Esophageal speech: double-contrast evaluation of the pharyngo-esophageal segment.

Alaryngeal voice is usually accompanied by esophageal speech; however, about 40% of laryngectomy patients are unable to achieve adequate esophageal speech and must rely on mechanical devices for communication. A technique was developed for performing double-contrast studies of the hypopharynx, cervical esophagus, and pharyngo-esophageal segment using thick barium and the air normally injected for speech. Simultaneous audio and video recordings obtained during esophageal speech allowed correlation of the quality of speech with the motion of the pharyngo-esophageal segment. In 35 patients with v

Predictive value of esophageal motility test in the proficiency of esophageal speech.

One method of speech rehabilitation following total laryngectomy surgery is esophageal speech therapy (EST). In this method, which has witnessed relatively low success rates at the end of therapy, identification of patients who can benefit from EST beforehand will be important for determining the appropriate method for alaryngeal speech rehabilitation and might be cost-effective, saving time and labor. To this end, this study conducted research on the feasibility of manometric data measured prior to therapy using an esophageal motility test (EMT) in order to determine the candidates most suita

[Clinical aspects of esophageal speech (author's transl)].

The quality of esophageal voice and speech after laryngectomy depends on several factors. Negative influences are produced by irradiation, resection of the hyoid bone and radical neck dissection. The factors important in esophageal speech were analyzed, with consideration given to the functional morphology of the pseudoglottis and the reconstructed hypopharynx. Phoniatric, laryngologic and radiologic aspects are considered.

[Voice rehabilitation with esophageal speech in the laryngectomized].

From January 1988 to December 1990, 186 patients with laryngeal cancer or hypopharyngeal cancer underwent total laryngectomy in our hospital. We studied voice restoration in 110 of these patients by means of a direct interview method at the end of 1991. Seventy-six cases were excluded because of death (70) or being lost to follow-up (6). About 80% of the patients (89/110) attended esophageal speech classes, with a success rate of 78% (69/89). Seventy-four percent (51/69) had no problems with daily oral communication using esophageal speech. Neither radiation therapy nor post operative pharyngo

Acoustic differentiation of laryngeal, esophageal, and tracheoesophageal speech.

This investigation was designed to determine if a multivariate acoustic classifier could effectively discriminate group membership for 15 tracheoesophageal, esophageal, and laryngeal speakers. Seven intensity, 10 frequency, and 13 duration measures were quantified from recorded voice samples. Using principal components analysis, a subset of the 13 least redundant acoustic and temporal measures was systematically selected from the 30 original measures and analyzed singly and jointly in terms of its ability to discriminate among the three speaker groups. Discriminant function analysis revealed p

Has Esophageal Speech Returned as an Increasingly Viable Postlaryngectomy Voice and Speech Rehabilitation Option?

PURPOSE: The literature on postlaryngectomy voice and speech rehabilitation is long-standing. Although multiple rehabilitation options have existed over the years, the acquisition and use of esophageal speech (ES) has decreased significantly over the past 40 years. This reduction coincides with the increased application of tracheoesophageal puncture (TEP) voice restoration. The literature suggests that voice acquisition failures observed secondary to TEP may represent a similar phenomenon that led to ES acquisition failures. METHOD: A comprehensive review of the literature on ES and TEP voice

Pitch control in esophageal and tracheoesophageal speech of Cantonese.

OBJECTIVE: Previous studies of English-speaking esophageal (SE) and tracheoesophageal (TE) speakers revealed a significantly lower voice fundamental frequency (F0) than normal laryngeal (NL) speakers. Studies of SE and TE speakers of a tone language, however, indicated discrepant findings. Tonal SE and TE speakers could produce comparable or even higher F0 than NL speakers. The present study examined the F0 characteristics associated with speech and nonspeech tasks produced by Cantonese SE and TE speakers. SUBJECTS AND METHODS: Speech produced by 15 superior SE, 15 superior TE and 15 NL speak

[Esophageal morphology with ultrasound imaging after laryngectomy: effect on quality of esophageal speech].

PURPOSE: In patients with aerodigestive cancer, laryngectomy places the esophagus in a superficial position, allowing easy ultrasound examination of this organ. The purpose of the present study was to assess oesophagus morphology after laryngectomy using echotomography and to compare this morphology with voice quality. MATERIAL AND METHODS: In the present study, ultrasonography was performed in 28 patients (delay from surgery: 3 +/- 3 years): 15 were operated using laryngectomy technique (LT) and 13 with pharyngolaryngectomy technique (PLT). Neural Correlates of Esophageal Speech: An fMRI Pilot Study.

OBJECTIVES: The esophageal speech is one of the possible alaryngeal voices resulting after total laryngectomy. Its production is made by the regurgitation of the air coming from the esophagus, sonorized through the passage from the walls of the upper esophageal sphincter. The neural correlates of this voice have never been investigated, while the neural control of laryngeal voice has been already documented by different studies. METHODS: Four patients using esophageal speech after total laryngectomy and four healthy controls underwent functional magnetic resonance imaging. The fMRI experiment

[Speech rehabilitation following total laryngectomy with tracheo-esophageal puncture].

