Neural Correlates of Oral Stereognosis-An fMRI Study.

Oral stereognosis is the ability to recognize, discriminate and localize a bolus in the oral cavity. Clinical observation indicates deficits in oral stereognosis in patients with vascular or neurodegenerative diseases particularly affecting the parietal lobes. However, the precise neural representation of oral stereognosis remains unclear whereas the neural network of manual stereognosis has already been identified. We hypothesize that oral and manual stereognosis share common neuronal substrates whilst also showing somatotopic distribution. Functional magnetic resonance images (fMRI; Siemens

The Effect of Treatment on Stereognosis in Children With Hemiplegic Cerebral Palsy.

PURPOSE: To determine if rehabilitation alone or combined with surgery or botulinum toxin injection improved stereognosis in children with hemiplegic cerebral palsy. METHODS: Inclusion criteria were children with spastic hemiplegic cerebral palsy who had stereognosis testing 2 separate times with documentation of intervening treatment. Sixty-three children were included, 30 girls and 33 boys at an average age of 9.1 years (range, 4.4-16.0 years). Twelve standardized objects were used for manual identification. Baseline and postintervention stereognosis results were recorded for the hemiplegic

The effect of altered hand position and motor skills on stereognosis.

PURPOSE: Poor stereognosis has been considered to be indicative of abnormal cerebral sensory function, and therefore a relative contraindication for upper extremity surgery in hemiplegic cerebral palsy. The effect of hand position and motor function on stereognosis has not been studied; decreased hand mobility can decrease a person's ability to recognize objects in the hand and can be an important component of stereognosis. METHODS: Twenty-one normal subjects had their nondominant arm positioned to simulate the hemiplegic hand posture of forearm pronation and wrist and digital flexion. Subjec

Evaluation of the Association of Oral Stereognosis with Malocclusion in Children.

AIM AND OBJECTIVE: To evaluate the association between oral stereognosis with malocclusion in children. MATERIALS AND METHODS: Stereognostic investigation was made with a set of seven different geometric configurations (circle, square, triangle, star, clover, diamond, and heart shapes) cut out from fresh, raw carrots using preformed iron molds. Any five of the seven geometric forms were randomly placed inside the mouth on the dorsum of the tongue by the investigator with the subject's eyes closed. A minimum of three shapes correctly identified out of the five offered to each child was conside

Comparison of cerebral activation involved in oral and manual stereognosis.

Brain activity associated with manual stereognosis has been the focus of increasing recent research effort. However, although oral stereognosis, defined as the ability to recognize and discriminate the food bolus in the mouth, is important for mastication and swallowing, there is little information available about the neural network relating to this function. In the present study, cerebral activation associated with oral stereognosis was evaluated as compared with manual stereognosis. Brain imaging data were acquired by functional MRI (fMRI). fMRI experiments were performed on 16 healthy right

Pioneering the concepts of stereognosis and polyradiculoneuritis: Octave Landry (1826-1865).

Octave Landry was one of a long list of fine 19th century clinicians who died very young and whose discoveries in physiology and descriptions of new clinical pictures helped found current-day neurology. In 1852, Landry proposed a new take on the physiology of sensation which laid the ground for the concepts of proprioception and stereognosis. He also described the clinical picture of a rapidly progressing ascending paralysis, which in 1859 prefigured Guillain-Barré syndrome. In discussing his very active life, we will mention the hydrotherapies in fashion at the time and the pleasures of Paris

Correlation of motor function and stereognosis impairment in upper limb cerebral palsy.

PURPOSE: To correlate motor function, as measured by the Jebsen-Taylor test, and sensory function, as measured by the 12-object stereognosis testing, in the hands of children with spastic hemiplegia due to cerebral palsy. METHODS: A chart review identified children with hemiplegic and triplegic cerebral palsy with stereognosis and Jebsen-Taylor testing between 1997 and 2008. Forty-one children were included in the study, including 22 girls and 19 boys, with an average age of 8.7 years (range, 6-16 years). The right side is affected in 23 children; 34 children have hemiplegic cerebral palsy, a

Hemispheric Dominance for Stereognosis in a Patient With an Infarct of the Left Postcentral Sensory Hand Area.

The concept of left hemispheric dominance for praxis, speech, and language has been one of the pillars of neurology since the mid-19th century. In 1906, Hermann Oppenheim reported a patient with bilateral stereoagnosia (astereognosis) caused by a left parietal lobe tumor and proposed that the left hemisphere was also dominant for stereognosis. Surprisingly, few cases of bilateral stereoagnosia caused by a unilateral cerebral lesion have been documented in the literature since then. Here we report a 75-year-old right-handed man who developed bilateral stereoagnosia after suffering a small infar

Effect of surgical reduction of the tongue on oral stereognosis, oral motor ability, and the rest position of the tongue and mandible.

