Beyond Medicare ACOs: Pediatric Alternative Payment Models and Financial Risk-Sharing.

Pediatric clinicians are increasingly asked to participate in alternative payment models (APMs) that hold clinicians financially accountable for population-level spending. Drawing heavily on evidence from Medicare, policymakers promote accountable care organizations and other risk-sharing arrangements as pathways to lower costs and higher quality. Yet the pediatric context differs in fundamental ways that limit the direct applicability of adult models. Child health care has fewer short-term opportunities for cost savings, and small numbers of children with medical complexity drive much spendin

Financial ''risk-sharing'' or refund programs in assisted reproduction: an Ethics Committee opinion.

Financial "risk-sharing" fee structures in assisted reproduction programs charge patients a higher initial fee that includes multiple cycles but offers a partial or complete refund if treatment fails. This opinion of the American Society for Reproductive Medicine Ethics Committee analyzes the ethical issues raised by these fee structures, including patient selection criteria, conflicts of interest, success rate transparency, and patient-informed consent. This document replaces the document of the same name, last published in 2016.

Health Shocks, Perceived Mortality Risk, and Financial Preferences: Longitudinal Evidence From Share.

Major health events are salient mortality cues and may reshape financial preferences in later life. However, evidence on the role of subjective mortality risk in shaping financial preferences is scarce. This study uses a longitudinal quasi-experimental design to examine whether incident heart attack, stroke, cancer, diabetes, or high blood pressure are followed by a change in perceived mortality risk and, subsequently, stated willingness to take financial risk and self-reported financial planning horizons. We use longitudinal data from Waves 4-9 of the Survey of Health, Ageing and Retirement i

[Risk-sharing scheme in Israel--Kuvan as an allegory].

Healthcare systems worldwide are dealing with the uncertainty characterizing new and expensive health technoLogies, particularly aspects involving drug effectiveness and the extent and doses required for utilization. Reducing this uncertainty can be achieved mainly by using either coverage with evidence development methods or risk-sharing schemes (RSS). In 2011, the first phenylketonuria (PKU) risk-sharing scheme was set up in Israel, through the public funding health services updating process. This was done in order to ensure that people with PKU could access PKU sole treatment--sapropterin d

Examining the Distinct Effects of Structural, Financial, and Information-Sharing Integration on Hospital Costs.

OBJECTIVE: Hospitals are pursuing integration strategies to contain costs, but evidence remains limited. This study examined the independent associations of 3 forms of integration-structural, financial, and information sharing-with hospital costs. STUDY DESIGN: Hospital fixed-effects panel regression using data from multiple sources-CMS Hospital Compare, Let's Get Healthy California, and the American Hospital Association Annual Surveys and Information Technology Supplements-for years 2014-2016. Development and Use of Gene Therapy Orphan Drugs-Ethical Needs for a Broader Cooperation Between the Pharmaceutical Industry and Society.

Gene therapy orphan medicinal products constitute a unique group of new drugs which in case of hereditary diseases are usually administered only once at an early age, in the hope to provide sufficient gene product to last for the entire life of the patients. The combination of an exceptionally large single payment and the life-long clinical follow-up needed for understanding the long-term benefits and safety of gene therapy, represent new types of scientific, financial, social and ethical challenges for the pharmaceutical industry, regulators and society. With special consideration of the uniq

Sociodemographic Factors Associated With HIV/HCV High-Risk Behaviors Among People Who Use Drugs on Methadone Maintenance Treatment: A 10-Year Observational Study.

Background: Sociodemographic factors have an impact worldwide on the behavior of people who use drugs (PWUD). This study attempts to clarify the sociodemographic factors related to HIV/HCV high-risk behaviors (injection drug use, syringe sharing, and multiple sex partners) among PWUD on methadone maintenance treatment (MMT) in the long term. Methods: The 13,300 PWUD recruited into the MMT program were followed during 2006-2015. Generalized estimating equations were used to examine the relationship between sociodemographic characteristics and HIV/HCV high-risk behaviors. Results: We found that

"Caballo": risk environments, drug sharing and the emergence of a hepatitis C virus epidemic among people who inject drugs in Puerto Rico.

BACKGROUND: Sharing drug injection equipment has been associated with the transmission of HCV among PWID through blood contained in the cooker and cotton used to prepare and divide up the drug solution. While epidemiologists often subsume this practice under the sharing of "ancillary equipment," more attention should be paid to the fact that indirect sharing takes place within the process of joint drug acquisition and preparation. METHODS: We employed an ethnographic approach observing active PWID (N = 33) in four rural towns in Puerto Rico in order to document drug sharing arrangements invol

Comparative policy analysis of national rare disease funding policies in Australia, Singapore, South Korea, the United Kingdom and the United States: a scoping review.

BACKGROUND: Rare diseases pose immense challenges for healthcare systems due to their low prevalence, associated disabilities, and attendant treatment costs. Advancements in gene therapy, such as treatments for Spinal Muscular Atrophy (SMA), have introduced novel therapeutic options, but the high costs, exemplified by Zolgensma® at US$2.1 million, present significant financial barriers. This scoping review aimed to compare the funding approaches for rare disease treatments across high-performing health systems in Australia, Singapore, South Korea, the United Kingdom (UK), and the United States

Evaluation of a risk-sharing agreement for atezolizumab treatment in patients with non-small cell lung cancer: a strategy to improve access in low-income countries.

