Schlemm's canal: the outflow 'vessel'.

In a healthy eye, the aqueous humour (AH) flows via the ciliary body and trabecular meshwork into the collector channels, which carry it to the episcleral veins. In glaucoma, a heterogeneous group of eye disorders affecting approximately 60 million individuals worldwide, the juxtacanalicular meshwork offers greater resistance to the outflow of the AH, leading to an increase in outflow resistance that gradually results in elevated intraocular pressure (IOP). The present review comprehensively covers the morphology of Schlemm's canal (SC) and AH pathways. The path of the AH from the anterior cha

Ab interno Schlemm's Canal Surgery.

In primary open-angle glaucoma, the site of greatest resistance to aqueous outflow is thought to be the trabecular meshwork (TM) and inner wall of Schlemm's canal. Augmentation of the conventional (trabecular) outflow pathway can facilitate physiologic outflow and subsequently lower intraocular pressure. The most recent approach to enhancing the conventional outflow pathway is via an internal approach to the TM and Schlemm's canal. Ab interno Schlemm's canal surgery includes 4 novel surgical approaches: (1) removal of the TM and inner wall of Schlemm's canal by an internal approach (ab interno

Schlemm's canal and primary open angle glaucoma: correlation between Schlemm's canal dimensions and outflow facility.

The purpose of the study was to examine the correlation between outflow facility and morphometric measurements of Schlemm's canal (SC) in normal and glaucomatous human eyes. Outflow facility was measured in ten normal and five glaucomatous (POAG) human eyes prior to perfusion-fixation at a pressure of 15 mmHg. Two separate sections per quadrant (eight sections per eye) were analysed using a morphometric analysis system. SC cross sectional area, perimeter, and inner wall length were measured. Inner wall endothelial nuclei were counted. When pooled data were examined, a statistically significant

Visual Function After Schlemm's Canal-Based MIGS.

Filtration surgery is highly effective in lowering intraocular pressure; however, it is associated with a higher risk of severe complications. Visual dysfunction may persist in relatively uneventful cases because of induced astigmatism or worsening optical aberrations. Therefore, for early- to moderate-stage glaucoma, an increasing number of surgeons are prioritizing surgical safety and preserving postoperative visual function by opting for minimally invasive glaucoma surgery (MIGS). Among the various MIGS techniques, canal-opening surgery-targeting aqueous outflow through the Schlemm's canal

Ageing of Schlemm's canal in nonglaucomatous subjects.

AIM: to study age-related Schlemm's canal endothelial changes and evaluate consequences on the filtration function. MATERIAL AND METHODS: the inner wall endothelium of Schlemm's canal was examined in 9 non-glaucomatous subjects aged between 32 and 75 years, by a combined technique of light and electron microscopy (scanning and transmission). Quantitative analysis included counts of bulges, pores, nuclei, giant vacuoles and other protruding structures, as well as measures of pores, giant vacuoles and Schlemm's canal size parameters (diameter and inner wall width). Outflow facility calculations

Flow of aqueous humor in the canal of Schlemm.

A mathematical model is presented for the flow of aqueous humor in Schlemm's canal in the eye. The model introduces a canal segment between two collector channels as a rectangular channel with porous upper wall. Two cases have been considered in the model: (I) the inner porous wall of the canal is rigid; (II) the inner wall is collapsible. Analytical solution of the governing equation in case I is straightforward, whereas the nonlinear equation in case II is solved by an iterative procedure. Aqueous fluid pressure and flow profiles in the proposed model are drawn, and the effects of important

Blood reflux in Schlemm's canal. I. Normal findings.

Blood may be refluxed into Schlemm's canal in the normal eye by occlusion of recipient veins in a sector of the conjunctiva and episclera. The blood makes the anterior margin of the canal distinct near the midportion of the trabecular mesh at the same place in which it is seen after hypotony. Circumferential flow in segments of the canal is possible when a pressure head exists to promote such flow. Evidence was found that the canal is not partially collapsed in the normal eye.

Schlemm's canal becomes smaller after successful filtration surgery.

OBJECTIVE: To determine whether filtration surgery causes secondary changes in the trabecular meshwork and Schlemm's canal. Successful filtration surgery allows most aqueous outflow to enter the filtration bleb, bypassing the meshwork and canal, and may result in underperfusion of these structures. METHODS: Eyes with primary open-angle glaucoma (POAG) that had undergone filtration surgery were studied and compared with eyes with POAG that had not undergone surgery. In addition, normal eyes and eyes with pseudoexfoliative glaucoma were studied for comparison. The trabecular meshwork and Schlem

Emulsified silicone oil droplets in the canal of Schlemm.

PURPOSE: To demonstrate the presence of emulsified silicone oil (SO) droplets in the canal of Schlemm with anterior segment optical coherence tomography (AS-OCT). METHODS: AS-OCT images from an eye that had undergone retinal detachment (RD) surgery three years ago were reviewed. At the time of surgery, SO had been used as the tamponading agent. RESULTS: The AS-OCT images demonstrated that minute emulsified SO droplets were present in the canal of Schlemm. CONCLUSION: Retention of SO in the eye for an extended period of time after RD surgery leads to secondary glaucoma. Presence of minute em

Circumferential Schlemm's Canal Surgery in Adult and Pediatric Glaucoma.

