BACKGROUND: Clear cell sarcoma is a rare, aggressive neoplasm with melanocytic differentiation, characterised by an EWSR1 fusion with ATF1, CREB1 or CREM. In addition to the classic deep-seated variant, a significantly rarer dermal/cutaneous form exists. First described in 2009, it is frequently misdiagnosed due to its histopathological and immunohistochemical overlap with melanoma.
OBJECTIVE: This review aims to summarise the current state of knowledge on cutaneous clear cell sarcoma, including pathogenesis, histopathology, immunohistochemistry, molecular pathology and differential diagnoses

We report a case of clear cell sarcoma in the third metatarsus of the right foot. This type of tumor is very rare and scantily reported in literature. A 42-year-old Caucasian male presented with a nodular ulcerated mass on the dorsal side of the left foot. X-rays demonstrated a nodular solid lesion which dislodged the third metatarsus. A biopsy revealed a neoplastic proliferation with a sarcoma clear cell profile; because of the aggressive nature of this type of neoplasm, we performed a trans-tibial amputation according to Bugess to achieve a better functionality for the patient. The present s

A 59-year-old woman presented with a large mediastinal mass. At thoracotomy, the mass was found tightly adherent to the esophageal wall and right lower lobe of the lung. Histological examination showed a solid tumor composed of closely packed nests of cells with clear and eosinophilic cytoplasm, which were strongly and diffusely positive for S100 protein but negative for HMB45 and Melan-A. The diagnosis of clear cell sarcoma was supported by demonstrating the presence of an EWS gene rearrangement by fluorescence in situ hybridization. There was no evidence that this lesion represented metastat

BACKGROUND: Clear cell sarcoma (CCS) is a rare soft tissue sarcoma that primarily affects individuals ages 20-40. It typically presents as a slow-growing, often painless, mass in the extremities. Wide resection is the mainstay of treatment, though local recurrence and metastatic disease are common despite surgery. This case series highlights our experience treating CCS within the military population.
MATERIALS AND METHODS: This is a retrospective case series of 3 patients treated at our institution between 2016 and 2026 with biopsy-confirmed CCS. Median survival, local recurrence, and inciden

In this editorial based on a case report, we delve into a seldom-seen occurrence of clear cell sarcoma featuring pancreatic metastasis in a 47-year-old male patient. Recognized for its typical tendency to metastasize to the lungs, bones, and brain, clear cell sarcoma rarely extends its reach to the pancreas. Despite the initial absence of discernible abnormalities during the patient's physical examination, the manifestation of abdominal pain prompted further investigation. Subsequent abdominal computed tomography brought to light the presence of a pancreatic tumor, culminating in the definitiv

Gastrointestinal clear cell sarcomas (GICCSs) are malignant mesenchymal tumour that occur in the wall of gastrointestinal tract, mostly in the ileum and rarely in the rectum. These tumours are highly invasive, and often have metastasized via the lymphatic system or bloodstream by the time of diagnosis. In this case, the patient was admitted for gastric fundus mass, who was subsequently found and confirmed by biopsy as clear cell sarcoma of rectum. Computed tomography (CT) and magnetic resonance imaging (MRI) showed multiple metastatic lesions in lymph node, lung, liver and bone. Furthermore, e

We report a case of primary clear-cell sarcoma (CCS) in the mediastinum. In October 2011, a 63-year-old man was admitted to our hospital for surgical resection. The tumor was completely excised by video-assisted thoracoscopic surgery. The tumor was well encapsulated and did not invade the pleura. Histological examination led to a final diagnosis of primary CCS in the mediastinum. The patient remains alive without evidence of recurrence at 15 months after surgery.

Clear cell sarcoma shares features with melanoma, but frequently shows EWSR1 rearrangements. It is an aggressive tumor typically occurring in the soft tissues of the extremities, with a gastrointestinal variant with less consistent melanocytic differentiation. It is extremely rare in the head and neck region, with no reported cases in the oral cavity. We report a case of an 82-year-old woman with a clear cell sarcoma arising in the tongue, with cervical lymph node metastases. Histologically, the tumor showed some features of gastrointestinal clear cell sarcoma. No osteoclast-type giant cells w

At 31 weeks gestation, a hydropic male fetus died in utero with metastatic disease from a renal clear cell sarcoma. The tumor had metastasized to para-aortic and mediastinal lymph nodes, lung, pleura, and liver, leading to superior vena cava obstruction and pulmonary hypoplasia. The pathologic findings and cytogenetic analysis of the fetus and tumor are presented. In addition, review of the literature reveals six other cases aged <6 months of age, including two extrarenal cases.

Clear cell sarcoma is a rare soft-tissue neoplasm, arising most commonly in the tendons and aponeuroses of young adults. We report here the first female case of clear cell sarcoma arising in the retroperitoneum with clinical features similar to those of malignant ovarian tumors. Aspects of clinical presentation, histopathologic evaluation, and treatment are described.

