Cesarean birth rate: small-geographic-area analysis.

Population-based analysis of cesarean section rates within 172 geographic areas in the Commonwealth of Massachusetts during fiscal year 1985 revealed a nearly normal distribution of observed-to-expected rates, implying that the forces that compel obstetricians to perform this surgical procedure are pervasive. However, a small number of areas were identified in which the number of procedures performed was significantly different from the state mean. During fiscal years 1982 to 1985, certain geographic subgroups consistently demonstrated incidence rates significantly greater than the state mean.

Small-area analysis of incidence and localisation of vulvar cancer.

Objective. Vulvar cancer is a rare disease mainly in older women. HPV and non-HPV induced vulvar cancer reflect two types of oncogenesis. Controversies exist on most recent developments in vulvar cancer incidence, patients, and disease characteristics. Changes in incidence, age of disease onset, and tumor site in women treated for primary vulvar cancer in a single German university hospital unit will be described. Methods. A retrospective analysis of patient records of women treated between 1994 and 2008 was performed. The fifteen-year-spanning period was divided into three five year-spanning

Small-area analysis: targeting high-risk areas for adolescent pregnancy prevention programs.

CONTEXT: Traditional methods of identifying areas in need of adolescent pregnancy prevention programs may miss small localities with high levels of adolescent childbearing. METHODS: Birthrates for 15-17-year-olds were computed for all California zip codes, and the zip codes with birthrates in the 75th percentile were identified. Panels of local experts in adolescent pregnancy reviewed these "hot spots" for accuracy and grouped them into potential project areas, based on their demographics, geography and political infrastructure. RESULTS: In all, 415 zip codes exceeded the 75th-percentile cut

Spatio-temporal analysis of small-area intestinal parasites infections in Ghana.

Intestinal parasites infection is a major public health burden in low and middle-income countries. In Ghana, it is amongst the top five morbidities. In order to optimize scarce resources, reliable information on its geographical distribution is needed to guide periodic mass drug administration to populations of high risk. We analyzed district level morbidities of intestinal parasites between 2010 and 2014 using exploratory spatial analysis and geostatistics. We found a significantly positive Moran's Index of spatial autocorrelation for each year, suggesting that adjoining districts have simila

[Ambulatory care-sensitive conditions in Germany: a small area analysis (2006-2009)].

PURPOSE: The paper aims (1) to identify and depict cartographically ambulatory care-sensitive conditions in Germany (based on data for the years 2006-2009) and (2) to discuss the implications. METHOD: The selection of ambulatory care-sensitive conditions (ACSC) was based on a literature review by Purdy et al. (2009) because a German catalogue of ACSC does not yet exist. Five of these indications were excluded due to limited data -access or a low number of cases. Additionally, 2 -diagnoses that are potentially relevant for Germany were included. Subsequently, diagnosis-specific hospitalisation

Extreme heat and hospital admissions in older adults: A small-area analysis in the Greater Boston metropolitan area.

Extreme heat (EH) is a leading cause of weather-related fatalities in the United States. In Massachusetts, average temperatures have increased by 1.9 °C since the 20th century, higher than the global average increase of 1.1 °C. EH disproportionately impacts communities, exacerbating social inequities. This study examined the risks of heat-related hospitalizations in each small area of the Greater Boston Metropolitan Area using Medicare data (2000-2016). EH events included daily heat index (HI), days with an HI above the 90th percentile, and heat waves (≥2 consecutive EH days). We applied a cas

[Faunistic analysis of the fleas on small mammals in the trans-Aral Sea area].

Materials on occurrence and abundance of 36 species of fleas on 17 species of small mammals are systematized. A great similarity between the faunas of ectoparasites of rodents and predators has been shown by means of special indices. The conception of parasitic field is formulated which implies the phenomenon of community of the fauna of plague vectors parasitic on its potential carriers. Some aspects of the formation of parasitic field and its possible effect the epizootic process are considered. It is established that the similarity between the faunas of fleas from different animals is ensur

Small-area contextual effects on children's dental caries in Alberta: a multilevel analysis.

OBJECTIVES: The objective of this study was to examine the presence of small-area-level effects on children's dental caries in Alberta, Canada, where dental public health programming is targeted in nature, based on an area-level measure of socio-economic circumstances. METHODS: This cross-sectional study included data on tooth decay (from an intra-oral examination conducted by dental hygienists at school) and socio-demographic and behavioural information (from a parent questionnaire) from 5677 grade 1 and 2 schoolchildren attending schools in public or Catholic school systems in Calgary and E

Risk adjustment for hospital use using social security data: cross sectional small area analysis.

OBJECTIVES: To identify demographic and socioeconomic determinants of need for acute hospital treatment at small area level. To establish whether there is a relation between poverty and use of inpatient services. To devise a risk adjustment formula for distributing public funds for hospital services using, as far as possible, variables that can be updated between censuses. DESIGN: Cross sectional analysis. Spatial interactive modelling was used to quantify the proximity of the population to health service facilities. Two stage weighted least squares regression was used to model use against su

A small area analysis of acute exposure to temperatures and mental health in North Carolina.

