Slit Ventricle Syndrome.

Slit ventricle syndrome (SVS) is a complication after ventriculoperitoneal shunt (VPS) or cystoperitoneal shunt(CPS), mostly due to excessive drainage of cerebrospinal. The disease is most often seen in children and has a complex pathogenesis. Clinical manifestations are mainly intermittent headache, slow refilling of the shunt reservoir, and slit-like ventricles on imaging. Surgery is the main treatment. We present a 22-year-old female patient with a previous 14-year history of CPS. The patient recently presented with typical symptoms but her ventricular morphology was normal. We performed VP

Antimigraine treatment for slit ventricle syndrome.

Slit ventricle syndrome is characterized by chronic or recurring headaches associated with subnormal ventricular volume in patients who have undergone shunt treatment for hydrocephalus. There appear to be at least three pathophysiological mechanisms that cause this syndrome: 1) intermittent shunt malfunction; 2) intracranial hypotension; and 3) paroxysms of increased intracranial pressure in the presence of normal shunt function. To treat seven patients with slit ventricle syndrome caused by paroxysms of elevated intracranial pressure, we successfully used antimigraine therapy rather than stan

[Collapsed ventricle syndrome (slit-ventricle). Apropos of 8 cases].

Slit-ventricle syndrome (SVS) may appear in shunted hydrocephalic children as a complication by chronic overdrainage. SVS is defined by clinical features (headache, nausea and/or vomiting, disturbance of consciousness), slow valve refilling and slit or slit-like ventricles at computerized tomography. Authors report eight cases of SVS and review physiopathological, clinical and therapeutic aspects of this syndrome. It is concluded that SVS at present time may be considered an iatrogenic complication and the placement of high-pressure valves as a prophylactic measure can be a useful system to av

Positional shunt assist for slit ventricle syndrome.

PURPOSE: Hydrocephalus is one of the most common pathologies in pediatric neurosurgery. One of the causes of recurring events of headaches among shunted children is "slit ventricle syndrome" (SVS). Several potential treatments have been proposed, yet SVS often represents a treatment challenge. The goal of the current series is to present our experience with adding a positional shunt-assist (SA) (Miethke, Aesculap) for the treatment of SVS. METHODS: Clinical data was retrospectively collected from all consecutive children with SVS that were treated with SA (Miethke, Aesculap) at our center. Su

[Slit ventricle--clinical syndrome or radiologic phenomenon?].

Between 1976 and 1982 336 patients with an age range from several days to 65 years, were treated by ventriculo-atrial or peritoneal shunt, because of internal hydrocephalus of different origins. Regular computerized tomographies demonstrated slit-ventricles in 36 patients. Twenty-eight patients were free of complaints. In 8 patients there were symptoms like nausea and vomiting. In 5 patients these complaints were explainable by other diseases. In only 3 patients these signs were caused by increased intracranial pressure because of ventricular-catheter obliteration. In these 3 cases an enlargem

Endoscopy in the treatment of slit ventricle syndrome.

The present study aimed to investigate the efficacy of endoscopy in the treatment of post-shunt placement for slit ventricle syndrome (SVS). Endoscopic surgery was performed on 18 patients with SVS between October 2004 and December 2012. Sex, age, causes of the hydrocephalus, ventricular size and imaging data were collected and analyzed. All patients were divided into two groups according to ventricular size and underwent endoscopic surgeries, including endoscopic third ventriculostomy (ETV), endoscopic aqueductoplasty and cystocisternostomy. All treated patients were observed postoperatively

Slit-ventricle syndrome masking shunt disconnection in adulthood.

A male in his 20s was referred to the neurosurgical service with headaches, vomiting and personality changes. He had a previous history of bilateral ventriculoperitoneal (VP) shunts and bitemporal decompressive surgery for neonatal intraventricular haemorrhage. There were appearances of slit-ventricles on his CT head scan typically associated with adequate cerebrospinal fluid drainage and normal intracranial pressure (ICP), which ordinarily will have limited further investigation. However, an X-ray shunt series demonstrated a right shunt disconnection at the neck and ICP monitoring confirmed r

Neuropathological findings in a child with slit ventricle syndrome.

The histopathological changes in shunted hydrocephalic children with slit ventricle syndrome have never been described. Periventricular gliosis is presumed to be an important feature. A girl who was shunted in infancy following meningitis developed headaches at 10 years of age and suffered a respiratory arrest, from which she was resuscitated. CT scan of the head showed very small ventricles. A diagnosis of slit ventricle syndrome was made. She died 33 h later. Autopsy revealed a large head and brain, small ventricles with glial adhesions, obstruction of the cerebral aqueduct, complete obstruc

Infantile hydrocephalus and the slit ventricle syndrome in early infancy.