The oral ability to recognize forms and oral motor ability were studied by means of two specific tests in 27 subjects, 10 to 23 years of age, before and after tongue reduction because of macroglossia. Recordings were made before and 6 and 12 months after the operation. At the same time the natural position of the head and of the cervical column, the craniocervical relation, the position of the tongue and the hyoid bone, and the rest position of the mandible were studied with profile roentgen-cephalometry. The surgical reduction of the tongue had a minor influence on the subject's performance i

A Proposal of a Design for the Bionic Hand: Describing the Integration, Motor Controlling System, Stereognosis, and Proprioception Sensory Feedback Components.

Despite recent advancements in bionic upper limb prostheses technology, the rejection rate by users remains unacceptably high. Various factors contribute to this issue, such as limited functionality, complex control mechanisms, and discomfort, with most of these concerns being documented solely through self-assessment surveys. In this article, we introduce our proposed four components for an integrated bionic hand aimed at making it closely resemble a natural hand. These components include an integrated intramedullary stem, a kineticomyographic motor control system, sensory feedback for stereo

Evaluation of the box and blocks test, stereognosis and item banks of activity and upper extremity function in youths with brachial plexus birth palsy.

BACKGROUND: One of the greatest limitations to measuring outcomes in pediatric orthopaedics is the lack of effective instruments. Computer adaptive testing, which uses large item banks, select only items that are relevant to a child's function based on a previous response and filters items that are too easy or too hard or simply not relevant to the child. In this way, computer adaptive testing provides for a meaningful, efficient, and precise method to evaluate patient-reported outcomes. Banks of items that assess activity and upper extremity (UE) function have been developed for children with

Sensibility deficiencies in the hands of children with spastic hemiplegia.

We evaluated 40 children with spastic hemiplegia due to cerebral palsy for sensory function and relative limb size in the affected and unaffected upper extremities. Sensory function of each limb was evaluated with respect to stereognosis (12 objects), two-point discrimination, and proprioception. Four size measurements of each limb were made: arm and forearm circumference and forearm and forearm-hand length. This study showed that 97% of the spastic limbs had a stereognosis deficit, 90% had a two-point discrimination deficit, and 46% had a proprioception deficit. Thus sensory deficits are the

Comparing tests of tactile sensibility: aspects relevant to testing children with spastic hemiplegia.

Children with hemiplegic cerebral palsy (OP) commonly exhibit tactile sensory deficiencies in their hands in addition to their motor problems. The aim of this study was to compare and evaluate the usefulness of some common tests of tactile sensibility for use with children with hemiplegic CP. Twenty-five children with hemiplegia aged between 5 and 18 years, and 19 control individuals participated. All children were examined with Semmes-Weinstein monofilaments, two-point discrimination (2PD), stereognosis of familiar objects, stereognosis of forms, and functional sensibility. Dexterity, spastic

Characteristics of bilateral hand function in individuals with unilateral dystonia due to perinatal stroke: sensory and motor aspects.

The authors assessed bilateral motor and sensory function in individuals with upper limb dystonia due to unilateral perinatal stroke and explored interrelationships of motor function and sensory ability. Reach kinematics and tactile sensation were measured in 7 participants with dystonia and 9 healthy volunteers. The dystonia group had poorer motor (hold time, reach time, shoulder/elbow correlation) and sensory (spatial discrimination, stereognosis) outcomes than the control group on the nondominant side. On the dominant side, only sensation (spatial discrimination, stereognosis) was poorer in

Toffee clearance and lingual sensory and motor activities in normal children and in children with articulation problems of speech.

The rate at which a standardized mouthful of toffee containing technetium (99mTc) tin colloid was cleared from the mouth during and after chewing was measured by counting the residual radioactivity in the mouth at short time intervals by external gamma counting. The clearance curve was resolved into three phases for each subject--chewing time (1), which coincided with the time to reach 10% of the initial counting rate, scavenging time (2), and a final slow diffusion phase (3). Although the mean chewing time was only 1.9 min, the technique allowed the pattern of clearance during this phase to b

Beyond motor ımpairment somatosensory function and body awareness as contributors to activity, participation and quality of life in children with cerebral palsy.

Hemiplegic Cerebral Palsy (CP) in childhood is characterized not only by motor deficits but also by accompanying sensory and perceptual impairments. Despite the recognized importance of somatosensory functions and body awareness, their associations with activity, participation, and quality of life remain relatively unexplored within an ICF-oriented framework. This study aimed to investigate the associations between somatosensory functions, body awareness, and functional outcomes, including activity performance, participation, and quality of life, in school-aged children with hemiplegic CP.
