[Sleep is the best medicine: what internists should know about sleep and sleep medicine].

Sleep is a fundamental biological function that affects nearly all organ systems, yet it has been insufficiently documented and considered in internal medicine practice to date. The aim of this review is to present the physiological basis of sleep, clinically relevant sleep disorders, and their significance for internal medicine conditions, as well as to outline practical approaches to prevention, diagnosis, and treatment. Sleep deprivation and sleep disorders are associated with an increased risk of hypertension, type 2 diabetes mellitus, obesity, cardiovascular events, and neurodegenerative

Hospital Sleep Medicine.

The integration of sleep medicine into hospital care is critical for addressing sleep-disordered breathing (SDB), a condition highly prevalent among hospitalized patients. Effective management of SDB is linked to improved patient outcomes, reduced recovery times, and decreased hospital readmission rates. Advancements in technology, such as wearable devices and noninvasive monitoring tools, have enhanced real-time assessment and management of SDB. Early detection and intervention through structured hospital sleep medicine programs have shown improvements in patient care and overall health care

Sleep Medicine-What's in a Name?

Sleep medicine has matured into a recognised medical discipline, characterised by defined diagnostic concepts, evidence-based treatments, and significant progress in understanding sleep physiology and disorders. Sleep and its disturbances impact virtually every aspect of health and well-being. The major categories of sleep disorders include insomnia, neurological and psychiatric sleep disorders, sleep-disordered breathing, and paediatric sleep disorders. Breakthroughs in biomedical research have deepened clinical expertise across each of these domains. Although sleep medicine has historically

The Development of Sleep Medicine: A Historical Sketch.

For centuries the scope of sleep disorders in medical writings was limited to those disturbances which were either perceived by the sleeper him- or herself as troublesome, such as insomnia, or which were recognized by an observer as strange behavioral acts during sleep, such as sleepwalking or sleep terrors. Awareness of other sleep disorders, which are caused by malfunction of a physiological system during sleep, such as sleep-related respiratory disorders, were widely unknown or ignored before sleep monitoring techniques became available, mainly in the second half of the 20(th) century. Chronic Opioid Therapy and Sleep: An American Academy of Sleep Medicine Position Statement.

There is a complex relationship among opioids, sleep and daytime function. Patients and medical providers should be aware that chronic opioid therapy can alter sleep architecture and sleep quality as well as contribute to daytime sleepiness. It is also important for medical providers to be cognizant of other adverse effects of chronic opioid use including the impact on respiratory function during sleep. Opioids are associated with several types of sleep-disordered breathing, including sleep-related hypoventilation, central sleep apnea (CSA), and obstructive sleep apnea (OSA). Appropriate scree

Integrating Sleep Knowledge Into the Anesthesiology Curriculum.

There is common ground between the specialties of anesthesiology and sleep medicine. Traditional sleep medicine curriculum for anesthesiology trainees has revolved around the discussion of obstructive sleep apnea (OSA) and its perioperative management. However, it is time to include a broader scope of sleep medicine-related topics that overlap these specialties into the core anesthesia residency curriculum. Five main core competency domains are proposed, including SLeep physiology; Evaluation of sleep health; Evaluation for sleep disorders and clinical implications; Professional and academic r

Sleep medicine in Africa: past, present, and future.

Interest in sleep and sleep disorders in Africa dates back thousands of years, influenced by various cultural and religious beliefs. However, the practice of sleep medicine as a specialty has been inadequate compared to other regions of the world. The objective of this study was to explore the current status of sleep medicine in Africa vis-à-vis education, professional societies, and facilities, and to identify challenges of the specialty in the region. A literature search of major electronic databases (PubMed, Google Scholar) was done. This revealed that there is a high prevalence of sleep di

Future of Sleep Medicine: Novel Approaches and Measures Derived from Physiologic Systems Dynamics (Part I).

