Shoulder arthroplasty has expanded in recent years with ongoing improvements in implant design and surgical techniques. Preoperative planning relies on radiographs, whereas computed tomography is preferred for bone assessment and MR imaging for soft tissues. Postoperative imaging assesses implant position and detects complications. Familiarity with expected imaging appearances and complications is essential.

Three types of shoulder prostheses are commonly used to cover the different indications of shoulder joint replacements: classical modular, inverse and cup. The modular prosthesis can replace the glenoid as well as the humeral head components. Exact visualization of the interface between the glenoid component and bone (or cement-bone interface) is necessary to detect osteolysis, which is relatively common. Therefore, it is mandatory to make true AP and axial projections. When describing the humeral component, the interface between bone and implant must again be considered. Additionally, the pos

PURPOSE: To analyse the morphology of the scapula relative to the reverse shoulder prosthesis.
METHODS: Scapulas of 46 women and 27 men aged 16 to 84 (mean, 53) years with proximal humeral fractures (n=52) or recurrent antero-inferior instability (n=21) were assessed using 3-dimensional computed tomography (CT). For comparison, 108 cadaveric scapulas with unknown epidemiology were assessed using a goniometer and a caliper. The length of the glenoid neck, the angle between the glenoid surface and the upper posterior column of the scapula, and the angles between the major craneocaudal glenoid a

Reverse shoulder prosthesis has become one of the most often used prosthetic implants in shoulder replacement surgery. It has a wide spectrum of indications, starting from comminuted humeral fractures and posttraumatic arthritis to arthritis caused by the rotator cuff loss. Its application at our hospital began in 2004, at first in few specific cases and with time in ever growing number of patients. Over 8 years, more than 250 reverse shoulder prostheses were implanted at our institution. In addition, our surgeons supervised its application in other hospitals all over Croatia. In the postopera

Periprosthetic fractures represent a severe complication after joint replacement. A case of comminuted displaced humeral fracture around the stem of a SMR reverse shoulder prosthesis is reported. The patient was a 81-year-old lady who had had a total shoulder replacement 28 months previously. The surgical solution consisted of a partial revision of the modular implant with conservation not only of the glenoid component but also of the prosthetic humeral body, which was well fixed in the humeral metaphysis. The humeral stem was removed and a long uncemented revision stem was implanted providing

Inferior scapular notching following reverse shoulder arthroplasty has been described using devices with a center of rotation located at the glenoid. Notching is typically seen within the first few months. To date, the only prosthesis without a reported case of scapular notching is the Encore Reverse Shoulder Prosthesis (RSP) (DJO Surgical, Austin, Texas). We present the first reported case of inferior scapular notching with the Encore RSP seen in a patient treated using an alternative glenosphere (36 minus 4) with a center of rotation 2 mm lateral to the glenoid. The glenosphere was implanted

BACKGROUND: The reverse shoulder prosthesis is a prosthesis that has been in clinical use in Europe since 1985 and was approved for use in the United States in 2004. This unique prosthesis has a baseplate attached to the glenoid, which holds a spherical component, while the humeral component includes a polyethylene insert that is flat. This design is the "reverse" configuration of that seen with a conventional arthroplasty, in which the spherical component is part of the humeral component. The indications for the reverse prosthesis are: (1) painful arthritis associated with irreparable rotator

BACKGROUND: Reverse shoulder arthroplasty has become the standard treatment for cuff tear arthropathy and complex fractures of the proximal humerus. The Delta Xtend prosthesis (DePuy Synthes, Warsaw, IN, USA) was launched in 2006 and has shown good short-term results. Longer-term results are not yet available.
METHODS: There were 126 primary Delta Xtend prostheses implanted in our center by 1 surgeon from October 2006 until December 2009. Of these, 38 patients died, 12 were lost to follow-up, and 2 needed early revision of the prosthesis. Follow-up of at least 8 years was available for 74 pat

BACKGROUND: This study evaluated the timing and location of radiologic bone adaptations related to shoulder arthroplasty using a single type of cementless short-stem shoulder prosthesis.
METHODS: Uncemented short-stem shoulder arthroplasties were evaluated in 52 patients at a mean age of 71.6 years (range, 58.1-86.6) with a minimum clinical and radiologic follow-up of 2 years (mean, 32 months; range, 23-52 months). All radiographs were analyzed for inclination of the stem, filling ratio of metaphysis and diaphysis, bone remodeling around the stem, radiolucent lines around the glenoid, and sub

