The painful shoulder: shoulder impingement syndrome.

Rotator cuff disorders are considered to be among the most common causes of shoulder pain and disability encountered in both primary and secondary care. The general pathology of subacromial impingment generally relates to a chronic repetitive process in which the conjoint tendon of the rotator cuff undergoes repetitive compression and micro trauma as it passes under the coracoacromial arch. However acute traumatic injuries may also lead to this condition. Diagnosis remains a clinical one, however advances in imaging modalities have enabled clinicians to have an increased understanding of the p

Impingement Syndrome of the Shoulder.

BACKGROUND: Shoulder pain is the third most common musculoskeletal complaint in orthopedic practice. It is usually due to a defect of the rotator cuff and/or an impingement syndrome. METHODS: This review is based on pertinent literature retrieved by a selective search of the Medline database. RESULTS: Patients with shoulder impingement syndrome suffer from painful entrapment of soft tissue whenever they elevate the arm. The pathological mechanism is a structural narrowing in the subacromial space. A multiplicity of potential etiologies makes the diagnosis more difficult; it is established by

Shoulder impingement syndromes.

OBJECTIVE: Not all shoulder pain conditions are a consequence of rotator cuff injuries secondary to anterior subacromial impingement. Additional causative forms have been identified and classified as posterosuperior glenoid rim, subcoracoid and suprascapular nerve (at spinoglenoid notch) impingement syndromes. MATERIAL AND METHODS: We reviewed 206 consecutive magnetic resonance examinations carried out with conventional T1- and T2-weighted spin-echo and gradient-echo sequences in patients complaining of shoulder pain. Adjunctive sequences were acquired with the involved arm positioned in abdu

Shoulder impingement/instability overlap syndrome.

Impingement syndrome is one of the most common shoulder problems seen by the orthopedist. Multidirectional instability is less common but can present with impingement symptoms. Patients having impingement symptoms due to multidirectional instability have a syndrome called "impingement/instability overlap." The purpose of this study is to retrospectively review 50 consecutive patients with a condition we have coined "impingement/instability overlap" syndrome. We discuss the criteria, by history and by physical examination, for inclusion in this category and the results of treatment. Our study s

[Diagnosis of shoulder impingement syndrome].

This article reviews the pathogenesis and clinical and imaging findings in shoulder impingement syndrome. Shoulder impingement is caused by compression of the supraspinatus tendon underneath the coracoacromial arch, mainly in forward flexion of the arm. Different stages of impingement syndrome are described. Stage I relates to edema and hemorrhage of the supraspinatus tendon. Stage II is characterized by bursal inflammation and fibrosis, as well as tendinopathy. In stage III there is a tear of the rotator cuff. Clinical signs may overlap. Moreover, calcifying tendinitis, fractures and pain ori

Shoulder impingement syndrome: radiographic evaluation.

The impingement syndrome is an important source of shoulder pain that occurs when the bony and soft-tissue structures of the superior aspect of the shoulder encroach upon the coracoacromial ligamentous arch during abduction of the arm. Subacromial bursitis, bicipital tendinitis, and rotator cuff disruptions are common sequelae of this abnormality. The presence of bony excrescences arising from the anteroinferior aspect of the acromion and of flattening and sclerosis of the greater tuberosity of the humerus is an important plain radiographic sign of the impingement syndrome. Fluoroscopy, should

Impingement syndrome in the athletic shoulder.

The repetitive overhead use of the arm required by many sports may lead to impingement in the vulnerable avascular region of the supraspinatus and biceps tendons. The impingement syndrome may spill over at any time to involve the adjacent biceps tendon, subacromial bursa, and acromioclavicular joint and, as a continuum, with the passage of time, may eventuate in degeneration and partial, even complete thickness, rotator cuff tears later in life. Physical findings, particularly a positive impingement sign, confirm the diagnosis. In the young competing athlete, therapeutic measures consist of ca

Subacromial lipoma causing shoulder impingement syndrome.

Subacromial lipoma represents a rare cause of subacromial impingement syndrome (SIS). A 49-year-old male patient presented to clinic with progressive right shoulder pain and limited movement, ongoing for approximately 1 month. Magnetic resonance imaging (MRI) revealed a lesion, compatible with lipoma, extending through subacromial space and pressing on supraspinatus muscle. After histopathological verification of lipoma, mass was excised. Postoperatively, patient completed 1 month physical therapy and rehabilitation program. Patient was free of pain at 4-month follow-up. Subacromial lipoma sho

Scapular behavior in shoulder impingement syndrome.

OBJECTIVE: To quantify the contribution of each scapular rotation to the scapular total range of motion (ROM) in both shoulders of persons with a unilateral shoulder impingement syndrome (SIS), to compare 3-dimensional (3D) scapular attitudes of their symptomatic and asymptomatic shoulders in flexion and in abduction, and to characterize the scapular behavior of these subjects by classifying them into subgroups based on scapular tilting differences between their symptomatic and asymptomatic shoulders. DESIGN: Comparisons of 3D scapular attitudes, scapular total ROM, and percentage of contribu

Posture in people with shoulder impingement syndrome.

