Tobacco smoking, harm reduction, and biomarkers.

The only known way to reduce cancer risk in smokers is complete cessation, but many smokers are unable or unwilling to quit. Consequently, tobacco companies are now marketing products that purport to reduce carcinogen exposure, with the implication that such products provide a safer way to smoke. Moreover, researchers are exploring ways to reduce the amount of cigarette smoke carcinogens to which the smokers are exposed. Although these methods are, in theory beneficial, it is possible that the perceived availability of "safe" ways to smoke will cause some former smokers to resume smoking and s

Heated tobacco product use frequency, smoking quit attempts, and smoking reduction among Mexican adult smokers.

INTRODUCTION: Heated tobacco products (HTPs) are promoted as less harmful than cigarettes; nonetheless, whether HTPs help smokers quit is uncertain. METHODS: Data from 4067 Mexican adult smokers surveyed longitudinally every four months (November 2019-March 2021) were analyzed. Mixed-effects multinomial models regressed HTP use frequency (no use=reference; monthly; weekly; and daily use) on sociodemographics and tobacco/nicotine-related variables. Among participants who completed at least two surveys (n=2900) over four months, the duration of their longest smoking quit attempt (SQA) between s

Self-initiated gradual smoking reduction among community correction smokers.

INTRODUCTION: Smoking remains the leading cause of preventable death in the United States. Many smoking cessation guidelines advise smokers to quit precipitately; however, most quit attempts involve a more gradual cessation. Characteristics of individuals who tend to reduce prior to quitting and the effectiveness of pre-quit reduction are not well understood. This study examined individual differences and smoking cessation outcomes between individuals who self-initiated gradual reduction in cigarettes per day (CPD) and those who did not reduce prior to quit date. METHODS: This study is a seco

Gastro-oesophageal reflux. Part 1: smoking and alcohol reduction.

INTRODUCTION: Gastro-oesophageal reflux disease (GORD) is defined as an abnormal reflux of the stomach contents into the oesophagus, which provokes symptoms and impairs the quality of life. GORD has a high prevalence and incurs costs to the healthcare system. This is the first paper in a series of three exploring the conservative, medical and surgical treatment of GORD. This first paper presents a review of the effect of smoking and alcohol on reflux symptoms and the impact of smoking and alcohol reduction on symptoms of GORD. METHODS: A search for English language studies on adults was condu

Relationship of DSM-IV-based depressive disorders to smoking cessation and smoking reduction in pregnant smokers.

This study investigated DSM-IV depressive disorders as predictors of smoking cessation and reduction in 81 pregnant smokers participating in a smoking cessation trial. Thirty-two percent of the sample met criteria for current dysthymia, major depressive disorder in partial remission, or minor depression. There was no significant reduction in smoking among women with or without current depressive disorders. Unexpectedly, as compared to women without depressive disorders, women with dysthymia significantly increased the mean number of cigarettes smoked (from 8 to 23 cigarettes per day during the

Perceived and internalized smoking stigma among pregnant women: association with smoking reduction, reactance to smoking health warnings, and intention to discuss smoking with health professionals.

BACKGROUND: Tobacco is an addictive substance associated with numerous serious health effects during pregnancy. Pregnant women who smoke face considerable social disapproval, which could be a barrier to seeking healthcare and ultimately to smoking cessation. This study explored how perceived stigma (i.e., smokers' beliefs about negative judgments of them) and internalized stigma (i.e., internalization of stigmatizing attitudes toward them) may be associated with smoking reduction during pregnancy, reactions to smoking health warnings, and the intention to discuss smoking with health profession

Smoking cessation and reduction in people with chronic mental illness.

The high prevalence of cigarette smoking and tobacco related morbidity and mortality in people with chronic mental illness is well documented. This review summarizes results from studies of smoking cessation treatments in people with schizophrenia, depression, anxiety disorders, and post-traumatic stress disorder. It also summarizes experimental studies aimed at identifying biopsychosocial mechanisms that underlie the high smoking rates seen in people with these disorders. Research indicates that smokers with chronic mental illness can quit with standard cessation approaches with minimal effec

Skin blood flow reduction induced by cigarette smoking: role of vasopressin.

The effect of vasopressin released by cigarette smoking on blood pressure (BP), heart rate (HR), and skin blood flow (SBF) was investigated in 12 normotensive habitual smokers. At a 1-wk interval, each subject smoked within 10 min two cigarettes before and after intravenous injection of either the specific vascular vasopressin antagonist d(CH2)5Tyr(Me)AVP (5 micrograms/kg) or its vehicle administered in double-blind fashion. SBF was assessed with a laser Doppler flowmeter. Smoking increased plasma vasopressin (P less than 0.01). In six subjects subsequently treated with the antagonist, plasma

Effect of Smoke Evacuator on Reduction of Volatile Organic Compounds and Particles in Surgical Smoke: A Randomized Controlled Trial.

