Displaced distal tibial Salter-Harris II fractures.

Salter-Harris II fractures of the distal tibia affect children frequently, and when they are displaced present a treatment dilemma. Treatment primarily aims to restore alignment and prevent premature physeal closure, as this can lead to angular deformity, limb length difference, or both. Current literature is of poor methodological quality and is contradictory as to whether conservative or surgical management is superior in avoiding complications and adverse outcomes. A state of clinical equipoise exists regarding whether displaced distal tibial Salter-Harris II fractures in children should be

Salter-Harris type II fractures of the capital femoral epiphysis.

Salter-Harris type II fractures of the capital femoral epiphysis have not been previously documented. The authors have treated three patients who sustained four such fractures. One child had a recurrent fracture two years after the first had healed satisfactorily. Two fractures were treated by spica cast immobilization, one fracture by closed reduction and internal fixation, and the other fracture healed without treatment. No patient developed avascular necrosis or other complications. Two of the children had an association with idiopathic slipped capital femoral epiphysis. An etiologic relati

Epiphyseal plate sparing fixation for Salter-Harris IV ankle fractures.

The long-term consequences of epiphyseal plate injuries of the ankle certainly can be of high morbidity, and are well documented in the literature. This is especially true of the displaced Salter-Harris type IV fracture. The primary areas to be addressed in this injury are establishment of the proper diagnosis, open reduction with fixation not crossing the physis, and close long-term followup.

Treatment and outcomes of distal tibia salter harris II fractures.

BACKGROUND: Distal Salter-Harris (SH) II fractures of the tibia are common injuries in the pediatric population. The purpose of this study is to evaluate our treatment and outcomes of SH II fractures of the distal tibia. METHODS: The study was approved by the medical school's institutional review board (IRB). Fifty-one distal tibia SH type II fractures were treated from 2003 to 2017. We performed a retrospective review of all patients. Patients with displacement less than 3 mm, on x-ray, were treated with a cast. Patients with displacement greater than or equal to 3 mm displacement were initi

Salter-Harris type III epiphyseal fractures of the proximal phalanx.

Of 823 pediatric hand fractures treated between 1981 through 1996, only 11 (1.3%) were Salter-Harris type III fractures of the proximal phalanx. We review the anatomic basis, etiology, age prevalence, treatment, and results of these epiphyseal injuries. The average age at injury was 14.9 years, with a narrow range from 14 years 2 months to 15 years 11 months. Nine of 11 fractures were displaced and treated with open reduction and internal fixation. At an average follow-up of 21 months, 9 of 11 had full joint motion. The other two had minimal loss of motion, and all had normal function and coll

Operative treatment of displaced Salter-Harris III distal tibial fractures.

Displaced, isolated Salter-Harris III fractures of the distal tibia are infrequent. Typically, the mechanism of injury is an external rotation stress to the adolescent ankle. The distal tibial growth plate fuses from medial to lateral during adolescence. If the anterior tibiofibular ligament remains intact, lateral distal epiphysis of the tibia. Closed reduction of displaced fractures is usually unsuccessful. In 5 displaced fractures, periosteal flaps were frequently found to have blocked closed reduction. Follow-up evaluation after open reduction and internal fixation revealed anatomic healin

Distal Tibia Salter-Harris Fractures: To Fix or Not to Fix.

It is important to review treatment considerations and provide a framework to guide decision making for nonsurgical and surgical treatment of distal tibia physeal fractures in pediatric patients. In most closed injuries, attempted closed reduction and immobilization is the initial treatment for stabilization of these fractures. Postreduction fracture alignment evaluation with a focus on residual fracture displacement at the articular surface and the physis combined with residual angulation will guide further treatment after closed reduction. Discussion of commonly occurring distal tibia physea

Salter-Harris type III fracture-dislocation of the proximal humerus.

Salter-Harris type III fractures of the proximal humerus are rare injuries. We report a Salter-Harris type III anterior fracture-dislocation of the proximal humerus in a 10-year-old boy that was open reduced and internally stabilized. A bone scan performed during the initial hospitalization and at 2-year follow-up revealed devascularization and subsequent revascularization of the humeral head. At 2-year follow-up, the patient had full motion of the shoulder, no pain, and arm strength equal to that of the contralateral side. Four cases of Salter-Harris type III fractures of the proximal humerus

Epiphyseal growth plate fracture: Salter and Harris type V.

One of the most easily misdiagnosed injuries, which can result in severe consequences in young patients, is a fracture involving the epiphyseal growth plate. The Salter and Harris type V fracture is a relatively uncommon injury resulting from a severe crushing force applied through the epiphysis to an area of the growth plate. It is most likely to occur in the region of the knee or ankle. The following is a case report of a comminuted compression fracture of the epiphysis of the fifth metatarsal head, with complete transverse midshaft fracture of the proximal phalanx of the fifth digit.

Isolated Salter-Harris Type II Fracture of the Distal Ulna.

