement efforts both locally and globally. The Risk Adjustment for Congenital Heart Surgery (RACHS-1) method was created to allow a refined understanding of differences in mortality among patients undergoing congenital heart surgery, as would typically be encountered within a pediatric population. RACHS-1 can be used to evaluate differences in mortality among groups of patients within a single dataset, such as variability among institutions. I