BACKGROUND: Using immune checkpoint inhibitors (IO) is a promising approach to maximize clinical benefits for patients with non-small cell lung cancer (NSCLC). PD-L1 expression serves as a predictive factor for treatment outcomes with IO. However, the high cost of this treatment creates significant barriers to access. Substantial evidence demonstrates the sustained clinical benefits experienced by patients who respond to immunotherapy. While IOs show promise in NSCLC treatment, their high cost poses access barriers. AIM: This study focused on a prospective cost analysis conducted at a high-sp

Medicare's 2026 to 2030 Transforming Episode Accountability Model (TEAM): Implications for Anesthesiologists.

Medicare's Transforming Episode Accountability Model (TEAM) represents a significant shift in surgical care reimbursement. TEAM's bundled payment introduces episode-level accountability for cost and outcomes, replacing fee-for-service for discrete activities. TEAM's risk-sharing payments will reshape the clinical and financial environment in which anesthesiologists practice, as their reimbursement is included within the bundle. Their role in perioperative decision-making can strongly influence the performance of hospitals operating under TEAM's risk-sharing arrangements. This article outlines

Funding linked to ongoing research: impact of the bosentan patient registry on pricing in Australia.

OBJECTIVES: Bosentan, a dual endothelin receptor antagonist (ERA), was the first oral drug approved for the treatment of pulmonary arterial hypertension (PAH), a rare disease with poor prognosis. In 2004 the Australian Department of Health agreed to fund bosentan on the pharmaceutical benefits scheme (PBS) on the condition that a registry was established to monitor mortality: if the observed mortality rate was higher than that claimed in the original funding submission then the price of bosentan would be reduced to maintain the original incremental cost-effectiveness ratio (ICER). When the Bough Breaks: The Financial Burden of Childbirth and Postpartum Care by Insurance Type.

UNLABELLED: Policy Points This study examines exposure to out-of-pocket (OOP) costs related to childbirth and postpartum care for those with a Medicaid-insured birth compared with those with a commercially insured birth and subsequent financial outcomes at 12 months postpartum. We find that Medicaid is highly protective against health care costs for childbirth and postpartum care relative to commercial insurance, particularly for birthing people with low income. We find persistent medical debt and worry at 12 months postpartum for Medicaid recipients who reported OOP childbirth expenses. CONT

Risk sharing arrangements for pharmaceuticals: potential considerations and recommendations for European payers.

BackgroundThere has been an increase in 'risk sharing' schemes for pharmaceuticals between healthcare institutions and pharmaceutical companies in Europe in recent years as an additional approach to provide continued comprehensive and equitable healthcare. There is though confusion surrounding the terminology as well as concerns with existing schemes.MethodsA literature review was undertaken to identify existing schemes supplemented with additional internal documents or web-based references known to the authors. This was combined with the extensive knowledge of health authority personnel from

Social reward and nonsocial reward processing across the adult lifespan: An interim multi-echo fMRI and diffusion dataset.

Social relationships change across the lifespan as social networks narrow and motivational priorities shift. These changes may affect, or reflect, differences in how older adults make decisions related to processing social and non-social rewards. While we have shown initial evidence that older adults have a blunted response to some features of social reward, further work in larger samples is needed to replicate our results and probe the extent to which age-related differences translate to real world consequences, such as financial exploitation. To address this gap, we are conducting a 5-year s

Cost-effectiveness of Financial Incentives for Patients and Physicians to Manage Low-Density Lipoprotein Cholesterol Levels.

IMPORTANCE: Financial incentives shared between physicians and patients were shown to significantly reduce low-density lipoprotein cholesterol (LDL-C) levels in a randomized clinical trial, but it is not known whether these health benefits are worth the added incentive and utilization costs required to achieve them. OBJECTIVE: To evaluate the long-term cost-effectiveness of financial incentives on LDL-C level control. DESIGN, SETTING, AND PARTICIPANTS: In this economic evaluation, a previously validated microsimulation computer model was parameterized using individual-level data from the ran

Divorced and separated parents during the COVID-19 pandemic.

The COVID-19 pandemic has caused significant stress for individuals, couples, and families. Divorced and separated couples with children face unique stresses amid the pandemic. This mixed-methods study explored these challenges among 296 divorced and separated parents: namely 204 women formerly partnered with men, 34 men formerly partnered with women, and 58 women formerly partnered with women, who were surveyed during Summer/Fall of 2020. Participants described legal, financial, and coparenting challenges. Those who were not yet divorced described difficulties filing for or finalizing their d

Planning for the Post-COVID Syndrome: How Payers Can Mitigate Long-Term Complications of the Pandemic.

As the COVID-19 pandemic continues to unfold, payers across the USA have stepped up to alleviate patients' financial burden by waiving cost-sharing for COVID-19 testing and treatment. However, there has been no substantive discussion of potential long-term effects of COVID-19 on patient health or their financial and policy implications. After recovery, patients remain at risk for lung disease, heart disease, frailty, and mental health disorders. There may also be long-term sequelae of adverse events that develop in the course of COVID-19 and its treatment. These complications are likely to pla

Meme asset wagering: Perceptions of risk, overconfidence, and gambling problems.

Several financial assets, such as shares of GameStop or Dogecoin cryptocurrency, became the focus of substantial speculation in early-2021 that resulted in high price volatility and trading volume. This "meme asset wagering" appears to be closely related to the emergence of zero-fee retail brokerages, high-leverage cryptocurrency exchanges, and social media investment communities that facilitate and encourage risky behavior. As an emerging form of financial risk-taking, little is known about participants in these markets. In this study, an internet-based sample (n = 643) was recruited to asses
