Presbycusic neuritic degeneration within the osseous spiral lamina.

OBJECTIVE: To describe a neglected anatomic variant occurring with presbycusis. STUDY DESIGN: Retrospective temporal bone histopathology study. METHODS: Quantitative analysis of peripheral hair cells, neurites, neurons, and the stria vascularis in temporal bones from individuals who had presbycusis. Fifty-three patients aged 65 years or older and with a down-sloping audiogram and clinical diagnosis of presbycusis were reviewed. Nine cases had normal hair and ganglion cell populations but reduced peripheral processes (neuritic presbycusis). These were compared with five normal-hearing control

How the Human Cochlea Moves: Biomechanical Modeling of a Wide, Layered Osseous Spiral Lamina.

PURPOSE: The human cochlear partition (CP) at the high-frequency region features a radially wide, layered osseous spiral lamina (OSL) and a soft-tissue bridge connecting it to the basilar membrane (BM). The OSL consists of two thin bony plates separated by a cavernous space that serves as a conduit for auditory nerve fibers. We used a finite element model with two fluid chambers, incorporating novel implementations of the CP features, to study the human cochlea. Model results were compared with experimental measurements of CP motion. METHODS: Model geometrical and material properties either c

Human vestibular schwannoma reduces density of auditory nerve fibers in the osseous spiral lamina.

Hearing loss in patients with vestibular schwannoma (VS) is commonly attributed to mechanical compression of the auditory nerve, though recent studies suggest that this retrocochlear pathology may be augmented by cochlear damage. Although VS-associated loss of inner hair cells, outer hair cells, and spiral ganglion cells has been reported, it is unclear to what extent auditory-nerve peripheral axons are damaged in VS patients. Understanding the degree of damage VSs cause to auditory nerve fibers (ANFs) is important for accurately modeling clinical outcomes of cochlear implantation, which is a

Aminoglycoside Increases Permeability of Osseous Spiral Laminae of Cochlea by Interrupting MMP-2 and MMP-9 Balance.

The spiral ganglion neurons (SGNs) located in the Rosenthal's canal of cochlea are essential target for cochlear implant. Previous studies found that the canaliculi perforantes, small pores on the surface of the osseous spiral lamina (OSL) of the scala tympanic (ST) of cochlea, may provide communication between the cochlear perilymph and SGNs. In this study, we found that chronic treatment of aminoglycosides antibiotics, which is well known to cause sensory cell damage in the cochlea, induced significant damage of bone lining cells on the OSLs and increased the permeability of the Rosenthal's

Finite element analysis of the osseous spiral lamina's influence on inner ear fluid flow during bone conduction stimulation.

Recent studies have investigated the anatomy and motion of the human cochlear partition, revealing insights into the flexible nature of the osseous spiral lamina (OSL). These investigations have primarily focused on air-conducted stimulation, leaving the impact of the OSL's flexibility during bone-conducted (BC) stimulation largely unexplored. By considering the OSL as either flexible or rigid in a finite element model of the human inner ear, we examined the effect of the OSL's flexibility on the fluid flow in the inner ear during BC stimulation, which was divided into contributors entering vi

Cochlear Implant Insertion Trauma Is Associated With Spiral Ganglion Neuron "Dead Zones" in the Human.

HYPOTHESIS SIGNIFICANT: Cochlear implant (CI) insertion trauma, as evidenced by fracture of the osseous spiral lamina (OSL), is associated with localized "dead zones" and focal spiral ganglion neuron (SGN) loss. BACKGROUND: Hearing and structure preservation approaches to CI insertion aim to minimize trauma and preserve residual SGNs. In cases of significant insertion trauma, peripheral axons running through the bony OSL inherently become damaged if the OSL is fractured. The current investigation sought to determine if the relative location of OSL fracture was associated with focal areas of S

Morphological changes in the normal and neomycin-perfused guinea pig cochlea following chronic prosthetic implantation.

The effects of chronic prosthetic implantation and interval electrical stimulation were studied in the normal and neomycin-perfused cochlea of the guinea pig. One group of guinea pigs was implanted with a multiple-electrode prosthesis in the scala tympani. During a 4-week period, the device was stimulated for 3 hours weekly with a continuous, 1 kHz sinusoidal current of constant intensity. A second group of guinea pigs underwent identical implantation and stimulation except that cochlear perfusion with .1 M neomycin was performed at the time of implantation. Current intensities ranged from .1

Quantitative evaluation of cochlear neurons and computer-aided three-dimensional reconstruction of spiral ganglion cells in humans with a peripheral loss of nerve fibres.

Quantitative data on human cochlear neuronal elements were collected from various regions in five patients with high-tone hearing loss due to presbycusis and in two patients with normal hearing. The number of nerve fibers was assessed in the spiral lamina and in the inner acoustic meatus together with counts of spiral ganglion cells. The results show that the number of neurons decreased peripherally, i.e., with increasing distance from the central nervous system in patients with high-tone hearing loss due to presbycusis. In two patients with normal hearing no significant difference in the numb

Inner ear morphologic changes resulting from cochlear implantation.

