Hip Vascularity: A Review of the Anatomy and Clinical Implications.

Throughout development, the vascular supply to the proximal femur and acetabulum undergoes a series of changes during which it is susceptible to injury. Before age 3 months, the ligamentum teres and lateral epiphyseal arteries are the dominant supply to the developing head. The dominant supply shifts to the lateral epiphyseal vessels by age 18 months. The distinct metaphyseal and epiphyseal circulations of the adult proximal femur form in adolescence when an increasingly rich metaphyseal circulation supplies the subphyseal region, terminating at the physeal plate. The acetabular blood supply d

Neurovascular structures of the ligament of the head of femur.

The ligament of the head of femur (LHF), or ligamentum teres, is believed to provide blood supply to the head of femur and mechanical stability to the hip joint. But these functions in the adult are often debated. The existence and distribution of neurovascular structures within the ligament are not widely documented. This study examined the blood vessels and nervous tissue within the LHF to determine whether the ligament may have a vascular and proprioceptive function at the hip joint. Histological sections from the LHF from 10 embalmed hips (six female, four male; mean age 80.4 ± 8.7 years)

Clinical Anatomy of the Ligament of the Head of Femur.

The ligament of the head of femur (LHF) has gained clinical attention recently and is reported to contribute to hip stability. This study explores its morphology and morphometry, information that may help inform surgical decision making. Gross anatomical dissections were undertaken on 229 embalmed hips from European (n = 105) and Thai (n = 124) adult cadavers to examine LHF anatomy. Ligament morphometry was statistically compared at different sites, between sexes and sides. The origin of ligamental arteries and absence of the ligament were documented. The LHF was pyramidal or quadrangular in s

The ligamentum teres femoris in orangutans.

OBJECTIVES: It is widely viewed that orangutans lack a ligamentum teres femoris (LTF) inserting on the femoral head because orangutans lack a distinct fovea capitis. Orangutans employ acrobatic quadrumanous clambering that requires a high level of hip joint mobility, and the absence of an LTF is believed to be an adaptation to increase hip mobility. However, there are conflicting reports in the literature about whether there may be a different LTF configuration in orangutans, perhaps with a ligament inserting on the femoral neck instead. Here we perform a dissection-based study of orangutan hi

The Role of the Artery of Ligamentum Teres in Revascularization in Legg-Calve-Perthes Disease.

INTRODUCTION: A recent cadaveric study supported that most immature hips are supplied by the artery of ligamentum teres and suggested this medial vascular source may influence the pattern of revascularization in Legg-Calve-Perthes disease (LCPD). The purposes of this study were to characterize the perfusion pattern of the capital femoral epiphysis and determine the role of the artery of ligamentum teres in early revascularization of LCPD. METHODS: Retrospective review of perfusion magnetic resonance imaging (pMRI) from 64 hips in early stage LCPD (Waldenström stage I to IIa) was performed. Tw

The position of the femoral fovea can indicate hip instability and highly correlates with lesions of the ligamentum teres: an observational study.

BACKGROUND: The aim of this study was to assess the reliability of Delta angle (DA) as a parameter of microinstability compared to established radiographic instability parameters. It was assessed whether the morphology of the Ligamentum teres (LT) was affected by DA, established instability parameters. Further, the correlation between clinical status and microinstability parameters was assessed. METHODS: Data was retrospectively analysed from a single centre database for hip preservations surgery. All patients underwent a comprehensive and standardized radiographic and clinical assessments. Si

Effect of ligamentum teres tear on the development of joint instability and articular cartilage damage: an in vivo rabbit study.

The contribution of the ligamentum teres to the stabilization of the hip joint and the clinical influence of a compromised ligamentum teres are not well known. This study aimed to investigate joint stability and cartilage damage in a rabbit model by surgically inducing a complete ligamentum teres tear. Twenty adult New Zealand rabbits were used in this study. Rabbits were divided into complete ligamentum teres tear with capsulotomy (n = 9, group I) and capsulotomy only (n = 10, group II) groups. Unilateral surgery was performed on the left hip. Joint instability was assessed by measuring the p

Evaluation of the distribution of mechanoreceptors in the hip joint with severe coxarthrosis in 9 patients: A histologic and stereological study.

