Rotaviruses are the most common cause of acute gastroenteritis in young children worldwide. Both licensed rotavirus vaccines (Rotarix™ [RV1] and RotaTeq™ [RV5]) are effective and safe. Studies from countries that have included RV1 or RV5 in the national immunization programs have demonstrated their safety and sustained efficacy under real-life circumstances. A significant decline in acute gastroenteritis-related deaths among Latin American children was observed after the introduction of RV1 and RV5 vaccines. Both vaccines were able to decrease the number of cases of rotavirus acute gastroenter

Since their discovery in the 1970s, the human rotaviruses have been recognized as the most important cause of acute infectious gastroenteritis among infants and children worldwide. Rotavirus has been found to infect almost all mammalian and avian species tested, and is primarily a disease of the young. In humans, rotavirus is the most frequent gastrointestinal pathogen in infants and children less than 2 years of age. In developing countries, the attack rate peaks at 6 months of age, whereas in developed areas of the world the virus is most commonly found among children 6-12 months of age. Rot

Group A rotavirus is the most common cause of severe gastroenteritis in young children globally, estimated to cause approximately 500,000 deaths each year, mainly in low-income countries. Two oral vaccines are currently licensed, a monovalent attenuated human rotavirus strain (Rotarix®, GSK Biologicals) and a pentavalent human-bovine reassortant vaccine (Rotateq®, Sanofi Pasteur MSD). On the basis of efficacy data from Europe and America, these vaccines were first recommended for routine immunization by WHO in 2006 in these countries. Rotaviruses are genetically highly variable, non-enveloped viruses with a double-stranded, segmented ribonucleic acid genome. They are a major cause of gastroenteritis worldwide. In children aged less than 5 years, they are the most frequent agent of severe acute diarrheal illnesses. In less developed countries, rotavirus diseases are one of the most frequent causes of death in infants and little children. Typically, symptomatic rotavirus diseases in infants (<5 years) and the elderly (>70 years) arise with sudden onset of watery diarrhoea with high risk of dehydration, accompanied by vomiting

Rotavirus gastroenteritis is one of the most important diseases in young children. There are 400,000 to 600,000 deaths annually among children under 5 years old in the world, especially in developing countries. Rotavirus vaccine has been developed since 1980s. However vaccine development and trial took a long time before the commercial available. One reason is the genetic variability of rotavirus, there are more than 10 serotypes (G types) in group A rotavirus. Another reason is side reaction such as intussusception which was recognized at the time of development of RotaShield. At present, Rot

Two live attenuated oral rotavirus vaccines were licensed in 2006 for prevention of severe acute gastroenteritis in children: Rotarix (GSK), a human rotavirus vaccine with G1P[8] serotype characteristics and RotaTeq (Merck), a bovine-human reassortant vaccine expressing human G1-4 and P[8] antigens. In prelicensure trials both vaccines showed high efficacy against severe RV gastroenteritis, low reactogenicity and no increased risk for intussusception (IS) (Ruiz-Palacios et al., 2006; Vesikari et al., 2006). Both vaccines have now been distributed in millions of doses. Post-marketing data have

The past few years have seen important developments in understanding the epidemiological and virological characteristics of rotaviruses, and rapid progress has been made in rotavirus vaccine development, but further challenges remain before a vaccine is introduced into widespread use. The licensure of the first rotavirus vaccine, a tetravalent rhesus-based rotavirus vaccine, in the United States in 1998, marked a significant advance in preventing the morbidity associated with rotavirus diarrhea. The association between the tetravalent rhesus-based rotavirus vaccine and intussusception has crea

Rotavirus is the leading cause of severe, dehydrating diarrhea worldwide and is responsible for more than 600 000 child deaths every year. In June 2009, the World Health Organization recommended inclusion of rotavirus vaccination in all national immunization programs. This universal endorsement of rotavirus vaccination should help the world's poorest countries to provide rotavirus vaccine to the poorest children, in order to achieve the Millennium Development Goal #4 of decreasing child mortality by two-thirds by 2015. This review will focus on the nature and epidemiology of the virus and on t