Insertion of electrodes into the cochlea can damage the spiral ligament, organ of Corti, osseous spiral lamina, and dendrites. The amount of damage depends on the method of insertion and the amount of surgical trauma. Damage is limited to the area immediately adjacent to the electrode. Damage in the cochlea and prolonged electrical stimulation do not affect ganglion cell population. The ganglion cell population responds directly to the electrical stimulus. Examination of three auditory nerves from three cases and the cochlear nuclei from one case also reveals no effect of prolonged electrical

Quantitative analysis of cochlear sensory cells and neuronal elements in man.

The myelinated radial fibres in the osseous spiral lamina and the myelinated fibres in the cochlear nerve in the internal auditory canal as well as the sensory cells were counted in cochleae from 15 dissected temporal bones from 8 patients. Light microscopy was carried out on semithin sections of epoxy resin embedded tissue. Audiometry had been performed within 6 months prior to death. Three patients had normal hearing for their age group, 2 had slight presbyacusis and the remaining 3 had sustained noise injury. All specimens clearly had fewer fibres in the spiral lamina than in the internal a

Cochlear hair cell and vascular changes in the guinea pig following high level pure-tone exposures.

Noise is thought to exert metabolic and/or mechanical stress on sensory and vascular tissues of the cochlea, the relative influence of the stressors being influenced by the intensity of the noise. Guinea pigs exposed to either of two pure-tone frequencies, 1.33 or 3.85 kHz for 6 hours at intensity levels ranging from 102 dB to 120 dB SPL, were studied for pathological changes in two spiral lamina vessels--the vessel of the basilar membrane (VSBM) and the vessel of the tympanic lip (VSTL). In general, animals sustaining mild to severe degrees of hair cell destruction one month after noise expos

Otosclerosis under microCT: New insights into the disease and its anatomy.

PURPOSE: Otospongiotic plaques can be seen on conventional computed tomography (CT) as focal lesions around the cochlea. However, the resolution remains insufficient to enable evaluation of intracochlear damage. MicroCT technology provides resolution at the single micron level, offering an exceptional amplified view of the otosclerotic cochlea. In this study, a non-decalcified otosclerotic cochlea was analyzed and reconstructed in three dimensions for the first time, using microCT technology. The pre-clinical relevance of this study is the demonstration of extensive pro-inflammatory buildup in

Histopathology of the Human Inner Ear in the p.L114P COCH Mutation (DFNA9).

The histopathology of the inner ear in a patient with hearing loss caused by the p.L114P COCH mutation and its correlation with the clinical phenotype are presented. To date, 23 COCH mutations causative of DFNA9 autosomal dominant sensorineural hearing loss and vestibular disorder have been reported, and the histopathology of the human inner ear has been described in 4 of these. The p.L114P COCH mutation was first described in a Korean family. We have identified the same mutation in a family of non-Asian ancestry in the USA, and the temporal bone histopathology and clinical findings are presen

Fast alterations of vascular endothelial growth factor (VEGF) expression and that of its receptors (Flt-1, Flk-1 and Neuropilin) in the cochlea of guinea pigs after moderate noise exposure.

Vascular endothelial growth factor (VEGF) is a vascular permeability regulating, proangiogenic factor with neuroprotective properties. Its expression in the inner ear has been demonstrated, but little is known concerning its subcellular distribution or potential involvement in sound perception and adaptation to noise. Therefore, we determined the expression patterns and levels of VEGF and the three VEGF-receptors FLK, FLT and Neuropilin in the cochlea of guinea pigs, and examined the alterations occurring after noise exposure. After 70 dB exposure, VEGF expression was found to be reduced in al

Immuno-surveillance and protection of the human cochlea.

BACKGROUND: Despite its location near infection-prone areas, the human inner ear demonstrates remarkable resilience. This suggests that there are inherent instruments deterring the invasion and spread of pathogens into the inner ear. Here, we combined high-resolution light microscopy, super-resolution immunohistochemistry (SR-SIM) and synchrotron phase contrast imaging (SR-PCI) to identify the protection and barrier systems in the various parts of the human inner ear, focusing on the lateral wall, spiral ganglion, and endolymphatic sac. MATERIALS AND METHODS: Light microscopy was conducted on

Electron microscopic findings in presbycusic degeneration of the basal turn of the human cochlea.

Three human temporal bones with presbycusis affecting the basal turn of the cochlea were studied by light and electron microscopy. Conditions in two ears examined by light microscopy were typical of primary neural degeneration, with a descending audiometric pattern, loss of cochlear neurons in the basal turn, and preservation of the organ of Corti. Ultrastructural analysis revealed normal hair cells and marked degenerative changes of the remaining neural fibers, expecially in the basal turn. These changes included a decrease in the number of synapses at the base of hair cells, accumulation of
