Editorial: Reflux, dyspepsia, and Rome III (or Rome IV?).

The paper by Xiao et al. in this issue of American Journal of Gastroenterology reports that patients with functional dyspepsia (FD) complaining of epigastric burning have a higher probability to present abnormal gastroesophageal acid reflux, as well as response to proton pump inhibitor therapy than those complaining of epigastric pain, bothersome postprandial fullness, or early satiety. No differences in the above parameters were detected when comparing patients with epigastric pain syndrome and postprandial distress syndrome, as proposed by the Rome III classification of FD. If confirmed, the

Splitting irritable bowel syndrome: from original Rome to Rome II criteria.

OBJECTIVES: Diagnosis of irritable bowel syndrome (IBS) and other functional bowel disorders (FBD) is based on symptom evaluation. Clinical criteria have changed over time, yielding different proportions of subjects fulfilling diagnostic requirements. According to new diagnostic criteria (Rome II), subjects considered some years ago to have IBS no longer do so. The aim of this article is to evaluate how patients diagnosed as having IBS according to original Rome criteria have been split, and to which clinical diagnosis they belong today. METHODS: Two hundred and eleven subjects meeting origin

Symptom Stability in Rome IV vs Rome III Irritable Bowel Syndrome.

INTRODUCTION: Irritable bowel syndrome (IBS) is a chronic functional bowel disorder, which follows a relapsing and remitting course. Little is known about how evolving definitions of IBS or treatment for the condition affect symptom stability. We conducted a 12-month longitudinal follow-up study of individuals who self-identified as having IBS to examine these issues. METHODS: We collected demographic, gastrointestinal symptom, mood, and psychological health data at baseline, and gastrointestinal symptom data at 12 months, from adults who self-identified as having IBS, registered with 3 organ

A case of loiasis in Rome.

BACKGROUND: Owing to the increase of an immigrant population and of Italian citizens travelling for tourism or on business, it is nowadays possible to observe clinical pictures characteristic of tropical regions, often with indistinct symptoms. One of these is Loa loa infestation, or loiasis, a form of filariasis caused by Loa loa and transmitted by the Chrysops fly. We present the case of a male immigrant from Cameroon. Characteristic symptoms were intense xerosis, mostly of the third inferior part of the legs, intensely pruritic, with numerous lesions from scratching. No benefit was obtained

Onychomycosis in Rome, Italy.

This report presents the results of a study conducted between 1985 and 1994 on onychomycosis observed in the city of Rome. Six thousand six hundred and eighty eight patients were examined during this period. Among them 1,762 (26.3%) were affected by fungal nail infections. Because the etiologic agents could not be isolated in 105 cases (6%), the results refer to 1,657 subjects (24.8% of the total), presenting with positive microscopic and cultural examinations. Thirty eight patients (2.3%) had onychomycosis of both their hands and feet. From an etiological point of view, 59.1% of the nail infe

Comparison of demographic, clinical, and psychological characteristics among patients with irritable bowel syndrome meeting the Rome clinical, Rome V, and Rome IV criteria.

BACKGROUND: The Rome clinical criteria for irritable bowel syndrome (IBS) were introduced to improve clinical applicability and represent an important advancement within the Rome V process. However, their clinical profiles relative to the standard Rome criteria remain unclear. METHODS: Consecutive adults with suspected IBS were prospectively recruited at a tertiary outpatient clinic in China. Participants completed symptom questionnaires and underwent clinical investigations to exclude organic disease. Patients were classified into three mutually exclusive groups: meeting Rome clinical criter

All Roads Lead to Rome: Update on Rome III Criteria and New Treatment Options.

The recently published Rome III criteria reflect current understanding of functional gastrointestinal disorders. These criteria include definitions of these conditions and their pathophysiologic subtypes and offer guidelines for their management. At the 2006 Annual Scientific Meeting of the American College of Gastroenterology, a panel of experts discussed these criteria as they pertain to irritable bowel syndrome, functional dyspepsia, and chronic constipation. This article reviews the panel's findings, highlights the differences between the Rome II and III criteria, and summarizes best treat

What's new in Rome IV?

The functional gastrointestinal disorders (FGIDs) are the most common disorders seen in Gastroenterology clinical practice. The Rome process has generated consensus definitions of FGIDS, allowing to subdivide patients into diagnostic categories based on the symptom pattern. The Rome IV consensus, presented in 2016, is the most recent update of this diagnostic scheme. This article summarizes the main changes, which include the use of epidemiology-based symptom thresholds to define FGIDs, characterization of FGIDS as disorders of Gut-Brain interaction, and updates of criteria for esophageal diso

Malaria and Miscarriage in Ancient Rome.

Many Roman authors have claimed that induced abortions were frequent among aristocratic women in their society. They assumed that abortion was a simple procedure, easy to perform, and generally harmless for the women involved. The truth of these claims is frequently accepted by modern scholars. This article will argue that most supposed "abortions" were miscarriages caused by various infectious diseases, especially malaria.

[State of ophthalmology in ancient Rome].

The article presents the state of medical knowledge of the peoples forming Etruscan and Roman civilization circa 600 years B. C. Medical practices of these peoples are discussed. It is underlined that some Etruscan views were adopted by the Romans who cared much about hygiene. However, the development of Roman medicine was strictly related to the Greek And Egyptian physicians to this country. Various conditions of ophthalmic inflammation and many ocular diseases like pterygium, entropionand tumor of the lids were known in the Roman society. What Is New in Rome IV.

Functional gastrointestinal disorders (FGIDs) are diagnosed and classified using the Rome criteria; the criteria may change over time as new scientific data emerge. The Rome IV was released in May 2016. The aim is to review the main changes in Rome IV. FGIDs are now called disorders of gut-brain interaction (DGBI). Rome IV has a multicultural rather than a Western-culture focus. There are new chapters including multicultural, age-gender-women's health, intestinal microenvironment, biopsychosocial, and centrally mediated disorders. New disorders have been included although not truly FGIDs, but