We review recent progress in understanding fundamental aspects of physiologic regulation during wake and sleep based on modern data-driven, analytical, and computational approaches with a focus on the complex dynamics of individual physiologic systems. The presented empirical findings indicate that sleep-wake and circadian cycles do not simply modulate basic physiologic functions but influence physiologic systems dynamics simultaneously over a broad range of time scales. The reviewed empirical approaches and derived measures represent novel mechanistic aspects of sleep and wake regulation, and

Pediatric pulmonary and sleep medicine - Best recent articles to read in 2025.

It is a challenge to select the "best" recent publications in a field. This is especially so when faced with a feast of outstanding manuscripts across a broad range of topics. I therefore reached out to a Who's Who of friends and colleagues in pediatric pulmonary and sleep medicine for suggestions, and I was delighted and overwhelmed by the response - please see the Acknowledgements for those who contributed ideas. Overwhelmed, by having to read 77 publications suggested by one or more colleagues and having to winnow the list down to a somewhat reasonable number. I chose to include all papers

A review of sleep disorders. The history and diagnosis of sleep disorders related to the dentist.

The last 4 decades have seen the emergence of a new medical specialty, sleep medicine. In response to an unfulfilled clinical need, and fueled by better understanding of the significance of sleep disorders, sleep medicine developed medical solutions for age-old problems with sleep and wakefulness. Sleep-disordered breathing has played a central role in this story. Therapy and management offered by the dentist has become recognized as an important aspect of care for patients with SDB. Dentists who offer this service need to become acquainted with the multifactorial nature of sleep medicine to s

Why could sleep medicine never do without polysomnography?

Given the clinical, methodological, conceptual and modelling challenges within the field of sleep medicine, polysomnography (PSG) has emerged as a central diagnostic tool over time. It has been highly beneficial to clinical practice over the years, thanks to the scientific data that it provides. More recently, sleep medicine has sought answers in precision medicine, broadening its quest for biomarkers that take into account environmental factors, big data, and nosological refinement. However, despite these innovative developments that are relatively independent of PSG, sleep medicine remains i

Pupillography in contemporary sleep medicine - A narrative review.

Excessive daytime sleepiness (EDS) is a common symptom of sleep disorders such as narcolepsy, obstructive sleep apnea, and hypersomnia. The most common tools for assessing EDS are various specialized questionnaires such as Epworth Sleepiness Scale (ESS) and Stanford Sleepiness Scale (SSS). However, the scores obtained from self-rating questionnaires do not seem to measure physiological sleepiness but rather a more complex phenomenon of subjective sleepiness modulated by other factors such as motivation, expectation, and capability of self-perception. The golden standard for measuring physiolog

[Sleep medicine differential diagnostics in psychiatry and psychotherapy].

Complaints about disturbed sleep or increased daytime sleepiness are among the most frequent symptoms reported to psychiatrists by patients. Such complaints can be symptoms of an underlying psychiatric disorder or indicative of a separate or comorbid sleep disorder. Hence, basic knowledge in the differential diagnosis of sleep medicine pathologies is pivotal for psychiatrists and psychotherapists. In the present overview following a description of the diagnostic methods, the diagnostic work-up according to the major symptomatic clusters, namely disturbances in initiating and maintaining sleep,

[Sleep medicine services in Germany].

The increase in sleep medicine services has paralleled the increased in nocturnal ventilation therapy in patients with sleep apnea. Even if the sleep medicine expertise did increase in the past, this is not sufficient to cover the increasing demand for sleep medicine services. To serve patients with nonrestorative sleep, a clinical algorithm showing the pathway from the clinical interview to sleep laboratory investigations is available. However, there is a gap in the services offered by family physicians, other medical specialists, and sleep medicine practitioners. For sleep apnea, the diagnos

Images in sleep medicine sleep-disordered breathing in Wolfram's syndrome - A near-fatal event.

Wolfram syndrome (WS) is a rare autosomal-recessive genetic disorder. The authors report a case of a patient with WS and undiagnosed/untreated obstructive sleep apnea (OSA) associated with prolonged periods of apnea and hypopnea and nocturnal hypoxemia, which may have predisposed him to the development of a near-fatal event during sleep. Addressing sleep-disordered breathing in patients with WS could improve their quality of life and potentially their longevity.
