Variation in medical claim denials: safety-net providers are hardest hit.

INTRODUCTION: Medical claim denials impose substantial costs on providers but may also reduce low-value care, resulting in lower expenditures and better quality. Little is known about how denial rates vary across payer and provider segments, including for safety-net providers. METHODS: Using a large, multipayer claims dataset from 2019, we examined initial denials, appeal patterns, and final denials by payer segment, service type, and provider safety-net status. RESULTS: Medicaid managed care had the highest initial denial rate for professional claims (15.1%), while Medicare Advantage had th

Health System Expansion and Changes in Medicare Beneficiary Utilization of Safety Net Providers.

BACKGROUND: Evidence is limited on insured patients' use of safety net providers as vertically integrated health systems spread throughout the United States. OBJECTIVES: To examine whether market-level health system penetration is associated with: (1) switches in Medicare beneficiaries' usual source of primary care from federally qualified health centers (FQHCs) to health systems; and (2) FQHCs' overall Medicare patient and visit volume. RESEARCH DESIGN: Beneficiary-level discrete-time survival analysis and market-level linear regression analysis using Medicare fee-for-service claims data fr

Defining trauma in complex care management: Safety-net providers' perspectives on structural vulnerability and time.

In this paper, we delineate how staff of two complex care management (CCM) programs in urban safety net hospitals in the United States understand trauma. We seek to (1) describe how staff in CCM programs talk about trauma in their patients' lives; (2) discuss how trauma concepts allow staff to understand patients' symptoms, health-related behaviors, and responses to care as results of structural conditions; and (3) delineate the mismatch between long-term needs of patients with histories of trauma and the short-term interventions that CCM programs provide. Observation and interview data gather

The role of a specialized memory clinic supporting primary care providers in a safety net health system.

BACKGROUND: Although most dementia care occurs in primary care, consultation with dementia specialty care is sometimes indicated. Access to dementia specialists is limited, particularly in resource-limited environments such as the public health safety net, which may require triaging referrals to preserve access for patients with needs that can not be met in a primary care setting. METHODS: The eConsult system for primary care providers to refer patients to a subspecialty memory clinic is described for a large safety net health system. Demographic and clinical characteristics are presented for

Public coverage provides vital safety net for children with special health care needs.

In 2003, an estimated 13.5 million American children had special health care needs, ranging from learning disorders to severe disabilities. Medicaid or the State Children's Health Insurance Program (SCHIP) covered nearly two out of five children with special health care needs, according to a new study by the Center for Studying Health System Change (HSC). Despite the critical safety net provided by public insurance to millions of special-needs children, an estimated 650,000 of these medically vulnerable children were uninsured in 2003. Many likely were eligible for public insurance but not enr

Knowledge and self-efficacy for caring for breast and colon cancer survivors among safety net primary care providers.

BACKGROUND: Primary care providers (PCPs) are critical to the provision of comprehensive care for cancer survivors, yet there is very little data on the practices and quality of survivorship care occurring in safety net primary care settings. This study aimed to assess the knowledge and attitudes of PCPs and preferences for care models for breast and colon cancer survivors in a safety net health network. METHODS: A modified National Cancer Institute Survey of Physician Attitudes Regarding the Care of Cancer Survivors was sent electronically to 220 PCPs in 12 primary care clinics in the San Fr

Providing a safety net: a qualitative study on supporting medical students during goals of care discussions.

BACKGROUND: Engaging in effective goals of care (GOC) discussions with patients is a critical skill for physicians. Medical students, however, often feel unprepared, unsupported and uncomfortable leading these conversations. We undertook the current study to explore senior medical students' experiences with GOC discussions during their clinical training, examining when and how (and when not and how not) GOC discussions impacted them and the influence of supervision on their GOC learning. METHODS: We used qualitative interpretive description as our methodology. Fourth-year medical students at

Do safety-net hospitals provide equitable care after decompressive surgery for acute cauda equina syndrome?

INTRODUCTION: Safety-net hospitals provide care to a substantial share of disadvantaged patient populations. Whether disparities exist between safety-net hospitals and their counterparts in performing emergent neurosurgical procedures has not yet been examined. OBJECTIVE: We used the Nationwide Inpatient Sample (NIS), a national all-payer inpatient healthcare database, to determine whether safety-net hospitals provide equitable care after decompressive surgery for acute cauda equina syndrome (CES). METHODS: The NIS from 2002 to 2011 was queried for patients with a diagnosis of acute CES who

Two-stage brachial-basilic transposition fistula provides superior patency rates for dialysis access in a safety-net population.

