Management of atypical squamous cells, low-grade squamous intraepithelial lesions, and cervical intraepithelial neoplasia 1.

In the American Society for Colposcopy and Cervical Pathology 2006 Consensus Guidelines, several changes in the management of mildly abnormal cervical cytology and histology were made. The most notable changes involve the management of adolescents, pregnant women, and postmenopausal women. For adolescents, management of atypical squamous cells of undetermined significance and low-grade squamous intraepithelial lesions is conservative, eliminating the need for immediate colposcopy. For pregnant women, options have been made to allow for deferral of colposcopy until pregnancy completion, whereas

Is the low-grade squamous intraepithelial lesion/atypical squamous cells cannot exclude high-grade squamous intraepithelial lesion category associated with cervical intraepithelial neoplasia 2?

OBJECTIVE: A number of cervical smears may exhibit unequivocal low-grade squamous intraepithelial lesions (LSIL) in association with atypical cells cytomorphologically suspicious, but not sufficient to be interpreted as high-grade squamous intraepithelial lesions (HSIL). These lesions are presently called LSIL, atypical squamous cells cannot exclude HSIL (LSIL/ASC-H). Previous studies have shown that LSIL/ASC-H and ASC-H are both equivocal for HSIL and have a high risk of underlying HSIL. However, in researching the literature only two studies were found which rendered the results as cervical

Low-Grade Squamous Intraepithelial Lesion or High-Grade Squamous Intraepithelial Lesion? Concordance Between the Interpretation of Low-Grade Squamous Intraepithelial Lesion and High-Grade Squamous Intraepithelial Lesion in Papanicolaou Tests: Results From the College of American Pathologists PAP Education Program.

CONTEXT.—: Obtaining diagnostic concordance for squamous intraepithelial lesions in cytology can be challenging. OBJECTIVE.—: To determine diagnostic concordance for biopsy-proven low-grade squamous intraepithelial lesion (LSIL) and high-grade squamous intraepithelial lesion (HSIL) Papanicolaou test slides in the College of American Pathologists PAP Education program. DESIGN.—: We analyzed 121 059 responses from 4251 LSIL and HSIL slides for the interval 2004 to 2013 using a nonlinear mixed-model fit for reference diagnosis, preparation type, and participant type. We evaluated interactions b

Anal Squamous Intraepithelial Lesions: A Primer for Gastroenterologists.

PURPOSE OF REVIEW: The purpose of this review is to provide gastroenterologists a basic understanding of what anal squamous intraepithelial lesions (ASILs) are, how they develop, who are the high-risk populations, who and how to screen, and management of these lesions. RECENT FINDINGS: Human papilloma virus (HPV) infection is the most common cause of squamous cell carcinoma of the anus. Anal intraepithelial neoplasia is a precursor to the development of anal squamous cell carcinoma, and screening can lead to early detection and cure. Despite the availability of HPV vaccination for primary pre

Treatment options for high-grade squamous intraepithelial lesions.

The introduction of testing for high-risk HPV types and P16 immunostaining of CIN2 histologic specimens allows for determination of the risk of progression versus regression for a woman with a particular cytologic or histologic specimen. Observation with serial cytological or colposcopic examinations is now appropriate for women with low-grade histologic lesions as well as pregnant and young women with certain high-grade histologic lesions. Current recommendations for management of high-grade lesions, the efficacy of treatment options (cryotherapy and LEEP), and the immediate (bleeding, infect

HPV and cervical squamous intraepithelial lesions: clinicopathological study.

The aim of our study is to determine the evidence of HPV infection, by either HPV-DNA test or histopathological examination, in patients with abnormal PAP-smear, for further clinical management. In 6-month period, we retrospectively selected a number of 103 patients in 16-54 years range of age that were investigated by complex diagnostic techniques supporting the evidence of HPV infection. Initially, these patients were evaluated cytological using the Bethesda system for PAP-smear interpretation along with colposcopic examination. A number of these patients were orientated to HPV testing (PCR)

Cytologic observations preceding high grade squamous intraepithelial lesions.

There is some evidence that a significant proportion (12%) of women, prospectively followed after negative cervical cytologic findings, develop tissue-proven high grade squamous intraepithelial neoplasia (HSIL) within a short period (up to 24 months). The present study was undertaken to address the validity of this statement since such a high prevalence of spontaneous HSIL, if true, may challenge the value of an annual cervicovaginal cytologic smear examination. Cytologic smears collected from 82 patients with tissue-proven HSIL and obtained in the preceding 24 months were evaluated. The vast

Clinical significance of the diagnosis of low-grade squamous intraepithelial lesion, cannot exclude high-grade squamous intraepithelial lesion.