We determined the outcome of 113 total shoulder replacement arthroplasties performed with a Neer prosthesis between 1975 and 1981. The operations were performed for the treatment of osteoarthritis, rheumatoid arthritis, and old fractures or dislocations with traumatic arthritis. The probability of implant survival was 93% after 10 years and 87% after 15 years. Complications requiring reoperation developed in 14 shoulders. Seventy-nine patients with 89 replacements were available for follow-up a minimum of 5 years after the operation (mean 12.2 years, range 5 to 17 years). Relief from moderate

BACKGROUND: Patients with disabling pain and loss of shoulder function with associated proximal humeral bone loss following shoulder arthroplasty have limited reliable treatment options. Our objective was to report the results, obtained as part of a prospective outcomes study, of the use of a reverse shoulder prosthesis-allograft composite in these patients.
METHODS: Between 2002 and 2005, 353 patients treated with a reverse shoulder prosthesis were enrolled in a prospective cohort study. Twenty-five patients received, in addition, a proximal humeral allograft for the management of severe pro

INTRODUCTION: Periprosthetic shoulder infections (PSIs) represent a serious complication following shoulder arthroplasty. No consensus exists regarding the optimal option. We conducted a retrospective case-control study to compare the outcomes of 2-stage revision shoulder arthroplasty and those of definitive articulating antibiotic spacer implantation with regards to eradication of the infection, improvement of pain and shoulder function.
MATERIALS AND METHODS: Thirty patients treated for an infected shoulder arthroplasty were retrospectively reviewed after a mean follow-up of 8 years (range,

The authors report the 2-year results of an uncemented resurfacing shoulder prosthesis in 47 patients with primary glenohumeral osteoarthritis who underwent a cementless humeral resurfacing arthroplasty between 2007 and 2009. Constant scores (corrected for sex and age), shoulder function, visual analog pain scales, Dutch Simple Shoulder Tests, and physical SF-12 scores improved significantly (P<.05) from preoperatively to 2 years postoperatively. Mental SF-12 scores remained the same. Complications included 1 traumatic lesser tuberosity avulsion fracture, 1 intra-articular loose body due to a

We report a rare case of reverse shoulder prosthesis implantation in a patient retrospectively showing a chronic tuberculous osteoarthritis and meningitis. In this atypical presentation, the patient presented only a chronic shoulder pain, with no clinical or imaging sign of tuberculosis. After the implantation of a reverse shoulder prosthesis, the patient presented a reactivation of a tuberculosis causing a meningitis. The diagnosis of shoulder osteoarthritis caused by M. tuberculosis was confirmed retrospectively on deep perioperative samples by standard culture, PCR, and positive pathology.

BACKGROUND: Implant registries have proved valuable in assessing the outcomes of arthroplasty procedures. Moreover, by identifying lesser quality implants they have indirectly improved the quality of care. The registry of prosthetic shoulder implants was established in 2008.
METHODS: It records information on all types of primary and revision arthroplasty procedures involving the glenohumeral joint, including reverse and total arthroplasty, hemiarthroplasty, resurfacing, removal, and any other surgical procedures that are required to manage these patients. The collected data include patient d

We present a case of Nocardia farcinica after placement of reverse shoulder prosthesis in a 73-year-old woman. One month after surgery, the patient was admitted to the hospital with a spontaneous drainage of the wound and complaints of aggravating pain in the operated shoulder. There was no history of an immunosuppressive disease or therapy. After cultivation and empiric therapy with flucloxacillin, Nocardia farcinica was found and treated with a combination of intravenous amikacin and ceftriaxone. Eight days after drainage, a rinse and replacement of the polyethylene cup and glenosphere was e

PURPOSE OF THE STUDY: Treatment of primary bone tumors of the proximal humerus is a major challenge, both in terms of oncological cure and limitation of functional handicap. Extracorporal radiation and reimplantation have provided satisfactory results for the treatment of bone tumors and the inverted shoulder prosthesis has be found to be effective in treating rotator cuff deficiency. We therefore assessed patients treated with a combination of these two treatments in order to search for a better functional outcome.
MATERIAL AND METHODS: Conservative resection of the tumor with reconstruction
