First-degree sinoatrial heart block: sinoatrial block in the sick-sinus syndrome.

Sinoatrial conduction time (SACT) was estimated from the delay in the atrial recovery period after premature depolarization applied in that portion of atrial diastole when increasing prematurity resulted in a constant recovery interval. In 20 normal patients SACT was 169 msec. +/- 91 (2 S.D.). At least nine of 19 patients with "sick-sinus syndrome" (SSS) demonstrated SACT that were longer than seen in these normal subjects. SACT was prolonged in seven of nine SSS patients with abnormal A-V nodal conduction. Among 10 SSS patients with normal A-V conduction, only two had prolonged SACT. This stu

Amiodarone-induced sinoatrial block.

We observed sinoatrial block due to chronic amiodarone administration in a 5-year-old boy with primary cardiomyopathy, Wolff-Parkinson-White syndrome and supraventricular tachycardia. Reduction in the dosage of amiodarone resulted in the disappearance of the sinoatrial block and the persistence of asymptomatic sinus bradycardia.

Sinoatrial block complicating legionnaire's disease.

A 59 year old woman presented with acute onset of fever, chills, diaphoresis, vague chest discomfort, and was found to be hypotensive and tachypnoeic. An electrocardiogram demonstrated sinoatrial block with a junctional rhythm between 50 and 80 beats/min. All cultures were negative and imaging studies unrevealing. Her urine tested positive for Legionella pneumophila antigen serotype 1 and she improved with antibiotic therapy.

Sinoatrial block during lithium treatment.

Lithium is known to produce T wave changes in the ECG, whereas effect upon the conducting system of the heart has not been described. In our patient lithium produced sinoauricular block and possibly tachycardia. The correlation between lithium treatment and tachyarrhythmias is discussed. When during lithium treatment block is found, the treatment should preferably be stopped, but if this is not possible it is important to realize that digitalis should not be used as a prophylactic drug against tachycardia, as it worsens the block and therefore increases the frequency of arrhythmia, even leadin

Exercise-Induced Complete Heart Block and Sinoatrial Exit Block in Baseline Bifascicular Block.

BACKGROUND Bifascicular block is typically asymptomatic and incidentally diagnosed, yet it may progress to advanced conduction disturbances. In patients with unexplained syncope and conduction abnormalities, exercise stress testing can diagnose transient conduction disease. While not routinely indicated for known atrioventricular block, stress testing may clarify etiology in patients without resting high-grade block. We present a case of syncope in a patient with bifascicular block, in whom exercise stress testing revealed complete atrioventricular block and sinoatrial exit block. CASE REPORT

Apparent bradycardia-dependent sinoatrial block associated with respiration.

In our previous patients, apparent bradycardia-dependent block has been shown in the atrioventricular (AV) junction and in the accessory pathway. It was suggested that these previous cases were not of true bradycardia-dependent block; namely, that, as a result of periodic increases in vagal tone associated with respiration, conductivity in the AV junction or in the accessory pathway was depressed to a greater degree than automaticity in the sinus node. In the present article, 3 patients with frequent sinoatrial (SA) block were reported. In 1 patient, sinus escape-capture bigeminy caused by SA

Sinoatrial conduction time in patients with atrioventricular block.

In 20 patients with atrioventricular (A-V) block, sinoatrial conduction time (SACT) was measured in order to established to incidence of sinoatrial conduction defects. 10 patients had complete A-V block, 5 patients second-degree A-V block type 1, 2 patients second-degree A-V block type II, nd 3 patients first-degree A-V block. Prolonged bidirectional SACT was found in 8 cases (40%). The mean value (+/- SD) of SACT was 206 +/- 69 ms. In comparison with SACT in a group of 11 patients without symptoms and signs of sinus node disorder (mean value 156 +/- 25 ms) the difference was significant (p le

[Functional state of the sinoatrial node in patients with intraventricular block].

The function of sinus node (SN) was studied in 101 subjects with intraventricular blocks (IVB) and 30 controls. The patients with IVB showed a significant increase in the corrected time of SN function recovery and that of sinoatrial conduction both before and after medicamentous++ autonomic block, as compared to the controls. In general, signs of SN dysfunction were detected in 47 (46.5%) subjects with IVB. In 18.8% of the examinees, SN dysfunction was due to abnormal autonomic exposures, 27.7% had abnormalities in SN proper or perinodal tissue. ++Tachy-bradycardia was found in 53.5% patients

Lyme carditis manifesting with sinoatrial exit block: a case report.

BACKGROUND: Lyme disease is the most common vector-borne disease in North America and Europe. Infection with the spirochete Borrelia burgdorferi complex can involve cardiac tissue causing Lyme carditis (LC). Due to the infection of conductive tissue, LC typically presents with varying degrees of atrioventricular conduction block. Here, we provide the first evidence that conductive tissue of the sinus node can be involved in LC resulting in higher degree sinoatrial (SA) block with concomitant syncope. CASE SUMMARY: We report the case of an otherwise healthy 31-year-old female presenting with L

[Recognition of first-degree sinoatrial block in the standard electrocardiogram].