Isolated distal ulna epiphyseal plate injuries are very rare and are often associated with early epiphyseal plate arrest. A 13-year-old boy sustained an isolated minimally displaced Salter-Harris type II fracture of the left distal ulna following a fall from a bicycle. The fracture was reduced, and a long arm plaster cast was applied for four weeks. At the six-month follow-up, the patient presented with a painless, full range of movement of the left wrist, but on radiological examination, a mild shortening of the ulna was detected. We plan to regularly evaluate this patient until distal epiphy

Displaced Salter-Harris I fracture of the distal ulna physis.

A 10-year-old girl presented to the emergency department having sustained a fall onto an outstretched left hand while playing soccer. Clinical and radiographical assessment identified a Salter-Harris I distal ulna fracture, as well as a buckle fracture of the distal radius. The injury was closed, and she had no neurovascular deficits on examination. She was brought to the operating theatre the following morning for closed reduction under general anaesthesia. Image intensification was used to confirm anatomical reduction, and an above-elbow moulded plaster-of-paris cast was applied. Follow-up c

Salter-Harris I fractures of the distal radius misdiagnosed as wrist sprain.

Over a period of 5 years, 65 children under 12 years of age were diagnosed as having "wrist sprain" at the American University of Beirut Medical Center, Emergency Department. Thirty-eight children were reevaluated; 33 patients (86.8%) were diagnosed as having a Salter-Harris I fracture of the distal radius that necessitated further treatment to relieve symptoms and allow early wrist function. The diagnosis of "wrist sprain" is often inadequate in establishing a proper treatment regimen. Rather, by taking a careful history, performing an exacting examination, and utilizing appropriate diagnosti

Monteggia equivalent fracture associated with Salter-Harris I fracture of the radial head.

BACKGROUND: Monteggia fracture is an uncommon lesion, which associates ulnar shaft fracture and radial head dislocation. Equivalent Monteggia fracture can occur by associated lesions such as olecranon fracture or radial neck or head fracture. CASE PRESENTATION: We present the case of a five-year-old girl, right handed, involved in domestic accident falling off a bed and sustained an isolated trauma of the left upper limb. Radiograph showed an unusual case of Monteggia equivalent lesion associating a fracture of the proximal third of the ulna shaft and a growth plate fracture Salter I of the p

An Anatomic Reduction Technique for Salter-Harris Type III Medial Femoral Condyle Fractures.

UNLABELLED: Salter-Harris type III (SH-III) distal femur fractures are challenging to manage given their intra-articular nature and physeal involvement. Inadequate reduction of these injuries can potentiate growth disturbances, including premature physeal closure and subsequent deformity. Existing literature lacks a description of a standardized reduction technique for this fracture pattern. In this report, we use a cadaver knee to demonstrate a structured reduction maneuver for the SH-III medial femoral condyle fracture, designed to facilitate anatomic alignment of the epiphyseal fragment via

A Salter-Harris II Distal Radius Fracture Irreducible by Closed Methods.

CASE: A 10-year-old boy presented with a Salter-Harris II distal radius fracture that was irreducible by closed methods. An open reduction was performed in the operating room where a sleeve of periosteum was found interposed between the fracture fragments. Successful reduction was performed without difficulty after the periosteum was removed from the fracture. CONCLUSION: Soft-tissue interposition must be kept in mind when having difficulty performing closed reductions of pediatric distal radius physeal fractures to avoid excessive reduction attempts.

Irreducible Salter-Harris type II fracture of the distal tibial epiphysis.

An 11-year-old boy was admitted to the hospital after he fell to the ground from the roof, complaining of pain in his left ankle. Radiographs showed a Salter-Harris type II fracture-separation of the distal tibial epiphysis together with a transverse bending fracture of the distal fibula. An attempt at closed reduction was unsuccessful. At open reduction, torn periosteum and the posterior tibial tendon were interposed between the tibial metaphysis and epiphysis. After the tendon was deflected to its normal position, fracture-separation of the distal tibial epiphysis was easily reduced. At the

Innominate artery pseudoaneurysm from a Salter-Harris fracture of the sternoclavicular joint.

Fractures and dislocations of the sternoclavicular joint (SCJ) are uncommon, accounting for <5% of all shoulder girdle injuries. They are relatively more common in the pediatric population than in the adult population and can often present concurrently as a posteriorly displaced medial clavicular dislocation with a fracture through the unfused physis. It is especially important to recognize this injury, because its management and potential sequelae are very different from those for fractures of the clavicle shaft. This type of injury frequently requires closed or open operative management beca

Isolated Salter-Harris Type III Physeal Fracture of the Distal Ulna.

We describe a case of isolated physeal fracture of ulna distal end in a 13-year-old boy. This fracture type is uncommon, especially Salter-Harris type III of this injury has not been reported. Plain radiographs showed a small vertical fracture line at the ulnar distal end and an enlargement of epiphyseal plate at the base of ulnar styloid process. The present case was successfully managed with conservative treatment because of its minimal displacement.

Compartment syndrome with an isolated Salter Harris II fracture of the distal tibia.

A 14-year-old boy sustained a Salter Harris II fracture to his right distal tibia after a fall from his skateboard. He rapidly went on to develop the signs and symptoms of compartment syndrome, and he underwent emergency fasciotomy. This resulted in relief of his symptoms. After this procedure, his fracture was fixed with a single anteroposterior screw. He made a full and uncomplicated recovery, with no clinical or radiological evidence of epiphyseal growth arrest.
