Pathogenic Sarcina in urine.

Sarcina is a Gram-positive anaerobic coccus that has been consistently reported in the upper gastrointestinal tract biopsies of patients with evidence of gastric stasis. The pathogenic role of Sarcina in humans is not entirely known. We describe an infant who was surgically treated for posterior urethral valve and the urine showed pathogenic Sarcina.

Sarcina ventriculi : Review of the Literature.

Sarcina ventriculi is an increasingly common gram-positive coccus, recognized in gastric biopsies, particularly of patients with delayed gastric emptying. It occurs most commonly in adult women and can be identified easily by its characteristic morphologic features, such as basophilic staining, cuboid shape, tetrad arrangement, red blood cell-sized packets, flattened cell walls, and refractile nature on light microscopy. Although the pathogenesis of the organism is debated, it has been implicated in cases of gastric perforation, emphysematous gastritis, and peritonitis as well as occurring in

Emphysematous Gastritis Associated with Sarcina ventriculi.

Emphysematous gastritis is a rare and lethal gastrointestinal emergency if not promptly identified and managed. In rare cases, emphysematous gastritis is associated with Sarcina ventriculi infection, usually in patients with delayed gastric emptying. Here we report a lethal case of S. ventriculi-associated emphysematous gastritis in the absence of delayed gastric emptying in which the diagnosis was confirmed postmortem. This case provides an opportunity to review the clinical presentation, pathophysiology, and management of emphysematous gastritis so that the condition can be promptly diagnose

Sarcina, a new threat in the bariatric era.

First identified in humans by Goodsir in 1842, Sarcina were already known to cause fatal abomasal bloat in animals. Their pathogenicity in humans has only recently been characterized. Sarcina is not inherently pathogenic but, with a gastric ulcer and delayed gastric emptying, can result in perforation. We present a case report of a 32-year-old woman status post-gastric banding presenting with epigastric pain. Upper endoscopy revealed a gastric ulcer near the band. After deflation, upper gastrointestinal series showed passage of contrast and no perforation. Ulcer biopsy showed gastric contents

Case Report: Sarcina ventriculi, a masquerade of motility.

Sarcina ventriculi is a rare bacterium that has the potential to cause severe disease in the gastric mucosa. According to our search, there are less than 100 prior published case reports. This case discusses a 69-year-old man who presented to the hospital with severe gastric distention noted on CT of the abdomen with intractable nausea and emesis, for which endoscopy was performed and ruled out gastric outlet obstruction, with biopsies resulting as S. ventriculi. This bacterium has previously been reported to cause gastric dysmotility, as well as severe side effects including gastric perforati

The significance of Sarcina in routine surgical pathology practice.

Sarcina was first described by Goodsir. The appearance of this bacterium is so characteristic that the diagnosis can be made on light microscopy. Although the original description of Sarcina was made more than 150 years ago, little is known about its role in various human diseases. This study was undertaken with the aim to analyze critically the reason for this sudden recent interest in human Sarcina infection. The results indicate that Sarcina is a histopathological marker of functional or anatomical causes of gastric stasis, and has a possible association with life-threatening emphysematous

Sarcina ventriculi associated gastritis: Mimicking lymphoma on endoscopy.

Sarcina Ventriculi is a gram-positive anaerobic coccus rarely reported in patients with delayed gastric emptying, emphysematous gastritis, gastric ulcers, and perforation. So far, less than 30 cases of sarcina isolated from the stomach have been reported. Herein, we describe a case of a 66-year-old male with a history of persistent epigastric pain and regurgitation. The upper gastrointestinal (GI) endoscopy revealed gastric erythema, edema, ulceration, and food bezoar. A gastric biopsy was done to rule out lymphoma. On histopathological examination, sarcina Ventriculi was identified. This orga

Gastric Sarcina ventriculi: A Report on Two Cases.

Background and Clinical Significance: Sarcina ventriculi is a rare Gram-positive coccus that thrives in acidic environments such as the human stomach. It has been increasingly identified in individuals with delayed gastric emptying and has been reported in association with various gastric disorders. However, its exact pathogenic role is not fully understood and remains controversial. Case Presentation: We present two cases of patients, one with a small bowel obstruction and the other with epigastric pain, both diagnosed with Sarcina ventriculi infection by histological examination of gastric b

Association of Gastric Sarcina With Malignant Pyloric Stenosis.