The natural history and histologic characteristics of a rare malignant neoplasm of tendons and aponeuroses are described, and a case is presented which originated in the right foot of a 19-year-old Caucasian woman who succumbed to the disease after 4 years. This case represents the first known example of metastasis to the oral cavity of this particular neoplasm.

Cutaneous clear cell sarcomas may be confused with melanomas as a result of overlapping histopathology and immunohistochemical staining. We report a case of a 41-year-old woman with a purported history of acral melanoma of the great toe. Twenty-one months after excision of the primary tumor, the patient developed a groin mass, diagnosed as metastatic melanoma on excision. Five months later, a biopsy of a lung mass was reported as metastatic melanoma. The patient was referred to our institution for treatment, which prompted molecular testing on the groin metastasis by targeted next-generation s

Clear cell sarcoma is a rare tumor with an unpredictable clinical course and usually poor prognosis. Almost all of the patients will have pulmonary metastases during the clinical course. Other more common metastatic sites are bones, lymph nodes, liver and spleen. Treatment of clear cell sarcoma is primarily surgical aiming to achieve uninvolved margins. Role of chemotherapy, radiotherapy and immunotherapy is controversial and remission of metastases has been reported rarely. In this study we report a case of clear cell sarcoma, with gastroduodenal and pulmonary metastases and remission of thes

A case of clear cell sarcoma (CCS) arising in the transverse colon is presented. The tumor consisted of sheets or small nests of epithelioid malignant cells possessing pleomorphic nuclei with one or more prominent nucleoli and ample clear or slightly eosinophilic cytoplasm. Some of the tumor cells contained various amounts of melanin pigments that were confirmed by histochemical and ultrastructural examinations. Immunohistochemical examination showed a positive immunoreactivity for HMB45 and S-100 protein. A metastatic nodule, which was found 9 months after surgery, showed similar histological

We report the case of a 67-year-old female with clear cell sarcoma (CCS) of the vulva. Grossly, the tumor was a partly exophytical vulvar mass, measuring 20 x 15 cm. At the time of presentation, the patient showed metastases to the lung, inguinal and pelvic lymph nodes. Histologically, the tumor consisted of oval or spindle cells with only mild nuclear pleomorphism and rare mitoses (up to 2/10 HPF). The cytoplasm was pale eosinophilic or clear. The tumor cells were arranged in confluent sheets. There were large areas of necrosis and surface ulceration. Immunohistochemically, the tumor cells sh

Peritoneal sarcomatosis was found in a 53-year-old male who had a history of resection of clear cell sarcoma (CCS) of the right wrist 7 years previously. Both the previous wrist tumor and the peritoneal disseminants consisted of small, spindle-shaped cells occasionally containing melanophages. Histologic features, histochemical demonstration of argentaffin granules, immunohistochemical reaction with HMB 45, and the demonstration of a chimeric transcript of EWS-ATF-1 established the diagnosis of CCS in the peritoneal tumors. As far as we are aware, this is the first case of a peritoneal sarcoma

Clear cell sarcoma (CCS) is a rare soft tissue neoplasm characterized by melanocytic differentiation and recurrent EWSR1 rearrangements. Owing to its overlapping morphology and immunophenotype with malignant melanoma (MM), distinction between these entities can be challenging, particularly in small biopsies or metastatic settings. Preferentially Expressed Antigen in Melanoma (PRAME) has emerged as a valuable marker in melanocytic pathology; however, its utility in CCS remains to be fully elucidated. We conducted a retrospective analysis of 26 CCS cases from 2 sarcoma referral centers, assessin

Clear cell sarcoma-like tumor of the gastrointestinal tract (CCSLGT) is a rare and relatively recently described entity. Here, we report a CCSLGT arising in the ileum of 28-year-old man. Histologically, the tumor composed of uniform, epithelioid or spindle cells that have eosinophilic cytoplasm and round, vesicular nucleus with single prominent nucleolus arranged in nests. No melanin pigment is identified. Lymph nodes and liver revealed metastasis. Immunohistochemically, tumor cells labeled with S100, SOX10, while negative for Cam 5.2, HMB-45, A103, CD117, DOG-1, SMA, desmin, chromogranin and

Clear cell sarcoma (CCS) typically presents as a tumor in the deep soft tissue of extremities, but when centered in the dermis it may be confused with a melanocytic nevus, primary nodular or metastatic melanoma. Compound variants of CCS, i.e. tumor cells present in both the epidermis and underlying soft tissue have not yet been described. Herein we report such a case, which initially presented as a nodule on the left wrist of a young woman at 19 years of age. The lesion was then interpreted as 'Spitz nevus, compound type'. Twelve years later the patient noticed an enlarged lymph node in the ri
