The control of colorectal cancer has become increasingly important because of its high incidence in the United States of more than 115,000 cases annually. Although 50% of neoplastic lesions of the colon arise in the distal 25 cm of the bowel, endoscopic examination of this area has not been generally accepted. Recent studies with flexible sigmoidoscopes as substitutes for the rigid proctosigmoidoscope, primarily by endoscopists, have shown an increased yield of neoplastic lesions, with excellent patient acceptance. Twenty-six patients were treated for a volvulus of the sigmoid colon between January, 1971, and June, 1979. Eight patients had an emergency resection and, although there was a high postoperative morbidity, there were no deaths in this group. An attempt to decompress the volvulus by means of a rigid sigmoidoscope was made in 18 patients. This was successful in 14 patients. Six of these patients suffered a recurrence, and perforation occurred in four patients, three of whom subsequently died. Because of the high risk of perforation, care should be taken with this procedure.

Forty-two patients underwent flexible sigmoidoscopy in the radiology department. After maximal insertion of the sigmoidoscope, a supine abdominal film documented the position of the tip of the instrument. Correlation with barium enema in all patients determined the extent of visualization of the colon relative to the length of insertion of the sigmoidoscope. Insertion to 60 cm was achieved in 31 (74%), 40-59 cm in six (14%), 25-39 cm in three (7%), and under 25 cm in two (5%), with a mean depth of insertion of 54 cm. Full insertion examined the entire sigmoid colon in 14 (45%) of 31 patients.

Proctosigmoidoscopic examinations were performed on 363 patients who had gastrointestinal but no colonic symptoms. Thirty-four, all over the age of 40, were found to have polypoid lesions, 24 of which were adenomatous. Air-contrast barium enemas were utilized to rule out higher lesions in the patients with one or more adenomatous polyps. One early carcinoma of the ascending colon was so discovered. The cost of finding an adenomatous polyp in a patient without colonic symptoms was calculated to be $523.75 and of a carcinoma $12,570.

The aims of the study were to determine what microscopic changes occur in the rectal mucosa of men who have had anal intercourse and to correlate the sigmoidoscopic and microscopic appearances. Emphasis is placed on the importance of early detection of colorectal cancer by the primary care physician using the 60 cm flexible sigmoidoscope. A training format for this procedure is proposed. Patients who had benign polyps and cancers were studied retrospectively to define whether use of the 60-cm flexible sigmoidoscope would markedly improve the diagnostic yield over the 25-cm rigid sigmoidoscope. Six hundred thirty-two patients were referred to the Colorectal Clinic from February 1983 to February 1986 for screening with the Pentax 65 cm flexible sigmoidoscope. Use of flexible endoscopes affords biopsy sampling of polypoid lesions of the colon, especially within the rectum and sigmoid colon by flexible fiberoptic sigmoidoscopes. Histologic examination of colonic biopsies usually allows division of polyps into neoplastic and non-neoplastic categories.
