We assessed hemisphere function in right-handed male chronic schizophrenic patients using dichotic listening tests. We evaluated digit, tonic and transitional tests in patients with paranoid schizophrenia (n = 8), patients with disorganized schizophrenia (n = 8) and in control subjects (n = 8). The dichotic listening analysis discriminated between paranoid and disorganized schizophrenia. In disorganized schizophrenia, functional impairment of both hemispheres was demonstrated, while in paranoid schizophrenia dysfunction was more prominent in the right hemisphere. These results indicate the pos

Owing to unresolved questions concerning the efficacy and safety of electroconvulsive therapy (ECT) in the treatment of schizophrenia, and widespread negative attitudes toward ECT, maintenance ECT (mECT) is generally considered only as a last resort. Nevertheless, in some clinical situations, the advantages of mECT may outweigh the risks and associated concerns. We report the case of a patient suffering from disorganized schizophrenia who had life-threatening hematological side effects to treatment with antipsychotic agents. Long-term mECT was administered and the patient achieved remission wi

INTRODUCTION: The middle longitudinal fascicle (MdLF) is a long association fiber connecting the superior temporal gyrus (STG) and temporal pole with the angular gyrus through the white matter of the STG, structures which are known to play a crucial role in the pathology of schizophrenia. Functions of MdLF are thought to be related to language and thought processing in the left hemisphere, and with attention in the right hemisphere. While deficits of these functions are core clinical features of schizophrenia, no study has investigated the structural abnormalities of MdLF in schizophrenia.

UNLABELLED: Most patients with schizophrenia will have subsequent relapses of the disorder, with continuous impairments in functioning. However, evidence is lacking on how symptoms influence functioning at different phases of the disease. This study aims to investigate the relationship between symptom dimensions and functioning at different phases: acute exacerbation, nonremission and remission.
METHODS: Patients with schizophrenia were grouped into acutely ill (n=89), not remitted (n=89), and remitted (n=69). Three exploratory stepwise linear regression analyses were performed for each phase

The dopamine D2 receptor gene has been proposed as a genetic risk factor for schizophrenia (Arinami et al., 1994). However, a number of replications failed to confirm the initial report. The finding of a stronger association considering schizophrenics with the absence of negative symptoms (Arinami et al., 1996) suggested that the influence of DRD2 variants should be analyzed more at the level of symptoms rather than syndromes. One hundred and four inpatients affected by schizophrenia (n = 99) and delusional disorder (n = 5) (DSM IV) were assessed at admission by the Operational Criteria for Ps

OBJECTIVE: The aim of this study was to compare the differential response to amisulpride in patients with paranoid versus disorganized schizophrenia.
METHOD: We reanalyzed the original data from five different randomized drug trials comparing Brief Psychiatric Rating Scale (BPRS) scores in a database containing 427 paranoid and 296 disorganized patients with schizophrenia.
RESULTS: Both the disorganized and the paranoid group showed a substantial improvement of the BPRS total score within the first 4 weeks. In the paranoid group, mean (±SD) BPRS reduction was 16.9 (±14.6) (t = 24.06, df = 42

We describe the cases of 2 sisters with adult metachromatic leukodystrophy (MLD). Whereas one sister presented with disorganized schizophrenia-like symptoms as the initial manifestation of MLD, the other remained symptom free except for a 4-week period of postpartum depression. In both patients, there was some residual activity of leukocyte arylsulfatase A (1.7% and 5.5% of normal), and a marked increase in urinary sulfatides was present, as measured by tandem mass spectrometry. An arylsulfatase A pseudodeficiency was therefore excluded. The most common mutations of the adult phenotype, Ile-17

BACKGROUND: Schizophrenia is a chronic mental disorder with a many-faced clinical presentation. Obsessive-compulsive symptoms are often part of it. The characteristics of the clinical picture and the course of schizophrenia are factors related to both the resistance and the manifestation of obsessive-compulsive symptoms. Our study aims to establish the relationship between the peculiarities of the schizophrenia process and the influence of resistance on the expression of obsessive-compulsive symptoms.
METHODS: A study was conducted on 105 patients with schizophrenia. Of them, 39 are men and 6

Recent discoveries involving pyramidal neurons with two zones of input integration (apical and basal), the contextual information they provide, and the neural circuitry and neuromodulation systems in which they are embedded, can contribute to our understanding of disorganized symptoms in schizophrenia, including changes in their expression from the prodromal to chronic stages of the disorder. We demonstrate that the concepts and findings inherent to apical integration theory provide evidence in support of some of the seminal understandings of schizophrenia. In particular, considering the funct