Increasing evidence suggests that temperatures adversely impact mental and behavioral disorders (MBD). This study explores the effects of temperatures on mental health outcomes using over 5.9 million MBD-related emergency department (ED) visits across three geographical regions of North Carolina (i.e., Mountains, Piedmont, and Coast) from 2016 to 2019. A distributed lag non-linear model (DLNM) with a generalized linear model and quasi-Poisson distribution adjusted for humidity, long-term seasonal time trends, and day of the week examined the acute impact (i.e., 7-day) of temperature on daily M

Small-area spatio-temporal analysis of cancer risk to support effective and equitable cancer prevention.

Cancer is rapidly increasing worldwide and urgent global action towards cancer control is required. Consistent with global trends, Canada is expected to experience a near doubling in new cases and cancer deaths between 2020-2040; population growth and ageing being the primary drivers. The projected increased cancer incidence and its associated costs is expected to further exacerbate socioeconomic inequities. Focused actions to prevent cancer, to detect it earlier when more treatable, and, to lower the risk of recurrence, must be prioritized. Almost half of all cancers are preventable, caused b

Role of individual and contextual effects in injury mortality: new evidence from small area analysis.

OBJECTIVE: To analyse the role of individual and contextual variables in injury mortality inequalities from a small area analysis perspective, looking at the data for the city of Barcelona (Spain) for 1992-98. SETTING: Barcelona (Spain). METHODS: All injury deaths in residents older than 19, which occurred in the period 1992-98 were included (n=4393). Age and sex specific mortality rates were calculated for each educational level and each cause of death (traffic injuries, falls, drug overdose, suicide, other injuries). The contextual variables included were the proportion of men unemployed,

A Bayesian unified framework for risk estimation and cluster identification in small area health data analysis.

Many statistical models have been proposed to analyse small area disease data with the aim of describing spatial variation in disease risk. In this paper, we propose a Bayesian hierarchical model that simultaneously allows for risk estimation and cluster identification. Our model formulation assumes that there is an unknown number of risk classes and small areas are assigned to a risk class by means of independent allocation variables. Therefore, areas within each cluster are assumed to share a common risk but they may be geographically separated. The posterior distribution of the parameter re

Small-area analysis of social inequalities in residential exposure to road traffic noise in Marseilles, France.

BACKGROUND: Few studies have focused on the social inequalities associated with environmental noise despite its significant potential health effects. This study analysed the associations between area socio-economic status (SES) and potential residential exposure to road traffic noise at a small-area level in Marseilles, second largest city in France. METHODS: We calculated two potential road noise exposure indicators (PNEI) at the census block level (for 24-h and night periods), with the noise propagation prediction model CadnaA. We built a deprivation index from census data to estimate SES a

Data-informed targets for suicide prevention: a small-area analysis of high-risk suicide regions in Australia.

PURPOSE: To investigate small-area variation in risks associated with suicide deaths across four regional communities in New South Wales, Australia, and to determine whether these areas have unique demographic and socioeconomic risk profiles that could inform targeted means restriction suicide prevention efforts. METHODS: Archival data on suicide mortality for all deaths in New South Wales, Australia, over the period 2006-2015 were geospatially attributed to four high-risk priority regions. Deaths in the four regions were compared to each other, and to NSW, on demographic factors, indicators

Small area analysis of HIV viral load suppression patterns in a high priority district (2012-2016), South Africa.

Globally, high viral load (VL) suppression rates are indicators of successful HIV treatment programs. Evaluation of these programmes at lower levels is likely to highlight variations that are masked at the provincial or national levels. This ecological study used routinely collected clinical and surveillance data on the HIV programme from 88 sampled Ekurhuleni wards. Between January 2012 and December 2016, 26 222 HIV VL tests for 2817 patients were conducted. We conducted a secondary analysis to determine the predictors of high VL suppression accounting for space and time random effects and es

Emergency Department Syndromic Surveillance to Monitor Tick-Borne Diseases: A 6-Year Small-Area Analysis in Northeastern Italy.

Tick-borne diseases (TBD) are endemic in Europe. However, surveillance is currently incomplete. Alternative strategies need to be considered. The aim of this study was to test an Emergency Department Syndromic Surveillance (EDSyS) system as a complementary data source to describe the impact of tick bites and TBD using a small-area analysis approach and to monitor the risk of TBD to target prevention. ED databases in the Local Health Authority 8 District (Veneto, Italy) were queried for tick-bite and TBD-related visits between January 2017 and December 2022. Hospitalisations were also collected

Changes in the Paneth cell population of human small intestine assessed by image analysis of the secretory granule area.

Estimates of the Paneth cell population in human jejunum and ileum were made using measurement of the granule area in micron2 by image analysis in a defined number of crypts. This figure was preferable to granule area per mm as there was a significant difference in crypts per mm between biopsies and surgical samples. In the jejunum no significant difference was found between normal children and adults with and without peptic ulcer. In adults with subtotal or partial villous atrophy the decrease in area was not statistically significant and there was no decrease in area in children with partial