BACKGROUND: Surgical smoke is an occupational health problem and is increasingly recognized as a potential source of virus transmission. Dedicated smoke evacuators are used to protect against surgical smoke exposure. We tested the hypothesis that using smoke evacuators would reduce volatile organic compounds and the number of particles in surgical smoke during the laparotomy procedure. STUDY DESIGN: A randomized, double-blind clinical trial was conducted in patients undergoing laparotomy from June 11, 2021, to March 30, 2022, to compare the effectiveness of smoke evacuators with a control (re

The impact of smoking reduction on food demand in people with excess weight.

Smoking cessation is commonly associated with increased food intake and weight gain, representing a substantial barrier to quitting, particularly for individuals with overweight or obesity. This study aimed to uncover the behavioral economic mechanisms underpinning this phenomenon by analyzing changes in food demand during smoking cessation treatment among people with overweight or obesity. We also explored whether baseline tobacco use and eating behaviors predicted these changes. In an eight-week clinical trial, 120 adults with tobacco use disorder and overweight or obesity received treatment

Cardiorespiratory hospitalisation and mortality reductions after smoking bans in Switzerland.

INTRODUCTION: Smoking bans are considered one of the most effective policies to reduce population exposure to tobacco smoke and prevent adverse health outcomes. However, evidence on the effect of contextual variables on the effectiveness of smoking bans is still lacking. AIMS: The patchwork of cantonal smoke-free laws in Switzerland was used as a quasi-experimental setting to assess changes after their introduction in: hospitalisations and mortality due to cardiorespiratory diseases in adults; total hospitalisations and hospitalisations due to respiratory disorders in children; and the modify

Reduction of EGF is associated with the delay of ulcer healing by cigarette smoking.

Cigarette smoking is associated with peptic ulcer diseases. Smokers have lower levels of salivary epidermal growth factor (EGF) than nonsmokers. We investigated whether reduction of EGF is involved in the delay of gastric ulcer healing by cigarette smoking. Rats with acetic acid-induced ulcers were exposed to cigarette smoke (0, 2, or 4% vol/vol) 1 day after ulcer induction. EGF level was elevated 1 day after ulcer induction in salivary glands and serum, and 4 days after ulcer induction in the gastric mucosa. However, cigarette smoke depressed these beneficial effects and EGF mRNA expression i

Closing the Harm Reduction Gap: EMPOWERing People Who Smoke.

Global tobacco control efforts, influenced substantially by the World Health Organization Framework Convention on Tobacco Control and the MPOWER package, have lowered smoking prevalence in some populations but have failed to address the lived realities of the people who continue to smoke. These individuals increasingly come from communities marked by socioeconomic disadvantage, lower educational attainment, and limited access to healthcare, contexts where structural constraints, chronic stress, and reduced opportunity shape tobacco use far more powerfully than individual "choice." Yet tobacco

Brief Advice on Smoking Reduction Versus Abrupt Quitting for Smoking Cessation in Chinese Smokers: A Cluster Randomized Controlled Trial.

AIMS: To compare the efficacy of brief advice about cut-down-to-quit (CDTQ) with that of brief advice about quit immediately (QI), as delivered by trained volunteers, without the use of pharmacological therapy, to outreach-recruited Chinese smokers in Hong Kong who intend to quit smoking. METHODS: Smokers (N = 1077) who enrolled in the Quit and Win Contest 2014 and intended to quit or reduce smoking were randomized in participation sessions to CDTQ (n = 559) and QI (n = 518) groups. Subjects in the CDTQ group received brief advice and a card about smoking reduction. Subjects in the QI group r

Craving Reduction and Smoking Outcomes in Psilocybin-Assisted and Standard Cessation Care.

OBJECTIVE: Smoking cessation outcomes are heterogeneous, and factors associated with these differences are not fully understood. We examined longitudinal smoking trajectories across distinct smoking profiles and evaluated whether early craving and affective symptom changes were associated with verified abstinence. METHODS: Participants in this observational cohort were classified as dependence-predominant or affective-predominant using measures of smoking severity and affective symptoms. Abstinence was defined as 7-day self-reported abstinence verified by exhaled CO <8 ppm. RESULTS: Psilocyb
