[Splenic neoplasms].

Carcinomas of the spleen are rare, whether primary or secondary. Two patients are described, one with a primary angiosarcoma. In this patient the first symptom was abdominal pain, caused by spontaneous rupture of the spleen. The other case was a woman with carcinoma of the breast which metastasized to the spleen. Both underwent splenectomy and adjuvant oncological treatment. Angiosarcomas constitute less than 1% of all sarcomas. Only about 60 cases of angiosarcomas in the spleen have been reported in the world literature. Spontaneous rupture of the spleen occurred in 30% of these cases. The pr

Splenic metastasis from gastrointestinal neoplasms: a review.

Spleen metastases from solid tumours are rather exceptional, especially for those located in the digestive tract. Although these lesions are usually associated with multivisceral disease at terminal stage, several cases of isolated lesions have also been described in the literature. Diagnosis of spleen lesions associated with multivisceral disease rarely influences patient's outcome. On the other hand, isolated, only-splenic lesions could be curatively treated, allowing physicians to obtain better patient's survival. The aim of this article is therefore to review and summarize a systematic sea

Pretransplant Splenic Irradiation in Patients With Myeloproliferative Neoplasms.

PURPOSE: Allogeneic hematopoietic cell transplantation (HCT) serves as the only curative treatment option for patients with myelofibrosis and other myeloproliferative neoplasms. Splenomegaly commonly manifests in patients with myeloproliferative neoplasms and can lead to delayed or poor engraftment, increased transfusion burden, and worse survival. Methods to decrease the effect of splenomegaly include splenectomy and splenic irradiation. We sought to report on clinical outcomes for patients treated with splenic irradiation as part of their transplant conditioning. METHODS AND MATERIALS: Pati

Marginal zone-related neoplasms of splenic and nodal origin.

BACKGROUND: The marginal zone is an anatomically distinct B-cell compartment of lymphoid tissue with an abundant antigenic influx. Among marginal zone-derived lymphomas the WHO classification listed, in addition to extranodal marginal zone B-cell lymphoma of MALT type, two other marginal zone B-cell neoplasms: splenic marginal zone B-cell lymphoma (+/- villous lymphocytes) and nodal marginal zone B-cell lymphoma (+/- monocytoid B cells). These two entities are well characterized histologically, but specific biological markers are lacking. Treatment options are heterogeneous, including a watch-

Splenic metastases from mucinous neoplasms of the appendix and colon.

AIMS AND BACKGROUND: Splenic metastases associated with mucinous intraabdominal tumors have been an enigma in the radiologic and oncology literature. These focal parenchymal defects from a non-metastasizing malignancy within an organ that rarely develops metastatic foci, even with high-grade cancer, were studied. METHODS: Information on 9 patients who underwent splenectomy with intraparenchymal splenic masses associated with appendiceal or colorectal mucinous tumors with peritoneal dissemination was collected. The CT scan, the histopathology and the clinical parameters of these patients were

Splenic involvement by peripheral T-cell and NK-cell neoplasms.

In contrast to B-cell lymphomas, the literature on the splenic pathology of peripheral T-cell and NK-cell lymphomas is limited. Several different patterns of splenic involvement can be observed in peripheral T-cell and NK-cell lymphomas: 1) solitary or multiple fleshy nodule, which is seen predominantly in tumors rich in large cells; 2) diffuse red pulp involvement, which is the commonest pattern; 3) colonization of periarteriolar shealth; and 4) patchy haphazard involvement. However, the miliary small nodule pattern commonly observed in low-grade B-cell lymphomas is practically never seen. In

Asymptomatic multiple splenic cysts in a pulmonary neoplasm patient.

Splenic cysts are rare, and their treatment remains challenging. A 66-year-old man scheduled to undergo surgical treatment for a pulmonary neoplasm was found with abdominal computed tomography and ultrasonography to have multiple cysts in the body of the spleen. He underwent pulmonary wedge resection, and histological examination showed that the lesion of the left lung was an adenocarcinoma. The patient recovered without complications after the operation. Because the splenic cysts were small and caused no abdominal symptoms, the patient was advised to undergo careful follow-up. Large splenic c

An Uncommon Confluence: Splenic Lymphangioma and Appendiceal Mucinous Neoplasm.

Splenic lymphangiomas, per se, are exceedingly rare, benign congenital malformations of lymphatic vessels, typically diagnosed in childhood or incidentally in adulthood. Appendiceal mucinous neoplasms (AMNs) are also infrequent, often asymptomatic, and usually discovered incidentally. To date, the co-occurrence of these two distinct primary lesions in the same patient has not been previously reported. We present the case of an 83-year-old Turkish female who underwent emergent abdominal surgery for an acute abdomen secondary to a strangulated left diaphragmatic herniation. Multiple perforations

Isolated splenic tuberculosis mimicking a solid splenic neoplasm in an immunocompetent patient: A surgical case report.