Rotavirus, the most common cause of severe diarrhea and a leading cause of mortality in children, has been a priority target for vaccine development for the past several years. The first rotavirus vaccine licensed in the United States was withdrawn because of an association of the vaccine with intussusception. However, the need for a vaccine is greatest in the developing world, because the benefits of preventing deaths due to rotavirus disease are substantially greater than the risk of intussusception. Early vaccines were based on animal strains. More recently developed and licensed vaccines a

Rotavirus is the main etiological agent that causes severe diarrheal diseases in newborns and young children up to two years. Every year, about one million children around the globe die of dehydration caused by Rotaviruses. The problem is even bigger in underdeveloped and developing countries. The results of our 18-month research, in the town Kragujevac and its surrounding area from December 1998 to May 2000 indicate that viruses are an important factor in the etiology of the acute diarrheal diseases in our population. In 124 children, aged 0 to 5, with the acute diarrheal diseases treated at

Encouraging results have been reported from several large trials of tetravalent rhesus rotavirus vaccine, with efficacy of 70-80% against severe disease. A recent Venezuelan study showed similar results to trials in USA and Europe. The vaccine may soon be licensed in USA. It provides the exciting prospect of a strategy to prevent one of the world's major child killers. Other candidate vaccines are under development including human-bovine reassortants, neonatal strains, non-replicating rotaviruses, vector vaccines and other genetically engineered products. Second and third generation rotavirus

On February 3, 2006, a bovine-based pentavalent rotavirus vaccine (RotaTeq, Merck & Co Inc, Whitehouse Station, NJ) was licensed by the US Food and Drug Administration for use in infants in the United States. The American Academy of Pediatrics recommends routine immunization of infants with 3 doses of pentavalent rotavirus vaccine administered orally at 2, 4, and 6 months of age. The first dose should be administered between 6 and 12 weeks of age; immunization should not be initiated for infants older than 12 weeks of age. Subsequent doses should be administered at 4- to 10-week intervals, and

The morbidity and mortality associated with rotavirus gastroenteritis are significant. Despite the past safety concerns with RotaShield, many physicians believed the need for rotavirus vaccination remains. Although most physicians were not aware of specific rotavirus gastroenteritis disease burden statistics, they perceive rotavirus gastroenteritis to pose a serious risk for children for which a vaccine would be beneficial. Proof of safety and efficacy, an ACIP/AAP/AAFP recommendation, and private insurance coverage/reimbursement were critical features that would allow physicians to overcome b

Rotavirus, a major etiological agent of acute diarrhea in children worldwide, has historically been linked to autoimmunity. In the last few years, several physiopathological approaches have been proposed to explain the leading mechanism triggering autoimmunity, from the old concept of molecular mimicry to the emerging theory of bystander activation and break of tolerance. Epidemiological and immunological data indicate a strong link between rotavirus infection and two of the autoimmune pathologies with the highest incidence: celiac disease and diabetes. The role for current oral rotavirus vacc

BACKGROUND: Infants born prematurely or with underlying conditions are at increased risk of severe rotavirus disease and associated complications. Given the theoretical risk of nosocomial transmission of vaccine-type rotavirus, rotavirus vaccination is recommended for infants at or after discharge from neonatal care settings. Because the first dose should be administered by 104 days of age, some infants may be age-ineligible for vaccination if delayed until discharge.
METHODS: This prospective cohort included infants admitted to an urban academic medical center between birth and 104 days who

Since the presentation of the clinical trial reports showing the excellent efficacy and safety of the two human rotavirus vaccines (Rotarix and RotaTeq), the human rotavirus vaccines have received worldwide attention. The two vaccines have been approved in more than 100 countries, and were included in routine immunization schedule in about 30 countries. The effectiveness of the two vaccines exceeded our expectations, and severe rotavirus gastroenteritis cases have been greatly reduced. Also in Japan, administration of Rotarix started just last November, and RotaTeq will be also started soon. O

This statement updates and replaces the 2007 American Academy of Pediatrics statement for prevention of rotavirus gastroenteritis. In February 2006, a live oral human-bovine reassortant rotavirus vaccine (RV5 [RotaTeq]) was licensed as a 3-dose series for use in infants in the United States. The American Academy of Pediatrics recommended routine use of RV5 in infants in the United States. In April 2008, a live, oral, human attenuated rotavirus vaccine (RV1 [Rotarix]) was licensed as a 2-dose series for use in infants in the United States. The American Academy of Pediatrics recommends routine i
