Sleep and sleep disorders.

Sleep is a complex neurological state, with its primary function of providing rest and restoring the body's energy levels. The importance of sleep could be seen from the fact that people spend about one-third of their lifespan in sleep. Normal human sleep is divided into non-rapid eye movement (NREM) and rapid eye movement (REM) sleep, and the alteration between NREM and REM occurs about 4-5 times during a night of normal sleep. Human NREM sleep could be classified into four stages, namely, stage I, II, III and IV, representing successively deeper stages of sleep. Sleep is an active rhythmic n

[Sleep and sleep disorders].

Disturbance of sleep is a symptom of heterogeneous origin. Hypersomnia (excessive sleep during daytime) must be carefully distinguished from hyposomnia. Hypersomnic patients should be referred to a neurologist for further diagnostic workup; hypnotics may be dangerous in these patients. Medical, psychiatric and other cases of hyposomnia should be identified and treated as part of a comprehensive therapy plan. Counselling and application of the rules of sleep hygiene should be considered. When drug therapy is indicated, benzodiazepines are preferable. Because chronic insomnia causes serious impa

Sleep loss overrides sleep-preparatory behavior to promote sleep.

STUDY OBJECTIVES: In the period preceding sleep, humans and other animals display a stereotyped repertoire of behaviors-including hygiene-related activities and preparing a place to sleep. Evidence suggests that this pre-sleep phase actively contributes to sleep initiation and quality. Nonetheless, individuals can sometimes fall asleep without preparation, even under undesirable circumstances. These abrupt transitions into sleep can have severe consequences, particularly in high-risk environments. Although progress has been made in identifying neuronal populations controlling sleep/wake states

Sleep in overweight adolescents: shorter sleep, poorer sleep quality, sleepiness, and sleep-disordered breathing.

OBJECTIVE: To document the sleep of overweight adolescents and to explore the degree to which weight-related sleep pathology might account for diminished psychosocial outcome. METHODS: Sixty children aged 10-16.9 from a weight-management clinic were compared to 22 healthy controls using comprehensive actigraphic, polysomnographic, and parent- and self-report questionnaire assessments. RESULTS: Overweight participants averaged more symptoms of sleep-disordered breathing, later sleep onset, shorter sleep time, and more disrupted sleep than controls. Although the groups did not differ in self-r

REM sleep obstructive sleep apnoea.

Obstructive sleep apnoea (OSA) can occur in both rapid eye movement (REM) and non-REM sleep or be limited to REM sleep, when the upper airway is most prone to collapse due to REM sleep atonia. Respiratory events are usually longer and more desaturating in REM than in NREM sleep. The prevalence of REM OSA is higher in women than in men and REM OSA usually occurs in the context of mild-moderate OSA based on the apnoea-hypopnoea index calculated for the entire sleep study. Studies have highlighted some detrimental consequences of REM OSA; for example, its frequent association with systemic hypert

Overview of sleep & sleep disorders.

Sleep is defined on the basis of behavioural and physiological criteria dividing it into two states: non rapid eye movement (NREM) sleep which is subdivided into three stages (N1, N2, N3); and rapid eye movement (REM) sleep characterized by rapid eye movements, muscle atonia and desynchronized EEG. Circadian rhythm of sleep-wakefulness is controlled by the master clock located in the suprachiasmatic nuclei of the hypothalamus. The neuroanatomical substrates of the NREM sleep are located principally in the ventrolateral preoptic nucleus of the hypothalamus and those of REM sleep are located in

Sleep, Sleep Apnea, and Epilepsy.

Sleep disorders occur commonly in patients with epilepsy, and can be responsible for symptoms of daytime somnolence and also can contribute to the intractability of epilepsy. The most important aspect of treating sleep disorders, especially sleep apnea, is the recognition of the problem. In a busy clinical practice, symptoms of sleep disorders are frequently overlooked or mistaken. Whenever sleep disruption or excessive daytime somnolence is potentially problematic, the patient should be referred to a sleep specialist and, if indicated, diagnostic testing performed (usually polysomnography wit

Sleep studies for sleep apnoea.

