Spinal cord ischemia is a rare disease with a poor prognosis. As with cerebral ischemia, spinal infarction typically presents with sudden symptom onset and variable clinical manifestation (depending on the lesion location). The majority of spinal ischemias occur in the supply territory of the anterior spinal artery, involving the anterior and middle third of the myelon. The most common cause of spinal ischemia is an arterial vessel occlusion, although venous ischemia and hypotensive watershed infarcts may also occur. Magnetic resonance imaging (MRI) represents the modality of choice for diagno

A spinal cord injury leads to disturbances of sensory and motor signals due to the damage to white matter and myelinated fiber tracts. Moreover, the damage to gray matter causes segmental loss of interneurons of dorsal horn and motoneurons and restricts the therapeutic options. Neuroprotective strategies have the potential to improve the neurological outcome of patients. To achieve this, concerns to anesthetics or analgesics as neuroprotective interventions have been accumulating to explore neuroprotection during perioperative period. This review includes consideration of: 1) basic concepts of

INTRODUCTION: Vascular pathology is considered to be uncommon, with a much lower incidence than cerebral vascular pathology.
PATIENTS AND METHODS: We reviewed the clinical histories of patients with a clinical diagnosis of spinal cord infarction-ischemia attended by the Neurology Department and the Neuropaediatric section at our centre between 1990 and 1997. We analyzed the clinical and investigational findings and compared them with those in the literature.
RESULTS: We studied the cases of 12 patients (nine adults and three children). There was only one case in which no etiological factor o

This article discusses the etiology of spinal cord ischemia (SCI) mechanisms that may lead to paraplegia during open and endovascular repair from an anatomical and physiological perspective as well as the role of various protective measures used in prevention of this dreadful complication of aortic surgery. There are many adjuncts that must be considered to reduce the risk of spinal cord injury, such as revascularisation of intercostal arteries, maintenance of high mean blood pressure, spinal cord drainage and a few new promising models like NIRS and MISACE which usefulness is yet to be determ

Subsequent to cardiac arrest, a 58-year-old man with intractable dysrhythmia and severe arteriosclerosis developed flaccid paraplegia, depressed deep tendon reflexes, and showed no pain or temperature sensation caudal to Th-7 in spite of completely intact proprioception and vibration sensation. An echocardiogram showed no clots or vegetation on the prosthetic valve and no thrombus in the left atrium or left ventricle. The patient's paraplegia was permanent, at least through a follow-up period of 2 years. These findings suggest that the etiology was spinal cord ischemia due to blood supply in t

Descriptive signs in radiology can aid in easier pattern recognition and quicker diagnosis. In spinal cord ischemia, paired anterior-horn T2-hyperintensities have traditionally been known as the "owl's eyes" or "snake eyes" sign. We discuss how these signs, while visually apt, convey no pathophysiologic context and propose renaming this finding the "snake bite sign". The image still evokes two punctate marks, yet the metaphor extends to a snake bite (two fang-like dots) rather than two bright foci (eyes) staring back at the viewer. Moreover, besides the sign metaphorically resembling a traumat

A 44-year-old male presented with symptoms of spinal cord compression secondary to metastatic prostate cancer. An urgent decompression at the cervical-thoracic region was performed, and there were no complications intraoperatively. Three hours postoperatively, the patient developed acute bilateral lower-limb paralysis (motor grade 0). Clinically, he was in class 3 hypovolemic shock. An urgent magnetic resonance imaging (MRI) was performed, showing no epidural hematoma. He was managed aggressively with medical therapy to improve his spinal cord perfusion. The patient improved significantly, and

Diagnosis of myelopathies of vascular origin is difficult and they are probably underdiagnosed at this time because of the lack of diagnostic tools. A recent report of a 58 year old patient who developed ASAS after an episode of cardiac arrest pointed out the importance of MRI and somatosensory evoked potentials (SEP) to support the diagnosis. MRI with T2 weighted imaging demonstrated diffuse signal abnormalities in both gray matter and surrounding white matter below T7. Furthermore, SEP latencies showed a delay between T6 and T7. Therefore, new technologies including MRI and SEP may improve t

A 62-year-old man had an acute, transient, flaccid paraplegia. Examination showed a primary cardiac tumor with emboli to major branches of the aorta. A myxoma was removed from the left atrium, and normal function returned. Left atrial myxoma should be suspected as a cause for embolism to the CNS.