BACKGROUND: Guidelines of the National Kidney Foundation recommending aggressive pursuit of autogenous fistulae for dialysis access in lieu of prosthetic arteriovenous grafts have stimulated a renewed interest in transposed brachial-basilic fistulae as an alternative technique for upper arm access in patients who may not be candidates for a lower arm radial-cephalic or forearm brachial-cephalic fistula. We hypothesized that in our safety-net population, where radial-cephalic and brachial-cephalic often are not possible, brachial-basilic would provide patency rates superior to arteriovenous gra

Sexually transmitted infection clinics as safety net providers: exploring the role of categorical sexually transmitted infection clinics in an era of health care reform.

BACKGROUND: For many individuals, the implementation of the US Affordable Care Act will involve a transition from public to private health care venues for sexually transmitted infection (STI) care and prevention. To anticipate challenges primary care providers may face and to inform the future role of publicly funded STI clinics, it is useful to consider their current functions. METHODS: Data collected by 40 STI clinics that are a part of the Sexually Transmitted Disease Surveillance Network were used to describe patient demographic and behavioral characteristics, STI diagnoses, and laborator

Filling the Gaps in an Inadequate Housing Safety Net: The Experiences of Informal Housing Providers and Implications for Their Housing Security, Health, and Well-Being.

The authors examine the experiences of informal housing providers, defined as those who provide housing and shelter to family, friends, and acquaintances in the context of a severe affordable rental housing crisis. Forty-five semistructured interviews were conducted with informal housing providers in and around New Haven, Connecticut, in 2021. The data describe the critical role informal housing providers play in addressing gaps in the housing safety net. Interviews also show the ways informal housing provision can strain already vulnerable households and threaten providers' own housing securi

Systematic Review Of US Health Care Safety-Net Definitions.

The health care safety net is critical to the care of patients with complex clinical and social needs. However, it is not uniformly defined across clinical settings, which may misdirect policies and funding. We performed a systematic review of peer-reviewed literature on safety-net definitions across the care continuum between March 2024 and March 2026. Our search produced 3,462 unique articles, with 2,532 articles ultimately included in the review. Safety-net definitions were categorized into a non-mutually exclusive framework that identified definitions based on population, institution, area

When Safety-Net Programs Compete: Medicaid, 340B, And The Battle Over Drug Discounts.

The Medicaid Drug Rebate Program and the 340B Drug Pricing Program are generally understood as separate efforts to promote drug affordability-one by directly subsidizing low-income patients through insurance coverage (Medicaid) and the other by indirectly subsidizing safety-net clinics and hospitals (340B). Yet they interact in ways that can unintentionally raise costs for Medicaid. This Policy Insight examines how this interaction occurs, introduces two policy strategies to mitigate the interaction, and summarizes the use of these strategies across states as of 2024. We conclude with recommen

Rural Federally Qualified Health Center Interventions Targeting Persons with Diabetes: A Systematic Review.

Diabetes and its complications are serious public health problems, disproportionately affecting rural Americans. Contributing to the literature, this systematic review examines intervention studies targeting people with diabetes (PWDs) receiving care in rural federally qualified health centers (FQHCs). The study (N=16) is informed by the Study Design, Data, Methods, Outcomes (SDMO) framework. Overall, findings revealed that rural FQHC interventions targeting PWDs were of serious or high risk of bias. Most studies (69%) used pre-experimental, pre-post intervention designs, often embedded within

Variation, Overlap, and Stability in Defining Safety Net Hospitals.

IMPORTANCE: The lack of universally accepted definitions for safety net hospitals (SNHs) has made it difficult to effectively design policies to support these hospitals and the populations they serve. OBJECTIVE: To evaluate the overlap, variation, and consistency across different definitions for SNH status. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used a hospital year-level dataset on short-term acute care US hospitals from 2014 to 2022. Hospital-level and area-level measures were used to define SNHs. Hospital characteristics under each definition, overlap across de

Trends in Substance Use Disorder-related Admissions at a Safety-net Hospital, 2008 - 2020.

OBJECTIVES: Safety-net hospitals disproportionately care for people with substance use disorders (SUDs), yet little is known about trends in hospital admissions related to specific substances. This study uses electronic health record data to describe trends in substance-specific admissions at a Midwest urban safety-net hospital. METHODS: We included all admissions from 2008 through 2020 and defined them as non-SUD (N = 154,477) or SUD-related (N = 63,667). We described patient characteristics and trends in substance-specific admissions. We estimated the association of SUD diagnoses with disch

Characterization of a hepatitis a outbreak in underserved urban tertiary safety net hospitals.

Outbreaks of Hepatitis A, caused by the Hepatitis A Virus (HAV), remain a worldwide health concern. We conducted a retrospective chart review to characterize patients with acute HAV during an outbreak at our urban tertiary care center to better characterize patients infected with HAV. We searched our electronic records for patients with positive HAV IgM antibodies during a period of outbreak in Philadelphia, May 2017-December 2019. Characteristics of patients were recorded. We searched an equal period of time prior to the outbreak, September 2014-April 2017, to compare the two patient populati