Sleep studies have grown to encompass a broad range of technologies employed to study and diagnose a variety of sleep disorders. From their inception in neurophysiology laboratories interested in investigating primary disorders of sleep architecture from psychiatric illness, their remit has widened such that their most common role is currently to diagnose secondary sleep disruption from respiratory, cardiovascular or other systemic causes. This review outlines the pathophysiology of obstructive sleep apnoea in particular and how sleep studies have improved our understanding of the complex dyna

Sleep Pharmacology: Medications Affecting Sleep and Medications to Facilitate Sleep.

Both therapeutic and nontherapeutic medications have a profound influence on sleep architecture, a vital component of overall health. Optimizing patient outcomes requires a clear understanding of how commonly prescribed drug classes affect the quality, duration, and structure of sleep. Clinicians should carefully consider these pharmacologic effects when selecting or modifying treatment regimens, particularly in patients with comorbidities or underlying sleep disorders. The widespread use of herbal and over-the-counter preparations further underscores the importance of comprehensive medication

Sleep in the age of technology: the use of sleep apps and perceived impact on sleep and sleep habits.

INTRODUCTION: The rapid development in sleep related technology, particularly the emergence of sleep apps, has generated a need to uncover user characteristics and perceived effects. METHODS: A sample of 1,002 adult participants living in Norway completed an online questionnaire on the use and perceived effects of sleep apps (response rate 19.8%). The questions pertained to demographics, including age, sex and educational level in addition to insomnia symptoms assessed with Bergen Insomnia Scale (BIS). The questionnaire also included 10 items focusing on perceived positive and negative effect

Sleep and sleep disorders in pregnancy.

Sleep problems are common in pregnant women. This review examines sleep in normal pregnancy; discusses the physiologic bases for alterations in sleep, including hormonal and mechanical factors; and correlates these factors with changes in sleep of pregnant women, as determined subjectively by surveys and objectively by polysomnographic studies. The changes in respiratory physiology during pregnancy, the possible predisposition of the pregnant woman to sleep-disordered breathing because of these changes, and results of published studies of sleep-disordered breathing in pregnancy are discussed.

Neuroimaging of sleep and sleep disorders.

Herein are presented the results of research in the area of sleep neuroimaging over the past year. Significant work has been performed to clarify the basic mechanisms of sleep in humans. New studies also extend prior observations regarding altered brain activation in response to sleep deprivation by adding information regarding vulnerability to sleep deprivation and regarding the influence of task difficulty on aberrant responses. Studies in sleep disorder medicine have yielded significant findings in insomnia, depression, and restless legs syndrome. Extensive advances have been made in the ar

Sleep and sleep disorders: an overview.

Sleep has been preserved during evolutionary history and is found in all mammalian species and birds. Despite the apparent strong survival advantage of sleep, and the observation in experimental animals that persistent sleep deprivation leads to rapid physical deterioration and death, the function of sleep is poorly understood. Modern neurophysiologic techniques have shown that sleep is not a homogeneous state but rather is divided into rapid eye movement and non-rapid eye movement stages. These two sleep states are as different from one another as sleep is from wakefulness. The advent of poly

Sleep Disorders: Sleep-Related Breathing Disorders.

Sleep-related breathing disorders or sleep-disordered breathing are characterized by abnormal respiration during sleep. They are grouped into obstructive sleep apnea (OSA), central sleep apnea, sleep-related hypoventilation, and sleep-related hypoxemia disorder. OSA is a common disorder encountered in the family medicine setting that is increasingly being recognized because of the obesity epidemic and greater public and physician awareness. OSA is characterized by recurrent episodes of partial or complete closure of the upper airway resulting in disturbed breathing during sleep. It is associat

Sleep Pharmacogenetics: Personalized Sleep-Wake Therapy.

Research spanning (genetically engineered) animal models, healthy volunteers, and sleep-disordered patients has identified the neurotransmitters and neuromodulators dopamine, serotonin, norepinephrine, histamine, hypocretin, melatonin, glutamate, acetylcholine, γ-amino-butyric acid, and adenosine as important players in the regulation and maintenance of sleep-wake-dependent changes in neuronal activity and the sleep-wake continuum. Dysregulation of these neurochemical systems leads to sleep-wake disorders. Most currently available pharmacological treatments are symptomatic rather than causal,