Due to its high mobility, hip joint plays a crucial role in executing many movements such as standing, sitting, running, crouching. The distribution of mechanoreceptors and neural elements in anatomical structures that provide stabilization of the hip joint is important in determining the optimal surgical incision site for the hip joint stabilizers in patients with coxarthrosis. Various studies have been conducted about the mechanoreceptors and distribution of neural elements in structures such as the transvers acetabular ligament, teres (round) ligament of femur, acetabular labrum and hip joi

The ligamentum capitis femoris: anatomic, magnetic resonance and computed tomography study.

The objective of the study was to describe the normal anatomy of the ligamentum capitis femoris and to determine the neurovascular structures potentially at risk during its reconstruction. Ten cadaveric specimens of the ligamentum capitis femoris (LCF) were dissected and photographed. Magnetic resonance (MR) and Computed tomography (CT) arthrography evaluation of the anatomy of the LCF in 30 hips were performed to measure length of the ligament and to study the proximity of neurovascular structures. The anatomical study showed that the LCF has a pyramidal structure and a banded appearance. The

Arthroscopic Reconstruction of the Ligamentum Teres: A Guide to Safe Tunnel Placement.

PURPOSE: To provide a quantitative guide to tunnel placement concurrently through the femur and acetabulum during a ligamentum teres reconstruction, minimizing the risk of injury to the obturator neurovascular bundle. METHODS: Nine human cadaveric pelvises, complete with femurs (mean age, 59.6 years; age range, 47-65 years), were studied. Before dissection, a 3-dimensional coordinate-measuring device was used to record the neutral orientation of the femur in the acetabulum. The specimens were then dissected free of all extra-articular soft tissue, except for the ligamentum teres and the obtur

Ligamentum Teres Impingement in Valgus Impacted Femoral Neck Fracture: A Case for Hip Arthroscopy.

CASE: Seven years after hip arthroscopy for cam impingement, a 47-year-old female patient suffered a valgus impacted femoral neck fracture of her left hip that was stabilized with cannulated screws. After fracture healing, she experienced persisting sharp groin pain that could be elicited in flexion/internal rotation and extension/adduction. We suspected a ligamentum teres impingement at the border of the acetabular fossa due to a post-traumatic fovea alta and performed an arthroscopic ligament resection. Symptoms completely resolved thereafter. CONCLUSION: Valgus impacted femoral neck fractu

Establishing a Consensus Definition of a Knee Fracture-Dislocation (Schenck Knee Dislocation V) Using a Global Modified Delphi Method.

BACKGROUND: Knee fracture-dislocations are complex injuries; however, there is no universally accepted definition of what constitutes a fracture-dislocation within the Schenck Knee Dislocation (KD) V subcategory. The purpose of this study was to establish a more precise definition for fracture patterns included within the Schenck KD V subcategory. METHODS: A series of clinical scenarios encompassing various fracture patterns in association with a bicruciate knee ligament injury was created by a working group of 8 surgeons. Utilizing a modified Delphi technique, 46 surgeons from 18 countries a

Combined intra-extra-articular technique for stabilisation of coxofemoral luxation. Preliminary results in two dogs.

OBJECTIVE: To report a new technique for repairing traumatic cranio-dorsal coxofemoral luxation in dogs. METHODS: Stabilisation of hip luxation was carried out in two dogs: a one-year-old male Border Collie and an eight-year-old female American Staffordshire Bull Terrier. A caudal approach was performed to expose the hip joint. Following this, a 2.5 mm hole was drilled through the acetabular wall at the original attachment of the round ligament, followed by a tunnel between the fovea capitis and the proximal third of the femur. Nylon tape was tied in a clove hitch knot around the femoral neck
