Distinguishing dural sinus thrombosis from benign intracranial hypertension.

Dural sinus thrombosis (DST) is a life threatening illness and is often overlooked but it must be considered in the differential diagnosis of a patient presenting with a significant headache. DST presents similarly to benign intracranial hypertension (BIH) with intracranial hypertension and headache. A case of a 23 year old woman with DST is described that was initially diagnosed as BIH. A Medline search to determine the examination of a patient with suspected BIH or DST yielded several articles on this topic but only two brief case reports in the emergency medicine literature. Any patient wit

Intracranial venous sinus thrombosis complicating AIDS-associated nephropathy.

An alert and oriented 27-year-old African American woman with AIDS presented with a 10-day history of fever, cough productive of yellow sputum, nausea, and vomiting and a 1-day history of excruciating headache and photophobia. Her condition rapidly deteriorated into a coma with decorticate and then decerebrate posture, and she died 3 weeks later. There was evidence of extensive intracranial venous sinus thrombosis (ICVST), renal vein thrombosis (RVT), and multiple cerebral hemorrhagic infarcts due to a hypercoagulable state complicating AIDS-associated nephrotic syndrome. This is the first rep

Open thrombectomy combined with thrombolysis in massive intracranial sinus thrombosis.

A case of complete thrombosis of the superior sagittal sinus in a pregnant women with multiple haemorrhages and hydrocephalus is described. Due to acute onset and progressive neurological deterioration, the decision was taken to remove the clot surgically. A sinotomy was performed removing the thrombotic material followed by local infusion of tissue plasminogen activator (t-PA). The combination of surgical removal and thrombolysis was life-saving for this young patient.

Complicated cerebral venous sinus thrombosis with intracranial hemorrhage and mastoiditis.

Cerebral venous sinus thrombosis (CVST) is a rare cause of stroke, occurring when a blood clot forms in any of the brain venous sinuses. Symptoms include neurological deficits, headache, seizures, and coma. There are many predisposing factors for CVST including prothrombotic conditions, oral contraceptives, pregnancy/puerperium, malignancy, infection, and head injury. Cerebral venous sinus thrombosis has no identifiable underlying etiology in about 12.5% to 33% of the cases. Diagnosis has become easier with newer imaging techniques, such as magnetic resonance venography. The treatment options

Benign intracranial hypertension, monoclonal gammopathy, and superior sagittal sinus thrombosis.

We report a case of benign intracranial hypertension associated with monoclonal gammopathy. The course was complicated by superior sagittal sinus thrombosis which we believe to be related to acetazolamide therapy. Response to volume expansion with colloid and crystalloid solutions was dramatic.

Intracranial Venous Sinus Thrombosis and Peripheral Venous Thrombosis Following Traumatic Brain Injury and Surgery.

Cerebral venous sinus thrombosis (CVST) is an uncommon cause of stroke. This case report describes a 47-year-old man who developed concurrent sigmoid sinus thrombosis and bilateral cephalic vein thrombosis following traumatic brain injury and surgical evacuation of an epidural hematoma. The patient presented with a new headache on postoperative day 5. Diagnosis was confirmed by CT angiography and Doppler ultrasound. He was successfully treated with subcutaneous enoxaparin, resulting in symptom resolution and radiologic improvement. This case highlights that CVST can co-occur with peripheral th

Management of a decompensated acute-on-chronic intracranial venous sinus thrombosis.

A 34-year-old female patient presented during the 10th week of her second gravidity with headache, nausea and vomiting 2 weeks before admission. Her medical history was remarkable for a heterozygous factor V Leiden mutation, elevated lipoprotein A, and a cerebral venous thrombosis (CVT) after oral contraceptive intake 15 years before. Magnetic resonance imaging (MRI) suggested acute and massive intracranial sinus thrombosis. Despite full-dose anticoagulation, the patient deteriorated clinically and eventually became comatose. Now, MRI/magnetic resonance angiography revealed vasogenic edema of

Central Venous Sinus Thrombosis with Spontaneous Intracranial Hypotension: A Case Report.

Spontaneous intracranial hypotension (SIH) is caused by leakage of cerebrospinal fluid (CSF) within the spinal column, and it is a rare risk factor for cerebral venous sinus thrombosis (CVST), which is a life-threatening condition. We present a case of SIH with further developed CVST causing intracranial hypertension. Discussions were made on the mechanisms of SIH, leading to CVST and the presentations and treatment of SIH. Clinicians should keep in mind the coexistence of CVST and SIH. Patients with CVST should be closely observed for any change in the headache pattern such as orthostatic hea

Geographically Remote Cerebral Venous Sinus Thrombosis in Patients with Intracranial Tumors.

