The incidence of chronically ill subjects with prolonged mechanical ventilation has significantly increased over the last decade. Many patients get discharge to Skilled Nursing Facilities with an artificial airway, which do not have the means to properly progress on weaning. In Portugal this prevalence is unknown. Our aim was to establish the prevalence of tracheostomized patients at SNF in the North of Portugal, characterizing these units and its population, in a cross-sectional study, through an online questionnaire answered on the same day. Of the 75 SNF, 30 answered: 13 long-term, 2 medium

INTRODUCTION:: Notifiable diseases (NDs) encompass conditions of high clinical severity and/or contagious. Being closed communities, long-term care facilities (LTCF) are places that deserve attention on their own, but one might be left wondering: what is the reality of NDs at Brazilian LTCFs?
OBJECTIVE:: To determine the prevalence and type of NDs at large LTCF.
METHOD:: Active search for NDs conducted by the Hospital Infection Control Committee (HICC) in 459 beds. Due to the low turnover of patients, the monthly list kept by the HICC on NDs was analyzed. Data were grouped into males and fem

This article describes our recommendation for adapting hospital-based RED (Reengineered Discharge) processes to skilled nursing facilities (SNFs). Using focus groups, the SNFs' discharge processes were assessed twice additionally, research staff then recorded field notes documenting discussions about facility discharge processes as they related to RED processes. Data were systematically analyzed using thematic analysis to identify recommendations for adapting RED to the SNF setting including (a) rapidly identifying, involving, and preparing family/caregivers to implement a patient focused SNF

The number of geriatric residents in skilled nursing facilities (SNF) receiving methadone maintenance is likely to increase as the drug abusing population ages. Currently, facilities specializing in the care of human immunodeficiency virus (HIV) diseases are predominantly affected, but over the next decade this will change. Residents on methadone have unique needs based on their drug history and current treatment demands and will present new challenges for the affected institutions. There are no current publications or data in the literature exploring the use of methadone for the aging nursing

Heart failure management in skilled nursing facilities (SNFs) is complicated by limited access to specialists, incomplete clinical documentation, and patients with complex comorbidities. Artificial intelligence clinical decision support systems have been developed mostly for acute hospital settings but not for SNF settings. We present ForeSight-HF, an artificial intelligence clinical decision support tool designed specifically for SNF heart failure management. Built on a large language model with HIPAA-compliant infrastructure, ForeSight-HF synthesizes unstructured clinical documentation to ge

BACKGROUND: A positive delirium screen at skilled-nursing facility (SNF) admission can trigger a simultaneous diagnosis of Alzheimer's Disease or related dementia (AD/ADRD) and lead to psychoactive medication treatment despite a lack of evidence supporting use.
METHODS: This was a nationwide historical cohort study of 849,086 Medicare enrollees from 2011-2013 who were admitted to the SNF from a hospital without a history of dementia. Delirium was determined through positive Confusion Assessment Method screen and incident AD/ADRD through active diagnosis or claims. Cox proportional hazard mode

BACKGROUND: Dual eligibles, persons who qualify for both Medicare and Medicaid coverage, often receive poorer quality care relative to other Medicare beneficiaries.
OBJECTIVES: To determine whether dual eligibles are discharged to lower quality post-acute skilled nursing facilities (SNFs) compared with Medicare-only beneficiaries.
RESEARCH DESIGN: Following the random utility maximization model, we specified a discharge function using a conditional logit model and tested how this discharge rule varied by dual-eligibility status.
SUBJECTS: A total of 692,875 Medicare fee-for-service patients

IMPORTANCE: For long-term residents of skilled nursing facilities (SNFs), greater use of telemedicine for specialist care may improve access to and overall quality of care. Although policy changes during the COVID-19 pandemic facilitated greater use of telemedicine, such use now is rare, and it is unclear why.
OBJECTIVE: To examine SNF clinical leaders' perceptions of telemedicine use and explanations of lower use among long-term residents.
DESIGN, SETTING, AND PARTICIPANTS: This survey study was conducted online and via postal mail from February 7 to July 28, 2025, among directors of nursin

INTRODUCTION: Skilled nursing facilities have one of the highest rates of occupational injury and illness among all industries. This study quantifies the burden of occupational injury and illness in this industry using data from a single state-based workers' compensation (WC) system.
METHODS: Ohio Bureau of Workers' Compensation claims from 2001 to 2012 were analyzed among privately owned, state-insured skilled nursing facilities and are presented as claim counts and rates per 100 full-time equivalents (FTE). Worker, employer, incident, and injury characteristics were examined among all claim

OBJECTIVE: Endocrinologists are faced with a growing elderly patient population with diabetes mellitus (DM), some of whom are in skilled nursing facilities (SNFs). Efforts at managing their DM is hampered by concerns for hypoglycemia. This study aimed to determine the frequency of hypo- and hyperglycemia in SNFs, and associated factors.
METHODS: We reviewed medical records of 200 consecutive residents admitted to two SNFs in the Cleveland area in 2014 with documented DM, aged ≥65 years. Data collected included blood glucose (BG) levels and DM regimens. Frequency of hyper- and hypoglycemic eve

Hospital readmission within 30 days of discharge is an important quality measure given that it represents a potentially preventable adverse outcome. Approximately, 20% of Medicare beneficiaries are readmitted within 30 days of discharge. Many strategies such as the hospital readmission reduction program have been proposed and implemented to reduce readmission rates. Prior research has shown that coordination of care could play a significant role in lowering readmissions. Although having a hospital-based skilled nursing facility (HBSNF) in a hospital could help in improving care for patients ne
