Mitral valve repair can sometimes require leaflet augmentation or a patch repair of a defect in the leaflet. No ideal patch material is currently available that can grow and adapt to the mitral leaflet. This report describes a new technique using autologous saphenous vein as patch material. The saphenous vein is untreated, living tissue and remains flexible as a patch or leaflet augmentation for up to 3 years on echocardiography.

PURPOSE: In patients with no-option critical limb-threatening ischemia, venous foot arterialization can be considered the last chance before major amputation. Up until now, a really significant limitation of endovascular arterialization compared with surgery was the possibility to obtain arterial flow into the foot only through the deep plantar network.
TECHNIQUE: Two 5-mm snares are placed: one in the proximal tibial artery and the other in the great saphenous vein. After passing through these snares with a needle and a guidewire and closing the snares, the guidewire is pulled through the pr

Our 20-year results with in situ saphenous vein grafting in the lower extremity were reviewed and compared wih those where reversed saphenous veins were employed. The ten-year patency rates for both types of grafts terminating at the popliteal artery ranged from 41% to 43% and showed no statistical difference. The reversed saphenous vein performed well in the femoropopliteal position because the vein is usually of the same size at the knee as in the groin and thus does not taper when reversed. We have identified from our study specific indications for the in situ operation. These indications a

An analysis of 200 consecutive patients treated by saphenous-vein bypass grafting has been carried out. One patient died in hospital, and one died nine months after surgery. Some of the factors which predispose to postoperative complications can be defined preoperatively. These are unstable angina, multiple vessels disease, and impaired left ventricular function. The complications of postoperative infarction, low cardiac output, and arrhythmia were higher in the presence of these factors. The use of cardioplegic myocardial protection lessened the incidence of complications in patients with imp

Aplasia or hypoplasia of great saphenous vein (GSV) is relatively common. Most of them are segmental and localized around the knee. They rarely extend to the inguinal area, yet in case of this, the anterior accessory saphenous vein (AASV) is the most common connecting vein. We report a case of a 22-year-old male who underwent surgery for pain and heaviness of the right calf. He had the hypoplasia of right GSV extended from below the popliteal crease to near the saphenofemoral junction with posterior ASV (PASV) as a connecting vein. Significant reflux was observed in PASV and GSV which are prox

BACKGROUND: Bypass surgery remains the gold standard for long and complex arterial occlusions in the lower limb. The vein is regarded superior to prosthetic conduits in peripheral arterial bypass surgery. However, this option is often limited because of previous bypass, stripping, or poor quality of the ipsilateral and/or contralateral great saphenous vein (GSV). Under these circumstances, the arm vein (AV) and small saphenous vein (SSV) are the only alternative autologous vein grafts.
METHODS: We analyzed all consecutive patients treated at an academic tertiary referral center between Januar

Successful varicose vein surgery depends on accurate preoperative assessment and individualized treatment for various combinations of venous insufficiency. Noninvasive duplex scanning is currently the gold standard in varicose vein evaluation. Flush ligation and division of the great saphenous vein and its tributaries, inverted downward stripping to below the knee combined with stab avulsion of varicosities, have yielded excellent results for patients with great saphenous vein reflux. Additionally, identifying and correcting incompetent calf perforating veins is necessary to achieve a satisfac

Sensory impairment in the cutaneous distribution of the saphenous nerve was measured in 30 patients after bilateral stripping of the long saphenous vein. The vein was stripped upwards in one leg and downwards in the other. Three months after operation a significantly greater incidence of sensory loss was found in those legs in which the vein was stripped upwards (13 out of 26) compared with those which had been stripped downwards (6 out of 26).We recommend that the long saphenous vein should be stripped downwards.

