[Skin and collagen vascular diseases].

Dermatology and rheumatology have in common that many rheumatologic diseases manifest with distinct clinical presentations on the skin. Since the skin is easily accessible both to physical examination and biopsy, rheumatologists appreciate to consult with dermatologists with experience in the clinical and pathological aspects of the rheumatic diseases with skin manifestations. Though the medical subspecialities show a tendency to an organ-particular focus, dermatologists traditionally have a multidisciplinary approach to patients and tend to communicate in a cross-disciplinary way. Molecular insights from skin biopsies: Deciphering microvascular pathology in peripheral vascular diseases.

BACKGROUND: The heterogeneous group of disorders called peripheral vascular diseases (PVDs) occurs outside the heart and brain tissue to cause ischemia and severe health complications. Diagnosis accuracy is essential in starting appropriate patient management at the proper time. Modern medicine considers skin biopsies crucial diagnostic tools that yield histopathological and molecular evidence for examining PVD-related microvascular changes. AIM: To evaluate skin biopsy applications in PVD diagnostics through artistic analysis of technical processes and examination of pathological and innovat

Role of Indocyanine Green Fluorescence to Assess Skin Vascularity in Peripheral Vascular Disease.

OBJECTIVES: The objective is to assess the level of amputation based on the skin vascularity using indocyanine fluorescence. BACKGROUND: Patients with chronic limb ischemia have a risk of amputation within 1 year and reamputation due to impaired wound healing postamputation. Therefore, the assessment of tissue perfusion and viability before the planned surgery can minimize the chances of reamputation. To qualify the level of tissue perfusion, near-infrared (NIR) fluorescence imaging is promising. METHODS: The study includes adult patients suffering from peripheral vascular disease and in nee

Skin Telangiectasia and the Identification of a Subset of Systemic Sclerosis Patients With Severe Vascular Disease.

OBJECTIVE: Cutaneous telangiectasia (CT) are common in systemic sclerosis (SSc) patients, but their ability to stratify patients by risk is poorly known. We aimed to determine whether the number and size of CT were associated with the pattern of microvascular lesions assessed by nailfold videocapillaroscopy (NVC) and markers reflecting the severity of SSc-related vasculopathy. METHODS: We performed a cross-sectional study, including consecutive SSc patients over a 6-month period. We also considered 3 predefined subsets of patients according to the number of hand or face CT: absence, ≤10, or >

The effects of age and peripheral vascular disease on the circulatory and mechanical response of skin to loading.

The skin and subcutaneous soft tissues of amputation residual limbs are required to withstand externally applied loads of greater magnitude than similar tissues of the intact lower limb. Increased age and poor circulatory status may contribute to the increased risk of tissue injury seen in this population. This study evaluates the effects of age and circulatory status as risk factors for skin injury resulting from externally applied forces. Twelve young control (YC), six elderly control (OC) and 11 subjects with peripheral vascular disease (PVD) were studied. After base-line ankle arm index (A

Occurrence of vascular IgA deposits in clinically normal skin of patients with renal disease.

Skin and kidney biopsies were performed on 262 patients with various nephropathies. In 45 skin biopsy specimens finely granular deposits of predominantly IgA and late-acting complement factors were detected in the walls of superficial capillaries, sometimes concomitantly with IgM, IgG, C4 or a combination of these proteins. Twelve of the 45 patients presented with anaphylactoid purpura and the majority of the other 33 patients had either recurrent macroscopic or microscopic hematuria. The renal lesions in 32 of these 45 patients consisted of focal segmental intracapillary proliferation. In 35

The measurement of skin blood flow in peripheral vascular disease by epicutaneous application of Xenon133.

Radioactive xenon133 applied epicutaneously was used to study the skin blood flow below the knee in sixteen normal subjects, in eight patients with peripheral vascular disease not requiring amputation, and in a blind study of twenty-nine patients requiring amputation. Following these twenty-nine amputations, the flow rates were made known and correlated with the rates of healing. When the flow rates were above 1.5 milliliters per minute per 100 grams of tissue (skin), the wounds generally healed. In thirteen other patients, Syme or below-the-knee amputations were performed on the basis of the

Collagen Vascular Diseases: A Review of Cutaneous and Systemic Lupus Erythematosus, Dermatomyositis, and Distinguishing Features in Skin of Color.

Collagen vascular diseases such as lupus erythematosus and dermatomyositis (DM) occur 2 to 3 times more often among patients with skin of color. In this article, the authors review DM and cutaneous lupus erythematosus, including acute cutaneous lupus erythematosus, subacute cutaneous lupus erythematosus, and discoid lupus erythematosus. They discuss the distinguishing features between these entities and highlight distinct presentations and management considerations in patients with skin of color to aid in prompt and correct diagnoses in this patient population.

Vascular calcification in skin and subcutaneous tissue in patients with chronic and end-stage kidney disease.

BACKGROUND: Vascular calcification (VC) is well described in large- and medium-sized vessels in patients with chronic kidney disease (CKD), especially in those with end-stage kidney disease (ESKD) on dialysis. Medial calcification is particularly prevalent in this population and contributes to arterial stiffness and increased cardiovascular mortality and morbidity. Apart from in the setting of calciphylaxis, few studies have assessed skin and subcutaneous calcification and associations with abnormalities of bone and mineral metabolism in patients with CKD. METHODS: We performed a single-centr

Targeted volume imaging reveals early vascular interactions of Lyme disease pathogen in skin.

Although the contours of the dissemination pathways of human pathogenic spirochetes in the vertebrate hosts are known, detailed high-resolution information on these processes remain lacking. In this study, we establish an efficient serial block-face scanning electron microscopy workflow incorporating semi-automatic AI-driven segmentation to investigate the architecture of early events following the deposition of Borrelia burgdorferi at the tick bite site in mice. We capture evidence of Borrelia penetrating the lymphatic endothelium via both transcellular and paracellular routes and observe its

The stromal vascular fraction mitigates bleomycin-induced skin fibrosis in mice by modulating vascular lesions and secreting antifibrotic factors at different stages of disease.

BACKGROUND: Systemic sclerosis (SSc) is a chronic autoimmune disease characterized by fibrosis of the skin and internal organs, leading to significant morbidity and reduced quality of life. Despite ongoing research, the underlying pathogenesis of SSc remains unclear, and treatment options are limited. Stromal vascular fraction (SVF), a naturally occurring cell population that includes mesenchymal stem cells (MSCs), has emerged as a potential therapeutic agent for various fibrotic diseases. This study aimed to investigate the therapeutic effects and underlying mechanisms of SVF in a bleomycin-i

Vascular cell adhesion molecule-1: expression in normal and diseased skin and regulation in vivo by interferon gamma.

BACKGROUND: Vascular cell adhesion molecule-1 (VCAM-1) is an endothelial protein with adhesive properties for inflammatory cells including lymphocytes. Its role in skin disease and regulation in vivo are uncertain. OBJECTIVE: Our purpose was to determine expression of VCAM-1 in normal and inflamed skin and the effect on this of the T-cell-derived cytokine interferon gamma (IFN-gamma). METHODS: VCAM-1 was detected immunohistochemically in frozen-section biopsy specimens of inflammatory dermatoses and skin tumors. Volunteers received intradermal IFN-gamma and underwent biopsy 2 hours to 6 days