Slit ventricle syndrome is well known as a complication in the treatment of hydrocephalus by shunting. It is generally considered to be a chronic (but not acute) complication, occurring years after the shunt procedure; there has been no report of this syndrome occurring before 1 year of age. The authors present infantile cases that developed a severe form of this syndrome shortly after shunt procedures and discuss the pathophysiology in comparison to experience with older cases. The causative factor was thought to be extremely low intracranial pressure with resultant microcephalus created by d

Hypertensive slit ventricle syndrome: pseudotumor cerebri with a malfunctioning shunt?

Symptomatic shunt malfunction without ventricular enlargement is known as slit ventricle syndrome (SVS). Patients presenting with this syndrome are not a homogeneous group. Of the 5 different types classified by Rekate, Type 1 is caused by CSF overdrainage and is associated with low pressures; Types 2 and 3 are associated with shunt blockage and elevated CSF pressures; Type 4 is cephalocranial disproportion that increases brain parenchymal pressure but not CSF pressure; and Type 5 is headache unrelated to shunt function. The low and normal CSF pressure types are relatively well understood, but

Slit ventricle syndrome: Historical considerations, diagnosis, pathophysiology, and treatment review.

After the introduction of shunt treatment for the management of childhood hydrocephalus, a wide variety of complications related to this treatment modality have been recognized. The entity of slit ventricle syndrome (alternatively, symptomatic ventricular coaptation) is one of them, is frequently encountered in the pediatric population and its symptom complex resembles that of shunt failure. We conducted research on PubMed®, MEDLINE®, and Web of Science®, using the keywords: "slit ventricles," "slit ventricle syndrome," "SVS" and "ventricular coaptation." The aim of our review was to trace the

Classification of slit-ventricle syndromes using intracranial pressure monitoring.

The term slit-ventricle syndrome appears to have different meanings to different authors. This study proposes a sublcassification of headaches in shunted children based on data obtained from chronic monitoring of intracranial pressure in 7 symptomatic children. Five distinct syndromes are identified: (1) intermittent, extremely low pressure headaches that are analogous to spinal headaches, (2) intermittent proximal obstruction, (3) shunt failure with small ventricles ('normal volume hydrocephalus'), (4) intracranial hypertension with working shunts (hydrocephalic pseudotumor), and (5) headache

Severe intracranial hypertension in slit ventricle syndrome managed using a cisterna magna-ventricle-peritoneum shunt.

OBJECT: Severely increased intracranial pressure (ICP) can be life threatening in patients who had previously undergone shunt treatment but who do not experience ventricular enlargement. The authors analyzed the utility of placing shunts into the cisterna magna concurrently with ventricular shunts in patients who were not candidates for lumboperitoneal (LP) shunt placement. METHODS: Ten patients treated with cisterna magna-ventricle-peritoneum (CMVP) shunts for complex problems of shunt function were reviewed retrospectively. All patients had documented increases in ICP and ventricles that di

Cranial expansion and aqueductoplasty for combined isolated fourth ventricle and slit-ventricle syndrome: a surgical alternative.

INTRODUCTION: An isolated fourth ventricle (IFV) is a rare entity observed in shunted patients and its treatment is still uncertain. Endoscopic aqueductoplasty has shown good results for restoring CSF flux between the third and fourth ventricles. However, it needs some grade of ventricular dilation to be performed. Some patients affected by IFV show slit-ventricle morphology in CT/MRI. Usually, the rise of opening pressure or the shunt externalization gets enough ventricular dilation. However, the lack of intracranial compliance in some patients makes these options unsuitable and high-ICP symp

Slit ventricle syndrome presenting with paroxysmal hypersomnia in an adult: case report.

The slit ventricle syndrome (SVS), defined as intermittent shunt malfunction without substantial ventricular enlargement, is usually observed in shunted children with small, slitlike ventricles. This syndrome has been attributed to recurrent obstruction of the ventricular catheter, which then causes an increase of intracranial pressure. Only rarely has the SVS been reported in adults. We describe a 29-year-old woman whose shunt malfunction presented with long-lasting paroxysmal hypersomnia and was diagnosed with computed tomographic evidence of small lateral ventricles. This episodic hypersomn