Speech rehabilitation after total laryngectomy has often been neglected, and traditional methods of esophageal voice production are frequently unsuccessful or disappointing. The development of the tracheoesophageal puncture (TEP) technique and the valved voice prosthesis by Singer and Blom in 1979 offered patients an alternative method for laryngeal speech rehabilitation. This technique is now used successfully in an increasing number of patients after total laryngectomy. We present our experience with TEP since 1988 in 8 men and 1 woman, aged 50-86 years, who had adequate follow-up. In 5, pri

[Comparative radiologic studies of artificial esophageal speech of laryngectomized patients].

Radiographs were taken of the pharyngo-oesophageal segment (PE segment) of 19 laryngectomees, 16 of whom had oesophageal speech. The voice production was studied by X-ray video-tape. Oesophageal speakers with a good voice showed a much wider upper oesophageal (subpseudoglottic) space and more prominent or round PE segments than poor speakers. Video recordings showed that good oesophageal speakers develop motion within the voice-producing system, characterized by maximum air intake and by active retrograde contractions of the oesophagus. In contrast, these structures showed little functional ad

Relationships between esophageal speech proficiency and surgical, biographical, and social factors.

The relationship between judged esophageal speech proficiency and detailed medical/surgical, biographical, and social factors was investigated among a large sample of alaryngeal speakers. The data suggested that more extensive surgery such as radical neck dissection did not lessen the subjects' ability to learn esophageal speech. Surprisingly, subjects who had undergone radical neck surgery had better voices in many cases than those who had undergone only a simple laryngectomy. Educational level, socioeconomic status, length of time spent in speech therapy, and the number of years since surger

Verbal performance of total laryngectomized patients rehabilitated with esophageal speech and tracheoesophageal speech: impacts on patient quality of life.

INTRODUCTION: Total laryngectomy has important psychophysical and social consequences for patients' quality of life because of the functional changes resulting from the removal of the larynx. Voice deprivation is perhaps the most relevant limiting factor in social relationships, increasing feelings of solitude and tending to drive individuals into social isolation. Multiple voice rehabilitation methods after total laryngectomy are available. This study aimed to determine the acoustic quality of the rehabilitated voice achieved with esophageal speech (ES) and tracheoesophageal speech (TES), and

Articulatory Contact Pressure during Bilabial Plosive Production in Esophageal and Tracheoesophageal Speech.

OBJECTIVES: The present study investigated the articulatory contact pressure during the production of bilabial plosives by esophageal (ES), tracheoesophageal (TE), and laryngeal speakers. METHODS: The peak contact pressure (PCP) during bilabial plosive production of /CVCVCVCVCV/ syllable strings of /p/ and /ph/ was obtained from 10 ES, 10 TE, and 10 laryngeal speakers of Cantonese. PCP values were obtained by using a pressure transduction system (Iowa Oral Performance Instrument) during speech production. RESULTS: The results showed that ES speakers exhibited a significantly greater PCP valu

The Role of Speech and Language Therapy in Assessing and Managing Pharyngo-esophageal Diverticula.

This study explores the contribution of Speech and Language Therapists (SLTs) to the assessment and management of patients presenting on videofluoroscopic swallow studies (VFSS) with a suspected pharyngo-oesophageal diverticulum. Records for all patients who attended for VFSS in an acute hospital over an eleven-year period were examined (N = 1820). Twenty patients were identified on VFSS as having a suspected diverticulum. Symptoms suggestive of a diverticulum were found during both bedside clinical examination and radiographic examination e.g. respiratory difficulties (n = 15; 75%), voice cha

Novel esophageal speech therapy method in total laryngectomized patients: biofeedback by intraesophageal impedance.

The loss of the best communication port after total laryngectomy surgery makes speech rehabilitation an important goal. Our aim was to improve the quality of esophageal speech (ES) using online esophageal multichannel intra-luminal impedance (MII) as a new biofeedback method. Twenty-six total laryngectomized patients were included. Before ES therapy, an esophageal motility test was carried out. MII catheters were placed in all subjects who were then randomized into two groups. Group 1 included 13 cases, who were retrained according to the classical method. Group 2 included 13 cases, who were r

[Effect of the extents of surgical resection in total laryngectomy on esophageal voice and speech rehabilitation].

The study of functional results of total laryngectomy cannot be separated from articulation of esophageal voice and speech. To achieve success of phoniatric rehabilitation in laringectomized patients, it is necessary to be well aware of pathological sequelae of the very operation, long-term adverse effect of oncological risk factors as well as postoperative anatomo-physiological changes. Anatomic results of total laryngectomy depend on the type of surgical intervention and probable radiotherapy. The extent of surgical procedure, primary dictated by oncological indications, has the essential ef
