OBJECTIVES: A nerve agent sarin (isopropyl methyl phosphonofluoridate) was released in Matsumoto city, Japan, on 27 June 1994. About 600 people were affected by the sarin, including seven who died. Fifty two rescuers engaged in helping the victims and 18 were affected. The aim was to investigate how the rescuers were affected by sarin.
METHODS: Health examinations and a questionnaire survey were conducted with all rescuers.
RESULTS: A rescuer who was one of the first engaged and who worked for about five hours in areas contaminated with sarin was admitted to hospital after poisoning; the oth

On the day of the disaster, 641 victims were seen at St. Luke's International Hospital. Among those, five victims arrived with cardiopulmonary or respiratory arrest with marked miosis and extremely low serum cholinesterase values; two died and three recovered completely. In addition to these five critical patients, 106 patients, including four pregnant women, were hospitalized with symptoms of mild to moderate exposure. Other victims had only mild symptoms and were released after 6 hours of observation. Major signs and symptoms in victims were miosis, headache, dyspnea, nausea, ocular pain, bl

We developed a new assay method for fluoride anion (F(-)) a specific metabolite of sarin. Trimethyifluorosilane (TMFS) was derivatized from F(-) with trimethylsilanol, and TMFS was detected with a GC-flame ionization detector (FID) and capillary column system. The linear range, detection limit and recovery rate were 0.02-10 ppm, 0. 01 ppm and 97.3-103.0%, respectively. The patients were reported to be exposed only once to the toxic substance (a bolus exposure). F(-) excretion in urine of the hospitalized patients demonstrated three or four peaks. Other common metabolites of sarin and by-produc

Organophosphate (OP) poisoning, which inhibits cholinesterase activity, leads to severe cholinergic symptoms. Effective and quick management of these symptoms is considered critical to the clinical outcome. Acute renal damage following exposure to OP insecticides has been reported. Similar complications might occur following exposure to OP nerve agents, however, this subject has been studied only sporadically. In the present study, the effect of the nerve agent sarin on renal function was examined in rats. A single dose of sarin ( approximately 0.9 LD(50)) led to a significant reduction (of 45

Rats surviving various single dose of the organophosphorus anticholinesterase nerve agents Soman and Sarin were examined by light microscopy at intervals up to 35 days post-exposure. Brain lesions, identical to those that have been reported elsewhere were present, as well as a previously unreported finding associated with Soman or Sarin intoxication: half of all animals that had brain lesions also had areas of myocardial degeneration and necrosis. Depending upon the point in time at which cardiac tissues were examined, findings varied from areas of acute myolysis and necrosis to areas undergoi

A patient severely poisoned with sarin in the Sarin Attack in Matsumoto is described. A 19-year-old man was exposed to sarin at 23:00 on 27 June, 1994. At 1:00 of the following day, a rescue team found and brought him to the hospital. His blood pressure was 150-80mmHg and the heart rate was 120/min with frequent premature ventricular contractions (PVC). His respiration was shallow and copious salivation and excretion from the respiratory tract were observed. Consciousness disturbance, generalized convulsion, severe miosis and fasciculation of tongue, facial muscle and extremities were also mar

Forty-six patients who were exposed to sarin consulted our hospital because of darkness of vision, and ocular pain, vomiting, dyspnea and headaches on June 27 and 28, 1994. Eighteen patients were admitted and 4 of them were in the critical state. There were 6 features: 1) depression of plasma cholinesterase activity (17 of 18 patients, 94%), 2) hypokalemia (4/18, 22%), 3) depression of triglyceride (12/18, 67%), 4) hypocapnia (5/17, 29%), 5) partial pressure of oxygen (PaO2) <80 mmHg, or requirement of O2 inhalation (15/18, 83%), 6) white blood cells (WBC) >9,000 per mm3 (13/18, 72%). Adult baboons (Papio sp.; 8-12 kg) were anesthesized with sodium pentobarbital (20 mg/kg iv). The animals were instrumented for measurement of mean blood pressure (MBP), pulmonary artery pressure (PAP), ECG, arterial and mixed venous blood gases, lung volumes, lung pressures, and efferent phrenic nerve activity. Bronchoalveolar lavage (BAL) was performed. Studies were done prior to exposure, at intervals during the first 4 hr postexposure, and at 4 and 28 days after exposure. Control animals received a sham exposure to 2-propanol (N = 5). Soman (pinacolyl methylphosphonofluoridate) at 13.14 mi