Cerebral venous sinus thrombosis (CVST) related to intracranial tumors has most commonly been recognized as an operative complication related to local operative factors such as retraction or direct venous injury. CVST may also be caused by tumor-related factors such as local mass effect but rarely occurs geographically remote from the site of the tumor. We report 6 cases treated at our institution of intracranial supratentorial tumors associated with CVST. In each case, the CVST was remote from the surgical site. In 3 cases CVST was noted at the time of resection, and 3 cases occurred in a del

Differential diagnosis of patients with intracranial sinus venous thrombosis related isolated intracranial hypertension from those with idiopathic intracranial hypertension.

In patients presenting with intracranial hypertension without hydrocephalus, mass lesions, and with normal cerebrospinal fluid (CSF) composition (pseudotumor cerebri syndrome), the diagnosis of intracranial sinus venous thrombosis (ISVT) has crucial etiological, therapeutic and prognostic implications. Utilizing two well-defined groups of pseudotumor cerebri patients, one with magnetic resonance imaging (MRI) or angiography confirmed ISVT (17 patients) and the other in whom ISVT has been excluded (idiopathic intracranial hypertension [IIH], 27 patients), we investigated the characteristics tha

Treatment of sagittal sinus thrombosis associated with cerebral hemorrhage and intracranial hypertension.

Two cases of complete sagittal sinus occlusion with multiple brain hemorrhages, elevated intracranial pressure, and disseminated intravascular coagulation are described. These patients were successfully managed using pentobarbital-induced coma to ameliorate intracranial pressure elevation. This therapy was combined with monitoring of intracranial pressure and intermittent drainage of cerebrospinal fluid to further control intracranial pressure elevations. Thrombus and coagulopathy resolved with pentobarbital alone in one patient and after pentobarbital plus heparin therapy in the second patien

Endovascular Treatment of Cerebral Venous Sinus Thrombosis and Insights into Intracranial Coagulopathy.

Cerebral venous sinus thrombosis (CVST) requires anticoagulation to promote vessel recanalization. Current anticoagulation paradigms utilize plasma tests from peripheral venous/arterial samples for therapeutic monitoring. We describe a medically-refractory case of CVST in a 35-year-old woman later found to have JAK2 mutation and essential thrombocytosis. Despite therapeutic anticoagulation levels, worsening cerebral edema and progression to coma prompted endovascular treatment. Failed endovascular thrombectomy attempts led to placement of 2 separate indwelling microcatheters for continuous inf

Idiopathic Intracranial Hypertension Masquerading as Cerebral Venous Sinus Thrombosis: A Diagnostic Challenge.

Idiopathic intracranial hypertension (IIH) is a syndrome identified by raised intracranial pressure (ICP), with absence of a mass lesion/hydrocephalus, typically affecting obese women of childbearing age.1 It may present with similar symptoms such as headache, visual disturbances, and papilledema, making clinical differentiation challenging.1 IIH primarily threatens vision, and treatment focuses on lowering ICP using medications like acetazolamide, serial lumbar punctures, or surgical interventions such as optic nerve sheath fenestration or CSF shunting in refractory cases.2 Misdiagnosing IIH

Thrombocytopenia and Intracranial Venous Sinus Thrombosis after "COVID-19 Vaccine AstraZeneca" Exposure.

BACKGROUND: As of 8 April 2021, a total of 2.9 million people have died with or from the coronavirus infection causing COVID-19 (Corona Virus Disease 2019). On 29 January 2021, the European Medicines Agency (EMA) approved a COVID-19 vaccine developed by Oxford University and AstraZeneca (AZD1222, ChAdOx1 nCoV-19, COVID-19 vaccine AstraZeneca, Vaxzevria, Covishield). While the vaccine prevents severe course of and death from COVID-19, the observation of pulmonary, abdominal, and intracranial venous thromboembolic events has raised concerns. OBJECTIVE: To describe the clinical manifestations an

"Idiopathic" intracranial hypertension caused by venous sinus thrombosis associated with contraceptive usage.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a life-threatening condition that may present with symptoms and signs identical to idiopathic intracranial hypertension (IIH). Both conditions consist of increased intracranial pressure (ICP); however, IIH shows no evidence of contributory intracranial pathology. Oral hormonally based contraceptive usage has been associated with both conditions as well. Because disc edema often occurs in CVST, and IIH and is evident in other sight- and life-threatening conditions such as intracranial mass lesions, neuroimaging and other ancillary testing m
