Panic attacks, hypochondriasis, and agoraphobia: a self-psychology formulation.

The clinical and theoretical material presented points to the value of looking at the syndrome of panic attacks, hypochondriasis, and agoraphobia from a self-psychology perspective. From this vantage, the nature of the constellation of symptoms and its relationship to premorbid personality characteristics can be understood and a coherent view of the syndrome can be constructed. Phenomenologically and pathogenetically, panic attacks, hypochondriasis, and agoraphobia are related to states of self-fragmentation. Despite longstanding faulty anxiety-regulatory mechanisms, from which arises the vuln

Threatened Identity: Self Psychology Informed Communication for Serious Illness Care.

Serious illness threatens both the body and the self: the coherent, continuous sense of who one is, from bodily integrity to psychosocial identity. When illness disrupts identity and patients confront mortality and dependence, a range of responses may follow, including behaviors such as relentless demands, devaluation of clinicians, insistence on additional treatment, or rage at perceived care failures. These presentations are often labeled "narcissistic," yet such a formulation can foreclose clinical understanding. Heinz Kohut's self psychology offers a more generative framework: one that und

Why the self is empty. Toward a historically situated psychology.

This article presents a contextualized treatment of the current configuration of self, some of the pathologies that plague it, and the technologies that attempt to heal it. Of particular interest is the historical shift from the Victorian, sexually restricted self to the post-World War II empty self. The empty self is soothed and made cohesive by becoming "filled up" with food, consumer products, and celebrities. Its historical antecedents, economic constituents, and political consequences are the focus of this article. The two professions most responsible for healing the empty self, advertisi

The psychology of leading together: free will beliefs, self-regulation, and self-efficacy in shared leadership.

INTRODUCTION: In this study, a quantitative research model has been adopted and the possible relationships among shared leadership, perceived self-regulation, free choice, and self-efficacy have been examined. METHODS: Snowball sampling surveyed 283 public and private white-collar workers online. Partial Least Squares Structural Equation Modeling (PLS-SEM) tested the model in question. Our measurement approach has strong internal consistency, convergent validity, and discriminant validity for all variables. RESULTS: In the structural model, perceived self-regulation substantially predicts se

Psychological models of self-mutilation.

The descriptive models of self-mutilation fall into three broad categories. The psychodynamic formulation; the second category includes the anxiety reduction model, the hostility model, the behavioral learning model and the appeal model; the third social learning category includes the group-epidemic model and aspects of the violence and punishment model. The three models support the view that there is no single cause or motive responsible for self-mutilating behavior. Having a number of factors in mind allows for flexibility and enables clinicians to test particular hypotheses during managemen

Genital self-amputation-its psychological urge.

Genital self-mutilation (GSM) is a rare phenomenon that can occur in patients with severe mental health illness. This case report highlights a rare case of self-inflicted bilateral testicular amputation and partial penile amputation in a patient who is a transwoman with a psychiatric history. The patient initially presented to urology in extremis with bilateral testicular amputation. The patient was resuscitated but required emergency surgery in the form of bilateral inguinal approach to ligate the cord and control haemostasis. The testes were not re-implanted as the patient refused and, after

Self/other organization in human psychological development.

Evidence from the evolution of human cultural behavior and learning, embryology and genetics of the brain, and the behavior of human infants indicates that the critical and uniquely human motives for cooperative imagination and joint interest in objects and tasks are determined by expression of genes and epigenetic neural systems elaboration long before birth, along with essential peripheral organs of perception and motor expression that will serve in communication by rhythmic facial, vocal, gestural, and body movement signals. These cerebral motives continue to exercise their influence on neu

Adolescent sexual offenders: a self-psychological perspective.

Following a request for assistance in formulating a treatment philosophy for adolescent sexual offenders, a qualitative study of seven adolescent offenders was designed with a view to elaborating pre-offense, and post-offense dynamics. The point of departure was the hypothesis that sexual offending had relation to object relations. It was further hypothesized that offenders' object relations and self-development had been disfigured in childhood and adolescent development. The developmental theories of Mahler, Stern, Winnicott, and Kohut were reviewed in order to shed light on the connection be

Psychological aspects of self-care and home dialysis.

Self-care and home dialysis encourages the patient's active participation in his treatment which may enhance coping and adaptation to his illness. The psychological issues of dependence-independence, activity-passivity, and control and mastery are frequently referred to in the literature as being important in understanding the chronic stress of illness and treatment. The measure of successful coping should be determined by adaptive or maladaptive value of behavior rather than traditional concepts of normality or abnormality. Treatment planning and patient selection should be tailored to the ps

Is Anorexia Nervosa a Disorder of the Self? A Psychological Approach.