BACKGROUND: The diagnosis of low-grade squamous intraepithelial lesion (LSIL), cannot exclude high-grade squamous intraepithelial lesion (LSIL-H) was not included in the 2001 Bethesda System. It is used in some institutions to diagnose cases that fulfill criteria for both the diagnosis of LSIL and atypical squamous cells, cannot exclude high-grade squamous intraepithelial lesion (ASC-H). In this study, the authors reviewed their experience with cases reported as LSIL-H during a 4-year interval. METHODS: Clinical information and histologic follow-up data were retrieved for Papanicolaou (Pap) t

Current review on squamous intraepithelial lesions of the larynx.

Squamous intraepithelial lesions (SILs) of the larynx, clinically usually defined as leukoplakia and chronic laryngitis, have remained the main controversial topic in laryngeal pathology for decades as regards classification, histological diagnosis and treatment. SILs are caused by smoking and alcohol abuse. There is also mounting evidence that gastroesophageal reflux is a potential aetiological factor. Human papillomavirus infection seems to play little if any role in laryngeal carcinogenesis. Histological classification of SILs is the central disputed aspect of these lesions. There are as ye

SIRT1 overexpression in cervical squamous intraepithelial lesions and invasive squamous cell carcinoma.

Invasive squamous cell carcinoma (SCC) of the cervix involves the progression of premalignant cervical intraepithelial neoplasia (CIN) and is associated with persistent human papillomavirus infection. Most CINs will regress, and the challenge is to identify the lesions likely to progress to invasive cancer. We evaluated Sirtuin 1 (SIRT1) expression in nonneoplastic cervix, CINs, and SCCs as a potential biomarker to predict disease progression. A total of 101 cases were selected including 29 CIN 1s, 32 CIN 2s, 16 CIN 3s, 2 microinvasive SCCs, and 22 invasive SCCs. Cervical nonneoplastic squamou

The significance of "low-grade squamous intraepithelial lesion, cannot exclude high-grade squamous intraepithelial lesion" as a distinct squamous abnormality category in Papanicolaou tests.

BACKGROUND: Early cytologic detection and treatment of high-grade squamous intraepithelial lesion (HSIL) is critical to cervical cancer prevention. The term atypical squamous cells (ASC), cannot exclude HSIL (ASC-H) was introduced in 2001 in the Bethesda System (TBS 2001) to define changes suggestive, but not diagnostic, of HSIL in the absence of unequivocal squamous intraepithelial lesion (SIL). Previous studies showed that women with ASC-H cytology are at an increased risk of harboring underlying histopathologic HSIL. TBS 2001, however, did not address the significance of finding ASC-H chang

Atypical squamous cells of undetermined significance (ASCUS), low-grade squamous intraepithelial lesion (LSIL), high-grade squamous intraepithelial lesion (HSIL) and histology.

OBJECTIVE: A new terminology for cytologic diagnosis of cervical lesions has been introduced by Bethesda System. This includes: 1) Atypical squamous cells of undetermined significance (ASCUS), 2) Low-grade squamous intraepithelial lesion (LSIL), 3) High-grade squamous intraepithelial lesion (HSIL), 4) squamo-cellular carcinoma. The aim of this study was to assess the correlation between the colpocytologic test (Pap psmear), the histologic response and colposcopy. METHODS: We re-examined the cytologic results of 447 patients who underwent routine cytologic tests, with a diagnosis of various gr

Cervical Cytokine Profile in High-Grade Squamous Intraepithelial Lesions.

OBJECTIVES: This study aimed to characterise the local cytokine milieu in the cervicovaginal environment of women with HSIL compared with healthy controls. METHODS: In this case-control study, cervical samples were collected from 84 women aged 20-49 years, including 57 patients with histologically confirmed HSIL and 27 healthy controls. Levels of IL-2, IL-4, IL-6, IL-10, TNF, IFN-γ and IL-17A were quantified in cervicovaginal lavage fluid using a Cytometric Bead Array. Cytokine ratios were calculated to assess the balance between pro-inflammatory and regulatory immune responses. Statistical c

Clinical and Morphological Study of Cervical Squamous Intraepithelial Lesions.

Squamous intraepithelial lesions (SILs) are cancer precursors targeted by secondary prevention of cervical cancer programs that are sometimes difficult to grade accurately. Mena is an actin regulatory protein involved in membrane protrusion, cell motility, in tumor invasion and metastasis. We studied retrospectively 68 cases of patients diagnosed with squamous intraepithelial lesions that received expedited treatment (treatment without colposcopic biopsy). We analyzed demographic, behavioral data, obstetrical and medical history, from the patients' medical charts and we studied the cervical fr

Correlation between squamous intraepithelial lesions (SILs) and bacterial vaginosis.

Bacterial vaginosis (BV) is a condition that seldom occurs in prepuberal girls or postmenopausal women, suggesting a hormonal component in its aetiology. The precise mechanisms by which BV arises are not fully understood. One proposed mechanism suggests that carcinogenic nitrosamines act either independently or via human papilloma virus (HPV). Human papillomavirus is known to be associated with the development of squamous intraepithelial lesion (SIL). Still today the relationship between BV and SIL is debated. Many confounding factors regarding the relationship between BV and SIL include the p