OBJECTIVE: To investigate whether endoscopic saphenous vein harvesting (EVH )reduces leg wound morbidity and improves rehabilitation of lower extremity as compared to open vein harvesting (OVH) in patients with coronary artery disease.
METHODS: The clinical data of 333 patients who underwent isolated coronary artery bypass graft surgery (CABG) between January 2007 and December 2011 were retrospectively analyzed. These patients were divided into EVH group (n=210) and OVH group (n=123). Parameters including age,sex,with/without diabetes mellitus, leg wound complications rate, postoperative hospita

PURPOSE: To determine why some vein grafts fail, we prospectively studied the relationship between the histologic condition of the greater saphenous vein (GSV) at the time of grafting and subsequent stenosis of the vein graft.
METHODS: Ninety-four remnant segments of GSVs were obtained at the time of infrainguinal bypass in 91 patients and were perfusion fixed before histologic and ultrastructural examination. All bypass grafts were evaluated clinically and by duplex ultrasonography at regular intervals from 1 to 30 months after operation. All 24 grafts that developed lesions that caused thro

This review discusses the morphological characteristics of the human great saphenous vein (SV) harvested for coronary artery bypass grafting (CABG). It focuses on the vein's luminal intima, which was examined using laser confocal microscopy (LCM), transmission electron microscopy (TEM), and scanning electron microscopy (SEM). Summarised findings are: (1) LCM observations revealed that the vessel-like profiles, formed by the intima of the peripheral parts of SV luminal folds, may create a false impression that these are vasa vasorum vessels terminating at the vein lumen. (2) The SV luminal inti

BACKGROUND: Great saphenous vein (GSV) incompetence, causing varicose veins and venous insufficiency, makes up the majority of lower-limb superficial venous diseases. Treatment options for GSV incompetence include surgery (also known as high ligation and stripping), laser and radiofrequency ablation, and ultrasound-guided foam sclerotherapy. Newer treatments include cyanoacrylate glue, mechanochemical ablation, and endovenous steam ablation. These techniques avoid the need for a general anaesthetic, and may result in fewer complications and improved quality of life (QoL). These treatments shou

Traumatic venous pseudoaneurysm is extremely rare. Only two cases of great saphenous venous (GSV) pseudoaneurysm have been reported. We present a case in which a gradually enlarged mass occurred at the calf after local injury. It was nonpulsatile and varied in size with posture. It was initially misdiagnosed by contrast MRI. Duplex ultrasound and surgical intervention eventually identified it as a GSV pseudoaneurysm. This case demonstrates the need for physicians to consider venous pseudoaneurysm as a possible diagnosis for nonpulsatile masses in patient with traumatic history, and to use Dupl

Aortocoronary saphenous vein graft is an effective treatment of coronary artery disease and a means of markedly improving long-term prognosis in certain patient subgroups. However, there is a significant failure rate with these conduits. Early failure occurs within the first 1-2 months, most likely from primary thrombosis. Intermediate failure is usually caused by the development of neointimal hyperplasia. Late failure occurs after 3-5 years and results from accelerated atherosclerosis. The impact of saphenous vein graft failure on cardiovascular outcomes is significant, and it is important to

Coronary revascularization using saphenous vein grafts is an important treatment modality for patients with severe coronary artery disease. Percutaneous intervention of these grafts is often the best option for patients who develop severe stenosis of the vein grafts. Use of adjunctive glycoprotein IIb/IIIa inhibitors does not confer added benefit with ischemic endpoints as compared with heparin alone, but it increases the risk of bleeding. Bivalirudin used as the primary anticoagulant lowers the risk of bleeding. No-reflow frequently complicates vein graft interventions but can be treated with

To establish a benchmark for the clinical efficacy of PTCA and the IMA graft in our institution, the data from 79 patients who underwent isolated SVBG to the LAD between July, 1971, and December, 1984, were analyzed. Follow-up averaged 96.16 months/patient. Actuarial freedom from reoperation was 100% at 12 months, 95% at 60 months, and 89.4% at 120 months. Actuarial freedom from cardiac death was 100% at 12 months, 94% at 60 months, and 87% at 120 months. Actuarial freedom from an LAD graft failure myocardial event was 100% at 12 month, 94% at 60 months, and 77% at 120 months. Our SVBG failure