The debate concerning the pathogenesis and the maintaining factors of eating disorders, anorexia nervosa in particular, is ongoing especially since therapeutic interventions do not result in satisfactory and enduring rates of remission. This paper presents a model for the pathogenesis of eating disorders, based on the hypothesis of a deficiency in the development of the self. We present the theory in light of new evidence concerning the role of attachment insecurity in the development and maintenance of eating disorders. In particular, we define the self in eating disorders in a comprehensive

The Effects of a Pain Psychology and Neuroscience Self-Evaluation Internet Intervention: A Randomized Controlled Trial.

OBJECTIVES: Many patients' chronic musculoskeletal pain is strongly influenced by central nervous system processes such as sensitization or amplification. Education about pain neuroscience can change patients' beliefs but has less consistent effects on pain outcomes. Patients may have greater clinical benefits if the educational intervention is personalized, and they evaluate various psychosocial risk factors with respect to their pain. We developed and tested a brief, internet-based Pain Psychology and Neuroscience (PPN) self-evaluation intervention. MATERIALS AND METHODS: From a patient reg

The psychological mechanism of Self-objectification: the interaction between sociocultural pressures and the self-system.

Self-objectification involves adopting an observer's perspective on the body and prioritizing appearance over internal attributes, which is most common in women. We propose that self-objectification arises from the interplay between sociocultural pressures and the self-system rather than from sociocultural forces alone. In this account, self-concept clarity functions as a susceptibility factor that conditions the internalization of appearance norms; internalization and upward social comparison then increase body surveillance and appearance-focused negative affect (e.g., body shame, dissatisfac

Measuring self-esteem in context: the importance of stability of self-esteem in psychological functioning.

In this article, I report on a research program that has focused on the joint roles of stability and level of self-esteem in various aspects of psychological functioning. Stability of self-esteem refers to the magnitude of short-term fluctuations that people experience in their current, contextually based feelings of self-worth. In contrast, level of self-esteem refers to representations of people's general, or typical, feelings of self-worth. A considerable amount of research reveals that self-esteem stability has predictive value beyond the predictive value of self-esteem level. Moreover, co

Self-Knowledge Accuracy and Psychological Adjustment: A Systematic Review.

The question of whether self-knowledge is linked to psychological adjustment requires nuanced consideration. Relevant theoretical frameworks pertain to domains of self-knowledge (ability, personality, social perceptions), types of self-knowledge (normative vs. profile), forms of psychological adjustment (intrapersonal vs. interpersonal), or interactions among these factors. In this systematic review, we outline these theoretical frameworks, specify their predicted pattern of association between self-knowledge and psychological adjustment, and evaluate the evidence. The findings do not support

An integration of feminist and self-psychological approaches to bulimia nervosa.

Theoretical frameworks have guided approaches to treatment for the eating disorders. While unitary models were characteristic of earlier eras, more recent formulations have attempted to improve understanding by integrating various schools of thought (1). Several of these have placed cultural factors in the context of individual and familial predispositions. A weakness of these models has been the relative lack of understanding of the means by which these cultural values create mechanisms for expression of these disorders in specific women. In this manuscript I will explore the theoretical fram

Association between Self-Stigma and psychological distress in epilepsy.

INTRODUCTION: Self-stigma is increasingly recognised as an important psychosocial burden among people with epilepsy (PWE), particularly among those experiencing depressive and anxiety symptoms. This study examined the association between self-stigma, psychological symptoms, self-esteem, work motivation, and quality of life in PWE. METHODS: In this cross-sectional study, adults with epilepsy were assessed using the Neurological Disorders Depression Inventory for Epilepsy (NDDI-E), Generalised Anxiety Disorder-7 (GAD-7), Epilepsy Self-Stigma Scale (ESSS), Rosenberg Self-Esteem Scale (RSES), Wor

Designing for AI literacy: psychological mechanisms and self-determination.

INTRODUCTION: This study examined how instructional approach (skills-based instruction [SBI] versus thinking-based instruction [TBI]) and task type (structured tasks [STT] versus open-ended tasks [OET]) influence university students' AI literacy performance through autonomy, competence, and relatedness. METHODS: A randomized 2 × 2 between-subjects experiment was conducted with 320 university students assigned equally to four conditions. Participants received video-based instruction and subsequently completed an AIGC image-generation task. AI literacy performance was assessed using an objectiv