This report concerns a patient affected by Sick Sinus Syndrome, whose ECG showed many episodes of second degree sino-atrial (SA) block. The long pause was always less than twice the normal PP interval, but the diagnostic criteria for Wenckebach SA block were not met. The atrial cycle, indeed, did not shorten progressively before the block. Furthermore even Schamroth's diagram, obtained by plotting the length of each PP cycle against the length of the following cycle, was not suggestive of Wenckebach SA block. In type 2 (Mobitz) second degree SA block, the long pause may appear slightly shorten

Complete sinoatrial block in two patients with bradycardia-tachycardia syndrome.

Electrophysiologic studies with recordings of sinus node electrograms were performed in two patients with bradycardia-tachycardia syndrome. In both patients, the rest electrocardiogram showed apparent sinus bradycardia. Patient 1 had frequent paroxysms of atrial tachycardia with long pauses of up to 10 seconds; Patient 2 had paroxysmal atrial flutter and atrial pauses of up to 8 seconds. Multiple, repetitive, low frequency deflections, with a cycle length ranging from 730 to 960 ms in Case 1 and 570 to 750 ms in Case 2, suggestive of sinus node electrograms, were recorded at a critical area at

Alternating sinus rhythm and intermittent sinoatrial block induced by propranolol.

Alternating sinus rhythm and intermittent sinoatrial (S-A) block was observed in a 57-year-old woman, under treatment for angina with 80 mg propranolol daily. The electrocardiogram showed alternation of long and short P-P intervals and occasional pauses. These pauses were always preceded by the short P-P intervals and were usually followed by one or two P-P intervals of 0.92-0.95 s representing the basic sinus cycle. Following these basic sinus cycles, alternating rhythm started with the longer P-P interval. The long P-P intervals ranged between 1.04-1.12 s and the short P-P intervals between

Prolonged sinoatrial block in an infant with respiratory syncytial viral bronchiolitis.

Complete heart block in children admitted to the pediatric intensive care unit with respiratory syncytial viral (RSV) infections has been described. This report describes a prolonged sinoatrial block exceeding 4 s in an infant with RSV, which, to the authors' knowledge, is the longest such event described in the published literature. This block was followed by shorter episodes within the next 24 h. An extensive workup showed no other known cause of bradycardia or sinoatrial block. The infant was discharged home with 48 h Holter monitoring, which was normal. At this writing, the infant has rema

Atrioventricular Wenckebach point and progression to atrioventricular block in sinoatrial disease.

The value of measurement of the atrioventricular (AV) Wenckebach point at rest as a predictor of progression to AV block was investigated prospectively. Twenty-four patients with sinoatrial disease without evidence of conduction disturbance on 12-lead ECG or 24-hour ambulatory monitoring were paced with Medtronic Activitrax II, Medtronic Legend, or Telectronics Meta MV systems in AAI or AAIR modes. Patients were monitored for symptoms and evidence of AV block on 24-hour tapes. The mean age of the patients was 67 years (range: 42-88). There were 11 males and 13 females. The mean follow-up time

The development of sinoatrial dysfunction in pacemaker patients with isolated atrioventricular block.

The purpose of this paper is the assessment of sinus node competence over time in patients with isolated atrioventricular block (AV block). Patients implanted with AV synchronous pacemakers for isolated AV block between December 1993 and June 1995 were prospectively evaluated at predischarge, 6 weeks, and subsequent 6 months follow-up with respect to atrial rate monitors/24-hour Holter and modified exercise test. Patients unable to maintain AV synchronous pacing or complete a modified exercise test were excluded. Sinus node competency is interpreted as: (1) absence of atrial brady- or tachyarr

[Danger of sinoatrial block and the use of antiarrhythmic agents in myocardial infarcts].

Nine cases of major ventricular arrhythmia (tachycardia (VT), fibrillation (VF), torsades de pointe) are reported in patients with sequellae of myocardial infarction but without residual angina or cardiac failure. --Six of these disturbances of excitability occurred after a bradycardia due to sino atrial block (SAB) which favoured the breakthrough of abnormal automatic foci. This form of the bradycardia-tachycardia syndrome was demonstrated by endocavitary electrophysiological exploration.. These were the only cases of major ventricular arrhythmia observed in a series of 88 SABs. Reputedly ben

Transient Sinoatrial and Atrioventricular Block in the Acute Phase of COVID-19 Infection.

BACKGROUND: Ongoing research on Coronavirus Disease 2019 (COVID-19) infection has revealed that it is associated with serious damage to many organs, not only the lungs. This infection can also affect the cardiovascular system and lead to serious cardiac pathologies. In this article, we present a case of transient bradyarrhythmia in a patient who was diagnosed with COVID-19 and recovered spontaneously with treatment. CASE REPORT: A 74-year-old male patient was hospitalized with symptomatic COVID-19 infection. At the time of hospitalization, the patient was asymptomatic from a cardiac standpoin

Sinoatrial and Atrioventricular Blocks: Who First Described Them and How? [Retrospectroscope].

The relationship among cardiac pacemakers is characterized by the fact that one pacemaker is usually dominant and all the others are subsidiary. The sinoatrial node acts as the dominant pacemaker, and all other potential pacemaker tissues are discharged by a conducted impulse before their respective diastolic depolarizations attain threshold. These pacemakers are called subsidiary to emphasize the fact that, under normal circumstances, they are engaged in conducting impulses, but, under abnormal circumstances, they may become actual pacemakers.
