Heavy users of an emergency department: psycho-social and medical characteristics, other health care contacts and the effect of a hospital social worker intervention.

A small proportion of the patients coming to emergency departments of general hospitals account for a substantial share of the department's resources by making repeated visits. Repeater behaviour is a complex product of many, sometimes concurrent factors. This study has focused on the medical and psycho-social factors. A group of patients with repeated visits to an emergency department of a middle-sized Stockholm hospital was studied in 1980. The repeater group had a heavier load of psycho-social problems than the source population. The repeater behaviour profile included: feeling of lonelines

Treating tuberculosis: the essential role of social work.

The resurgence of tuberculosis gives cause for alarm. Compliance with an effective medication regimen leads to cure; yet, many patients fail to comply. This paper discusses factors affecting compliance and the essential role of social work, from an ecological perspective, when intervening to increase compliance. The authors relate comments made by patients at the only hospital originally and still serving as a TB sanatorium in relation to literature on compliance. Changes at the hospital include the development of a social work department which uses the ecological perspective in working with p

The experience and preparation of pediatric sibling bone marrow donors.

To assist sibling bone marrow donors with the psychological and emotional distress that they may experience as donors, a sibling bone marrow donor program was developed at The Hospital for Sick Children. These donors feel overwhelming responsibility for their siblings' survival, which can lead to psychological distress. The donors are engaged in age-appropriate medical play and are encouraged to discuss their feelings and concerns about their role. After the marrow harvest, donors receive a certificate, and either they or their parents evaluate the program. Thus far, 97.5% have rated the progr

[Medical response to precariousness and poverty].

If there is a medical response to precariousness and extreme poverty, there is no illness that is specific of the poor. Diseases that are commonly found here result from the state of being homeless and often addicted. One possible medical response to the problem is to establish specialized institutions as, in Paris, the Nanterre hospital, the Social SAMU (Service d'aide médicale urgente) medical dormitories, and several outpatient clinics for precarious people. The present debate on a "two-tiers medicine" must be considered with pragmatism. Specific structures have to be imagined like places f

Interdisciplinary expectations of the medical social worker in the hospital setting.

Nearly 500 physicians, nurses, and social workers affiliated with four general hospitals participated in a mailed questionnaire survey designed to compare interprofessional expectations of the medical social worker role in the hospital. As hypothesized, physicians and nurses expect the exclusive role of the social worker to be more limited to working with families, resolving social-environmental problems, and providing referrals than social workers expect of their role. In general, physicians and nurses do not disagree with medical social workers on what social workers expect to do in the hosp

Clinical social work in a medical setting.

This paper, using case vignette and discussion, will elaborate on two specific functions of the clinical social worker practicing in a medical setting. The first, helping patient, family and staff to acknowledge the psychological impact of physical illness; the second is helping a family to accept that there may be no organic etiology for a symptom, and that the symptom may be psychogenic in origin. Barriers to performing these functions are discussed as well as reasons why the social worker is in an ideal position to integrate medical and psychological information into a coherent and practica

The Abortion and Sterilization Act in 1975--experience of the Johannesburg Hospital Pregnancy Advisory Clinic.

The experience of a Pregnancy Advisory Service operating in terms of the Abortion and Sterilization Act of 1975 is reviewed. A total of 454 applications for abortion were considered, of which 198 (44%) were accepted and the abortions performed. The urgent need for sex education at schools, colleges and universities is stressed, in view of the large number of scholars and students under the age of 17 who seek an abortion. There are also patients who discontinue contraception for a variety of reasons, on medical advice or otherwise, without an alternative being suggested or sought. Motivation of

Stop the escalation before it begins by using the pediatric Behavior Response Team protocol.

In today's world, clinicians need to be prepared to care for challenging patients and families that are struggling with the stress of illness and hospitalization and have inadequate coping skills. The University of Michigan Health System (UMHS) has developed a protocol identifying a team with representatives from psychiatry, security, and risk management to provide a rapid response in situations that historically have resulted in, at worst, sentinel/adverse events and at best, service disasters. The pediatric BRT protocol formalizes the purpose of the team, how staff should access them, and th

Respite care for HIV-affected families.

Anonymous testing of all newborn infants in 1988 revealed that one of every 61 babies born in New York City was HIV positive (New York City Department of Health, 1988). With the availability of new treatment, HIV positive children are living longer, often into their school years, and most are able to live at home. The majority of children acquire HIV through maternal transmission. Frequently, multiple family members are HIV positive. Parents, or other caregivers, are overwhelmed by depression, anxiety and grief, and urgently need childcare assistance. Finding childcare is difficult since famil

Everyone's business: developing an integrated model of care to respond to child abuse in a pediatric hospital setting.

In pediatric hospitals, social work plays a central role in the prevention, identification, and management of child abuse. Children who are suspected of having been abused or neglected require an evaluation of their psychosocial situation. As an integral member of the health care team, the social worker is well placed to undertake comprehensive psychosocial assessments including information on the child's development, parental capacity, family, and community supports. Current practice approaches have seen a shift away from a narrow, "expert" approach to child protection. This article describes

Improving access, efficacy, and cost effectiveness of alcohol rehabilitation for poor patients.

Numerous alcoholics and alcohol abusers unknown to other agencies enter hospitals for treatment of alcohol-related illnesses. Two well-constructed studies focusing on poor clients enrolled in alcohol rehabilitation programs do not duplicate the long-term medical cost savings typical for higher income populations. Study limitations are indicated. Alcohol rehabilitation might avert or reduce potentially even higher long-term costs for poor clients as well as a host of social costs. It is important to demonstrate and improve the accessibility, efficacy, and cost effectiveness of alcohol treatment

Evaluating an activity intervention with hemodialysis patients in Israel.

As a life-threatening and potentially disabling disease, End Stage Renal Disease and its treatment cause stress as well as other psychosocial problems for patients and their families. This paper examines the results of an innovative activity-based intervention aimed at reducing some of the psychosocial repercussions of hemodialysis. A modified withdrawal/reversal design was employed to compare patients participating in the intervention and those who did not, at two points in time. The findings confirmed that dialysis patients in general have relatively high levels of psychological distress, di

Domestic violence documentation project 2012.

One in four women presenting to Emergency Departments in Australia have experienced domestic violence in their lives but there are no specialist services for victims of domestic violence in the state of New South Wales, population of 7.25 million. Fundamental forensic medical and nursing skills developed for the comprehensive assessment of complainants of sexual assault were utilised in the examination of victims of domestic violence in a trial project at Nepean Hospital, Sydney. The project was then reviewed via a series of qualitative patient and police interviews along with an analysis of c

Geriatric care in the United Kingdom: an American perspective.

Geriatric care is a complex and challenging aspect of health care. The United Kingdom, with 13.6% of its population over 65 years of age, compared to 10.1% in the United States, has given special attention to this field. Characteristics of the British system include financing through the National Health Service, existence of a specialty of geriatrics, absence of "nursing homes," emphasis on home care, and avoidance of "heroics" for the terminally ill. While British life expectancy at birth is longer than that for Americans, the opposite is now true for those over 65. The British have made effo

Social services referral of adolescent trauma patients admitted following alcohol-related injury.

Alcohol screening of severely injured patients should be universal. Hospitalization following alcohol-related injury provides an opportunity for intervention to reduce recidivism. This study examines the frequency of social services referral of an alcohol positive cohort of adolescent trauma patients. This was a retrospective analysis of data collected from 1994 through 1998 by the National Pediatric Trauma Registry. All patients between the ages of 12 and 17 who had a blood alcohol level (BAL) measured were analyzed. Patients receiving referral to the department of social services, family cou