OBJECTIVES: The significance of cognitive functioning deficits in schizophrenia is already well acknowledged. The relationships among lower and higher order cognitive skills and symptoms, as well as their relevance to holding a paid job are relatively less researched, though. The purpose of this study was to conduct an analysis concerning these issues.
METHODS: This study included 62 individuals with schizophrenia, 33 of them employed in paid sheltered jobs and 29 attending occupational therapy. They were assessed with tests of cognitive functions including communication skills. Psychopatholo

Background: Schizophrenia is a complex disorder characterized by positive symptoms (e.g., hallucinations), negative symptoms (e.g., social withdrawal), and disorganized symptoms (e.g., thought disorder). Alongside these, cognitive and depressive symptoms often emerge, with depressive symptoms sometimes dominating the clinical picture. Understanding the factors that influence the development of depressive symptoms in schizophrenia could clarify the dynamics between depressive and psychotic symptoms and guide clinical interventions. Methods: A total of 105 patients with schizophrenia (66 women,

OBJECTIVE: Psychosocial trauma during childhood is associated with schizophrenia vulnerability. The pattern of grey matter decrease is similar to brain alterations seen in schizophrenia. Our objective was to explore the links between childhood trauma, brain morphology and schizophrenia symptoms.
METHOD: Twenty-one patients with schizophrenia stabilized with atypical antipsychotic monotherapy and 30 healthy control subjects completed the study. Anatomical MRI images were analysed using optimized voxel-based morphometry (VBM). Childhood trauma was assessed with the Childhood Trauma Questionnair

BACKGROUND: Orexin A and B, a pair of hypothalamic neuropeptides also named hypocretin 1 and 2, play a role in the regulation of arousal, appetite, reward, attention, and cognition. Animal studies showed that antipsychotics can activate orexin neurons in a manner correlated with their weight gain liability. However, little is known about the role of orexin in patients with schizophrenia. This study aimed to investigate the correlation of plasma orexin level with clinical symptom profile, neurocognitive functioning and weight gain liability of the antipsychotics taken in patients with schizophr

Visual shape completion is a canonical perceptual organization process that integrates spatially distributed edge information into unified representations of objects. People with schizophrenia show difficulty in discriminating completed shapes, but the brain networks and functional connections underlying this perceptual difference remain poorly understood. Also unclear is whether brain network differences in schizophrenia occur in related illnesses or vary with illness features transdiagnostically. To address these topics, we scanned (functional magnetic resonance imaging, fMRI) people with sc

Working memory (WM) deficits are key features of schizophrenia and are associated with significant functional impairment. The precise mechanisms of WM and their relationship between WM deficits with other clinical symptoms of schizophrenia remain unclear. Contemporary models propose that WM requires synchronous activity across brain regions within a distributed network, including lateral prefrontal cortex (PFC) and task-relevant posterior sensory cortical regions. This suggests that WM deficits in patients may be due to PFC functional connectivity (FC) impairments rather than activation impair

OBJECTIVE: Executive dysfunction is a hallmark cognitive deficit in schizophrenia, yet phenotypic heterogeneity within the disorder complicates clinical translation. This study aimed to identify discrete executive profiles within a large Korean schizophrenia cohort using the Wisconsin Card Sorting Test (WCST) and to evaluate their clinical and functional correlates.
METHOD: We analyzed WCST performance in 276 individuals with schizophrenia and 361 healthy controls. A principal component analysis (PCA)-derived executive composite was validated against chart-reviewed clinical variables in a pat

BACKGROUND AND HYPOTHESIS: Deficit schizophrenia (DS) represents a distinct entity characterized by primary and enduring negative symptoms, yet the neurobiological differences between DS and non-DS (NDS) remain undetermined. Using a gradient-based approach, we hypothesize that DS and NDS will exhibit convergent and divergent brain functional hierarchy patterns, each with a specific underlying neurotransmitter architecture.
STUDY DESIGN: Resting-state functional magnetic resonance imaging images were acquired from 44 treatment-naive DS, 55 treatment-naive NDS, and 60 matched healthy controls (

OBJECTIVE: Although deficits in cognitive control are thought to contribute to the diverse cognitive and behavioral abnormalities in individuals with schizophrenia, the neural mechanisms underlying these deficits remain unclear. In this event-related functional magnetic resonance imaging (fMRI) study, the authors tested the hypothesis that during cognitive control tasks, impaired activation of the dorsolateral prefrontal cortex in schizophrenia patients is associated with disrupted coordinated activity between this prefrontal region and a distributed brain network that supports cognitive contr
