This article reviews the history of insomnia pharmacotherapy, documenting the evolution that has occurred over time in the increasing availability of medications with novel mechanisms of action that more specifically target the neural systems that modulate sleep/wake function. This evolution provides an increasing capacity to improve the effectiveness of insomnia pharmacotherapy by allowing the selection of medications that specifically target the particular type of sleep difficulty present in each patient. As a result, they can achieve a therapeutic effect with fewer effects on aspects of bra

Effective management of insomnia begins with recognition and adequate assessment. Family doctors and other health care providers such as practice nurses and psychologists should routinely enquire about sleep habits as a component of overall health assessment. Identification and treatment of primary psychiatric disorders, medical conditions, circadian disorders, or specific physiological sleep disorders--eg, sleep apnoea and periodic limb movement disorder--are essential steps in management of insomnia. Conditioned aspects of insomnia can be primary (psychophysiological insomnia) or may complic

Idiopathic insomnia (Childhood-onset insomnia) which belongs to Intrinsic Sleep Disorders in ICSD, has some dysfunction in sleep-wake center in brain. Sleep-wake center consists of two parts, one is sleep center which is present in the anterior parts of hypothalamus, and the other is wake center which is present in the ascending reticular activating system including posterior parts of hypothalamus. Idiopathic insomnia patients are said to have hyperactivity in wake center or hypoactivity in sleep center. And these patients have sleep disturbances such as inability to fall asleep, frequent awak

Sleep problems are amongst the most common triggers of migraine and non-migraine primary headache. Despite a majority of sleep problems being psychological in nature, there is a paucity of quantitative research on the psychological factors involved in sleep-related headaches. This is the first study to examine the link between maladaptive sleep beliefs and headaches. 542 participants completed an online battery measuring headache, sleep, and psychological distress. Avoidance of sleep-related headache triggers was associated with more headaches, as mediated by increased sensitivity to the trigg

Sleep and circadian disturbances in bipolar disorder are common and persistent within and between illness episodes. Insomnia, hypersomnia, reduced need for sleep, sleep schedule variability and circadian rhythm disorders are frequently observed. In this article, recent research is reviewed suggesting that the presence of sleep disturbance is associated with functional impairment, interacts with other physical and environmental systems (e.g. physical activity, light exposure), and may attenuate response to treatment. Established and emerging treatments for various sleep disturbances are reviewe

A 'Direct to Psychology Insomnia' pathway was developed for implementation within a multidisciplinary sleep disorders service in a tertiary hospital in Brisbane, Australia. The project was informed by implementation science principles and methodology to re-design the model of care (MoC). A consensus group workshop using the Nominal Group Technique (NGT) with 12 multidisciplinary staff was undertaken to develop the new MoC. The workshop explored inclusion and exclusion criteria for a Direct to Psychology pathway including patient flow and enablers. The team endorsed a MoC that was acceptable to

Sleep is important for our survival. Research suggests that getting 'good' sleep is a problem for a proportion of adolescents. The paper advocates for holistic treatment of sleep disorders incorporating expertise from a multidisciplinary team. Much of the assessment and treatment of sleep disorders in adolescents comes from research within adult populations, therefore, there is a need for further research to be completed within the adolescent population to ensure there is a robust evidence base for assessment and treatment of sleep disorders.

Insomnia is one of the most frequently encountered diseases in clinical practice, and about half of the patients have a long-term course. Therefore, proactive prevention against chronicity is required, using a non-pharmacological approach for insomnia (sleep hygiene). Pharmacological treatment, needs to decrease the risk of appearance of rebound insomnia, the risk of the patient falling, drug dependence, and cognitive dysfunctions caused by hypnotics. In view of this, it is recommended to use novel sleep medications such as orexin receptor antagonists and melatonin receptor agonists.

Insomnia is a distinct disorder that is common, yet underrecognized and undertreated in primary care. Treating insomnia has been shown to improve outcomes, including reduced risk of developing cardiovascular and mental health disorders. Insomnia is influenced by the brain's regulation of sleep and wake, which are mutually exclusive events. Insomnia should be treated as a distinct condition, even when occurring with a comorbid diagnosis such as depression or anxiety. Clinicians should implement a multimodal approach to insomnia management, including nonpharmacologic interventions and pharmacolo

Studies show a high frequency of comorbidity of insomnia and nightmares, which aggravate and chronicle each other. The mechanisms of their mutual influence are different. Insomnia changes the nature of dreams, both indirectly through emotional hyperactivation characteristic of insomnia, and as a result of frequent awakenings. However, frequent recurring unpleasant and frightening dreams can themselves increase the frequency of nocturnal awakenings and worsen the quality of sleep. The comorbidity of insomnia and nightmares should be taken into account when choosing a therapeutic approach in thi

Napping, when its timing and duration are designed properly, has the potential to improve our daily lives. Laboratory findings indicate that scheduled napping promotes waking function after normal sleep at night, and also counteracts decreased alertness and performance under conditions of sleep deprivation. Since these effects are evident even with naps shorter than 30 min, shiftwork problems may be alleviated by the short nap at the workplace. Multiple short naps are effective in managing excessive daytime sleepiness in narcoleptic patients under medication. The therapeutic usefulness of napp

The importance polysomnography (PSG) in the diagnosis and treatment process of insomnia disorder (ID) remains highly disputed. This review summarises the state of the science regarding PSG indications and findings in ID, and the indications to conduct PSG in ID as stated by relevant guidelines. It then highlights the most relevant questions regarding the topic, including the relevance of ID subtyping, to allow an individualised pharmacological or psychotherapeutic treatment approach.

BACKGROUND: Several epidemiological studies on the association between primary headaches and insomnia have been published in recent years. Both disorders are frequent, and our purpose was to review results from population-based studies exploring this association.
METHODS: We performed a literature search in PubMed for "insomnia" (or sleep disturbance) and "headache" (or migraine) linked with "epidemiology." Two hundred and eight records were identified. Three longitudinal and 10 cross-sectional studies met our inclusion criteria: population-based design with at least 200 participants includin

Movements during sleep characterize sleep disorders, which can disturb sleep or its onset, impacting sleep quantity and quality. Video-polysomnography is the current gold standard to assess movements during sleep, but its availability is limited. Using data recorded with a 3D time of flight sensor, we developed a novel method of encoding temporal and spatial information of automatically identified movements during sleep. In a cohort of 20 insomnia patients and 18 controls, we showed that this novel method holds important information able to discriminate the groups. Future studies will explore

Sleep disruption is common in older adults and is associated with many poor health outcomes. It is vital for providers to understand insomnia and other sleep disorders in this population. This article outlines age-related changes in sleep, and medical, psychiatric, environmental, and psychosocial factors that may impact sleep. It addresses the evaluation of sleep symptoms and diagnosis of sleep disorders. It aims to examine the evidence for non-pharmacological and pharmacologic treatment options for insomnia while weighing factors particularly germane to the aging adult..

Exploding head syndrome is a rare sleep disorder, characterized by an explosive feeling in the head, that occurs during the sleep-wake transition. Usually the attacks are painless, but the fear caused by the attack can result in awakening and insomnia when it is generated frequently. It has been suspected that exploding head syndrome is related to emotional stress, because most patients report stressful life situations in periods when attacks are intense and frequent. The benign character and good prognosis of exploding head syndrome are the most likely reasons why it has not become a subject