INTRODUCTION AND IMPORTANCE: Tuberculosis is the leading cause of death by an infectious agent globally, especially among immunocompromised patients. Extrapulmonary tuberculosis (EPTB) can occur as a sequela of pulmonary tuberculosis through lympho-hematogenous or miliary spread, or can occur in isolation. EPTB rarely occurs in isolation in a single organ, especially in immunocompetent patients. Limited data exist on the incidence of isolated splenic tuberculosis in immunocompetent patients. CASE PRESENTATION: This report highlights a case of a 45-year-old immunocompetent female with a one-mo

Splenic marginal zone lymphoma. A distinct B-cell neoplasm.

The splenic marginal zone is a morphologically and perhaps immunologically distinct B-cell compartment. Lymphomas arising from cells of the splenic marginal zone are rare. Here we describe the morphologic, immunologic, and clinical features of 14 cases. Patient age ranged from 35 to 79 years (median, 68 years) with a male-to-female ratio of 1:1.8. The spleen was uniformly enlarged (median, 1,540 g; range, 388-3,845 g) in all patients, the neoplastic infiltrate had a nodular pattern in three cases, nodular and diffuse in seven cases, and diffuse in four cases. The neoplastic cells had small to

Giant chronic splenic infarction mimicking splenic neoplasm in a patient with rheumatic heart disease: a case report.

BACKGROUND: Giant chronic splenic infarction is an extremely rare condition with insidious onset and atypical imaging features, carrying a high risk of misdiagnosis as splenic neoplasm. This case highlights the atypical clinical features and important diagnostic considerations of this uncommon disorder. CASE DESCRIPTION: A 73-year-old female presented with recurrent epigastric pain for 2 years, with a medical history of rheumatic heart disease and hypertension. Contrast-enhanced abdominal computed tomography showed a huge splenic mass measuring approximately 18.0 × 14.2 × 19.1 cm with heterog

Clonal X-chromosome inactivation suggests that splenic cord capillary hemangioma is a true neoplasm and not a subtype of splenic hamartoma.

Splenic hamartoma is a rare tumor-like lesion composed of structurally disorganized red pulp elements. It has been hypothesized that two other splenic lesions, cord capillary hemangioma and myoid angioendothelioma, may fall within the spectrum of splenic hamartoma, simply representing morphological variants. In this study, we compared the vascular and stromal composition of cord capillary hemangioma and myoid angioendothelioma with those of classical hamartoma. In addition, we assessed the clonal vs polyclonal nature of the lesions in nine female cases by performing clonality analysis for X-ch

[Solitary splenic neoplasm as an unusual presentation in an adolescent with sporadic Burkitt lymphoma].

UNLABELLED: Burkitt lymphoma is a non-Hodgkin B-cell lymphoma with a high prevalence in the pediatric population. Abdominal manifestations are well known in sporadic Burkitt lymphoma and vary from nonspecific symptoms to intestinal obstruction due to intussusception; however, mass-like splenic involvement has been scarcely described. OBJECTIVE: To present a case of a patient with a splenic mass whose histopathological analysis revealed Burkitt lymphoma. CLINICAL CASE: A 13-year-old female patient presented with abdominal pain, progressive weight loss, and fever. Imaging studies showed a sple

Primary Splenic Angiosarcoma: Clinical and Imaging Manifestations of This Rare Aggressive Neoplasm.

Primary splenic angiosarcoma is a rare and fatal neoplasm arising from vascular endothelial cells within the spleen. With an incidence of 2 cases per 10 million people worldwide, the diagnosis and treatment of this rare entity is unfamiliar and challenging. We describe the case of a previously healthy 45-year-old woman who presented with vague upper-abdominal pain and was found to have a splenic mass on computed tomography. The patient underwent laparoscopic splenectomy and was found to have splenic angiosarcoma on microscopic evaluation. Although specific radiologic diagnosis is not possible,

Improving diagnosis of atraumatic splenic lesions, part II: benign neoplasms/nonneoplastic mass-like lesions.

Focal atraumatic splenic lesions often pose a diagnostic challenge on cross-sectional imaging. They can be categorized based on etiology as nonneoplastic, benign neoplastic (discussed in Part II), and malignant neoplastic lesions or on prevalence as common, uncommon, and rare lesions. Familiarity with pertinent clinical parameters, etiology, pathology, prevalence and ancillary features such as splenomegaly, concomitant hepatic involvement, and extrasplenic findings, in addition to knowledge of imaging spectra of the lesions, can improve diagnostic confidence. Consideration of these factors tog