Sarcina ventriculi (SV) is a Gram-positive cocci that thrives in the acidic stomach environment and may cause gastrointestinal symptoms. A 65-year-old woman with a history of Helicobacter pylori gastritis and diabetes presented with abdominal pain, vomiting, diarrhea, and weight loss. Initial esophagogastroduodenoscopy revealed pyloric stenosis with thickened prepyloric gastric folds, and endoscopic biopsy revealed SV without malignancy. Owing to persistent symptoms, endoscopic ultrasound was done with repeat biopsies and was nondiagnostic. Subsequently, a robotic gastrojejunostomy was done du

Sarcina ventriculi Bacteremia Complicating Aspiration Pneumonia: A Case Report.

Sarcina ventriculi, a large anaerobic Gram-positive coccus that clusters in tetrads, is most commonly detected histologically in gastric biopsy specimens from patients with gastrointestinal disorders. Herein, we describe a rare case of bacteremia caused by S. ventriculi in an 89-year-old man. The patient had a history of cerebral infarction, atrophic gastritis, and chronic obstructive pulmonary disease and was receiving home oxygen therapy. He was admitted to our hospital with a right femoral neck fracture. Three days after femoral surgery, he developed aspiration pneumonia, and S. ventriculi

John Goodsir: discovering Sarcina ventriculi and diagnosing Darwin's dyspepsia.

In 1842, when John Goodsir was Conservator to the Museum of the RCSEd, he saw a 19-year-old male patient who vomited a large volume of acidic, fermented-smelling, watery fluid every morning. Under his microscope, Goodsir found the vomitus to be populated with a micro-organism he named Sarcina ventriculi, which he considered to be causative. In so-doing, Goodsir became one of the first people to link a specific micro-organism with a disease. Goodsir recommended small doses of creosote as an antiseptic and claimed that the boy was eventually cured of the vomiting condition. In August of 1863 Cha

A Rare Cause of Upper Gastrointestinal Bleeding: Sarcina ventriculi.

We describe a patient who presented with hematemesis and was found to have unusually well-demarcated erythematous mucosa with a 2-3 cm irregular nonbleeding necrotic ulcer in the gastric body on esophagogastroduodenoscopy. Biopsy and pathologic examination of the tissue indicated infection with a rare bacterium, Sarcina ventriculi, prompting treatment with an unproven combination of 4 agents: metronidazole, ciprofloxacin, sucralfate, and pantoprazole. Repeat esophagogastroduodenoscopy 8 weeks later revealed complete resolution of the ulceration and surrounding erythema. These results may contr

Clinicopathologic Characterization of Sarcina ventriculi in the Upper Gastrointestinal Tract.

The bacterium Sarcina ventriculi (SV) is rarely identified in the upper gastrointestinal (GI) tract and has been associated with diverse clinical presentations. We aimed to characterize the clinicopathologic features of SV in the GI tract. Seventeen specimens (3 gastrectomy and 14 biopsy specimens) with histologic diagnosis of SV were identified and analyzed. The patients (9 female, 8 male) had a median age of 65 (range 32-86) years. Five (30%) patients presented acutely with GI bleeding or altered mental status. Other relevant symptoms included abdominal pain (n = 6, 35%), diarrhea (n = 4, 24

Sarcina organisms in the gastrointestinal tract: a clinicopathologic and molecular study.

Sarcina organisms were first observed in and recorded from the stomach contents of a patient suffering from vomiting by John Goodsir in 1842. Since that time, their fine structure, phylogenetic classification, and biochemical characteristics have been described. Although numerous cases of fatal disease have been attributed to this organism in the veterinary literature, only a few human cases have been documented. As a result, whether this organism causes disease in humans has not been definitively established. We report the clinicopathologic findings in a series of 5 patients with Sarcina-like

Fatal Outcome of Emphysematous Gastritis due to Sarcina ventriculi Infection.

Sarcina ventriculi is an increasingly common bacterium with a variable pathogenic role. It is often an incidental finding in asymptomatic patients but can also lead to life-threatening conditions, such as gastric perforation and emphysematous gastritis. We report a case of a 14-year-old boy with fatal emphysematous gastritis caused by S. ventriculi, who presented with abdominal pain and vomiting. His medical history included infantile left hemiparesis and operation of patent ductus arteriosus. Emergency surgery was scheduled due to the extremely dilated stomach and gastric pneumatosis, and a t