Sarin (GB, O-isopropyl methylphosphonofluoridate) is a potent organophosphorus (OP) nerve agent that inhibits acetylcholinesterase (AChE) irreversibly. The subsequent build-up of acetylcholine (ACh) in the central nervous system (CNS) provokes seizures and, at sufficient doses, centrally-mediated respiratory arrest. Accumulation of ACh at peripheral autonomic synapses leads to peripheral signs of intoxication and overstimulation of the muscarinic and nicotinic receptors, which is described as "cholinergic crisis" (i.e. diarrhea, sweating, salivation, miosis, bronchoconstriction). Exposure to hi

Since plasma is generally employed for amino acid analysis, we compared amino acid levels in plasma with those in serum for healthy individuals and examined the influence of separation and storage conditions on the stability of the samples. Then, we determined the amino acid levels of frozen serum samples obtained from sarin poisoned patients. A. Comparison of Amino Acid Levels in Plasma and Those in Serum Blood was collected from 5 healthy individuals. Then, heparinated plasma and serum were separated by centrifugation immediately after blood collection. Serum was also separated by centrifuga

Organic phosphate pesticides were used worldwide after World War II and experiences on poisoning and treatment have been accumulated. An organic phosphate "nerve agent" Sarin was used in two terrorist attacks in Japan in the 1990s. Sarin effects on humans were well documented in these two incidents. Sarin gas inhalation caused instantaneous death by respiratory arrest in several victims in Matsumoto. Severely injured victims presenting with coma and generalized convulsion were resuscitated and recovered rapidly without sequelae. Miosis and blurred-dark vision, ocular pain, copious secretions f

Forensic chemical analysis aims not only to confirm the presence of an illicit chemical agent but also to determine its origin, synthesis route, and production conditions to bring evidence for a criminal investigation. In the production of organophosphorus-nerve agents and their precursors, phosphorous-containing species arise at different stages of synthesis and reflect specific reaction conditions. However, current methods for analysing methylphosphonic dichloride (DC) (precursor of sarin) impurities are often limited by the high reactivity and low concentrations of key phosphorus-containing

The possibility of mass terrorism has become increasingly apparent. Accurate and relevant teaching tools are needed for healthcare givers and emergency personnel of all experience. We describe one of these tools, BODY Simulation, and its use in training caregivers to respond to chemical terrorism. We have implemented two chemical agents--cyanide and sarin, the latter a nerve agent--in a detailed whole-body model and simulation. In the simulation, each agent was administered to a healthy young adult, first without therapy, then with therapy, for a total of four runs. We recorded several variabl

1. The central and peripheral anticholinergic activities of a series of drugs comprising atropine, hyoscine, caramiphen and one of its analogues, and three glycollic acid esters, have been measured.2. The ability of the same drugs used alone, and in conjunction with N-methyl pyridinium-2-aldoxime methanesulphonate (P2S), to protect mice, rats and guinea-pigs from the lethal effects of sarin has been assessed.3. No correlation was found to exist between central or peripheral anticholinergic activity and ability to protect from sarin.4. On the indirectly stimulated isolated rat phrenic nerve-dia

Two terrorist attacks with the nerve agent Sarin affected citizens in Matsumoto and Tokyo, Japan in 1994 and 1995, killing 19 and injuring more the 6000. Sarin, a very potent organophosphate nerve agent, inhibits acetylcholinesterase (AchE) activity within the central, peripheral, and autonomic nervous systems. Acute and long-term Sarin effects upon humans were well documented in these two events. Sarin gas inhalation caused instantaneous death by respiratory arrest in 4 victims in Matsumoto. In Tokyo, two died in station yards and another ten victims died in hospitals within a few hours to 3

Behavioral responses to atropine in rats exposed to the potent anticholinesterase agents Soman and Sarin were studied. Atropine itself produced limb-shakes and certain stereotyped activities, in a dose-dependent manner. The neurotoxicity elicited by Soman and Sarin was antagonized by atropine, while at the same time the responses induced by the latter were attenuated. In contrast, where rats were challenged with atropine 6-72 h after giving single doses of Soman or Sarin, the limb-shake myoclonus was markedly enhanced. The atropine-induced stereotypies were not, however, significantly affected

The neurotoxicity and lethality of Soman and Sarin, after single and repeated treatment at 50-60% of their LD50 doses in rats were investigated. Single treatment with Soman (100 micrograms/kg) and Sarin (120 micrograms/kg) produced severe tremors, convulsions and hypothermia, in some rats only, while the others did not show toxicity, i.e. an all or none effect. Soman and Sarin (100-120 micrograms/kg) caused, respectively, 89-93% and 26-48% inhibition of AChE at 6 hr and 56-68% and 17-39% inhibition at 24 h after single injections. Repeated treatment with Soman (90 micrograms/kg) and Sarin (100