BACKGROUND: The posture of people with shoulder impingement syndrome (SIS) is a result of adaptive defensive posturing to decrease the intensity of pain in the affected joint. The aim of this work is to characterise trunk and shoulder girdle positioning in patients with SIS. MATERIAL AND METHOD: The study involved 58 patients treated for SIS in the years 2004-2006. Symptoms had been present for 40 months on average. A photogrammetric study was performed with the use of a MORA 4G system. It consisted in measuring lordosis and kyphosis, as well as the symmetry of some selected anthropometric po

Overuse and impingement syndromes of the shoulder in the athlete.

Overuse and impingement syndromes in the shoulders of athletes are predominantly caused by instability of the glenohumeral joint. Glenohumeral joint instability is usually acquired from repetitive overuse of the rotator cuff and shoulder girdle muscles, or injury of the static and dynamic stabilizers of the glenohumeral joint. Congenital hypermobility of the joint may also contribute to these syndromes in some individuals. The throwing action may lead to a cascade of injuries to the static and dynamic stabilizers of the posterosuperior glenohumeral joint, caused by the repetitive, high-energy

Relationship Between Shoulder Impingement Syndrome and Thoracic Posture.

BACKGROUND: Shoulder impingement syndrome (SIS) is the most common form of shoulder pain and a persistent musculoskeletal problem. Conservative and invasive treatments, aimed at the shoulder joint, have had limited success. Research suggests shoulder function is related to thoracic posture, but it is unknown whether thoracic posture is associated with SIS. OBJECTIVE: The objective of this study was to investigate whether there is a relationship between SIS and thoracic posture. DESIGN: This was a case control study. METHODS: Thoracic posture of 39 participants with SIS and 39 age-, gender-,

Kinesthetic sense of the shoulder in patients with impingement syndrome.

A proprioceptive deficit is an important determinant of disability in various shoulder disorders, such as instability and osteoarthrosis. In 15 patients with impingement syndrome stage II (Neer 1983), who were treated by arthroscopic subacromial decompression, we measured movement sense by determining threshold levels for the perception of motion of the shoulder. The patients were placed in a specially designed chair allowing continuous passive motion of the shoulder joint, while avoiding cutaneous, auditory and visual stimuli. To assess movement detection thresholds, passive abduction movemen

Anterior acromioplasty for treatment of the shoulder impingement syndrome.

Between 1975 and 1979, anterior acromioplasty was performed in 65 patients with 66 involved shoulders. The procedure appears to be safe and reasonably effective. Of the 65 patients in this study, 50 had chronic tendon inflammation with fibrosis, and 16 also had a small supraspinatus tendon tear. These 16 had rotator cuff repairs. Twenty-six patients were also treated with distal clavicle excision, and seven had tenodesis of the long head of the biceps brachii. The average age of the patients was 50 years (range, 23-75 years). All patients were followed for an average of eight years (range, thr

Functional assessment for shoulder impingement syndrome after anterior acromioplasty.

BACKGROUND: Impingement syndrome is a common cause of shoulder pain. Conservative treatment should always be initially considered. But for those patients with persistent symptoms, open acromioplasty is a reliable method to relieve pain and improve function. METHODS: From May 1989 to September 1994, a series of 36 anterior acromioplasties for impingement syndrome of the shoulder in 34 patients was retrospectively analyzed. Among them, rotator cuff tear was found in 25, and concomitant cuff repair was performed in 20. Five shoulders were not repaired because of a massive tear with marked cuff r

Subdeltoid lipoma causing shoulder impingement syndrome - a case report.

The impingement syndrome is defined by the compression of the rotator cuff tendons against the coracoacromial arch. Several factors contribute to this condition and they are classified as structural or functional factors. The former are changes in the coracoacromial arch, proximal humerus, bursa and rotator cuff, and the latter are related to the mechanism of the upper limb by means of synchronized activity and balanced between the rotator cuff and scapular girdle muscles. The authors report here a case of parosteal lipoma of the proximal humerus, located between the muscles deltoid, teres min

[Acromial osteophyte in shoulder impingement syndrome. Diagnosis and prevalence].

INTRODUCTION: The shoulder impingement syndrome (SIS) is caused by the compression of the rotator cuff tendon tear and the subacromial bursa between the humeral head and the structures that conforms the subacromial arc. OBJECTIVE: To determine a proportion of the acromial spur in the SIS comparing it with the general population. A second one would be to evaluate the quality of the radiological technique. MATERIAL AND METHODS: A comparative cross-sectional study was made in 148 patients, divided in two groups: one conformed by 52 persons, for patients with SIS and the other, conformed by 96,

Diagnostic value of clinical tests for shoulder impingement syndrome.

Results of three clinical tests for detecting shoulder impingement syndrome (Neer's, Hawkins', and Yocum's tests) and four tests for determining the location of the rotator cuff lesion (Jobe's test [supraspinatus], Patte's test [infraspinatus], lift-off test [subscapularis], and palm-up test [long head of the biceps brachii]) were compared to intraoperatively observed anatomic lesions in 55 consecutive patients who had surgery for Neer's syndrome. For Jobe's and Patte's tests, both pain (denoting tendinitis) and functional impairment (denoting tendon rupture) were evaluated. All clinical tests
