Spine Conditions: Occupational Spine Conditions.

Neck and back conditions have significant effects on employee health and productivity. More than $7 billion in lost revenue in the United States annually can be attributed to workplace back pain among employees ages 40 to 65 years. According to the Bureau of Labor Statistics, in 2012 back pain was the most prevalent musculoskeletal condition resulting in workplace absenteeism. The incidence of these conditions is higher among individuals in certain professions, such as bus drivers, police officers, and correctional officers. Risk factors include obesity, depression, nicotine dependence, and al

Spine Conditions: Cervical Spine Conditions.

There are a variety of causes of neck pain, including trauma and degenerative changes. The history of onset helps to direct evaluation, including the need for imaging or ancillary testing. Patterns of pain, weakness, and other specific physical examination findings also aid in diagnosis. Management of most soft tissue injuries involves early mobilization, whereas fractures are managed based on patterns of instability. Acute spinal cord injury requires emergent care, to include possible decompressive surgery. The role of hypothermia and corticosteroids in these patients remains unclear. Cervica

Upper Cervical Spine Trauma: WFNS Spine Committee Recommendations.

Craniovertebral junction (CVJ) trauma is a challenging clinical condition. Being a highly mobile functional unit at the junction of the skull and the vertebral column, traumatic events in this area may produce devastating neurological complications and death. Additionally, many of the CVJ traumatic injuries can be left undiagnosed or even raise difficult treatment dilemmas. We present a literature review in the format of recommendations on the diagnosis and management of different scenarios for upper cervical trauma and produce recommendations, which can be applicable to various areas of the g

[Influence of macromastia on cervical spine and thoracic spine].

Macromastia refers to an excessively large breast volume, which can occur on one or both sides and is disproportionate to the patient's body proportions. Patients with macromastia often suffer from chronic pain of the breasts, shoulders, back and neck, degenerative changes of the spine, increased kyphosis of the thoracic spine, increased lordosis of the cervical and lumbar spine, headache, neurological problems such as dysesthesia of the ulnar nerve and carpal tunnel syndrome. Reduction mammoplasty is the gold standard in the treatment of symptomatic macromastia. A resection weight of more tha

Spine infections.

Infections of the spine represent a rare but potentially debilitating and neurologically devastating condition for patients. Early diagnosis, imaging, and intervention may prevent some of the more critical complications that may ensue from this disease process, including alignment abnormalities, central canal compromise, nerve root impingement, vascular complications, and spinal cord injury. This article reviews the underlying pathophysiologic basis of infection, clinical manifestations, and imaging modalities used to diagnose infections of the spine and spinal cord.

Spine imaging.

Imaging of the vertebral column and spinal cord has evolved from plain film evaluation to computed tomography (CT), CT in conjunction with myelography, and most recently magnetic resonance imaging (MRI). Currently, MRI has essentially replaced other imaging modalities in evaluation of the spine because of its superiority in demonstrating anatomic detail and multiplanar capability. The following is a review of the various imaging modalities available and their application in the evaluation of common spinal disorders.

Spine duplication.

Spinal column duplication in association with other vertebral, gastrointestinal, and genitourinary tract anomalies is reported in a female infant. An embryologic explanation is postulated, in which the combination of anomalies is attributed to a single embryologic insult occurring the latter half of the third week of gestation.

Degenerative Spine.

Spine degeneration represents a continuum of biomechanically related alterations that evolve over time. Disc degeneration is partly attributable to physiologic aging or senescence and partly due to pathologic conditions and this distinction is sometimes not very clear. Standard MR imaging is the mainstay for evaluating spine degeneration and newer noninvasive quantitative MR imaging techniques are rapidly emerging to detect early biochemical changes before the onset of structural changes. Degenerative disc disease although being a common and remarkable health problem, is still not completely u

Spine trauma.

Spinal fractures represent 3% to 6% of all skeletal injuries. Spine trauma is a complex diagnostic area in which the radiological assessment is crucial. Plain radiography is often used as the initial diagnostic modality. However, stabilization of the acutely injured spine is a primary concern. In this respect, computed tomography (CT) is vastly superior to plain film in terms of speed and accuracy. In many trauma centers, CT has replaced plain film as the primary modality for evaluation of spinal trauma. Magnetic resonance imaging is not indicated for all cases of spinal trauma but provides de

Lumbar Spine Mucormycosis Mimicking Pott's Spine: A Case Report.

BACKGROUND: Spinal mucormycosis is an exceptionally rare form of invasive fungal infection, with fewer than 12 confirmed cases reported in the prior literature. Immunocompromised patients are disproportionately affected, and mortality exceeds 50% in this population. Chronic liver disease as a primary predisposing condition is underrepresented in the existing case series. CASE PRESENTATION: A 54-year-old male with Child-Pugh Grade C chronic liver cirrhosis, diabetes mellitus, and hypothyroidism presented with progressive low back pain and bilateral lower limb weakness of one week duration. Kyphosis After Thoracolumbar Spine Fractures: WFNS Spine Committee Recommendations.

Thoracolumbar fractures change the biomechanics of the spine. Load distribution causes kyphosis by the time. Treatment of posttraumatic kyphosis is still controversial. We reviewed the literature between 2010 and 2020 using a search with keywords "thoracolumbar fracture and kyphosis." We removed osteoporotic fractures, ankylosing spondylitis fractures, non-English language papers, case reports, and low-quality case series. Up-to-date information on posttraumatic kyphosis management was reviewed to reach an agreement in a consensus meeting of the World Federation of Neurosurgical Societies (WFN

Rigid spine syndrome and rigid spine sign in myopathies.

We studied eight patients with rigid spine syndrome aged 8 to 20 years at the time of first examination. Muscle weakness, rigid spine, and flexion contracture of elbows and ankles were noted in the first 6 years of age. Radiological study of the cervical spine revealed considerable reduction not only of flexion, but also of extension, of the neck. The "alligator sign" was demonstrated. Progression of scoliosis and of contractures reduced the functional capacity in six patients. A restrictive ventilatory syndrome was observed in all, while central apneas with oxygen desaturation were shown in t

Anatomy of deer spine and its comparison to the human spine.

The anatomical parameters of the thoracic and lumbar regions of the deer spine were evaluated and compared with the existing data of the human spine. The objective was to create a database for the anatomical parameters of the deer spine, with a view to establish deer spine as a valid model for human spine biomechanical experiments in vitro. To date, the literature has supported the use of both calf and sheep spines as a suitable model for human spine experiments as the difficulty in procuring the human cadaveric spines is well appreciated. With the advent of Bovine Spongiform Encephalopathy (B

Multiple thoracic spine fractures complicating ankylosing hyperostosis of the spine.

Ankylosing hyperostosis of the spine is a distinct clinical entity that merits diagnosis and recognition of its structural limitations. The case of multiple thoracic spine fractures presented here emphasizes the potentially increased susceptibility of involved spines to trauma and also suggests the need to caution diagnosed patients from placing their spines at undue risk.