PURPOSE: We identified urodynamic patterns of lower urinary tract dysfunction in children after anterior spinal artery injury.
MATERIALS AND METHODS: Between 1981 and 1993, 6 boys and 1 girl in whom ischemic spinal cord injuries developed after umbilical artery catheterization (4), cardiovascular surgery (2) and spontaneous bleeding from an arteriovenous fistula (1) were evaluated radiologically, neurologically and urodynamically.
RESULTS: Neurological assessment revealed a motor level from T8 to S1 in all 7 children, whereas only 3 had sensory denervation ranging from T8 to L4. Urodynamic s

Coronary artery bypass graft (CABG) surgery is associated with a low risk of paraplegia. Spinal cord ischemia causing paraplegia following off-pump coronary artery bypass (OPCAB) surgery has not been described previously. We now describe a patient who developed paraplegia following OPCAB and review the literature on spinal cord ischemia following CABG surgery.

Neurological complications after lumbar spine manipulation are uncommon. The cause is usually a herniated disk or displaced bony structure. We report a case of paraplegia that developed a few hours after manipulation of the lumbar spine. Magnetic resonance imaging was consistent with ischemia of the caudal spinal cord. No disk fragment or bony structure impinging on the spinal cord was seen. Spinal cord ischemia may deserve to be added to the list of possible adverse events after lumbar spine manipulation.

Morbidity of infrainguinal bypass surgery is mostly related to cardiac complication, graft thrombosis, and wound infections. Neurologic complications in these patients are unusual and generally related to traumatic peripheral nerve injury or ischemia. Anterior spinal cord ischemia, manifest clinically as flaccid paraplegia or lower extremity paraparesis, is a complication usually associated with aortic surgery. Reported here is a case of an 81-year-old male who developed spinal cord ischemia after infrainguinal bypass under epidural anesthesia.

We studied 9 patients with clinical evidence of spinal cord ischemia or infarction who had altered vesicourethral function. Of the patients 6 had some preservation of bladder and lower extremity sensation, a clinical pattern consistent with the anterior spinal artery syndrome. A total of 3 patients had a clinical picture more consistent with complete transverse myelopathy, since they had no preservation of bladder or lower extremity sensation. Despite sensory levels well above the sacral spinal cord segments, 6 patients had detrusor areflexia (4 with bethanechol supersensitivity and 4 with neu

OBJECTIVE: This study tested to see if trehalose, a cytoprotective disaccharide, protects against spinal cord ischemia in a rabbit model.
METHODS: The infrarenal aorta was mobilized in four groups of 10 rabbits. In groups I, II, and III, it was clamped proximally and distally for 20 minutes. In group I, the clamped aorta was infused at 2.5 L/min for 2 hours with lactated Ringer's (LR) solution. In group II, the clamped aorta was infused with 5% trehalose in LR. LR was administered intravenously (2.0 mL/min) in groups I and II starting 30 minutes before clamping. In group III, 5% trehalose in

The pressure difference between mean distal aortic pressure (MDAP) and cerebrospinal fluid pressure (CSFP), defined as relative spinal cord perfusion pressure (SCPP), and somatosensory evoked potentials (SEP) were monitored intraoperatively to detect and prevent intraoperative ischemic spinal cord injury in 19 patients who required cross-clamping of the descending thoracic aorta. Temporary axillo-femoral shunt utilizing a 10 mm woven Dacron tube graft was employed in 10 patients and partial cardiopulmonary bypass in nine. Ischemic SEP changes were seen in five patients. Two patients, whose SCP

The relationship between the evoked spinal cord potential (ESP) and the histological findings of the spinal cord after thoracic aortic cross-clamp was studied. Thoracic aorta was cross-clamped in 23 dogs and ESP was monitored before, during, and after cross-clamping. Incidence of paraplegia and histological findings were studied after the dogs recovered from the procedure. Aortic cross-clamp was maintained for 60 minutes in 20 dogs (Group A). And cross-clamp was released 10 minutes after the amplitude of ESP became lower than 20% of control in 3 dogs. (Group B). In group A, three types of ESP

To explore the mechanism regarding the regulation of spinal cord ischemia (SCI) in rats by mild hypothermia. A SCI rat model was established through aorta occlusion, and in some cases, the rats were intervened with mild hypothermia, after which motor function, microglia activation, and M1/M2 polarization in rats were measured. Also, the expression of inflammatory cytokines (IL-1β, IL-6 and TNF-α) and neuronal apoptosis were examined. Lipopolysaccharide (LPS)-induced M1 microglia and IL-4-induced M2 microglia were intrathecally injected into rats to evaluate the effect of microglial polarizatio
