The response of the hospital doctor to the health requirements of a changing society.

Before the advent of the National Health Service in 1948 most hospital doctors were in close contact with the primary care of, at least, a section of the community both by their continuing close contacts with general practitioners and as a result of often having spent some part of their careers as general practitioners. Since 1948 this awareness of the health aspirations of the community has weakened as a result of advances in the care of disease which have made the hospital doctor increasingly dependent on sophisticated facilities and, thus, focused his attention on hospital care rather than

Recommendations on pre-hospital & early hospital management of acute heart failure: a consensus paper from the Heart Failure Association of the European Society of Cardiology, the European Society of Emergency Medicine and the Society of Academic Emergency Medicine.

Acute heart failure is a fatal syndrome. Emergency physicians, cardiologists, intensivists, nurses and other health care providers have to cooperate to provide optimal benefit. However, many treatment decisions are opinion-based and few are evidenced-based. This consensus paper provides guidance to practicing physicians and nurses to manage acute heart failure in the pre-hospital and hospital setting. Criteria of hospitalization and of discharge are described. Gaps in knowledge and perspectives in the management of acute heart failure are also detailed. This consensus paper on acute heart fail

Should the geriatrician host the party? Person-centred hospital care in ageing societies requires placing geriatrics at the centre of care.

As populations age, multimorbidity and frailty increasingly characterize hospital admissions, exposing the limits of traditional disease-specific and organ-centred models of care. Older adults with complex care needs face higher risks of hospitalization, readmission, functional decline, and fragmented care pathways, while hospitals remain insufficiently aligned with these realities. Despite strong evidence for the clinical and economic benefits of Comprehensive Geriatric Assessment (CGA) and geriatric-led care, geriatrics often remains peripheral to everyday clinical practice and hospital gove

The British Rhinological Society multidisciplinary consensus recommendations on the hospital management of epistaxis.

OBJECTIVE: Epistaxis is a common ENT emergency in the UK; however, despite the high incidence, there are currently no nationally accepted guidelines for its management. This paper seeks to recommend evidence-based best practice for the hospital management of epistaxis in adults. METHODS: Recommendations were developed using an Appraisal of Guidelines for Research and Evaluation ('AGREE II') framework. A multifaceted systematic review of the relevant literature was performed and a multidisciplinary consensus event held. Management recommendations were generated that linked the level of support

Introduction of a board certification system for the Japanese Society of General Hospital Psychiatry.

The importance of board certification systems is increasing in parallel with changes in the social conditions surrounding general medical practice. The same is also the case for consultation-liaison psychiatry, a subspecialty of psychiatry. As a consequence, in April 2001 a board certification system for the Japanese Society of General Hospital Psychiatry was introduced. The clinical abilities required for liaison psychiatrists in this system can be summarized as follows: the ability to adequately treat patients with physical/psychiatric comorbidity or somatization, the ability to form an appr

Differences in management of bronchiolitis between hospitals in The Netherlands. Dutch Paediatric Respiratory Society.

UNLABELLED: To study the variation in management of acute bronchiolitis in the Netherlands, a questionnaire on the use of diagnostic and therapeutic procedures and prescription of drugs after discharge was mailed to all 110 hospital-based paediatric practices in the Netherlands. A 100% response rate was achieved. There was a great deal of variation in management of bronchiolitis between respondents. The most commonly applied diagnostic procedures were immunofluorescence staining of nasopharyngeal secretions for respiratory syncytial virus (100% of respondents), blood gas analysis (93%), leucoc

Management of opioid use disorder and associated conditions among hospitalized adults: A Consensus Statement from the Society of Hospital Medicine.

Hospital-based clinicians frequently care for patients with opioid withdrawal or opioid use disorder (OUD) and are well-positioned to identify and initiate treatment for these patients. With rising numbers of hospitalizations related to opioid use and opioid-related overdose, the Society of Hospital Medicine convened a working group to develop a Consensus Statement on the management of OUD and associated conditions among hospitalized adults. The guidance statement is intended for clinicians practicing medicine in the inpatient setting (e.g., hospitalists, primary care physicians, family physic

Improving the Safety of Opioid Use for Acute Noncancer Pain in Hospitalized Adults: A Consensus Statement From the Society of Hospital Medicine.

Hospital-based clinicians frequently treat acute, noncancer pain. Although opioids may be beneficial in this setting, the benefits must be balanced with the risks of adverse events, including inadvertent overdose and prolonged opioid use, physical dependence, or development of opioid use disorder. In an era of epidemic opioid use and related harms, the Society of Hospital Medicine (SHM) convened a working group to develop a consensus statement on opioid use for adults hospitalized with acute, noncancer pain, outside of the palliative, end-of-life, and intensive care settings. The guidance is i

Management of diabetes in patients hospitalized for acute cardiac events: Joint position paper from the French Society of Cardiology and the French-speaking Diabetes Society.

Patients with type 2 diabetes, but also older patients with type 1 diabetes, are at major risk of cardiovascular morbidity and death. After an acute cardiac event, the prognosis of patients with diabetes is impaired, with clear increases in in-hospital and long-term morbidity and deaths. Both hyper- and hypoglycaemia are deleterious after an acute cardiac event, and the decision to start intravenous insulin is often challenging. Moreover, some antidiabetic treatments have cardioprotective effects, and the onset of an acute cardiac event provides an opportunity to shift to these treatments. The

Hospital-to-community care pathways for patients with heart failure, an inter-society consensus document.

Heart failure (HF) remains one of the most prevalent and burdensome chronic conditions worldwide, with rising incidence and poor prognosis despite major therapeutic advances. Effective management requires a comprehensive, multidisciplinary approach that ensures diagnostic accuracy, therapeutic optimization, and continuity of care from hospital to community settings. This consensus document outlines an integrated model for HF care in Italy, developed to improve patient outcomes and resource efficiency. HF should be diagnosed and phenotyped through clinical evaluation, electrocardiography, echoc