Purpose: Although trabeculectomy, and to a less extent glaucoma drainage devices are currently the gold standard in glaucoma surgery, there is a rising interest in addressing the more physiological pathway of aqueous outflow through surgeries that target Schlemm's canal (SC). These surgeries have the advantage of a lower risk of hypotony and bleb-related complications commonly associated with conventional glaucoma surgery. In this article we review new developments and the different techniques of circumferential angle surgery in adult and pediatric glaucoma, as well as procedures which target t

Altered mechanobiology of Schlemm's canal endothelial cells in glaucoma.

Increased flow resistance is responsible for the elevated intraocular pressure characteristic of glaucoma, but the cause of this resistance increase is not known. We tested the hypothesis that altered biomechanical behavior of Schlemm's canal (SC) cells contributes to this dysfunction. We used atomic force microscopy, optical magnetic twisting cytometry, and a unique cell perfusion apparatus to examine cultured endothelial cells isolated from the inner wall of SC of healthy and glaucomatous human eyes. Here we establish the existence of a reduced tendency for pore formation in the glaucomatous

Pathological Mechanism and Clinical Therapy Progress of Schlemm's Canal.

Schlemm's canal (SC) is a small circular canal in the deep part of the sclera at the junction of the sclera and cornea. As an integral component of the aqueous humor outflow, its structure and function are essential in regulating intraocular pressure (IOP). If SC develops lesions, the drainage of aqueous humor would be obstructed, leading to increased intraocular pressure and injury to the optic nerve. With the rapid development of minimally invasive glaucoma surgery, an increasing number of surgeons became familiar with SC, and the area generated substantial academic attention. The pathologic

Flap-sparing canaloplasty: a modified approach to Schlemm's canal.

PURPOSE: To present a modified surgical technique for canaloplasty with the Stegmann Canal Expander obviating the need for lamellar scleral dissection. METHODS: After limbal peritomy of the conjunctiva, the sclera is not dissected in the classic lamellar fashion with superficial and deep scleral flaps but is successively 'scratched' down by radial incision until Schlemm's canal (SC) is opened (trench cut). Following canal opening and viscodilation with a microcannula and sodium hyaluronidate, one Stegmann Canal Expander is implanted on either side of the canal ostia and the scleral incision i

Augmentation of the conventional (trabecular) outflow pathway would facilitate physiologic outflow and subsequently lower intraocular pressure. Ab interno Schlemm's canal surgery including two novel surgical modalities, Trabectome (trabeculotomy internal approach) and Trabecular Micro-bypass Stent (iStent), is designed to reduce intraocular pressure by this approach. Numerical simulation of the flow of aqueous humor in the Schlemm's canal.

Schlemm's canal (SC) is a ring structure responsible for maintaining fluid homeostasis in the anterior chamber of the eye by draining aqueous humor (AH) from the trabecular meshwork (TM) into the collecting channel (CC). Obstruction of the AH flow in this conventional pathway leads to an elevation of intraocular pressure in glaucoma, which is internationally recognized as the most common cause of blindness. Although there are obvious morphological differences between patients and healthy individuals, the AH drainage capacity of SC with specific parameters is difficult to quantitatively evaluat

Shear Stress in Schlemm's Canal as a Sensor of Intraocular Pressure.

Elevated intraocular pressure (IOP) narrows Schlemm's canal (SC), theoretically increasing luminal shear stress. Using engineered adenoviruses containing a functional fragment of the shear-responsive endothelial nitric oxide synthase (eNOS) promoter, we tested effects of shear stress and elevated flow rate on reporter expression in vitro and ex vivo. Cultured human umbilical vein endothelial cells (HUVECs) and SC cells were transduced with adenovirus containing eNOS promoter driving secreted alkaline phosphatase (SEAP) or green fluorescent protein (GFP) and subjected to shear stress. Localization of myocilin to the golgi apparatus in Schlemm's canal cells.

PURPOSE: Biochemical and genetic evidence suggests that overexpression of or mutations in myocilin within the cells of the aqueous humor outflow pathway play a significant role in the development of steroid-induced and several other open-angle glaucomas. As a baseline to understanding the normal and pathologic function of myocilin, we determined the subcellular localization of myocilin in steroid-treated human Schlemm's canal endothelial (SC) cells. METHODS: SC cells were grown to confluence, treated with dexamethasone for 10 days, and then stained using antibodies against myocilin, tubulin,

Microinvasive Glaucoma Surgery: A Review of Schlemm's Canal-Based Procedures.

Microinvasive glaucoma surgery has gained popularity over the past decade. It can be performed using three different mechanisms. In the present review, we focused on Schlemm's canal (SC)-based surgery, which increases aqueous humor (AH) outflow into the aqueous veins by either removal of the trabecular meshwork (TM) or an increase in the tension in the TM. In primary open-angle glaucoma (POAG), the TM is the most likely region for increased AH outflow resistance. Theoretically, removal of the TM can improve the AH outflow; hence, glaucoma specialists focus on microsurgical dissection of the TM
